1.Application Value of Detecting Serum Micrornas-210, miRNA-23b in the Patients with the Femoral Head Necrosis

Objective To investigate the application value of detection of serum miRNA-210,miRNA-23b in the patients with the femoral head necrosis.Methods The serum and clinical data of 135 patients with the internal fixation operation for the femoral neck fracture were collected.The patients were divided into the necrosis group (27 cases) and non-necrosis group (108 cases),used the real-time fluorescent quantitative PCR (Real time-PCR,RT-PCR) technology to detect serum miRNA-210,miRNA-23b expression in comparatively analysizing the relationship between the changes of serum miRNA 210,23b and occurrence of the femoral head necrosis.Results The serum microRNA-210,-23b expression in the necrosis group were 0.19±0.03,0.21±0.05,which in the non-necrosis group were 0.92±0.18 and 0.87±0.17.The serum microRNA-210,-23b expression in the necrosis group were significantly lower than those in the non-necrosis group,the difference had statistical significance (P<0.01).The analysis results from the receiver-operating characteristic curve (ROC curve) showed that AUC of serum miRNA-210,miRNA-23b expression were 0.790 (95%CI:0.687~0.892,P=0.000) and 0.743 (95%CI:0.633~0.854,P=0.000),respectively.The sensitivity and specificity of miRNA-210 and miRNA-23b at the best threshold in diagnosis of the femoral head necrosis were 95.7% and 93.9%,91.5% and 92.3%,respectively.The analysis results showed that the serum miRNA-210,-23b expression had clinical significance in diagnosis of the femoral head necrosis.Conclusion Detection of serum miRNA-210,-23b expression can predict occurrence of the femoral head necrosis,estimate development condition of the desease.
2.Comparison of cardiomyocyte apoptosis and early postoperative recovery between propofol-and midazolam-combined anesthesia in patients undergoing cardiac valve replacement

Objective To compare the cardiomyocyte apoptosis and early postoperative recovery in patients undergoing cardiac valve replacement under propofol-or midazolam-combined anesthesia.Methods Forty NYHA class Ⅱ or Ⅲ patients (aged 48-64 yr and weighing 45-78 kg) undergoing cardiac valve replacement with cardiopulmonary bypass (CPB) were randomly divided into midazolam group (Group M) and propofol group (Group P) (n=20each). The patients were premedicated with morphine 0.1 mg/kg i.v. and scopolamine 0.3 mg i.v. Anesthesia was induced with midazolam 0.2 mg/kg (in Group M) or propofol 2 mg/kg (in Group P) combined with fentanyl 10 μg/kg and vecuronium 0.1 mg/kg, and maintained with propofol 5 mg. kg-1·h-1 (in Group P) or midazolam 0.1 mg·kg-1·h-1(in Group M) and intermittent i.v. boluses of fentanyl and vecuronium after tracheal intubation. The patients were mechanically ventilated with PETCO2 maintained at 35-45 mm Hg. Myocardial tissues were obtained from the right atrium before and after CPB for determination of apoptosis in cardiomyocytes (by TUNEL). The apoptotic index was calculated. The expression of caspase-3 and caspase-9 was determined by immunohistochemical avidin-biotin-peroxidase complex (ABC) technique staining. The mean airway pressure (MAP) and heart rate (HR) were monitored. Aortic cross-clamping time, surgical and CPB times, spontaneous recovery of normal heart beat, emergence from anesthesia, extubation time and duration of ICU stay were recorded and compared between the two groups.Results The percentage of spontaneous recovery of normal heart beat after release of aortic cross clamp was significantly higher and the need for dobutamine support was significantly less in Group P than in Group M ( P < 0.05). The emergence from anesthesia was significantly more rapid, the extubation time and the ICU stay were significantly shorter in Group P than in Group M (P<0.05). There were no significant differences in apoptosis index and expression of caspase-3 and caspase-9 before CPB between the two groups. The apoptosis index and expression of caspase-3 and caspase-9 were significantly increased after CPB in Group M and significantly higher than those after CPB in Group P (P<0.05).Conclusion Cardiomyocyte apoptosis in patients undergoing CPB can be inhibited and the postoperative recovery is more rapid under propofol-combined anesthesia.
3.Inhibition of HER-2 expression by RNA interference in osteosarcoma cells

