OBJECTIVE: To examine the nerve supply of external anal sphinctor muscle which contributes for the anal or urinary continence. METHOD: In the present study, sacral arc reflex was examined by an electrical stimulation of the penile shaft and recording of the compound muscle action potential from the external anal sphincter muscle with surface electrode, and pudendal nerve conduction was done by using a magnetic stimulation of proximal and distal cauda equina and recording from the external anal sphincter muscle. RESULTS: The results showed that the mean sacral arc reflex latency was 36.88+/-3.59 msec, and mean amplitude was 223+/-51.86 V with an electrical stimualtion. The mean latency by a magnetic stimulation of the proximal cauda equina was 6.34+/-1.29 msec, and the mean amplitude was 138+/-56 V. The mean latency by a magnetic stimualtion of the distal cauda equina was 3.67+/-0.44 msec, mean amplitude was 534+/-380 V. CONCLUSION: The pudendal nerve conduction study using a magnetic stimulation is a useful evaluation tool for the fecal incontinence of peripheral type, and neurogenic bladder.

PURPOSE: We reviewed surgical results in a group of women after Raz bladder neck suspension using questionnaire based outcomes analysis. MATERIALS AND METHOD: Of 71 patients who underwent Raz bladder neck suspension 55 had completed the questionnaire. Interviewees mean age was 54 years(range: 41-72 years) and mean observation time was 26.2 months(range: 3-77 months). RESULTS: According to outcomes analysis 39 patients(70.9%) were cured, 4(7.2%) improved, 11(20%) same and 1(1.8%) became worse. Overall improvement was found to be 43 patients(78.2%). 40(72.7%) patients replied 'satisfactory' for the Raz bladder neck suspension. Overall 'success rate' was estimated as 74.5%. No significant statistical correlation was found between success rate of Raz bladder neck suspension and various factors such as patients age, urge incontinence or follow up length. Of the 55 patients 7(12.7%) reported daily pad use. CONCLUSIONS: With these questionnaire based outcome analysis there was an overall success rate of 74.5%. We strongly emphasize the need for standardized questionnaire based outcome analyses for the evaluation of incontience surgery.

PURPOSE: The purpose of this study was to assess the usefulness of MRI in the evaluation of vascular changes of the meningioma. MATERIALS AND METHODS: We retrospectively studied the MRI findings in 20 cases of intracranial meningiomas. The images were assessed for vascular hilum, vascular rim, sinus invasion, and vascular encasement. Cerebral angiograms were obtained in 15 cases and compared with MRI findings. RESULTS: MRI was as accurate as angiography in detection of vascular hilum(MRl=8/20, Anglogram=9/15), vascular rim(MRl=14/20, Angiogram=9/15) and dural sinus invasion(MRl=4/6, Angiogram=5/6). MRI was superior to angiography in detection of vascular encasement(MRl=2/3, Angiogram=0/2) and cavernous sinus invasion(MRI=3/3, Angiogram=0/2). CONCLUSION: MRI is an excellent noninvasive imaging modality in evaluating vascular changes of omas.

We examined 16 patients with unilateral tongue deviation using magnetic stimulator in order to evaluate central hypoglossal nerve palsy following brain injury. Surface recording electrodes were placed at the apex and anterolateral one thirds of tongue. Magnetic stimulation was performed at vertex and occiput. On occiput stimulation, the mean latency was 3.77+/-0.36 msec in affected side and 3.89+/-0.47 msec in sound side for male patients, and 3.9+/-0.61 msec, 3.90+/-0.55 msec respectively for female patients. The mean amplitude was 0.85+/-0.63 mV in affected side and 2.64+/-2.32 mV in sound side for male patients and 1.00. 8273;0.23 mV, 3.56+/-0.40 mV respectively for female patients. There was significant difference between affected side and sound side for amplitude. On vertex stimulation, the mean latency was 8.61+/-0.83 msec in affected side and 7.50+/-0.80 msec in sound side for male patients, and 9.66+/-1.14 msec, 6.48+/-0.44 msec respectively for female patients. The mean amplitude was 0.77+/-0.59 mV in affected side and 1.23?1.08 mV in sound side for male patients and 0.52+/-0.23 mV, 1.15+/-0.64 mV respectively for female patients. There was significant difference between affected side and sound side for latency and amplitude.

