1.A Case of Nonadrenal Retroperitoneal Ganglioneuroma.

Ganglioneuromas of retroperitoneal cavity which not originate from adrenal gland are very rare tumor, and usually recognized incidental to some other aliment or on a routine physical examination. Ganglioneuromas are generally considered to be the mature form of a triad of tumors arising from neural crest tissue which include malignant neuroblastoma and ganglioneuroblastoma. We report one case of retroperitoneal non-adrenal ganglioneuroma with review of literature.

4.The Prognostic Role of Body Temperature in Acute Ischemic or Hemorrhagic Stroke Patients.

BACKGROUND: Hyperthermia is known to be one of the important poor prognostic factors of ischemic stroke. However, the prognostic role of fever in hemorrhagic stroke is not clear. We investigated whether the prognostic role of fever is influenced by the type of stroke. METHODS: We analyzed the linical data of patients who were admitted to our hospital within three days after the onset of stroke during 28 months. We divided the patients into three groups according to the degree of maximum temperature recorded during the first seven days; no fever (< OR=37.2 degrees C), low grade (37.3~38.0 degrees C) or high grade (> OR=38.1degrees C) fever. Using the modified Rankin disability scale (MRDS) at discharge, clinical outcome was dichotomized into favorable (MRDS 0~2) and unfavorable (MRDS 3~5) categories. The size of lesion, age, sex, alcohol, diabetes, hypertension, initial glucose level, hypercholesterolemia, heart disease, smoking, previous stroke, and initial National Institutes of Health Stroke Scale (NIHSS) were investigated. RESULTS: Of 217 patients, 46.1% had no fever, 36.9% had low grade fever, and 17.1% had high grade fever. Fever was strongly correlated with outcome in patients with stroke of any type (p<0.05). The size of lesion was also significantly correlated with the degree of fever in patients with ischemic stroke (p=0.01). Multivariate analysis in the logistic model revealed that fever and NIHSS were independent predictors for a poor prognosis. CONCLUSIONS: Fever in patients with acute stroke is an independent poor prognostic factor regardless of stroke type.

5.Surgical Management of Stress Urinary Incontinence.

A group of 30 patients with stress urinary incontinence who had been admitted to the Taegu Fatima Hospital during 5 years period from November, 1951 to October, 1956 were subjected for the clinical evaluation in respect of particular attention to surgical management. The results are as follows: 1. Patients varied in age from 34 to 65 years. All patients were multiparous with an average of 4.6 previous deliveries. 2. Clinical symptom classification (Grade I-Grade IV) of stress urinary incontinence reported by Ball was used. There were symptom distributions of Grade II in 13 patients, Grade 1 in 10, Grade III in 5 and Grade IV in 2. 3. According to Green`s classification, patients with stress urinary incontinence were classified into two basic type of urethrovesical anatomy on the cystourethrogram with respect to type 1 and type II. There were anatomic distributions of type 1 in 7 patients and type II in 8, while 15 had cystocele plus uterine prolapse. 4. With minimum follow up of 3 months, overall success rate was 93% in the surgical management (28 out of 30 patients). Success rates according to the each surgical procedure were 91% in the Pereyra procedure (10 out of 11), 83% in Afterior colporrhaphy (5 out of 6) and 100% in Anterior-Posterior colporrhaphy (10), MMK operation (2) and Stamey procedure (1). 5. Postoperative complications were minimal and infrequent except for urinary retention in 17 out of 30 patients (57%) which were managed by catheterization.

8.The Study of the Clinical, Histopathological Features of Pilomatricoma and the Differentiation to Normal Hair Follicles.

BACKGROUND: Pilomatricoma is a benign appendageal tumor characterized by basaloid and shadow cells. Differentiation towards normal hair follicles is often observed in this condition. OBJECTIVE: The purpose of this study was to analyze the clinical and histopathological features of pilomatricoma and to confirm diverse types of differentiation towards normal hair follicles, and its origin. METHODS: We retrospectively reviewed medical records and histopathological findings of 64 patients who had visited our hospital in the last 7 years. The evolutional stages of tumor and differentiation were analyzed through histopathological findings. RESULTS: The majority of the tumors occurred in the first two decades (69.8%). Unlike previous reports, the male to female ratio was 1.1:1. The mean duration was 11.8 months. The predilection sites were the upper extremities (40.8%), face (33.8%), neck (15.5%), back (2.8%), scalp (2.8%), and thigh (2.8%) in that order. Histopathologically, most of the tumors were located in the lower dermis (76%) and enclosed by pseudocapsules. The evolution of tumor was divided into four groups- early, fully developed, early regressive, and late regressive stage, as previously reported. Our 71 cases were classified as: 2 early, 16 fully developed, 34 early regressive, and 19 late regressive stage. Calcification (60.5%) and ossification (8.4%) were observed, and the ratio increased as the tumor aged. Inflammatory cell infiltration and multinucleated giant cells were observed disorderly. Differentiation to normal hair follicles was observed: 14 cases to infundibulum, 10 isthmus, 17 outer root sheath, and 11 inner root sheath. CONCLUSION: Pilomatricoma is a benign tumor that differentiates toward the hair cortex. Basaloid and shadow cells correspond to hair matrix cells and immature hair cortex cells, respectively. Basaloid cell can also differentiate to normal hair follicles.

9.A Case of Chronic Cyclitis.

Since Ernst Fuchs had described the chronic cyclitis as cyclitis in 1892, there have been many reports about the chronic cyclitis with the development of the binocular indirect ophthalmoscope and the Schepens' scleral depressor and gonioscope. The chronic cyclitis comes incidious onset with minimal symptoms of hazy vision and vitreous opacity. The authors experienced a case of chronic cyclitis of the left eye of 33 year old male patient who has been treated with systemic corticosteroid and antihistamines. Here, we briefly report it referring the literature about the chronic cyclitis.

10.The Effect of Cyclic Compressive Strain on the Differentiation of Osteoblast-Like Cells (MC3T3-E1) in 3-D Scaffold.

We examined the effect of an applied cyclic compressive strain on the growth and differentiation of MC3T3-E1 cultured in a three-dimensional chitosan scaffold. The specially designed testing apparatus for mechanical stimulus was developed for uniaxial cyclic compressive strain. Cyclic compressive strain was applied over a period of 17 days with 150 cycles per day at a frequency of 0.5hz. Strain magnitude was 2.5% of the scaffold length. Control group and mechanically stimulated group were incubated and harvested at the indicated times. (day 3, 7, 10, 14, 17) The total amount of protein and alkaline phosphatase activity were examined. The total amount of protein of the control group was higher than that of the mechanically stimulated group. This was due to cell death for the nodule formation and calcium deposit of the mechanical stimuli group which resulted in cell differentiation. The alkaline phosphatase activity increased slightly in the control group. However, in the mechanical stimuli group, it increased significantly and reached its peak level on day 7 and subsequently its activity dropped to a level that was higher than the level at day 4(p < 0.05). Conclusively, it can be noted that the mechanical stimulus significantly accelerated the differentiation of MC3T3-E1 cells.
