1.Detection and its clinical significance of CD+4 CDHi25 CDLo127 regulatory T cells in peripheral blood of patients with thyroid neoplasm

Objective To analyze CD+4 CDHi25 CDLo127 regulatory T cells (Treg) in peripheral blood of patients with thyroid nodules and differentiated thyroid cancer and their change regularity, and to investigate the immunosuppression mechanism. Methods The peripheral blood was collected from 175 patients with thyroid nodules, including 43 patients with differentiated thyroid cancer and 132 patients with nodular goiter.By using monoclonal antibodies, the blood samples were evaluated with the flow cytomertry for lymphocyte subsets and Treg cells. Results The results showed the prevalence of the CD+4 CDHi25 CDLo127 Treg in differentiated thyroid cancer group [(6.48±1.49) %] and nodular goiter group [(6.23+1.67) %] was higher than those in the healthy group [(5.62±1.48) %], and the difference was significant(P < 0.005), but there was no significant difference between the nodular goiter group and differentiated thyroid cancer group (P >0.005).Conclusion It is concluded that the relative increase of CD+4 CDHi25 CDLo127 Treg in peripheral blood of patients with nodular goiter and differentiated thyroid cancer may be related to immunosuppression and tumor progression.
2.Clinical research on Huang Bai capsules combined with Chinese medicine fumigation on the treatment of chronic anal fissure

Objective To research the effect of Huang Bai capsules combined with Chinese medicine fumigation on treatment of chronic anal fissure.Methods 84 patients with chronic anal fissure from the second hospital of Shandong from January 2014~June 2016 were divided into two groups randomly.The control group was treated with Nitroglycerin and Lining gel and the observation group was treated with Huang Bai capsules combined with Chinese medicine fumigation.Treatment efficacy,symptom improvement,VAS score,gap healing time,ARP and RRP were compared between the two groups.Results The total effective rate in the observation group 95.24%is higher than that in the control group 78.57%(P<0.05).The proportion of bleeding and constipation after treatmen were lower than that before treatment(P<0.05).The proportion of bleeding and constipation in the observation group(9.52%and 11.90%)were lower than those in the control group(28.57%and 33.33%)(P<0.05).③ VAS score and gap healing time in the observation group were(1.18±0.20)points and(14.92±2.05)days,which were better than those in the control group(2.39±0.22)points and(19.16±2.28)days(P<0.05).④ARP and RRP in the observation group were(7.88±0.40)kPa and(2.61±0.31)kPa,which were lower than those in the control group(8.59±0.55)kPa and(3.09±0.45)kPa(P<0.05).Conclusion It can speed up crack healing and improve the effect of patients with anal fissure which Huang Bai capsules combined with Chinese medicine fumigation were used in the treatment.
3.Numerical simulation analysis of hemodynamic values of budd-chiari syndrome based on MR image

Objective The hemodynamic mechanism of Budd-Chiari syndrome ( BCS ) has become the research hotspot in recent years.The aim of this study was to discuss the hemodynamic characteristics of BCS through 3D numerical simulation for inferior vena cava stenosis based on the fluid dynamics ( CFD) method. Methods 3D model was established from a patient with Budd-Chiari syndrome based on MR image with Ansys software .The numerical simulation of this model was performed by the CFD . Results The 3D model of inferior vena cava functionally demonstrated the change procedure of hemodynamic characteristics of BCS .Vortex was found above the narrow area , and blood flow velocity achieved maximum in the center of the stenosis throughout the coronal section graph.The static pressure value gradually declined at the narrow area entrance to the narrowest place , and achieved minimum value at the exit.The maximum wall shear stress existed in the stenosis . Conclusion Specific 3D computational hemodynamic model can show the hemodynamic characteristics of BCS , and its hemodynamic parameters could be used for clinical practice .It will facilitate the study on correlation of complex hemodynamic parameters and morphology changes of inferior vena cava vascular .
4.Expression and clinical significance of diacylglycerol kinase a and protein kinase C in hepatocarcinoma

