2.Radiological evaluation of cortical rim sign of the kidney

The rim sign in renovascular compromise may be defined as a thin nephrographic rim outlinning a kidney with another wise faint nephrogram. Renal infarction with gelform and blood clots was induced experimentally in 5 dogsand high dose IVP and CT were performed to evaluate renal cortical rim sign after renal infarction. One expireddue to anesthetic technical failure, and one was embolized partially. So we could be obtain the result on 3 dogs.The reults were as follows; 1. The high dose IVP films show cortical rim sign on 24 hrs and 72 hrs films in twodogs. The CT films show rim sign in 3 dogs. 2. The rim sign may be visualized, if the renal artery is occuludedenough to drop perfusion presssure below the authoregulatory range. 3. The high dose IVP and early nephrogramphase are necessary for the rim sign.

3.Electromechanical and Bioelectrical Charateristics of Fracture Healing

It is widely blieved that the new bone formation is provoked by fine electrical stress to bone. There have been maay experimental studies that showed much efforts to define these small amounts of electricity, and these fine electricities are being used in the orthopedic field for the treatment of various fractures, nonunions and pseudsrthrosia The auther has checked the electrical potentials changing in the course of fractvre hesling in rabbits femur. Comparing these data with that of the normal control group, the following results were obtained. 1. In the normal rabbit's femur the electrical potentisl was 13.6mV less on the concave side than on the convex side. This finding can be explained by the existence of stress generated potential or piexoelectricity. 2. Electrcal potentials on the fractured femoral sites were lowered significantly from that of the control group. So auther could confirm the existence of bioelectrical generated by increased electrical activity of the living bone. 3. The electrical potential of the fractured femoral site has been decreased 50% and 20% as low as that of the normal site at 8 weeks respectively. It is expected that these lowered electrical potentials may become dose to the normal values when the changed metabolic process due to the fractured bone is no longer functioning and it is believed to take human 2 years to get the normal value.

5.A Case of Meningioma in Temporo-occipital Lobe without Dural Attachment in a 14-yer-old Girl: Case Report.

Meningiomas are thought to arise from the arachnoid cap or meningothelial cells. This frequent apposition to the dura mater probably accounts for the usual attachment of the neoplasm to this layer. We report a case of intraparenchymal meningioma without dural attachment in a 14-year-old girl. A survey of the literature reveals that the majority of cases of meningiomas without dural attachment occur either in children or below the tentorium, and most of them located in the ventricle, the cranial vault. Intraparenchymal meningioma without dural attachment, however, are extremely rare. The uncommon locations of meningiomas at sites distant from the duramater is believed to reflect the rare occurrence of arachnoid cap cells in the Virchow-Robin spaces along the cerebral vasculature or in the pial layers distant from the dura mater.

6.Clinical Parameters Associated with Vasospasm in Early-Operated Aneurysmal Subarachnoid Hemorrhage.

Much controversies still exist with respect to the risk factors influencing symptomatic cerebral vasospasm. In our study, the patient group had some unique characteristics, ie.:42.5%(65) of the 153 patients were aged 60 years or older(mean;55.6 years) and consequently the proportion of Hunt-Hess grade 3-5 was higher than grade 1-2(57.5% vs 42.5%). The authors analyzed the relationship between vasospasm and Hunt & Hess grade, Fisher grade, hydrocehalus, age & hypertension among these 153 early operative cases after a 3 year follow up study. The incidence of vasospasm was significantly higher in the Hunt-Hess grade III, IV, V and the Fisher grade III, IV, compared to grade I & II, and also in preoperative hydrocehalus but it was insignificant in old and hypertensive patients. These results suggest initial severe direct insults and complicating preoperative hydrocephalus increase the chance of postoperative symptomatic vasospasm in this older aged, poorer grade study group.

7.A Comparative Study on MIB-1, AgNORs and PCNA Expressions in Astrocytic Tumors.

