4.The Concept Analysis of Hope: Among Cancer Patients Undergoing Chemotherapy.

The main objectives of this study were to analyze the concept of hope, so to provide basic data to develop a valid instrument to measure hope, and to develop hope enhancing nursing intervention a program for cancer patients. The hybrid model approach was applied in three phases, the theoretical phase, the empirical phase, and the analytic phase. The study was developed on universal attributes explaining generalized hope and specific hope, which were revealed in a comprehensive review of the literature. In the empirical phase, eight cancer patients undergoing chemotherapy were interviewed to reveal causes, motivation, and their resource of hope according to The Hope Assessment Guide (Farren, Herth, & Popovich, 1995). In the analytical phase, the results of the two previous stages of the study were compared. The results were as follows: In the theoretical phase, six dimensions of hope emerged; affective, cognitive, behavioral, affiliative, temporal and contextual dimension. The antecedent of hope was loss, crisis, uncertainity, and stress. The consequences were renewal, development of new methods, safety, peace and transcendental competence. In the empirical phase, these six dimensions emerged as theoretical phases were verified and specified as these descriptive terms: feeling, intention, expectation, activity, relation, future- orientation, reality and goal-setting. The antecedent factor of hope was occurrence or recurrence of cancer. The consequence of hope was ability to cope with real condition, feeling of safety and comfort, peace, development of new strategy and recovery of disease. The major content of hope in this phase was related to specific hope, but it was also influenced on by general hope. In the analytic phase, general and specific hope was renamed as trait and state hope. All attributes emerged at the empirical phases, and also emerged at the theoretical phase. However, cognitive and contextual dimensions were revised and specified. In conclusion, the concept of hope is divided into trait hope and state hope, and state hope is an anticipatory expectation that occurs at the time of a stressful stimulus, such as being diagnosed with cancer. Hope is a multidimensional dynamic energized mental state which has the dimensions of affective, cognitive, behavioral, affiliative, temporal and contextual. There should be further studies to develope the state and trait hope scale according to definition and attributes of hope investigated in this study. In addition, considering results of the empirical phase, the family is very a important factor as a resource of hope, so it is necessary to consider family in implementing a nursing intervention program to enhance hope.

5.Improving Access to the IUD: Experiments in Koyang.

The experimental study has been conducted in six townships of Koyang County, Kyonggi Province to identify more efficient methods of carrying the Intra-Uterine Device (IUD) into rural areas since May 1965, covering in all 76,810 inhabitants with 10,073 eligible women in 13,947 households. IUD acceptance rates are improved remarkably in two townships with activities of the mobile service and stationary clinic, impling that the IUD program could be intensified if IUD services are extended to the village either by mobile service or stationary services. The most of the eligible women wanting IUD in the future preferred to have IUD insertion by female physician on the before survey, but the results of the study shows no difference in IUD insertions by female and male physicians. Reasons to choose a clinic for IUD insertion are most likely to be accessibility or availability and reliability of the service with a little shyness problem. It indicates that the sex of physician inserter is not important in Korea. IUD termination rates are least for postpartum, and are trivially different between those inserted within ten days after onset of menstruation and those inserted later. IUD termination rates in terms of the qualification of inserter show that Ob-Gyn specialist is with the lowest termination rates, but it is interesting that nurse working alone with the postpartum cases did best of all and suggested that nurses can potentially insert IUD's as efficiently as do physicians if the proper training is given to them.

6.Epidemilogical Study on Orientia tsutsugamushi Infection of Wild Rodents in Korea.

This study was carried out to determine the seropositivity for Orientia tsutsugamushi infection among wild rodents captured in farms and mountains of Korea. A total 965 wild rodents of 7 species and 48 Crocidura laciura were captured in farms and mountains of 6 province during 1997~2000 in Korea. Serological evidence for O. tsutsugamushi infection was obtained using O. tsutsugamushi antigens by indirect immunofluorescent antibody technique (IFA). Four hundred eighty four Apodemus agrariusus, 246 Apodemus peninsulae, 179 Eothenomys regulus, 44 Microtus fortis, 6 Micromys minutus, 5 Mus musculus, and 1 Cricetula triton were captured in Gyeonggi, Gangwon, Chungnam, Gyeongbuk, Gyeongnam, and Jeonbuk province. Among them, 59 (12.2%) A. agrariusus, 22 (8.9%) A. peninsulae, 12 (6.7%) E. regulus, 5 (11.4%) M. fortis and 1 (16.7%) M. minutus were IF antibody positive against O. tsutsugamushi Gilliam or Karp strain.

