4.Solitary Bone Cyst of the Capitate: A Case Report

Solitary bone cyst are usually found in the juxta-epiphyseal region of metaphysis of long bones of children. They are rarely seen in non-tubular bones. The following case describes what we believe to be a solitary cyst of the capitate. Cyst was treated by radical curettage and packing with autogenous iliac bone. The most recent evaluation 7 months after surgery showed no discomfort.

6.Do Metoclopramide with Dexamethasone Act Synergistically in Control of Postoperative Nausea and Vomiting?.

BACKGROUND: Better effect of antiemetic drugs can be obtained by combination of multiple antiemetic drugs that have different mechanisms of action. However, if the combined drugs have similar mechanisms of action, the incidence of side effects may be increase. This study was conducted to evaluate the ability of using combination therapy consisting of metoclopramide and dexamethasone in control of postoperative nausea and vomiting. METHODS: Sixty adult female patients undergoing major gynecological operation under the general anesthesia were randomly divided into two groups according to administered drugs. The patients received a single IV dose of metoclopramide (10 mg, Group I) or metoclopramide plus dexamethasone (10 mg 8 mg, Group II) when as operator sutured the skin wound. The incidences of nausea and vomiting were assessed during the first 12, 24, and 48 hours after recovery from anesthesia. RESULTS: There were no prominent incidences of postoperative nausea and vomiting between two groups within postoperative 12 hours. But there were significantly increased incidences of nausea and vomiting in Group I (26.7%, 13.3%) compared to Group II ( 8.0%, 3.2%) during postoperative 24 hours and 48 hours. CONCLUSIONS: Combination therapy of metoclopramide with dexamethasone seemes to have no synergic effect in control of postoperative nausea and vomiting.

7.A case of tongue volume measurement with the plaster tongue model for partial glossectomy in macroglossia.

The tongue is located inside the oral cavity at rest, and the structural interrelations between the tongue and the oral cavity is essentially important for normal tongue function. Macroglossia is clinically diagnosed whenever the tongue does not fit the oral cavity, thus patients complain of noisy breathing, drooling, speech and swallowing dysfunction, malocclusion, and social ridicule. Partial glossectomy for macroglossia volume has been assumed to be difficult. In this study, we attempted to measure the tongue volume with the plaster tongue model in a 7-year-old macroglossia patient. This method is reliable and simple for measuring the tongue volume in macroglossia.

8.Pityriasis Rotunda with Familial Occurrence.

Pityriasis rotunda is an uncommon chronic dermatosis characterized by multiple, round or oval, hyperpigmented or hypopigmented patches that have a fine scale on the trunk and extremities. Most of the cases reported predominantly occurred in Oriental and black patients in association with internal disease. However, in Caucasians it has been documented in healthy persons usually as a familial tendency. We report a case of pityriasis rotunda which showed familial occurrence and had no underlying disease.

9.FRACTURE STRENGTH AND MARGINAL FIT OF IN-CERAM, COPY-MILLED IN-CERAM, AND IPS EMPRESS 2 ALL-CERAMIC BRIDGES.

All-ceramic restorations have become an attractive alternative to porcelain-fused-to-metal crowns. In-Ceram, and more recently IPS Empress 2 were introduced as a new all-ceramic system for single crowns and 3-unit fixed partial dentures. But their strength and marginal fit are still an important issue. This study evaluated the fracture resistance and marginal fit of three systems of 3 unit all-ceramic bridge fabricated on prepared maxillary anterior resin teeth in vitro. The 3 all-ceramic bridge systems were: (1) a glass-infiltrated, sintered alumina system (In-Ceram) fabricated conventionally, (2) the same system with copy-milled alumina cores (copy-milled In-Ceram), (3) a heat pressed, lithium disilicate reinforced glass-ceramic system (IPS Empress 2). Ten bridges of each system with standardized design of framework were fabricated. All specimens of each system were compressed at 55degree at the palatal surface of pontic until catastrophic fracture occurred. Another seven bridges of each system were fabricated with standard method. All of the bridge-die complexes were embedded in epoxy resin and sectioned buccolingually and mesiodistally. The absolute marginal discrepancy was measured with stereomicroscope at x50 power. The following results were obtained: 1. There was no significant difference in the fracture strength among the 3 systems studied. 2. The Weibull modulus of copy-milled In-Ceram was higher than that of In-Ceram and IPS Empress 2 bridges. 3. Copy-milled In-Ceram (112micrometer) exhibited significantly greater marginal discrepancy than In-Ceram (97micrometer), and IPS Empress 2 (94micrometer at P=0.05. 4. The lingual surfaces of the ceramic crowns showed smaller marginal discrepancies than mesial and distal points. There was no significant difference between teeth (incisor, canine) at P=0.05. 5. All-ceramic bridges of three systems appeared to exhibit sufficient initial strength and acceptable marginal fit values to allow clinical application.

10.Factors Affecting Final Adult Height in Patients with Congenital Hypothyroidism.

PURPOSE: Thyroid hormone is essential for normal growth and development. The aim of this study was to evaluate the factors affecting final adult height in patients with congenital hypothyroidism. METHODS: The study group was comprised of 42 patients who were diagnosed as congenital hypothyroidism and attained final adult height. Retrospectively, we reviewed medical records as to clinical and laboratory data. We analyzed the influence of various factors on final adult height(FAH) in patients with congenital hypothyroidism. RESULTS: The mean chronologic age at initiation of treatment was 5.85+/-4.32 years and the FAH deviation score(SDS) was -1.21+/-1.14. The age at initiation of treatment, the chronologic age, the mean dose of L-thyroxine of current treatment, and the bone age delay at initiation of treatment were negatively related to the FAH SDS(P<0.05). The height SDS at initiation of treatment and the height SDS at initiation of puberty were positively related to the FAH SDS(P<0.05). Analyzing according to etiology, the FAH SDS of dyshormonogenesis, thyroid aplasia, thyroid ectopia, and thyroid hypoplasia were 0.16+/-0.27, -1.15+/-0.97, -1.45+/-1.07, and -2.70+/-1.70 respectively(P<0.05). CONCLUSION: The younger the age at initiation of treatment and the chronologic age, and the more the mean dose of L-thyroxine of current treatment and the bone age delay at initiation of treatment, The higher the final adult height SDS. The higher the height SDS at initiation of treatment and the height SDS at initiation of puberty, the final adult height SDS were the higher.