ObjectiveTo investigate the expression and the clinical significance of diacylglycerol kinase α (DGKα) and protein kinase C (PKC) in human hepatocellular carcinoma (HCC).MethodsDGKα and PKC expressions in the samples from 60 pathologically confirmed HCC patients were analyzed by immunohistochemistry. The relationship between DGKo expression and clinical pathology factors was analyzed.ResultsThe expression positive rates of DGKo and PKC were highest in normal liver tissues [90.0% (9/10) and 100.0% (10/10)].The positive rates were 81.7 % (49/60) and 71.7 % (43/60) in HCC tissues,respectively,and were 58.3 % (35/60) and 61.7 % (37/60) in carcinoma adjacent tissues,respectively.In three liver tissues,the positive rates of DGKα and PKC were significantly different (P <0.05).The location of both kinases in hepatocytes translocated from cytoplasm/nucleus to membrane.The expressions of DGKα and PKC were positively correlation(r =0.495, P < 0.05), The DGKα expression was correlated to differentiation type,portal venous tumor thrombus and TNM staging(all P < 0.05).ConclusionDGKa is expressed with high activity in advanced stage and poorly differentiated HCC. It may be promote the pathological process of HCC.
5.Development of web-based literature database for Yersinia pestis

Objective To build a literature database that systematically sorts out and integrates related knowledge of Yersinia pestis researches, which will improve the efficiency of future researches on Y.pestis.Methods EndNote X5 software was used as a literature collection tool in this research.Computer languages, including Perl and PHP, were employed to standardize the text data, establish the entity relationship model, code the webpage and build the basic literature database.The tool combination of Apache+PHP+MySQL that is usually used in development of small or medium-scale websites was selected to build a dynamic website.Results and Conclusion The Y.pestis literature database(YPKD) was constructed (accessed by http://101.201.51.148/ypkd/.), from which users can quickly search or browse the required literature, download the required full text, and obtain the latest online information on Y.pestis.The database can serve as a model for establishing literature databases of other crucial pathogens.
6.Construction and identification of the replication defective adenoviral vectors carrying mouse SCp2shRNA

Objectives It was constructed that the replication defective adenoviral vectors carried the short hairpin sequences of mouse SCP2.And we will make a further study of mechanism between SCP2 gene and cholesterol stone in gallbladder.Methods The short hairpin sequences of mouse SCP2 were cloned by two-step PCR,and connected together with the plasmid pDC312.The Admax Adenoviral Vector System was used to generate the replication defective adenoviral vectors,which were purified by CsCl method.The processes of TCID50 were applied to detect the titers of the adenoviral vectors.Furthermore,Protein levels of SCP2 were determined by Western blot analysis,and the levels of SCP2、CYP7A1、HMGCR mRNA from the hepa1-6 cell of mouse were measured by the usage of RT-PCR.Results SCP2mRNA and SCP2 protein were down-regulated by the replication defective adenoviral vectors carried the SCP2-shRNA.With the decreasing SCP2mRNA,the levels of HMGCRmRNA were down-regulated at same the time,while CYP7A1mRNA were up-regulated.Conclusions The replication defective adenoviral vectors carried SCP2-shRNA were constructed successfully.The lower levels of SCP2 could affect the activities of HMG-CoA reductase and CYP7-a enzyme,which caused the variations of cholesterol metabolism and then decreased the formation of cholesterol stone.
7.Progression from ocular to generalized myasthenia gravis in adults

Objective To investigate the time and rate of generalization developed from ocular symptoms in adults to generalized myasthenia gravis (GMG).Methods The data from 456 adult patients who started on ocular symptoms during Jan 2005 to Jan 2012 were retrospectively studied,with a follow-up period of 3 months to 17 years.The data included transfer time,Osserman type,difference between steroids group and no-steroids group.Results (1) During the follow-up period,197 (43.2%) patients developed to generalized myasthenia gravis,58.4% (115/197)in one year after onset of ocular symptoms,74.1% (146/197) in 2 years and 80.7% (159/197) in 3 years.86.8% (171/197) developed to Osserman Ⅱ a or Ⅱ b.(2) Of 233 patients untreated with steroids,134 (57.5%) developed to GMG,while 63 (28.3%) of 223 patients who were treated with steroids developed to GMG (x2 =38.57,P < 0.01).There were no difference between two groups in transfer time and Osserman type.Conclusion MG initiated from ocular syndromes in adults easily develops to GMG in 2 years.Patients treated with steroids have low chance of generalization.
8.The technique of guide-wire loop in interventional therapy of patients with Budd-Chiari syndrome with hepatic vein obstruction

