1.An Epidemiological Study of 100 Cases of Lung Cancer in Korean.

An epidemiological study is to observe and analyse 100 cases of lung cancer in Korean, which had been diagnosed actually at the general hospitals in Korea from 1970 through 1974, to discover the tendency of the lung cancer by sex, age, occupation, duration of clinical symptoms before diagnosis, site of cancer in lung, size of cancer, occurrence, treatment, prognosis, smoking and pathological findings as follows. 1. Males were affected 5.7 times more than females, the peak incidence of lung cancer were 50 years old age group. 2. The occupations of the cases were commerce, agriculture, industry, laborers and teachers, etc. in thht order. 3. The average duration symptoms from onset to diagnosis were 5.5 months. 4. The cancers were affected more right side than left side of the lungs, Particularly the most of them were affected in the upper lobes of the lungs. 5. The average diameter of the tumors was 5.5crX6.5cm, However the largest of them was 10cm X 13cm. 6. The most cases of the lung cancers were occurred primarily. 7. Out of the 100 cases, 58 cases had no treatment, 23 cases had palliative-symptomatic treatment, of cases by special chemo-therapy, 7 cases by surgery, and 5 cases by cobalt 60 or X-gay. 8. In the view point of prognosis, the most cases of the lung cancers died within one year atfer diagnosis. 9. The smoking was one of the causative factor of lung cancer, but there was no significant diferentiation by means of the amount of smoking. 10. The most cases of the lung cancers diangosed histo-pathologically, were epidermoid(squamous) type.

2.The Efficacy of Combination Therapy of 5 alpha -Reductase Inhibitor and of-Adrenergic Blocker in Benign Prostate Hyperplasia.

PURPOSE: Benign prostate hyperplasia(BPH) can be treated with alpha1-adrenergic blocker that relaxes prostate smooth muscle or 5 alpha-reductase inhibitor that reduces serum dirtydrotestosterone. The efficacy of the combination of 5 alpha -reductase inhibitor(finasteride) and alpha1-adrenergic blocker(doxazosin) was evaluated in patients with benign prostate hyperplasia. MATERIALS AND METHODS: Eighty five patients with BPH was treated and followed over 6 months and divided into three groups: Group 1(doxazosin 3mg/day), Group 2(finasteride 5mg) and Group 3(combination of both drugs). Initially, all patients were evaluated by international Prostatic Symptom Score(IPSS: irritative, obstructive, sum, life quality), uroflowmetry, residual urine, serum prostate specific antigen(PSA) and prostate weight by transrectal ultrasonography. IPSS, uroflowmetry and complications were evaluated every month. Residual urine and PSA were assessed at every 3 months, prostate weight at every 6 months. RESULTS: In Group 1 and 3, IPSS were more decreased than In Group 2 immediately(p < 0.001). In Group 1 and 3, maximal flow rate was more increased than in group 2 immediately(p < 0.001). There was no difference of mean change of residual urine among three group. In Group 2 and 3, serum prostate specific antigen and prostate weight were more decreased than in Group 1 (p < 0.001). CONCLUSIONS: In medical treatment of BPH, the combination therapy of alpha1-adrenergic blocker and 5alpha-reductase inhibitor shows early symptomatic improvement and decreased prostate weight without significant complications.

7.Treatment of Gap Nonunion of the Tibia by the Ilizarov Method

Twenty four patients with gap nonunions of the tibia were treated with the Ilizarov intercalary bone transport method. The purpose of this retrospective study is to evaluate the results and complications in gap nonunions of the tibia treated using the Ilizarov method. There were 22 males and two females with an average age of the 35.8 years(range, 23-63 years). The mean follow up period was 30.2 months(range, 17-70 months). Ten of the 24 patients had no external shortening with bone gap, 13 external shortening with gap and one hemicircumferential defect. On average, the size of bone loss measured 8.2 cm(range, 3-18 cm). Twenty-one of 24 patients were treated by whole segment internal transport(fifteen were proximal fragment transport, five were distal fragment transport, one was proximal and distal fragment transport), one patient by anterior hemicircumferential corticotomy and partial bone fragment internal transport and tow patients by fibula transfer. All patients healed with solid bony union, although twelve patients required bone grafts, eleven at the docking site and one at the distraction site. On average, the healing index was 49 days/cm(range, 18-82 days/cm). There was on relationship between the percentage transport(size of the distraction gap/the transporting fragment length x 100) and healing index. Numerous complications were encountered, most commonly delayed union and pin site infection, Conclusively, the application of Ilizarov techniques to gap nonunions of the tibia was very effective, but required correct technique and careful follow-up examination.

