1.A Case of Adenofibroma of Left Renal Pelvis.

2.A Clinical Observation on Tumors of the Genitourinary Tract.

A clinical observation was made on 216 cases of genitourinary tract tumors of the impatients in the Department of Urology, Chosun University Hospital during the 10 years, from January 1972 to December 1981. The results were as follows: 1. During this period, of 1282 cases hospitalized, 216 cases (180 males, 36 females) had tumors of the genitourinary tract, at the rate of 16.8%. 2. The male to female ratio was 5:1 (180:36). 3. The majority of the patients(88.4%) were distributed over age of 40 years. 4. The prostate was the most commonly involved organ (51.3%) and the tumors of the bladder, kidney, testis, penis, pelvis, ureter, urethra were in order. 5. Histopathologically, B.P.H. was observed in 43.1% of all operative tumors, transitional cell carcinoma of bladder in 28.1% and renal cell carcinoma in 5.6%. 6. Operations were performed in 160 patients (74.1%).

3.Adreno-Hepatic Fusion: A case report.

Adreno-hepatic fusion is rare condition defined as adhesion of the liver and right adrenal cortex with close intermingling of the respective parenchyme. It is suggested to be an aging phenomenon, because its incidence is much higher in older age group. Clinically it may pose a problem of operability of the organ involved. We report a case of incidentally found adreno-hepatic fusion in a 49 year old female patient with adenocarcinoma of the sigmoid colon. The segementectomy of VIII segement of the liver was done due to a 6 4 cm sized metastatic nodule of adenocarcioma. Pathological examination of the liver revealed an ovoid shaped, 1 0.5 cm sized adrenal cortical tissue. It was subcapsularly located and about 1cm apart from the metastatic adenocarcinoma with an intervening normal hepatic tissue. The adrenal tissue was mainly composed of zona fasciculata without medullary tissue. In the interphase, the adrenal tissue and liver tissue were admixed closely and partially septated by thin fibrous tissue. There was no inflammatory response to the heterotropically located adrenal tissue and there was no symptom related to the adrenal gland.

4.The Efficacy of Combination Therapy of 5 alpha -Reductase Inhibitor and of-Adrenergic Blocker in Benign Prostate Hyperplasia.

PURPOSE: Benign prostate hyperplasia(BPH) can be treated with alpha1-adrenergic blocker that relaxes prostate smooth muscle or 5 alpha-reductase inhibitor that reduces serum dirtydrotestosterone. The efficacy of the combination of 5 alpha -reductase inhibitor(finasteride) and alpha1-adrenergic blocker(doxazosin) was evaluated in patients with benign prostate hyperplasia. MATERIALS AND METHODS: Eighty five patients with BPH was treated and followed over 6 months and divided into three groups: Group 1(doxazosin 3mg/day), Group 2(finasteride 5mg) and Group 3(combination of both drugs). Initially, all patients were evaluated by international Prostatic Symptom Score(IPSS: irritative, obstructive, sum, life quality), uroflowmetry, residual urine, serum prostate specific antigen(PSA) and prostate weight by transrectal ultrasonography. IPSS, uroflowmetry and complications were evaluated every month. Residual urine and PSA were assessed at every 3 months, prostate weight at every 6 months. RESULTS: In Group 1 and 3, IPSS were more decreased than In Group 2 immediately(p < 0.001). In Group 1 and 3, maximal flow rate was more increased than in group 2 immediately(p < 0.001). There was no difference of mean change of residual urine among three group. In Group 2 and 3, serum prostate specific antigen and prostate weight were more decreased than in Group 1 (p < 0.001). CONCLUSIONS: In medical treatment of BPH, the combination therapy of alpha1-adrenergic blocker and 5alpha-reductase inhibitor shows early symptomatic improvement and decreased prostate weight without significant complications.

7.Mechanisms of HPV-16-induced Transformation in Human Epithelial Cells in Culture.

Human epithelial cell line immortalized by Ad12-SV40 hybrid virus was transfected with plasmid containing HPV-16 via calcium-phosphate method. Subsequently, 8 clonal cell lines were obtained after G418 selection. Among these clonal cells, clonal cell-4(C-4) and clonal cell-6(G-6) showed increases of tumorigenic cellular properties such as saturation density, soft agar colony formation and cell aggregation. Morphological alteration such as appearane of foci was observed on these two clones after passage 6 and 7(50 to 55 days after transfection). When clonal cells and control cells were treated with MNNG(0.01ug/ml), both C-4 and C-6 showed increases of tumorigenic cellular properties and the level of increase was much more elevated as compared to clonal cells prior to MNNG treatment. Appearance of foci formation was observed in C-4 and C-6 after passage-6. After passage-8, control cells and all clonal cells showed morphological alterations. It seems that treatment of cells containing HPV-16 DNA with MNNG increases tumorigenic properties of the cells and expedite morphological transformations. (continue)

9.The Expression of Epidermal Growth Factor Receptor mRNA by in Situ Hybridization and the Prognosis in Transitional Cell Carcinoma of Bladder.

Recently many molecular biologic techniques have been tried to determine a prognostic indicator in various malignancies by evaluating the capacity of cellular differentiation and proliferation and the degree of expression of oncogene products. It is suggested that the activation of EGFR may play a role in normal and abnormal cellular proliferation. Many reports have demonstrated that EGFR is overexpressed in genitourinary, breast, lung, head and neck tumors, and possibly used as tumor marker. In this study we measured EGFR mRNA by in situ hybridization in 64 patients with transitional cell carcinoma of bladder and 14 cases of normal bladder to evaluate its relation to the prognosis. The positive staining of EGFR mRNA by in situ hybridization was observed in 42.8% of normal urothelium and 100% of transitional cell carcinoma of bladder. The strong positive staining of EGFR mRNA was observed in 57.5% of superficial cancer of bladder and 80.6% of invasive one(Ta 25%, T1 62.1%, T2 72.7%, T3 76.9%, T4 100% and Grade I 35%, Grade II 75.9%, Grade III 94.4%, respectively). As tumor stage and grade was increased, the corresponding strong positivity of EGFR mRNA was significant. Progression to invasive cancer occurred in 3 patients with superficial cancer and 2 of them had strong positive EGFR mRNA finding in original specimen. Tumor recurrences occurred in 15 patients with superficial cancer and 8 of them had strong positive EGFR mRNA finding in original specimen. In 20 of the 25 patients(80%) who died of bladder cancer during the study tumors were EGFR mRNA strong positive. The death rate of patients with strong positive EGFR mRNA was significantly greater than that of patient with weak positive EGFR mRNA tumor. In conclusion, The EGFR mRNA expression of transitional cell carcinoma of bladder using in situ hybridization technique may be a useful tumor marker and prognostic indicator.
