1.Comparative Assessment of Immunohistochemical and Zieh1-Neelsen Stains for Demonstration of Mycobacterium Tuberculosis.

To obtain a useful method for the identification of mycobacteria in tissue section, we evaluated 118 cases of tuberculosis: 48 pulm onary, 14 lymph nodal and 56 synovial tuberculosis. Seventy nine of these cases underwent the culture study. Sections stained with anti-Mycobacterium bovis were compared with the results of the Zieh1-Neelsen stain and culture. The immunohistochemical stain for Mycobacterium bovis in al examined cases was not any more sensitive than the Zieh1-Neelsen stain(p>0.05). Neverthless, the immunohistochemical stain was a useful method for the localization of mycobacteria because of the striking contrast between its background and the wider dimension of a positive area. Immunoreactive areas demonstrated a few intact mycobacteria showing a positive reaction in the Zieh1-Neelsen stain. In conclusion, double staining method using the immunohischemical stain for Mycobacterium bovis and the Zieh1-Neelsen stain is an efficient technique in oder to confirm the diagnosis of tuberculosis.
2.Adrenocortical Oncocytoma: A case report.

Adrenal gland is a rare location for an oncocytic neoplasm. In English literature less than 10 cases of adrenocortical oncocytoma have been reported. We have experienced a case of adrenocortical oncocytoma in a 35-year-old man which was detected incidentally during the ultra-sonographic evaluation of the abdomen for a routine physical examination. This case did not demonstrate any clinical evidence of adrenocortical abnomalities, such as virilization or hypertension. Grossly, the tumor was light to dark tan on cut surface. Light-microscopic examination revealed tumor cells with abundant lipid- sparse eosinophilic cytoplasm and occasional pleomorphic nuclei. Mitotic figures were less than 5/50 HPFs. Tumor cells were positive for vimentin but negative for pancytokeratin, CAM 5.2, chromogranin and synaptophysin. Ultrastructural examination demonstrated abundant mitochondria containing occasional intramitochondrial dense bodies or inclusions.

3.The Aphyllophorales of Mungyong Saejae.

To evaluate the establishment of Mungyong Saejae Natural Ecology Park located in the northwestern Gyongbuk Province, a scientific survey for the mushroom flora of the park was carried out from May to December of 1999. A checklist of the Aphyllophorales collected from the park was prepared. The list included 67 species of 44 genera belonging to nine families in the Aphyllophorales. Among them, seven species, Antrodia malicola, Ceriporia purpurea, Oligoporus leucospongia, Perenniporia tephropora, Phanerochaete xerophila, Sistotrema diademiferum and Vuilleminia comedens, were confirmed as new to Korea and are registered here as unrecorded species along with descriptions and microscopic drawings.

4.An Empirical Study on Burnout in Clinical Nurses: Focused on Bullying and Negative Affectivity.

PURPOSE: The purpose of this study was to identify the effects of bullying, negative affectivity and burnout in the nurses. METHODS: The sample consisted of 389 nurses. Data were analyzed using frequency, percentage, mean, standard deviation, t-test, ANOVA, Scheffe, Pearson Correlation Analysis and Hierarchical Multiple Regression. RESULTS: The control variables of age, marital status, education level, current position, health status and work unit explained 30.2%(F=28.9, p<.001) of variance in burnout. The control variables, bullying and negative affectivity collectively explained 49.7% of variance in burnout. CONCLUSION: The results indicate that the demographic factors influencing burnout are age, education level, health status, work unit, while bullying and negative affectivity in the work places are factors that influence burnout. These findings can be utilized to develop strategies to reduce bullying and negative affectivity.

5.A Case of Papillary Adenoma of the Nipple Showing Histopathologic Findings of Syringocystadenoma Papilliferum.

