Objective To explore the competency of community health administrators in a survey of such administrators at large in Shanghai. Methods McClelland's Competency Dictionary was used to set two benchmark positions, while the behavioral event interviewing and questionnaires were made to survey 19 community health service centers in Shanghai. Results The competency model benchmarking on the head of a medical branch consists of 6 character clusters, 11 characteristic items, 27 evaluation dimensions and 27 typical behavior descriptions. The model benchmarking on the community team leader consists of 5 character clusters, 8 characteristic items, 21 evaluation dimensions and 21 typical behavior descriptions. Conclusion Findings of the survey provide objective criteria for competence appraisal of community health administrators, and serve as reference for formulating their development plans as well.

Objective To observe the clinical efficacy of fire-needle acupuncture in treating myofascial pain syndrome (MPS). Method Seventy-two patients with MPS were randomized into a treatment group and a control group, 36 in each group. The treatment group was intervened by fire-needle acupuncture, while the control group was by ordinary acupuncture. The short-form McGill Pain Questionnaire (MPQ) was observed before and after the intervention, and the clinical efficacy was compared. Result The total effective rate and markedly-effective rate were respectively 94.4% and 77.8% in the treatment group versus 80.6% and 30.6%in the control group. There was a significant difference in comparing the markedly-effective rate between the two groups (P<0.01). The MPQ item scores [pain rating index (PRI), present pain intensity (PPI), and visual analogue scale (VAS)] were significantly changed after intervention in both groups (P<0.05). After intervention, the MPQ item scores of the treatment group were significantly different from that of the control group (P<0.05). Conclusion Fire-needle acupuncture is an effective method in treating MPS.

Objective To investigate the role of PKCδin free fatty acid-induced endothelial cell apoptosis.Methods In addi-tion,we looked for evidence of apoptosis-related PKCδsignal pathway.Human umbilical vein endothelial cells were treated with va-rious concentrations of free fatty acids and transiently transfected with PKCδsiRNA to inhibit PKCδ expression.Cell proliferation was determined through colorimetric assays,and apoptosis was quantified using flow cytometry.Protein expression was determined from cell lysates use Western blots with antibodies against p-PKCδ Tyr512,PKCδ.Statistical analyses were performed.Results Free fatty acids had multiple effects on human umbilical vein endothelial cells,including concentration-dependent inhibition of cell proliferation,induction of apoptosis,increased Fas expression,and increased PKCδ expression and phosphorylation.Inhibition of PKCδmRNA expression by PKCδsiRNA led to a reduction in free fatty acid-induced apoptosis.Conclusion The free fatty acids-induced apoptosis in endothelial cells are possibly mediated by PKCδ.

Objective To discuss the correlation factors of locally recurrent gastric carcinoma. Methods Clinical, pathological and periodical follow-up datas of 50 cases with locally recurrent gastric carcinoma after radical resection from 1991 to 2005 were studied retrospectively, logistic regression analysis was used to disscuse the possible correlation factors of locally recurrent gastric carcinoma. Results 66.0% of 50 cases had recurrenced within 2 years after operations; Tumor pathological stages, positive lymph nodes and radical resection extent were independent correlation factors of locally recurrent gastric carcinoma. Conclusions Most recurrences would happen within 2 years after radical resection for gastric carcinoma; Early diagnosis of reccurrent gastric carcinoma depends on periodic follow-up observation especially gastroscope; Tumor pathological stages, positive lymph nodes and radical resection extent are independent correlation factors of locally recurrent gastric carcinoma.

Objective To investigate the effect of estrogen on gliosis, synaptic proliferation and reorganization in hippocampus of rats with chronic epilepsy induced by pentylenetetrazol (PTZ). Methods Thirty-two Wistar rats were randomly divided into normal control group, PTZ kindling epilepsy group, estrogen+PTZ group and estrogen +Tamoxifen (mixed estrogen antagonist)+PTZ group. The PTZ kindling epilepsy group was intraperitoneally injected with 35 mg/kg PTZ once a day, then the behavior of the rats in one hour was observed. The estrogen interfered group was intramuscluarly injected of 0.6 ?g/kg ?-estradiol 6 h before PTZ injection, 1/3 d. The estrogen+Tamoxifen interfered group was injected of Tamoxifen into lateral cerebral ventricle 1 h before estrogen injection. If the severe epileptic seizures occurred three times, the rats were ready for detecting the expression of filial fibrillary acidic protein (GFAP) and synaptophysin (P38) by immunohistochemical technique. Results Epileptic seizure occoured earlier and more severely in most rats of estrogen interfered group than no estrogen interfered group. Rats in all model groups were found more astrocyte proliferation and positive synaptophysin staining, especially in the CA2, CA3 and hilar areas of dentate gyrus (DG) of hippocampus than control group. Objective To explore the application of case-based discussion combined with translational medicine teaching method in clinical teaching of gastrointestinal surgery.Methods Totally 100 undergraduates of clinical medicine in grade 2007 and 100 undergraduates in grade 2008 from the second military medical university were divided into experimental class (50 students,25 in discussion class and 25 in comprehensive class) and control class (50 students).Method of case-based discussion was applied in discussion class; method of case-based discussion combined with translational medicine was employed in comprehensive class and traditional method was used in control class.Results Scores in experimental class were superior to those in control class ; questionnaire survey showed that 90％ students thought the new model was better than the traditional model; the interests of students in exploring clinical problem in experimental class were increased (90％ vs.50％ ) ;the sense of mission and responsibility was Strengthened (86％ vs.64％ ) ;the scientific research thinking was more active (80％ vs.48％ ) ; the individual ability and quality were enhanced (76％ vs.50％ ),the depth and width of study were markedly increased (80％ vs.52％ ),the consciousness of independent study and thinking was strengthened (88％ vs.54％ ).Comprehensive class had more obvious improvements in scientific research thinking,individual ability and quality,depth and width of study,independent study and thinking.Conclusions The new teaching model which combined translational medicine with case discussion breaks through the traditional model and adapts to the requirements of modem medicine,it can enhance the teaching effect and worth promoting in educational reform.

