2.A Case of Pulmonary Infarction Probably Related with Henoch-Schönlein Purpura.

We report a case of pulmonary infarction probably related with Henoch-Schönlein purpura, which presented with purpura on both lower extremities in a 27-year-old woman. The purpura had developed 4 days previously and it had no itching or tend'erness. She complained of mild fatigue, chest tightness, but no dyspnea or hemoptysis. On routine examination, chest radiography showed a poorly defined nodular opacity on the left lung field, and it was diagnosed by a follow up chest CT as a pulmonary infarction secondary to pulmonary vasculitis.

3.Expression of CD44v6 in Cervical Cancer.

CD44 is a cell-surface glycoprotein postulated to play a role in tumor cell metastasis. Aberrant expression of the cell adhesion molecule CD44 has been detected in human tumors and the expression of specific isoforms(splice variants) has been shown to be associated with metastasis and poor prognosis in human malignancies. We used variant exon sequence-specific monoclonal antibody to epitope encoded by exon v6 of human variant CD44 to study the expression of CD44 splice variant by immunochemistry in fifty nine samples of human cervical cancer. twenty seven tissue samples of cervical intraepithelial neoplasia(CIN) and normal cervix were included in this study. CD44v6 was stained positive in the basal and parabasal layer of normal epithelial cells homogenously but was absent in the stromal cells. The intensity of CD44v6 staining was the strongest in invasive squamous cell carcinoma followed by normal cervical epithelium, CIN, adenocarcinoma. In the malignant samples, heterogeneity in staining intensity among different clusters of tumor cells was observed. Furthermore the intensity of staining was stronger in proportion to stage, depth of invasion, lymphovascular invasion(p<0.05), and lymph node metastasis(p=NS). This study suggest that the expression of CD44v6 adhesion molecule may be useful value in predict the high stage, depth of invasion, lymphovascular invasion and lymph node metastasis probably.

4.The Prevalence and Associated Factors of Kidney Disease in Korean Rheumatoid Arthritis Patients.

OBJECTIVE: To determine the prevalence and associated factors of kidney disease in Korean rheumatoid arthritis (RA) patient, and describe the profile of RA drugs prescribed in RA patients. METHODS: A total of 284 patients at the division of rheumatology with confirmed RA were prospectively included in this study from May 1 to August 31, 2008. Renal function was assessed using Cockcroft-Gault (CG) and abbreviated Modification of Diet in Renal Disease (aMDRD) study formulae, and classified by the National Kidney Foundation (NKF) classification. Comparisons was were performed by the two sample t-test, chi-square-test and binary logistic regression analysis. RESULTS: Of 281 patients with RA, only 7 (2.5%) patients had abnormal serum creatinine (sCr) levels. According to the NFK classification, the prevalence of kidney disease using aMDRD and CG formula was 23.8% and 31.7%, respectively. Among the patients with eGRF <60 mL/min/m2 according to aMDRD or CG formulae, 91.8~100% received at least one drug that was potentially nephrotoxic. RA patients with renal dysfunction was associated with advanced age, body mass index (BMI), antinuclear antibody (ANA). CONCLUSION: Estimation of renal function with CG or aMDRD formulae is important in RA patients, particularly in those with low BMI or old age. Also, appropriate dosage adjustment is needed in patients with renal dysfunction.

5.'Medical Knowledge' and 'Tradition' of Colonial Korea: Focused on Kudo's "Gynecology"-based Knowledge.