[Objective]To investigate whether RNA interference(RNAi)could suppress the expression of HER-2 in osteosarcoma OS-732 cells.[Method]OS-732 cells were transsfected using chemically synthesized double stranded RNA(dsRNA)formulated with Lipofectamine 2000.The HER-2 gene expression inhibition was assessed by RT-PCR,Western Blot and immunocytochemistry.The chemosensitivity of transfected cells to cisplatin was determined by MTT measurement.[Result]Sequence specific siRNAs targeting HER-2 down-regulated the expression of HER-2 significantly.Currently,a range of effects on cell physiology,such as growth inhibition or apoptosis,was observed,Introduction of the siRNA into HER-2 positive tumor lines in vitro greatly reduced the expression of the HER-2 mRNA,effects lasted for 10 days,On the 9th day of interference,HER-2 protein expression was down-regulated in interference group.After RNAi,HER-2-siRNA increased thesensitivity of OS-732 to cisplatin by 10-fold.[Conclusion]Sequence specific siRNAs targeting HER-2 can suppress the expression of HER-2,and significantly inhibiting cellular proliferation,increasing the sensitivity of OS-732 to eisplatin.The successful application of HER-2 siRNA for inhibition of proliferation in HER-2 overexpressed cells extends the list of available therapeutic modalities in the treatment of human osteosarcoma.

5.Use of spiral CT with 3D reconstruction for fractures of tri-ankle joint

[Objective]To investigate the clinical use of spiral CT with 3D Reconstruction for fractures of the tri-ankle joint.[Method]Fifty-six cases of tri-ankle joint fractures treated in our hospital from 1999 to 2004 were studied respectively.The ankle joint X-ray and spiral CT with 3D reconstruction were taken for all the cases preoperatively.The results were analyzed and compared with finding in operation .Scanning parameters of spiral CT were: 2 to 5mm collimation,3 to 5mm table speed and 1.5 to 3mm reconstruction interval.[Result]Fractures were demonstrated definitely by spiral CT in all the cases.Analyses of the results showed the superiority of spiral CT with 3D reconstruction in the fracture classification.Exact assessment of fragments and total agreement of the preoperative results were compared with findings in operation.[Conclusion]Operative treatment and rehabilitation may provide satisfactory fracture reduction and clinical results for ankle joint fracture.Well-timed treatment,correct fracture pattern estimation,proper internal fixation and rehabilitation are important to achieve better long term results.

Proteomics is complementary to genomics because it focuses on the large scared gene products, which are the active agents in cells. As almost all the drugs are directed against proteins, proteomics have been widely used in pharmacological research.Proteomics profiling in drug target identification, toxicological and drug resistances research was reviewed.
7.Clinical analysis of 113 cases of tuberculous pleurisy in children

Objective To investigate the clinical characteristics of tuberculous pleurisy(TP) in children in order to provide a good solution to the diagnosis and treatment of this disease.Methods Retrospective analysis was performed on the clinical data of 113 cases confirmed with TP who were admitted to Children's Hospital of Chongqing Medical University from January 2009 to December 2014.Results Among the 113 pediatric inpatients with TP,85 cases (75.2%) were sick for less than 1 month;92.9% (105/113 cases) age ≥≥5 years old by onset;and 21.2% (24/113 cases) patients demonstrated the contact with tuberculosis ,among whom 70.8% (17/24 cases) attributed to linear relatives.The primary clinical manifestations included fever[96.5% (109/113 cases)], cough [76.1% (86/113 cases)] and chest pain[38.9% (44/113 cases)] ,with as high as 89.0% (87/109 cases) of the patients experiencing moderate or ardent fever.The positive rates of acid-fast bacilli smear and mycobacterium tuberculosis culture were 12.2% (17/139 cases) and 22.7% (15/66 cases), respectively;among the 41 patients who received pleural biopsy,40 cases (97.6%) were identified with the typical pathological changes in tuberculosis;and all 11 patients who received acid fast stain test on pleural tissues were positive to the test.Pulmonary parenchymatous lesions were revealed on CT examination in 89.1% patients(98/113 cases),82.4% (90/109 cases) of the patients had normal body temperature within a short period,and reduced effusion was revealed in 86 out of 98 patients who received repeated chest imaging examinations.Conclusions TP in children has acute onset typically, and the diagnosis of TP in children is more difficult than in adults,requiring considering multiple factors.Contact history with tubercular patients, etiological examination, pleural biopsy and chest CT are essential for the diagnosis of this disease.
8.Role of CREB-binding protein in cyclooxygenase 2 expression in spinal cord in a rat model of neuropathic pain