Object To study the growth speed and dynamic spore production of CY-8202, which is an asexual strain of entomopathogenic fungi isloated from Cordyceps sinensis (Berk.) Sacc., to explore the method of artificial culture of CY-8208 strain, to assay its antibiotic activity and spectrem and to provide the experimental basis for studies of its active components. Methods Variations in colony diameter of the cordyceps hypha cultured on the fungi media were measured. The spore-count was used to determine the dynamic colony spore outputs of the hypha on several fungi media and water agar. The agar-piece method was used to test its antibiotic activity.Results There were linear relationships between the colony extensions and the culture times on common fungi media such as PDA medium, etc.. The amounts of spore produced by the single colony of the fungi were more than 10 7 and gradually increased, but the rates of increase tended to be gentle after 200 h. Antimicrobial tests against 22 microbial strains showed strong inhibition of gram positve bacteria including Bacillus subtilis, Micrococcus tetragenus and Staphylococcus albus, and to gram negative bacteria, including Proteus vulgaris, Salmonella typhi, Aerobacter aerogenes and Salmonella sp., as well as weak inhibition of three mold strains and one actinomycetes strain, but no inhibition was observed in four yeast tested.Conclusion The growth activity and the spore-production ability of Cordyceps hypha are two important factors to infect validly its insect-host. For the growth activity, its strong penetration seems more important than its fast growth. The ability to product numerous spores of Cordyceps hypha may be an important mark of its strong infectivity to insect-host. The antimicrobial tests show that CY-8202 may secrete some metabolites which have a more broad-spectrum antibacterial activity than cordycepin isolated initially from Cordyceps militaris.

Objective To study the effect of astrocyte on synapse formation and the molecular mechanism. Methods Cortical astrocytes were isolated and purified from neonatal rats.On the 2h,7th day,14th day and 21th day after passage,we counted the number of astrocytes and the culture medium(astrocyte-conditioned medium,ACM)was harvested to measure the concentration of estrogen(E 2)by using ELISA techniques.Based on the model of pure culures of neonatal cortical neurons,the experimental groups were designed as follows:1.pure neuron cultures(group N);2.ACM cultures(group A);3.mixed cultures(group M);4.E 2 cultures(group E 2);5.ACM+Tamoxifen(estrogen receptor antagonist)cultures(group A+T);6.Tamoxifen cultures(group T).Then synaptic puncta in every group was stained and counted through immunofluorescence,and we also compared the differences in puncta number among those six groups(at 9th day in culture,number/per neuron). Results The numbers of astrocytes were:1?10 4/ml, 1.1?10 6/ml, 1.4?10 6/ml, 1.5?10 6/ml; The concentrations of E 2 were:(ng/L):0, 117?22, 266?22,252?27 respectively.No estrogen was detected in the primary culture medium.The concenteration of estrogen increased in correspondence with the culturing days and reached the peak around at the 14th day, then decreased gradually but kept at a certain high level,and the numbers of synaptic puncta of per neuron in group N,A,M,E 2,A+T,T were:14?3;79?5;83?8;80?6;32?3;29?3 respectively.The treatment of pure neuron culture with ACM increased the number of synapses on per neuron by up to 6 fold by comparison with pure neuronal culture.Exogenic estradiol added into pure neurons can mimick the effect of the ACM.Tamoxifen which is antagonist of estrogen receptor could decrease the effect of ACM by 75%.Conclusion The astrocytes of neonatal rat cortex do secrete E 2.Astrocyte-derived estrogen may be the molecule regulating the synaptic formation through estrogen receptors.