This article attempts to illuminate the ways in which Kudo's medical knowledge based on 'gynecological science' constructed the cultural 'traditions' of colonial Korea. Kudo appears to have been quite an influential figure in colonial Korea in that his writings on the relationship between women's crime, gynecological science and the Choson society granted a significant amount of intellectual authority. Here, I examine Kudo's position within colonial Korea as a producer and propagator of medical knowledge, and then see how women's bodies were understood according to his gynecological knowledge. It also traces the ways in which Kudo's gynecological knowledge represents Choson society and in turn invents the 'traditions' of Chosn. Kudo's knowledge of "gynecology" which had been formed while it traveled the states such as Japan, Germany and France served as an important reference for his representation of colonial Korean society. Kudo was a proponent of biological evolution, particularly the rules of 'atavism' put forth by the criminal anthropologist Cesare Lombroso, and argued that an unique social environment caused 'alteration of sexual urges' and primitive cruelty in Choson women. According to Kudo, The social environment was none other than the practice of 'early marriage,' which went against the physiology of women. To Kudo, 'early marriage' was an old 'tradition' of Choson and the cause of heinous crimes, as well as an unmistakable indicator of both the primitiveness and savageness of Chosn. While Lombroso considered personal factors such as stress as the cause of women's crimes, Kudo saw Choson women's crimes as a national characteristic. Moreover, he compared the occurrence rate of husband murders by provinces, based on which he categorized the northern population of Choson as barbaric Manchurian and the southern population as the superior Japanese, a combination of racism and scientific knowledge. Kudo's writings provide an insight into the appropriation of Western medical theories and criminal anthropological knowledge by a non-Western colony as well as the ambivalence and contradictions underlying Japanese empire as in the use of concepts like 'difference' and 'unity.' According to today's standards, Kudo's physiological arguments can hardly avoid being called pseudo science, which confirms that the power and authority of science standing on 'objectivity' and 'universality' are actually dependent on social contexts that are constantly being readjusted. In the end, the cultural 'traditions' of a nation/state often taken for granted are social constructions born out of transnational crossing points of knowledges, and on the basis of these constructs are the concepts of differences between nations/states. And one of the core references for these differences in colonial Korea was Western science/medical knowledge.

6.Three Cases of Bee-sting Granuloma.

Most insects including the honeybee cause erythema and edema at the site of a sting in non-allergic individuals. This reaction usually resolves within a few days. However, in certain cases, the persistence of inciting materials in the cutaneous tissue may promote a local inflammatory foreign body reaction. Localized nodular lesions were seen in our two patients and erythemstous patches were present in one patient due to rarely encountered complications induced by retained sting materials. In these cases histopathology showed multiple foci of foreign body granuloma containing yellow refraetile material in the center surrounded by histiocytes and foreign body giant cells. The granulomatous skin lesions in these patients were treated with intralesional injections of triamcinolone acetonide and partial excisions of the nodular masses were performed.

7.Spectral Analysis of Heart Rate and Blood Pressure Variability during Hemorrhage in Thiopental-anesthetized Rats.

BACKGROUND: This study was aimed to elucidate the effect of thiopental anesthesia on circulatory response to hemorrhage in rats by power spectral analysis of heart rate and blood pressure variability. METHODS: Sixteen male Sprague-Dawley rats weighing 350~475 g were divided into thiopental (50 mg/kg, ip)-anesthetized (T, n=10) and conscious (C, n=6) groups. Hemorrhage was induced with a withdrawal pump from the femoral artery at 3 ml/kg/min for 5 min. Arterial pressure was measured with a pressure transducer connected to the contralateral femoral artery for 5 min before, during and after hemorrhage. The blood pressure signal digitized at 500 Hz through a data acquisition system was analyzed with fast Fourier transform algorithm to yield power spectra of sytolic (SP) and diastolic (DP) blood pressure and instantaneous heart rate (HR). Powers of very low frequency (VLF, 0.02~0.26 Hz), low frequency (LF, 0.26~0.75 Hz) and high frequency (HF, 0.75~5.00 Hz) band were expressed as percent of total power. RESULTS: Before Hemorrhage blood pressure was lower in T (113 6/83 6 mmHg) than in C (157 4/101 2) rats, but was not changed by hemorrhage in both groups. Before Hemorrhage HR was lower in T (331 22 beats/min) than in C (378 27) rats. HR was significantly increased to 412 26 beats/min in C, but was not changed in T during hemorrhage. Total powers of Before Hemorrhage blood pressure and HR variability were lower in T than in C. During hemorrhage, total power of blood pressure variability tended to increase in both groups, and that of DP was significantly increased during hemorrhage in T. Total power of HR was significantly increased in C, but was not changed in T. T rats showed lower LF and higher HF power of blood pressure, and lower VLF and LF and higher HF power of HR than C rats. During hemorrhage, both groups showed no significant change in spectral distribution of HR power, except for a significant increase in LF of HR after hemorrhage in C. Spectral distribution of blood pressure power was not changed by hemorrhage in C. In T rats, VLF of DP was significantly increased and HF of DP was decreased during hemorrhage. CONCLUSIONS: It suggests that autonomic activity, especially cardiac sympathetic activity is increased in response to hemorrhage in C. Thiopental anesthesia depresses overall autonomic activity, especially sympathetic activity, and vasomotor tone. Hemorrhage under thiopental anesthesia induces depression of baroreceptor reflex activity, while increased levels of vasoconstrictor hormones help to maintain the blood pressure.

