1.Curative effect of mosapride and Shugan Jieyu capsule in the treatment of functional dyspepsia and Influence on brain-gut peptide

Objective To investigate the curative effect and mechanism of mosapride and Shugan Jieyu capsule in the treatment of functional dyspepsia(FD).Methods One hundred and ten cases patients with FD were divided into two groups according to the digital random table method,with 55 cases in each group.The control group took mosapride tablets orally(5 mg/times,3 times/d,30 min before meals),the treatment group took with Shugan Jieyu capsule orally(2 pellets/times,2 times/d,orally with warm water on morning and evening)on the basis of treatment of the control group.Two groups were treated continuously for 4 weeks.The symptoms score and total effective before and after treatment were compared,and plasma Leptin(Leptin),Ghrelin and gastric dynamic element(MTL)levels of the two groups before and after treatment were compared.The two groups were followed up for 6 months after treatment,the recurrence rate of the two groups was compared.Results The symptoms integral of the treatment group after treatment was lower than that of the control group((4.97±1.85)points vs.(7.35±2.28)points,t=6.011,P<0.01),the total effective rate was higher than that of the control group(94.5%(52/55)vs.81.8%(45/55),x2=4.274,P=0.039),the improvement of plasma Leptin((13.10±2.07)μg/L vs.(14.66±2.11)μg/L,t=3.914,P=0.001),Ghrelin((3.52±0.70)ng/L vs.(2.95±0.67)ng/L,t=4.363,P<0.01),MTL((281.47±61.09)ng/L vs.(242.31±65.28)ng/L,t=3.248,P=0.002)levels were better than those of the control group,the recurrence rate of 6 months was lower than that of the control group(7.5%(5/53)vs.24.4%(11/45),x2=4.124,P=0.041),the difference was statistically significant.Conclusion Mosapride tablets combined with Shugan Jieyu capsule can benefit the liver symptom,and also can reduce the recurrence rate,it has curative effect on FD,its mechanism may be related to the improvement of BGP levels disorder in patients with FD.
2.Clinical study on valsartan and methylprednisolone combined with cyclophosphamide for treatment of IgA nephropathy with prevailing proteinuria

Objective To evaluate the clinical efficacy and safety of valsartan and methylprednisolone com-bined with cyclophosphamide for treatment of IgA nephropathy with prevailing proteinuria.Methods 118 case of IgA nephropathy patients with prevailing proteinuria were divided into the research group(60 cases)and control group (58 cases)according to random number table.Patients in the control group were given valsartan capsules(80mg/d) and methylprednisolone tablets(1mg·kg -1 ·d -1 ),patients in the research group were given cyclophosphamide tab-lets on the basis of control group(0.5 ~1.0g/m2 ,1 time /month,a total of 10 times).All the patients were treated for 12 months.The clinical efficacy,24h urine protein quantitative and plasma -albumin(ALB)were observed before and after treatment.Results After treatment,the clinical efficacy of research group and control group were 79.33% and 43.10%,the data of research group was significantly higher than that of the control group(χ2 =19.401,P <0.01), the 24h urine protein quantitative of the research group was significantly decreased from (2.0 ±1.3)g/24h before treatment to (0.9 ±0.2)g/24h in the 12 month after treatment(t =7.184,P <0.01),the ALB of the research group was significantly increased from (32.2 ±4.2)g/L before treatment to (39.5 ±4.8)g/L in the 12 month after treat-ment(t =5.335,P <0.01),the 24h urine protein quantitative of the research group afer 4,6,8,10,12 month were significantly lower than those in the control group(t =3.461,3.854,4.021,3.976,3.365,all P <0.05),the ALB of research group afer 8,10,12 month were significantly higher than those in control group(t =3.850,5.020,5.560,all P <0.05).Conclusion Valsartan combined methylprednisolone can more effectively reduce urinary protein and pro-tect renal function for the treatment of IgA nephropathy.
3.Application of performance evaluation in bonus distribution in nurses working in otolaryngology department

Objective To explore the application effect of performance evaluation in bonus distribution in nurses working in otolaryngology department,and seek for the perfect solution of bonus distribution,motivate nurses' work enthusiasm,and therefore be helpful for the enhanced nursing quality andsafety.Methods Bonus coefficient was calculated according to technical title (make up 40%),position(make up 30%) and performance assessment(make up 30%).The occurrence of nursing defects,patients satisfaction degree,the scores of nursing performance,the scores of theoretical test and skill examination,and nurse satisfaction degree were compared between pre-and post-implementation of system.Results The above items showed significant differences before and after the practice of performance evaluation.Conclusions Bonus distribution according to performance evaluation can be effective in the improvement of nursing quality,patients' satisfaction degree and nurses' work enthusiasm,therefore be helpful for the enhanced nursing management level in otolaryngology department.
4.RP-HPLC determination of paeoniflorin,baicalin,palmatine hydrochloride and berberine hydrochloride in Kuntai Capsules