Objective To investigate the value of guide-wire loop in interventional therapy of patients with Budd-Chiari syndrome with hepatic vein obstruction.Methods A retrospective study was conducted on 25 patients with Budd-Chiari syndrome (BCS) with hepatic vein obstruction treated from May 2011 to August 2014.The technique of guide-wire loop was used in these patients.The pressure of the hepatic vein was measured before and after treatment.The difference in the pressure was analyzed by the t test.Results All the patients were treated successfully using guide-wire loop angioplasty.No complications of bleeding,pericardial tamponade and liver capsule hemorrhage were observed.After treatment,the pressure of the hepatic vein reduced from (48.3± 8.0) cmH2O to (20.9 ± 3.8) cmH2O (t =26.82,P < 0.05);The symptoms and physical signs of the patients were relieved or disappeared.BCS-related symptoms reappeared on follow-up in 4 patients,2 were treated by balloon dilation successfully.1 patient was treated with transjugular intrahepatic portosystemic shunt (TIPS) because of decompensated liver cirrhosis.1 patient with ankylosing spondylitis had treatment failure and the symptoms relapsed for the third time.Conclusion The technique of guide-wire loop in interventional therapy of patients with Budd-Chiari syndrome with hepatic vein obstruction is safe and effective,and it can improve the overall success rate of treatment.
9.Clinical features of right temporal lobe variant of semantic dementia

Objective To identify the clinical profile and neuro-imaging findings of the right temporal lobe variant of semantic dementia (RTLV),and to understand the relationship between anatomic and function by comparison to the left temporal lobe atrophy.Methods Of 70 patients with diagnosis of frontotemporal dementia in our memory clinic,three patients with right temporal lobe atrophy were identified based on the MRI scans.We obtained the profile of cognitive function,behavior and personality changes in these 3 patients and compared them with those in 5 semantic dementia (SD) patients with predominant leftsided temporal lobe atrophy.We also underwent 18F-fluoro-2-deoxy-D-glucose positron emission tomography (18F-FDG-PET) with statistical parametric mapping analysis in these 3 patients.Results In RTLV patients,the most prominent cognitive deficits were impairment of prosopagnosia,and getting lost.Variety behavioral symptoms including apathy,social disinhibition,stereotypy,compulsive behaviors were more prevalent in early course for patients with RTLV,which were different from SD patients with semantic loss began with anomia or single-word comprehension.18F-FDG-PET scan showed decreased metabolism mainly in right temporal lobe and in other brain regions with varying degrees.Conclusions The RTLV is clinical heterogeneous.Prosopagnosia,behavioral changes and getting lost are the main features in the early course.It therefore,might be proposed a separate clinical entity.
10.Clinical study of interventional embolization for the treatment of tiny intracranial aneurysms

Objective To explore the therapeutic efficacy of endovascular embolization for the treatment of tiny intracranial aneurysms (≤3.0 mm) and to discuss its technical skill. Methods During the period from Dec. 2010 to July 2013, a total of 12 patients with tiny intracranial aneurysms (≤3.0 mm) were admitted to authors’ hospital to receive endovascular embolization therapy. Of the 12 patients, narrow-necked aneurysm (neck-to-body ratio ≤0.5) was seen in 7 and embolization with coils only was carried out, while wide-necked aneurysm (neck-to-body ratio > 0.5) was seen in 5 and stent-assisted coils embolization was adopted. Based on Raymond grading standard, the immediate therapeutic results were evaluated, and the procedure-related complications were recorded. Thirty days after the treatment , GOS grade was used to assess the results. Follow-up evaluation with angiography or through call was conducted. Results Successful embolization of the aneurysm was obtained in all the 12 aneurysms , with a success rate of 100%. Angiography performed immediately after the treatment showed that complete embolization was achieved in 8 aneurysms (66.7%) and residual aneurysm cavity was seen in 4 aneurysms (33.3%). GOS grading indicated that 12 cases belonged to grade V. The 12 patients were followed up for 1 - 12 months through telephone , and no re-bleeding occurred. Conclusion For the treatment of tiny intracranial aneurysms (≤ 3.0 mm), endovascular embolization is feasible although its safety and efficacy need to be further observed.