8.Mullerian Adenosarcoma of the Ovary with Sex Cord-Stromal Differentiation: A case report.

Mullerian adenosarcoma is a tumor composed of a mixture of glandular and stromal elements in which the glandular component appear to be neoplastic but, histologically, benign with the stromal component showing varying degrees of malingancy. We report a case of ovarian m llerian adenosarcoma with sex cord stroma differentiation in the stromal components. A 57 year-old female who presented with palpable mass in the right lower abdomen had undergone through salingo-oophorectomy. Grossly, the ovary was multicystic, and partly showed a solid appearance with multiple polypoid projections into the dilated cystic spaces. On microscopic examination, the tumor consisted of benign to borderline epithelial glands that were lined by variety of mullerian epithelia and sarcomatous component with sex cord-stromal elements, which include sertoliform tubules, Leydig cell like clusters, and granulosa cells.

10.Experimental Study of Changes of beta-catenin, PCNA, Substance P after Peripheral Nerve Compression in Rat.

The ultrastructural changes of sciatic nerve and immunohistochemical changes of beta-catenin, PCNA, substance P were studied at the proximal segment of rat sciatic nerve after compression injury. We used 90 Sprague Dawley rats and the sciatic nerve compressed using silicon tube. We divided experimental groups which were the compression group for 1 hour (1C), for 2 hours (2C), and for 3 hours (3C), the release group for 1 day (1C1R) and 3 days (1C3R) after the compression for 1 hour, the release group for 1 day (2C1R) and 3 days (2C3R) after the compression for 2 hours, the release group for 1 day (3C1R) and 3 days (3C3R) after the compression for 3 hours. The rats were sacrified and took the sciatic nerve specimen. The specimens were investigated under the light microscope after hematoxylin & eosin, toluidin blue, and immunohistochemical stainings. In the H & E finding, the axon of the 1C disappeared, but recovered at the 1C3R. The part of nerve fibers at the 2C were swollen, but began to be partially recovered at 2C3R. Most nerve fibers were enlarged at the 3C, but markedly decreased at the 3C1R. The beta-catenin reaction disappeared at the 1C, but almost recovered at the 1C3R. This reaction of the 2C disappeared in the large fibers, but began to be recovered in the small fibers at the 2C1R. This reaction of the 3C disappeared in the large fibers, but began to be recovered at the 3C1R and 3C3R. The PCNA reaction prominently appeared at the 1C3R and 2C3R, the more prominent reaction at the 3C1R, and markedly increased reaction at the 3C3R. The substance P reaction of the 1C1R was mild positive, and the 2C1R and 3C1R were strong positive. In the toluidin blue staining, the myelin sheaths near the perineurium began to be thickened at the 1C, but almost recovered at the 1C3R. Many myelin sheaths became to be very thickened at the 2C and 3C, but almost recovered at the 2C3R and 3C3R. In the electron microscopic findings, the myelin sheaths of the 1C underwent the demyelination with the separated lamellae and the increase microtubules. At the 1C3R, the axolemma was attached on the myelin sheath and the axon was recovered. the myelin sheaths of the 2C underwent the demyelination with the separated axolemma. At the 2C1R, the myelin sheath was recovered by the developing Schwann cells, many intraaxonal mitochondria of demyelinated nerve fibers. At the 2C3R, the myelin sheath tended to be recovered by the increased rough endoplasmic reticulum and mitochondria of Schwann cells, many intraaxonal mitochondria of demyelinated nerve fibers. The myelin sheaths of the 3C began to be underwent severe demyelination from the middle portion of the sheath and the vacuolization of intraaxonal mitochondria. At the 3C1R, the myelin sheaths were recovered and contained many extended microtubules, mitochondria, and small granules. At the 3C3R, severe demyelinated nerve fibers were recovered by increasing microtubules. The proximal retrograde degeneration of sciatic nerve by the acute compression appeared the loss of the axons and the swelling of nerve fibers. The beta-catenin reaction was disappeared by the compression, but recovered by releasing. This reaction may be played a important role of the recover of demyelination. The PCNA reaction of Schwann cells was increased by the nerve compression. In the substance P finding, the pain after the compression appeared at the 1 day after releasing. Electron microscopic changes after sciatic nerve compression were the demyelination, the separated lamellae and the increase of intraaxonal microtubules. After releasing, the nerve fibers were recovered by developing Schwann cell, the intraaxonal mitochondria, and the transported granules through extending microtubules.