7.Use of Mother's Class in Promoting IUD Acceptance and Its Effectiveness.

This paper is a result of experimental studies on family planning in Koyang county, a rural setting of Korea. The Koyang experience has shown that the acceptance of IUD is heavily dependent on two factors: the distance from the village to the IUD service clinic and the extent of side reactions: and unfavorable rumors about its use in the community. While the problem of distance was solved by sending out mobile teams or by establishing new clinics in the villages, the problem of side reaction and unfavorable rumors still remained. This study was set up to see whether the organization of mothers' classes could be a means to promote the acceptance and retention of IUDs. The mothers' class was organized in each village (hamlet), and the membership was opened to every women in the village. It was hypothesized that the mothers, class with the proper educational materials will work as an adequate educational program to offset rumors and to make a favorable social climate for accepting and retaining the IUD. The field experiment was conducted according to the "before-after" design with a control group. It was found that the educational program in the mothers' class was effective in promoting the acceptance and retention of contraceptive practice, particularly the IUD.

8.The Correlation between Apoptotic Index and Gleason Grade in Prostate Carcinoma.

PURPOSE: Apoptosis is a process of natural cell death that can occur under both normal and neoplastic conditions and increased apoptosis in neoplastic conditions is a feature of the increasing malignant potential. We investigated the possible relationship between the apoptotic index(AI) and Gleason grade, T stage of prostatic carcinoma. MATERIALS AND METHODS: A total of 31 patients with untreated prostatic carcinoma diagnosed by TURP, transrectal needle biopsy and radical prostatectomy was evaluated. We used in situ end labeling method for apoptotic staining. AI was compared with Gleason grade and T stage. RESULTS: The values of mean AI according to Gleason grade were grade I, 1.3+/- 0.13, II, 1.38+/-0.11, III, 1.92+/-0.20, IV, 2.0+/-0.20, V, 2.01 +/-0.28. A positive correlation was noted between apoptotic index and Gleason grade, but not between apoptotic index and T stage. CONCLUSIONS: We found a positive correlation between AI and Gleason grade and the possibility that AI may serve as an important morphological marker to predict the behavior of prostatic carcinoma.

9.Cystic Teratoma of the Sacrococcygeal Region in Adult: A Case Report.

Sacrococcygeal teratoma is the most common solid tumor to occur in neonates, in whom it mostly takes the form of an obvious exophytic mass. They are, however, rarely found in adults. We report the CT and histopathologic findings of an adult cystic teratoma arising from the lower sacrococcygeal region that was discovered incidentally. Unenhanced CT scans showed an oval-shaped, cystic lesion with hyperattenuating content and no contrast enhancement. After surgery, the gross specimen was seen to be a cystic lesion filled with mucin. Microscopy revealed three germ cell layers in the cystic wall and the lesion was confirmed as cystic teratoma.

10.Effects of Back Massage on Immune Response, Symptom Distress and Mood State of Patients Undergoing Allogeneic Hematopoietic Stem Cell Transplantation.

PURPOSE: To examine the effect of back massage on immune response, symptom distress, and mood state of patients undergoing allogeneic hematopoietic stem cell transplantation (allogeneic HSCT). METHODS: Subjects were thirty-seven patients undergoing sibling allogeneic HSCT (including 16 in the experimental group and 21 in the control group). Experimental subjects participated in an intervention group of back massage for 10 minutes, once a day and 5 times a week, from one week prior to the HSCT to the third week after the HSCT or a control group. A non-equivalent pretest-posttest design was used. t-test and Repeated measures ANOVA were used to examine group differences by using SAS. RESULTS: No significant group differences were found in Immune response (CD4+, CD8+,CD19+, CD56+) and symptom distress. The experimental group had significantly less mood state (anxiety, confusion) than the control group. CONCLUSION: The back massage for the patients undergoing allogeneic HSCT may be effective in altering the anxiety and confusion during hematopoietic stem cell transplantation. However, this study did not provide evidence in improving immune response and symptom distress.