AIM: To develop an HPLC method for analyzing the contents of paeoniflorin,baicalin,palmatine hydrochloride and berberine hydrochloride in Kuntai Capsules(Praeparata Radix Rehmannial,Rhizoma Coptidis,Radix Paeoniae,Radix Scutellariae,Colla Asini corii,Poria).METHODS: Four compounds were determirned with an Agilent ZORBAX SB C18 column(4.6 mm ? 250 mm,5 ?m).Acetonitrile-0.4% phosphoric acid was used as the mobile phase at a flow rate of 1.0 mL/min.The mobile phase was in linear gradient elution.The detection wavelength was 230 nm.RESULTS: The linear ranges were within 0.016 58-0.331 6 ?g(r = 0.999 9) for paeoniflorin,0.059 80-1.196 0 ?g(r = 0.999 9)for baicalin,0.012 91-0.258 2 ?g(r = 0.999 9)for palmatine hydrochloride,0.041 98-0.839 6 ?g(r = 0.999 9) for berberine hydrochloride.The average recoveries (n = 6) were 98.91% (RSD = 1.29% ),99.20% (RSD = 0.92% ),97.86% (RSD = 0.89% ),98.63% (RSD = 0.62% ),respectively.CONCLUSION: The method is simple,reliable,sensitive and can be applied to having controls over the preparation.
5.Analysis of 20 cases of patients with diabetes merged oral and maxillofacial space infection

Objective To discuss the methods of treating diabetes merged oral and maxillofacial space infec-tion and their effects.Methods Clinical data of 20 patients with diabetes merged oral and maxillofacial space infec-tion were retrosepectively analyzed.Results All 20 cases of patients with diabetes merged oral and maxillofacial space infection were cured.The mean length of treatment was (18.4 ±2.8)days.There were 20 cases of positive results of bacterial culture,the positive rate was 100%,and there were 15 cases of streptococcal infection,4 cases of klebsiella pneumoniae infection,1 case of candida albicans infection.20 cases of patients was given incision and drainage,4 cases given incision of trachea.Conclusion It was easier for patients with diabetes having maxillofacial space infection,and more serious illness,more difficult for treating.Local infection should be treated,and systemic conditions should be improved as well.The risks of occurrence of serious complications should be reduced.

7.Determination of adenosine in Compound Houtou Capsule by HPLC

Objective: To establish the determination method of adenosine in Compound Houtou Capsule. Methods: HPLC was performed to determine adenosine on DISCOVERY column, using methanol-0.05 mol/L potassium dihydrogen phosphate solution (13∶87,v∶v) as a mobile phase and detection wavelength at 259nm.Results: The linearity of this method was good, and the average recovery was 100.72%, RSD =0.22%. Conclusion: This method was simple and accurate, and available for the quality control of Compound Houtou Capsule.
8.Clinical effect and mechanism of hirudin in treating lower limb venous thrombosis after fracture

Objective To analyse the clinical effect and mechanism of hirudin in the treatment of patients with lower limb venous thrombosis after fracture.Methods 56 patients who were diagnosed with lower limb venous thrombosis after fracture in our hospital were collected.All patients were randomly divided into experimental group (n:28) and control group (n:28).12h after fracture operation, patients in control group were treated with subcutaneous injection of low molecular weight heparin calcium, patients in experimental group were treated with Maixuekang capsule ( hirudin) orally on the basis of control group.Before and after the treatment, the plasma levels of thrombin antithrombin complex ( TAT) , thrombin activated fiber soluble inhibitor (TAFI), D-Dimer (D-D), fibrinogen (FIB), thrombin time (TT), prothrombin time (PT), activated partial thromboplastin time (APTT) and clinical effect were detected in all patients.Results After treatment, the total effective rate of experimental group ( 92.86%) was significantly higher than that of the control group ( 71.43%) ( P<0.05 );compared with control group, the plasma levels of TAT, TAFI were lower in the experimental group (P<0.05); the plasma levels of D-D, FIB were lower in the experimental group (P<0.05); the plasma levels of TT,PT,APTT were higher in the experimental group ( P<0.05 ) .Conclusion The hirudin could inhibit thrombin activation and coagulation function in fracture patients with lower limb venous thrombosis, decrease the plasma D-D and FIB levels for improving blood hypercoagulable state and preventing thrombosis,thus improve the clinical efficacy.
