3.Esophageal Gland Duct Adenoma.

Benign ductal or glandular neoplasms of the esophagus unrelated to Barrett esophagus are extremely rare. Only 9 cases have been reported in the English language literature. We now report a case of esophageal gland duct adenoma incidentally found in a 73-year-old man. A 0.8 cm-sized, polypoid submucosal lesion in the distal esophagus was removed. Histologically, the lesion was well circumscribed and consisted of several ducts or cysts with focal papillary configurations. Interstitial lymphocytic infiltration with germinal centers was also observed. The lining cells of ducts or cysts were composed of two layers: an inner intensely eosinophilic luminal duct cell layer and an outer myoepithelial cell layer that was accentuated by alpha-smooth muscle actin. There was no significant nuclear atypia or mitosis. Mucin production was occasionally observed in a few goblet cells. To the best of our knowledge, this is the first case of benign ductal or glandular neoplasm of the esophagus among Koreans.

4.A Survey on Use of Environment-Friendly Agricultural Products for School Food Service by Dietitians in Chungnam Province.

The purpose of this study is to analyze the school dietitian's cognition, practical use of environment-friendly agricultural products to provide fundamental information for the qualitative improvement in the school food service. The results of the study are as follows: 1. It was clearly recognized that the subsidy for the school food service is generally insufficient, the amount of additional appropriate subsidy for the school food service was different according to type of school and roughly 300~600 won in each of foods needs to be supported. 2. The environmentfriendly agricultural products that were often used in the school food service were vegetables and grains. 3. In the analysis of cognition and problems toward using environment-friendly agricultural products, the dietitians recognized that environment-friendly agricultural products should be appropriately used. A reason that environmentfriendly agricultural products were better than general agricultural products was composed of a use of less additive, good ingredient, and good outer condition. 4. In the analysis of countermeasure on increasing environment-friendly agricultural products in the school meal service, the response to that more additional subsidy was necessary for the school food service. The highest and the most positive effects after using environment-friendly agricultural products were students' health improvement and improvement in parents' satisfaction with the school food service.

PURPOSE: This study was done to provide information about issues of clinical competency from the perspective of new graduate nurses and to make suggestions for improving undergraduate education programmes. It was also done to clarify learning experiences between fundamentals of nursing and the other major areas of nursing. METHOD: For this study, 7 new graduates and 8 experienced nurses participated. The data were collected through in-depth interviews and analysed in terms of Strauss and Corbin's grounded theory methodology. RESULT: Core category and main categories, were delineated. Clinical nursing competency was found to be a dynamic process with each participant actively engaged in acquiring problem solving ability in diverse clinical settings. These findings have value in understanding the embedded meaning of clinical nursing competency. CONCLUSION: Therefore, the educational programs reflecting the experience of new nursing staff should be developed.

7.A Case of Dysgerminoma Associated with 46, XY Pure Gonadal Dysgenesis.

Dysgerminoma developed in a 21-year-old phenotypic female patient with 46,XY pure gonadal dysgenesis, Swyer syndrome. This patient presented with pelvic mass associated with abdominal pain and primay amenorrhea. Clinical characteristics showed a typical stigmata of gonadal dysgenesis: primary amenorrhea, sexual infantilism, a small uterus and left streak gonad. A 46,XY karyotype was made by lymphocyte culture. The patient was counseled to undergo operation, chemotherapy and hormon therapy. She underwent bilateral gonadectomy with total hysterectomy, partial omentectomy and multiple pelvic wall random biopsy. Histological examination revealed dysgenetic gonads with dysgerminoma. After surgery, the patient received chemotherapy and also was started on hormone replacement therapy. She is currently alive with no evidence of disease after 19 months from surgery.

Sjogren's syndrome is a chronic,slowly progressive,autoimmune disease in which the exocrine glands are damaged by lymphocytic infiltration,resulting in xerostomia and xerophthalmia.Fever in patients with Sjogren's syndrome is rarely manifested with the exception of cases with complicated illness such as lymphoma.We experienced a 70-year-old male patient with Sjogren's syndrome who was presented with fever of unknown origin.Despite of thorough investigation,other diseases to cause fever could not be found.Fever and other clinical features were improved with empirical steroid therapy.

9.4 Cases of Duchenne type of Progressive Muscular Dystrophy.

4 cases of Duchenne type of Muscular Dystrophy have been experienced in brothers of 6 years old boy, 8 years old boy whose limb girdle was involved, brothers of 9 years old boy and other brothers of 9 years old boy. Ina all cases, serum CPK levels were significantly increased and typical waddling gait and Gower's sigh were noticed. The muscle biopsy findings and electromygraphic findings were also compatible with progressive muscular dystrophy.We report with brief review of literatures.