Objective To evaluate the role of CREB-binding protein (CBP) in cyclooxygenase 2 (COX-2) expression in spinal cord in a rat model of neuropathic pain (NP).Methods Forty male SD rats weighing 220-250 g in which intrathecal catheter was successfully implanted were randomly divided into 4 groups (n=10 each):sham operation group (group Sham),group NP,negative control group (group NC) and group CBP.NP was induced by CCI at 5 days after successful implantation of the intrathecal catheter.Normal saline,negative lentivirus vector (RNAi-NC-LV) and CBP shRNA lentivirus vector (CBP RNAi-LV) 20 μl were injected intrathecally at 7 days after CCI in groups NP,NC and CBP,respectively.Paw withdrawal threshold to mechanical stimuli (MWT)and paw withdrawal latency to thermal stimulus (TWL) were measured at 1 day before operation and at 3,5,7,10,12 and 14 days after operation.The rats were sacrificed after the measurement of MWT and TWL at 14 days after operation and the lumbar segment of the spinal cord was then removed for determination of the expression of CBP,acetylated histone H3/H4 (Ac-H3,Ac-H4) and COX-2 protein (by immunohistochemistry),and CBP and COX-2 mRNA (by RF-PCR).Results Compared with group Sham,MWT and TWL were significantly decreased at different time points after operation in groups NP,NCand CBP,and the expression of CBP,Ac-H3,Ac-H4 and COX-2 protein,and CBP and COX-2 mRNA was up-regulated at 14 days after operation in groups NP and NC (P < 0.05).Compared with group NP,MWT and TWL were significantly increased at 10,12 and 14 days after operation and the expression of CBP,Ac-H3,Ac-H4 and COX-2 protein,and CBP and COX-2 mRNA was downregulated at 14 days after operation in group CBP (P < 0.05).Conclusion CBP is involved in the development of NP by up-regulating COX-2 expression and increasing histone H3 and H4 acetylation in spinal cord.
9.Flow cytometry analysis of the protective effect of platelet factor-4 on γ -ray irradiated mouse hemopoiesis

Aim To investigate radioprotective effects of platelet factor 4 (PF4) on hemopoiesis of mice after γ -ray irradiation and their mechanisms. Methods The mice were injected with PF4 twice at 6h intervals; and 20 h after the second injection they were given one irradiation of 7.5Gy 60Co γ -ray. 8d later the colony forming unit of granulocyte-macrophage (CFU-GM), colony forming of unit spleen (FU-S), number and DNA contents in bone marrow cells (BMCs) were examined by flow cytometry. Results 8d after irradiation, a significant difference was shown between DNA contents in BMCs and cell cycles of the PF4-injected mice and those of the irradiated mice (P∨ 0.01), whereas little difference was found between those of the irradiated mice and the controls (P∧ 0.05). Conclusion PF4 can obviously protect hemopoietic stem cells (HSCs) of mice from radiation injury and promote proliferation of HSCs after radiation.
10.Effect of intraurethral irrigation with honeysuckle water-decoction on formation of bacterial biofilm on the surface of catheter

Objective To investigate the effect of intraurethral irrigation with honeysuckle water-decoction on formation of bacterial biofilm on the surface of the catheter. Methods Eighty male patients,who had no history of catheterization and no urinary infection with negative results of urine bacterial culture,were enrolled in the study. All patients were randomly divided into 4 groups with 20 cases in each: group A (patients underwent intraurethral irrigation with honeysuckle water-decoction); group B (intraurethral irrigation with 25% PVP); group C (intraurethral irrigation with normal saline ) and group D (no intraurethral irrigation). The urethral catheters were removed at the 7th day and the bacterial biofilm on the surface of the catheter was examined by scanning electron microscopy (SEM). Results SEM demonstrated uneven- distributed matrixes coated on the surface of the catheter, and bacteria adhered to the matrixes. The average thickness of biofilm in group A, B, C and D was (17.876 ±11.788)μn, (31.090±15. 006)μm,(33.716±11.927)μm, (67.762±23.588)μm, respectively. The average thickness of biofilm in group A, B, C was significantly thinner than that in group D (P<0.01), and the thickness in group A was thinner than that in group B and C (P<0.01), but there was no significance difference between group B and group C (P>0.01). Conclusions Intraurethral irrigation with honeysuckle water-decoction can effectively reduce the formation of bacterial biofilm on the surface of the catheter.
