1.Use of separate single-tooth implant restorations to replace two or more consecutive posterior teeth: a prospective cohort study for up to 1 year.

PURPOSE: The aim of this study was to evaluate the periodontal and prosthodontic complications of multiple freestanding implants in the posterior jaws for up to 1 year of function. MATERIAL AND METHODS: Eight patients received 20 implants posterior to canines. Two or more implants were consecutively inserted to each patient. Single crowns were delivered onto the implants. Marginal bone loss, implant mobility, probing depth, and screw loosening were examined to evaluate the clinical success of such restorations for maximum 1 year of functional loading. RESULTS: All the implants performed well during the observation period. Neither periodontal nor prosthodontic complications were found except a slight porcelain chipping. While the marginal bone level was on average 0.09 mm lower around the implant after 6 months of loading, it was 0.15 mm higher after 1 year. CONCLUSION: Within the limits of this investigation, separate single-tooth implant restorations to replace consecutive missing teeth may clinically function well in the posterior jaw.

2.THE EFFECT OF AUTOLOGOUS PLATELET-RICH PLASMA (PRP) ON BONE FORMATION AROUND DENTAL IMPLANT IN THE RABBIT : A HISTOMORPHOMETRIC AND REMOVAL TORQUE STUDY.

Platelet-rich plasma (PRP) has been known to increase the rate and degree of bone formation by virtue of growth factors in concentrated platelets. Although its great healing effect on bone defect or pre-implantation site preparation in conjunction with bone substitute has been reported, the effect associated with implant is unknown. The purpose of this study was to investigate the effect of PRP on rapid osseointegration of endosseous dental implants in the rabbit tibiae. Twenty two adult female New Zealand white rabbits, weighing approximately 2.7-3.3kg, were used for this study. Twelve of the 22 animals were used for histomorphometric analysis and ten of the 22 were for removal torque test. Each animal received two implants in each tibia (two treated with PRP and two as control) and was given fluorochrome intramuscularly. For histomorphometric analysis, rabbits were divided into four groups according to the healing period. At 1 week, 2 weeks, 4 weeks and 8 weeks postoperatively, each three animals were sacrificed serially and the amount and rate of bone formation around dental implant were examined on the undecalcified sections under fluorescent microscope, polarized microscope and light microscope connected to a personal computer equipped with image analysis system. For removal torque test, rabbits were divided into two groups and removal torque tests were performed at 4 weeks, 10 weeks after implant placement. In total, 88 screw shaped, commercially pure titanium implants (Neoplant, Neobiotech, Seoul, Korea) were used in this study. Labeling pattern reflected differences of two groups in bone formation rate at each period. Histomorphometrically, PRP group showed significantly higher bone volume within threads compared to control group at 2 weeks (70.30+/-4.96% vs. 50.68+/-6.33%; P<.01) and 4 weeks (82.59+/-5.94% vs. 72.94+/-4.57%; P<.05). PRP group at 1, 2 and 4 weeks revealed similar degree of bone volume formation comparable to control group at 2, 4 and 8 weeks, respectively. On the other hand, while PRP group showed higher bone-implant contact (47.37+/-8.09%) than control group (33.16+/-13.47%) at 2 weeks, there were no significant differences between PRP group and control group for any experimental period. Removal torque values also showed no significant differences between PRP group and control group at any experimental period (P>.05). These findings imply that PRP could induce rapid, more bone formation around implant during early healing period and get faster secondary stability for reducing healing period, though it has not induced bone maturation enough to resist functional loading.

3.Results of immediate loading for implant restoration in partially edentulous patients: a 6-month preliminary prospective study using SinusQuick(TM) EB implant system.

STATEMENT OF PROBLEM: Many dental clinicians are concerned about immediate loading of inserted implants. However, there have been few clinical studies surveying the success rates of immediate loading, based on Korean implant systems. PURPOSE: The aim of this study was to evaluate the outcome of immediate functional loading of the implant (SinusQuick(TM) EB, Neobiotech Co., Seoul, Korea) in partially edentulous maxilla or mandible. MATERIAL AND METHODS: Total 15 implants were placed. Within 2 weeks after implant insertion, provisional implant-supported fixed partial dentures were delivered to the patients. Quantitatively, marginal bone loss was measured at the time of immediate loading, after 3-months of continued loading and at the last follow-up. The mean follow-up period was 4.8 months. RESULTS: Mean marginal bone loss from implant surgery to early loading, 3-months follow-up and last follow-up was 0.03 +/- 0.07 mm, 0.16 +/- 0.17 mm and 0.29 +/- 0.19 mm. No implant failed up to 6 months after insertion, resulting in a 100% survival rate. CONCLUSION: Immediate loading exhibited high success rate in partial edentulism for up to 6 months. Well-controlled long term clinical studies with large sample size are necessary to confirm this finding.

4.A Study on the Comparison of Video Display Terminal(VDT) Subjective Symptoms for Computer-addicted and Non-addicted Adolescents.

