4.A Case of Lofgren' s Syndrome.

Sarcoidosis is a rare d:sease in Korea and its association with erythema nodosurn is even rarer. Recently we saw a patient of bihilar adenopathy, who had arthralgia and erythematous nodules of both lower legs. A skin biopsy obtained from the erythematous nodule of the lower leg showed septal panniculitis, consitent with erythema nodosum, and a biopsy from the mediastinal lymph node revealed a pathology consistent with sarcoidosis. This is the first case of Lofgrens syndrome confirmed by histology ir, Korea.

We report a case of successful heart transplantation after 67 days of support with venoarterial extracorporeal membrane oxygenation (ECMO) in a patient who underwent surgery for type A aortic dissection and myocardial infarction complicated by irreversible myocardial damage and a deep sternal wound infection. During ECMO support, left heart vent and distal limb perfusion were performed. Mediastinitis was treated with mediastinal washout and irrigation. Multiple complications from peripheral ECMO were successfully managed.

8.An Experimental Study and Clinical Investigation on Rotational Deformity in Supracondylar Fracture of the Humerus in Children

In children, the supracondylar fractures of the humerus are common in elbow region, and often associated with various complications and problems in treatment (ie,Volkmann's ischemic contracture, cubitus valgus or varus deformity, rotational deformity). The results of reduction can be measured by Baumann's angle, carrying angle, trochlea-capitellum angle, and displacement of distal radial styloid process on radiological study. So we have made experiments on the process which various three dimensional displacements reflected into two dimensional Baumann's angle and humerocapitellar angle. And we investigated the process of rotation, through various radiological studies by means of molded models of humerus. The results were as follows:l. In experiments, AP & lateral X-ray studies showed no change in rotation of proximal segment of fracture with fixed distal segment of fracture. 2. On the other hand, AP & lateral X-ray showed significant change, in rotation of proximal fracture segment with distal segment of fracture. 3. Supracondylar rotation angle(S.R.A.)depicted as the ratio of the length of proximal fracture width minus that of the distal fracture width in lateral view, to the length of distal fracture width in AP view minus that of the distal fracture width in lateral view. 4. According to the modified Mitchell result, S.R.A. to the result of treatment was average 7.6 degree in excellent group, 15.2 degree in good group and 29.3 degree in unsatisfactory group.

9.Subclavian Artery Laceration Caused by Pigtail Catheter Removal in a Patient with Pneumothorax.

We report a case of subclavian artery laceration caused by the removal of a pigtail pleural drainage catheter in a patient with a pneumothorax. The patient was successfully resuscitated through diagnostic angiography with subsequent balloon occlusion and primary repair of the injured subclavian artery. Although pigtail drainage of a pneumothorax is known to be safe and effective, proper insertion and removal techniques should be emphasized to reduce the risk of complications.

10.Anti-inflammatory Effect of Lycopene on Chronic Bacterial Prostatitis Rat Model.

PURPOSE: Chronic bacterial prostatitis (CBP) is the most common urological disease in adult males, with antibiotic therapy being the gold standard for its treatment. However, long-term therapy results in many side effects as well as bacterial resistance. For these reasons, there is a need for a new treatment modality to replace traditional antibiotic therapy. Lycopene, an extract of tomatoes, has antioxidant effects against various bacteria and synergistic effects with antibiotics. We evaluate the synergistic effects of lycopene on the treatment of CBP in an animal model. MATERIALS AND METHODS: Forty five rats demonstrating CBP were randomly divided into 4 groups; the control, lycopene, ciprofloxacin and lycopene with ciprofloxacin groups. All drug treatments were conducted over a period of 2 weeks. After treatment, the results were analyzed, with the microbiological cultures and histological findings of the prostate and urine samples compared with the control group and between each group. RESULTS: The uses of ciprofloxacin, and lycopene with ciprofloxacin showed statistically significant decreases in bacterial growth and improvements in prostatic inflammation compared with the control group (p<0.05). The lycopene with ciprofloxacin group showed a statistically significant decrease in bacterial growth and improvements in prostatic inflammation compared with the ciprofloxacin group (p<0.05). CONCLUSIONS: These results suggest that lycopene may be an effective material in the treatment of CBP. Especially, the combination treatment of lycopene and ciprofloxacin has synergistic effects. Therefore, it is suggest that the combination of lycopene and ciprofloxacin may be effective in the treatment of CBP, and with a higher success rate.