Cell cycle associated nuclear proteins such as proliferating cell nuclear antigen(PCNA), argyrophilic nucleolar organizer regions(AgNORs) and the family of nuclear proteins identified by the Ki-67 epitope, have been primarily utilized for estimating of growth potential of neoplasms. Although PCNA and AgNORs staining are possible in the paraffin-embedded tissue, Ki-67 staining had been only possible on frozen sections. Recently monoclonal antibody MIB-1 is available, and reacts with the Ki-67 epitope in paraffin-embedded tissue. Twenty eight astrocytic tumors in paraffin-embedded, archival materials were stained by immunohistochemical technique for the MIB-1, PCNA, and by silver colloid stain for AgNORs. The MIB-1 labeling indicies(LI) ranged from 2 to 25%(10+/-7.58) for 10 glioblastomas; from 2 to 15%(7+/-3.74) for 11 anaplastic astrocytomas; and from 1 to 5%(3+/-1.91) for low grade astrocytomas. Glioblastomas and anaplastic astrocytomas exhibited significantly higher MIB-1 LI than their benign counterparts(p<0.05). The AgNORs count per cell ranged from 1.3 to 3.1(1.96+/-0.57) for 10 glioblastomas: from 1.2 to 3.1(1.9+/-0.64) for 11 anaplastic astrocytomas: and from 0.8 to 1.5(1.2+/-0.26) for low grade astrocytomas. Glioblastomas and anaplastic astrocytomas exhibited significantly higher AgNORs count than their benign counterparts(p<0.05). The PCNA LI ranged from 10 to 40%(24.5+/-10.39) for 10 glioblastomas; from 5 to 20%(11.6+/-5.24) for 11 anaplastic astrocytomas; and from 5 to 10%(7.1+/-2.67) for low grade astrocytomas. The differences of PCNA LI between glioblastomas and anaplastic astrocytomas(p<0.01), and between glioblastomas and low grade astrocytomas(p<0. 001) were statistically significant. Linear regression analysis showed correlations between MIB-1 LI and AgNORs count(Spearmans r=0.4306, p<0.05), between PCNA LI and AgNORs count(Spearman's r=0.586, p<0.05) and between PCNA and MIB-1 LI(Spearman's r=0.4523, p<0.05). These findings suggest that LI of MIB-1, PCNA and AgNORs count are correlated each other, and can be used as helpful markers for differentiating astrocytic tumors in addition to conventional staining methods.

9.Effects of Improved Methods of Endotracheal suction and Hand Washing on Incidence of Nosocomial Pneumonia in the Neurosurgical Intensive Care Unit.

BACKGROUND: This study aimed at identifying the effects of improved methods of endotracheal suction and hand washing on incidence of nosocomial pneumonia (NP) in the neurosurgical ICU. METHOD: Repeated training of nurses about hand washing and adherence of the single use of suction catheter and sterile normal saline of endotracheal suction was provided. Then hand washing compliance of nurses before and after training was investigated. and the incidence of NP in all patients hospitalized for more than 72 hours in the neurosurgical ICU was investigated from Sep 1, 2000 to Jan 31, 2001 (2nd surveillance). The NP rate of that period was compared with the incidence of the 1st surveillance period, from Sep 1, 1999 to Jan 31, 2000. The data were analyzed with chi-square-test. Fisher's exact test and logistic regression analysis by SAS 8.0. RESULT: The proportion of hand washing performance increased from 15.6% to 46.8%. The rates of NP per patient during 1st surveillance and 2nd surveillance were 10.3%, 3.9%(p=0.01) patient-days rates of NP were 10.0 cases/1,000 patient days, 4.1 cases/1,000 patient days(p=0.04), and ventilator-days rates of NP were 67.6 cases/1.000 ventilator days, 16.2 cases/1,000 ventilator days(p=0.00) respectively. Out of 29 isolates, 18 were methicillin-resistant Staphylococcus aureus, which was the most frequently isolated microorganism. The risk factors associated with incidence of NP were age, length of stay, Glasgow coma scale, diabetes mellitus, duration of intubation, insertion of endotracheal tube, duration of tracheostomy, presence of tracheostomy, duration of ventilator, use of ventilator, and insertion of nasogastric tube. CONCLUSION: These results showed that single use of suction catheters and sterile normal saline and increased compliance of hand washing were important factors to decrease the incidence of NP.