PURPOSE: The purpose of this study was to compare subjective symptoms of VDT between computer-addicted and non-addicted adolescents. METHOD: A descriptive survey design was used and 646 students in one middle and two high schools were selected as participants. RESULT: The VDT subjective symptoms and degree of severity differed according to whether the students were computer-addicted or not, and in all symptoms, general, musculoskeletal, eye and mental, the mean score for subjective symptoms was higher in the addicted group than in the non-addicted group. The score for VDT subjective symptoms was highest in the addicted group for girls and students who were not healthy. The most frequent physical symptom reported by students who visited the school health room for a health problem after using the computer was headache. The most frequent type of treatment at the school health room was treatment of the symptom. CONCLUSIONS: This study suggests that students must acquire correct habits in computer use and be careful not to be addicted to the computer in order to avoid VDT syndrome. For this, educational authorities should develop computer-related health education programs and start the programs from the lower grades of elementary school.

5.Influence of Verbal Abuse on Job Stress for Special Unit Nurses and General Ward Nurses in General Hospitals.

PURPOSE: It is expected that there will be a difference in job stress between special unit (operating room, anesthesia room) nurses and general ward nurses in general hospitals, but factors influencing job stress have not been examined. This study reports on research to examine the sources of verbal abuse in the workplace. METHODS: Participants were 243 nurses (special unit 112, general ward 132) working in a hospital in I city. Data collection period was March 18 to March 25, 2016 and questionnaires were used to collect data. Comparison of differences in influence of verbal abuse on job stress between special unit and general ward nurses was analyzed with multiple hierarchical regression. RESULTS: In special unit nurses, verbal abuse from doctors and/or nurses influenced their job stress (Adj. R²=.545). In general ward nurses, verbal abuse from patients and/or care-givers influenced their job stress (Adj. R²=.241). CONCLUSION: Findings indicate a need to campaign for increase respect among nurse-doctor, nurse-nurse, and nurse-patients/care givers because verbal abuse from doctors, nurses, and patients/care-givers causes negative attitudes in nurses. General ward nurses in particular should provide sufficient explanation to patients about nursing services because verbal abuse from patients/care-givers is related to their complaints about nursing care.

6.A Novel Urotensin II Receptor Antagonist, KR-36996 Inhibits Smooth Muscle Proliferation through ERK/ROS Pathway.

Urotensin II (UII) is a mitogenic and hypertrophic agent that can induce the proliferation of vascular cells. UII inhibition has been considered as beneficial strategy for atherosclerosis and restenosis. However, currently there is no therapeutics clinically available for atherosclerosis or restenosis. In this study, we evaluated the effects of a newly synthesized UII receptor (UT) antagonist, KR-36996, on the proliferation of SMCs in vitro and neointima formation in vivo in comparison with GSK-1440115, a known potent UT antagonist. In primary human aortic SMCs (HASMCs), UII (50 nM) induced proliferation was significantly inhibited by KR-36996 at 1, 10, and 100 nM which showed greater potency (IC₅₀: 3.5 nM) than GSK-1440115 (IC₅₀: 82.3 nM). UII-induced proliferation of HASMC cells was inhibited by U0126, an ERK1/2 inhibitor, but not by SP600125 (inhibitor of JNK) or SB202190 (inhibitor of p38 MAPK). UII increased the phosphorylation level of ERK1/2. Such increase was significantly inhibited by KR-36996. UII-induced proliferation was also inhibited by trolox, a scavenger for reactive oxygen species (ROS). UII-induced ROS generation was also decreased by KR-36996 treatment. In a carotid artery ligation mouse model, intimal thickening was dramatically suppressed by oral treatment with KR-36996 (30 mg/kg) which showed better efficacy than GSK-1440115. These results suggest that KR-36996 is a better candidate than GSK-1440115 in preventing vascular proliferation in the pathogenesis of atherosclerosis and restenosis.

7.Factors Influencing Competence in the Intensive and Critical Care Nursing of Senior Nursing Students.

PURPOSE: This study was conducted to investigate the factors influencing the competence in intensive and critical care nursing of senior nursing students. METHODS: A descriptive survey was used to examine the factors that influence competence in intensive and critical care nursing. The data were collected using self-administered questionnaires; 237 senior nursing students were asked about general characteristics, critical thinking, communication competence, self-esteem, self-resilience, and self-leadership. RESULTS: Multiple regression analysis indicated that self-leadership, self-esteem, and confidence in intensive care unit practice predicted the competence in intensive and critical care nursing (37.8% of explanatory power). Critical thinking, communication competence, and self-resilience were not shown to be significant predictors of, but were positively correlated with, the competence in intensive and critical care nursing. CONCLUSION: Based on the results, it is suggested that self-leadership, self-esteem, and confidence in intensive care unit practice should be considered in nursing education in order to improve nursing students' level of competence in intensive and critical care nursing.