Leiomyoma is a neoplasm of smooth muscle relatively common in the stomach, but very rare in the duodenal location. Recurrent bouts of severe melana remain the most frequent presenting symptom for leiomyoma. A 69-year-old man presented to us with a 4-day history of melena. Endoscopy revealed norinal mucosa covered ovoid elevated lesion with central linear depression and ulcer at the second portion of duodenum. Endoscopic biopsy showed chronic inflammation only. Duodenal mass was excised through a duodenotomy. Pathological diagnosis was leiomycena.

To find the severity and the type of the cervicobrachial syndrome, through physical examinations and laboratory tests were made toward 137 patients complaining the cervicobrachial syndrome (CBS) through the health interview among 313 workers occupied in a microwave-oven assembling factory from December 1993 to February 1994. Results were as follows. 1. The prevalence of spontaneous pain was 94.9%, while 78.1% suffered from muscle tenderness, 63.5% suffered from muscle induration and 62.0% showed kinesalgia. 54.7% of patients showed positive cervical X-ray findings and 28.5% showed positive Morley test. 2. The muscle induration was associated with the total duration of the job among female workers. 3. The severity of neck tenderness was negatively correlated with the strength of the grip at both hands, positively correlated with the duration of overtime in male and the severity of the wrist tenderness was negatively correlated with the height in female. 4. X-ray findings, muscle induration, Morley test results, muscle tenderness on the neck and muscle tenderness on the shoulder were associated with subjective symptoms of the workers.5. The muscle tenderness was associated with the number of the complaints at upper extremities among the sufferings in daily life significantly and the electromyogram (EMG) finding was associated with; the number of the complaints: about neuropsychological system among the sufferings in daily life significantly. In conclusion, the results of the physical examination were related with the subjective symptoms of the cervicobrachial syndrome. The muscle induration, the muscle tenderness, cervical X-ray findings and the Morley test results were more useful for the diagnosis of CBS. More efforts to objectify the subjective symptoms were asked.

PURPOSE: Idoxifene is currently entering phase II clinical trials for the treatment of advanced breast cancer. The radiolabeled idoxifene using 123I provides an opportunity for clinical pharmacology with single photon emission computed tomography (SPECT). The purpose of this study was to prepare radiolabeled idoxifene using 123I and to determine its cell uptake of breast cancer cell line. MATERIALS AND METHODS: With a view to evaluating new anticancer drugs, we are investigating the novel antiestrogen pyrrolidino- 4-iodotamoxifen (idoxifene). [123I]Idoxifene has been prepared in no-carrier-added form using a tributyl stannylated precursor which has been synthesized by means of (2-chloroethoxy)benzene with (+/-)-2- phenylbutanoic acid on the basis of previously reported standard methods. The biodistribution and dynamic behavior of the compound were investigated using the comparative breast cancer cell line, MCF-7 (estrogen receptor-positive) and MDA-MB-468 (non-estrogen receptor). RESULTS AND CONCLUSION: Acylation of (2-chloroethoxy)benzene with (+/-)-2-phenylbutanoic acid gave the versatile ketone (81%) which reacted with 1,4-diiodobenzene to give triphenylethylene as a mixture of E and Z geometric isomers, which were separated by the recrystallization in ethanol. The E-isomer was treated with pyrrolidine to give idoxifene (67%). In order to incorporate radioactive iodine into the 4-position, the 4-stannylated precursor was prepared (30%). The yield of radioiodination was 90-92% with a high radiochemical purity greater than 98%. The ratio of tumor uptake of the breast cancer cell line between MCF-7 and MDA-MB-468 was about 1.7.

Rupture of the internal carotid artery (ICA) during endoscopic sinus surgery is a rare complication. However, it can potentially result in death within minutes. In the event of a traumatic injury to the ICA during sphenoid sinus exploration, it is very difficult to control the bleeding. We present a case of carotid-cavernous fistula after an accidentally-developed ICA bleed during endoscopic sphenoidotomy. The patient was successfully treated with endovascular embolization techniques that included detachable microcoils.