A 24-year-old woman was seen for evaluation of an skin lesion on thiright nipple. Examination revealed a solitary 1.5 x 1cm-sized, erythematous, oozing, eroded andruted, verrucous nodule on the right nipple. A small amount of serous discharge was found but there was no pain or tenderness. Histopathologic findings showed cystic invaginations extending down vard from the epidermis and papillary projections extending into the lumina of the invagination. Pupiae were lined by two rows of cells. In stroma, there were marked inflammatory infiltrates containing any plasma cells. The overlying dermis showed an adenomatous growth pattern with a prolifertion of round, oval or irregular shaped ducts and fibrotic stroma.

6.Effects of Workplace Bullying, Job Stress, Self-esteem, and Burnout on the Intention of University Hospital Nurses to Keep Nursing Job.

PURPOSE: Medical institutions and their patients benefit from continued employment of nurses. In this study an assessment was done of important factors that influence nurses' intention to leaving their jobs. METHODS: The sample consisted of 229 university hospital nurses. Data were analyzed using frequency, percentage, mean, standard deviation, t-test, ANOVA, Scheffé, Pearson's Correlation Analysis, and Hierarchical Multiple Regression. RESULTS: The control variables, including age, current position, and health status explained 18% (F=16.37, p<.001) of variance in retention intention. The control variables, bullying, job stress, self-esteem, and burnout collectively explained 27% of variance in retention intention. CONCLUSION: The findings indicate that the factors influencing retention intention are age, current position, and health status, while self-esteem and burnout in work places are new and more recent factors that impact retention intention. These findings can be utilized to develop strategies to increase self-esteem and retention intention.

8.Granulomatous(Lobular) Mastitis in a Pregnant Woman: A case report.

Granulomatous(lobular) mastitis is a distinct disease entity of unknown etiology which is characterized by noncaseating granulomatous lobulocentric inflammation. We describe a rare case of granulomatous(lobular) mastitis of a 36 year-old pregnant woman a review of the literature. The mass which was discovered in the third month of her pregnancy, began as a localized, nontender mass on the left breast and persisted during her entire pregnancy. It decreased slightly in size when she began taking post-partum bromocriptine. Clinically and mammographically, the mass was highly suspected as a carcinoma with axillary lymph node metastasis. Fine needle aspiration smears revealed numerous aggregates of granulomas composed of epithelioid histiocytes admixed with multinucleated giant cells of Langhans' and foreign body type, and collections of polymorphonuclear leukocytes. Ziehl-Neelsen, silver methenamine and PAS stain were negative for acid-fast bacilli, fungus, and bacilli on the smear respectively. Histologically, granulomatous inflammation was centered on the breast lobules. Caseation necrosis was absent, instead, numerous microabscesses were formed in the center of the granulomas. Cultures of the fresh tissue for the AFB, aerobic and anaerobic bacteria, and fungus were all negative. Excision of the mass was performed without further treatment and there was no recurrence of the mass 6 months postoperatively. An autoimmune mechanism, infection, and some association with oral contraceptives have been suggested as etiologic factors in the literature.

10.Effects of Perineal Care with Aroma Essential Oil on the Decrease of Urinary Tract Infection in Patients with Indwelling Urinary Catheter.

PURPOSE: This study was designed to verify effects of perineal care with aroma essential oil on urinary tract infection(UTI) in patients with indwelling urinary catheter. METHOD: The research design was non equivalent control group pretest-post test design. The data were collected from March to August, 2005 at C university hospital in Seoul. The subjects were 43 patients who had a indwelling catheter in neurosurgery intensive care unit(NSICU). They were assigned to two groups, 21 subjects to the experimental group and 22 subjects to the control group. The experimental group received perineal care with 3% blending essential oil for 4 days. RESULTS: In the experimental group, white blood cells and bacteria in urine were significantly lower than control group. Bacteria in urine culture was lower in the experimental group at post test. CONCLUSION: Perineal care with aroma essential oil could be an effective intervention to reduce the UTI of patients in NSICU.