8.Inhibition of MMP-13 mRNA expression by ginseng saponin in fetal rat calvarial cells.

There is a potential role of collagenase-3 in alveolar bone loss and periodontal disease progression, we need to develope or find chemotherapeutic drugs or herbal agents which may regulate the expression of MMP-13. Ginseng saponin, one of the major components of Korea ginseng(panax ginseng) root, has many various biologic effects, such as cytotoxic effect, tumoricidal effects, cytokine regulations, and protein biosynthesis effect. The purpose of this study was to determine the effects of Korea red ginseng saponin on MMP-13 gene expression in osteoblasts. The experimental groups were cultured with ginseng saponin in concentration of 1.0, 10, 25, 50, 100, 250 and 500microgram/ml for MTT assay. Primary rat calvarial cells were pre-treated for 1 hour with ginseng saponin(100 microgram/ml) and then stimulated with IL-1beta(1.0ng/ml) and PTH (10 nM). MMP-13 gene expression was evaluated by RT-PCR. The results were as follows: Ginseng saponin was cytotoxic to osteoblast at concentration exceeding 250microgram/ml for longer than 24 hours in tissue culture(p<0.01). In RT-PCR analysis, steady state MMP-13 mRNA levels were increased approximately 350% by IL-1beta, and 400% by PTH when normalized to untreated control. IL-1beta-indued MMP-13 mRNA expression was reduced 50% by pre- treatment with ginseng saponin. But ginseng saponin didn't inhibit MMP-13 expression from PTH stimulated cells. This results suggest that ginseng saponin inhibit IL-1beta-indued MMP-13 mRNA expres- sion.

9.Prognostic Role of High-sensitivity Cardiac Troponin I and Soluble Suppression of Tumorigenicity-2 in Surgical Intensive Care Unit Patients Undergoing Non-cardiac Surgery.

BACKGROUND: The prognostic utility of cardiac biomarkers, high-sensitivity cardiac troponin I (hs-cTnI) and soluble suppression of tumorigenicity-2 (sST2), in non-cardiac surgery is not well-defined. We evaluated hs-cTnI and sST2 as predictors of 30-day major adverse cardiac events (MACE) in patients admitted to the surgical intensive care unit (SICU) following major non-cardiac surgery. METHODS: hs-cTnI and sST2 concentrations were measured in 175 SICU patients immediately following surgery and for three days postoperatively. The results were analyzed in relation to 30-day MACE and were compared with the revised Goldman cardiac risk index (RCRI) score. RESULTS: Overall, 30-day MACE was observed in 16 (9.1%) patients. hs-cTnI and sST2 concentrations differed significantly between the two groups with and without 30-day MACE (P < 0.05). The maximum concentration of sST2 was an independent predictor of 30-day MACE (odds ratio=1.016, P=0.008). The optimal cut-off values of hs-cTnI and sST2 for predicting 30-day MACE were 53.0 ng/L and 182.5 ng/mL, respectively. A combination of hs-cTnI and sST2 predicted 30-day MACE better than the RCRI score. Moreover, 30-day MACE was observed more frequently with increasing numbers of above-optimal cut-off hs-cTnI and sST2 values (P < 0.0001). Reclassification analyses indicated that the addition of biomarkers to RCRI scores improved the prediction of 30-day MACE. CONCLUSIONS: This study demonstrates the utility of hs-cTnI and sST2 in predicting 30-day MACE following non-cardiac surgery. Cardiac biomarkers would provide enhanced risk stratification in addition to clinical RCRI scores for patients undergoing major non-cardiac surgery.

10.Clinical Study of Signet Ring Cell Carcinomas of the Stomach.

From among the 1485 patients with gastric cancer who had gastric resections at the Department of General Surgery, Chonnam National University Hospital, from January 1986 to December 1995, a clinical study was made of 104 patients (7%) who had signet ring cell carcinomas. Patients with this form of cancer tended to be younger and female; the tumors were smaller and involved the stomach body and antrum rather than the cardia, but none of these findings was significantly different from the findings for other types of gastric cancer. Mucosal and submucosal tumor were more prominent in gastric canner, and lymph-node metastases were statistically less likely to be present in signet ring cell carcinoma (p<0.05). The tumor stage of signet ring cell carcinomas tended statistically to be early (I) rather than advanced (III, IV) in comparison with those of other types of gastric cancer. The ploidy pattern of signet ring cell carcinomas was tended to be diploidy (80.7%) rather than aneuploidy (19.3%), and this finding was statistically significant compared with those of other types. Surgery was more curative in cases of signet ring cell carcinoma (92.3%) than in cases of other types of gastric cancer (73.4%), and the 5-year survival rates were 43.7% for patients with signet ring cell cancer and 32.8% for those with other types of gastric cancer (p<0.05). In patients with signet ring cell gastric cancer, the lesion is less extensive; thus, these patients probably can expect a longer survival time and good prognosis.