8.EFFECT OF GOLD ELECTRODEPOSIT ON THE BOND STRENGTH BETWEEN ALLOYS AND VENEERED RESIN.

The purpose of this experiment was to determind whether the gold electrodeposit on Pd-Ag and Ni-Cr alloys influences on the shear bond strength between veneering resin and silicoated metal surface. All the metal specimens were sandblasted with 250microneter aluminum oxide and followed by silicoating and resin veneering. According to the metal surfaces to be veneered, experimental groups were divided into five. Group Prec : Gold alloy without gold coating Group Semi : Pd-Ag alloy without gold coating Group Base : Ni-Cr alloy without gold coating Group Semi-G : Pd-Ag alloy with gold coating Group Base-G : Ni-Cr alloy with gold coating All specimens were thermocycled 1,000 times at temperature of 5degrees C to 55degrees C. The effects of gold electrodeposit on the shear bond strength between resin and metal interface were measured and fractured surface of the resin veneered metal was examined under the scaning electron microscope. The following results were obtained 1. The shear bond strength between resin and metal was 64.51+/-11.11Kg/cm2in Prec group, 62.77+/-11.23Kg/cm2in Base group and 58.97+/-9.20 Kg/cm2in Semi Group. There was no significant difference among the groups. 2. The bond strength in groups Semi-G and Base-G decreased about 17%, compared to the non-gold-electrodeposit groups(Semi, Base). 3. In groups of non electrodeposit(Prec, Semi, Base), fracture occurred at the interface between alloy and resin, while fracture interface was observed between gold coating and resin in group Semi-G, and between metal substrate and gold coating in group Base-G resp[ectively.

9.T-cell Lymphoma of the prostate: Remission with Doxorubicin-Based Combination Chemotherapy.

Lymphoma of the prostate is exceedingly rare. Much more common than primary lymphoma of the prostate id secondary involvement of malignant lymphomas originating at other sites. A variety of treatments have bee n used, including prostatectomy, radiotherapy or combinations of chemotherapy and radiotherapy. Generally, the prognosis is very poor, and there is currently no consensus regarding treatment. We report on a patient with T-cell lymphoma who presented with symptoms of bladder neck obstruction. The patient has been asymptomatic and under complete remission after completion of doxorubicin-based chemotherapy for 2 cycles.

10.Study for Usefulness of Total Alkaline Phosphatase as a Biochemical Markers of Bone Turnover in Healthy Menopausal Women.

OBJECTIVE: Total alkaline phosphatase (TALP) has been common bone formation marker of managing individual cases of highly abnormal bone remodeling, such as paget's disease. However, the lack of bone specificity of TALP balances such as bone loss resulting from postmenopausal estrogen deficiency and osteoporosis. This study investigated usefulness of TALP as a biochemical markers of bone turn over in healthy menopausal women. METHODS: The levels of urinary deoxypyridinoline (DPD), serum total alkaline phosphatase (TALP), osteocalcin (OC) were determined. Bone mineral density (BMD) were also measured by dual energy X-ray absorptiometry (DEXA). We compared the biochemical markers of bone turnover and serum TALP among 3 group classified by menopausal state, and among 3 groups classified by WHO classification of osteoporosis. Also we analyzed the correlation between bone markers with BMD and bone markers with age. RESULTS: All biochemical markers of bone turnover (TALP, DPD, OC) were increased significantly with age. There were negative correlation between Biochemical markers of bone turnover and spinal BMD (TALP; r=-0.440, p<0.01, DPD; r=-0.380, p<0.001, OC; r=-0.328, p<0.001, respectively). Biochemical markers of bone turnover (TALP, DPD, OC) in osteoporosis group were significantly higher than osteopenia and normal BMD group. The TALP significantly increased in osteopenia group compared normal BMD group but not DPD and OC. Biochemical markers of bone turnover (TALP, DPD, OC) were increased from premenopause to postmenopause. There were positive correlation between DPD and OC (r=0.365, p<0.001). TALP correlated well with DPD (r=0.490, p<0.001) and OC (r=0.433, p<0.001). CONCLUSION: Because of these association, we concluded that total alkaline phosphatase is a useful biochemical markers of bone turnover and reflects the bone turnover in healthy menopausal women characterized by more subtle remodeling imbalances such as bone loss resulting from postmenopausal estrogen deficiency and osteoporosis.
