1.Exploration in intensive management mode of hospital

The paper elucidates the theory and connotation of intensive management of hospital,discusses the domestic and foreign research status on hospital management,and studies the management mode of hospital by analyzing the internal and external environment of hospitals in China.By referring to the experience of foreign experts and the different concepts put forward by domestic experts,introducing medical value chain theory,and attaching importance to the contact with patients and the basic core of medical service.Chinese hospitals can integrate the internal operation system of hospital using scientific and technological means to form a new type of dynamic organization structure and implement intensive management of medical input and output of quality benefit.
2.Value of ultrasonography in choledocholithotomy with fibroptic choledochoscope

Objective To explore the feasibility and value of ultrasonography during choledocholithotomy with fibroptic choledochoscope. Methods Fifty-one cases of intrahepatic bile duct stones after cholecystomsy as diagnosed by pre-operative examination with ultrasound, CT, magnetic resonance imaging from Jan. 2007 to Dec. 2008, underwent conventional lithotomy through fibroptic choledochoscope first, and then high-frequency probing with ultrasonography was done directly over the surface of liver to detect the existence of any residual stone in the bile duct. The position of residual stone, if there was any, was then accurately determined and removed through fibroptic choledochoscope under the guidance of ultrasonography. Results Of 51 patients who had undergone lithotomy through fibroptic choledochoscope, 12 patients were found to have residual stones in their bile ducts after operation. The rate of complete calculus removal was 76.5%, and of residual stones was 23.5%. Of the 12 patients with residual stones, 7 patients were found to have residual stones in their intrahepatic bile ducts complicated with deformed bile duct, 5 patients were found to have residual stones in their intrahepatic bile ducts complicated with stricture of bile duct. All residual stones in intrahepatic bile ducts of 12 patients were accurately located and then completely removed under the guidance of operative ultrasonography through fibroptic choedochoscope without any complications. Conclusions The difficulties of residual stones after lithotomy may be overcome by localization with ultrasonography and removal under the guidance of atfrasonography, so the technique is of practical value in clinical application.
3.Application of percutaneous transhepatic multiple biliary drainage introduced under ultrasound guidance in treatment of advanced portal cholangiocarcinoma

Objective To evaluate the feasibility and value of percutaneous transhepatic multiple biliary drainage introduced under ultrasound guidance in treatment of advanced portal cholangiocarcinoma. Methods Thirty-two patients were accurately diagnosed as advanced portal cholangio carcinoma according to preoperational examination of ultrasound,enhanced computed tomography,nuclear magnetic resonance or endoscopic retrograde cholangiopancreatography (ERCP) in the hospital from Jan. 2006 to Dec. 2008. After making a reasonable therapeutic plan,3 or 4 drainage-tubes were successfully inserted in intrahepatic bile duct in turn using Seldinger technique under ultrasound guidance. Thus,retained bile was effectively drained from intrahepatic bile ducts. The changes in jaundice index after treatment and complications were observed,follow-up was performed during the patients' surviving period,and therapeutic effect was evaluated comprehensively. Results Of the 32 patients drained through multiple bile drainage-tubes,19 received 3 draining tubes simultaneously,and 13 with 4 bile drainage-tubes,with a total of 109 bile drainage-tubes inserted into all the patients. Bile drainage-tubes were inserted into all target bile ducts successfully in a 100% successful rate. 4-8 weeks after operation,the jaundice disappeared completely in 28 cases and decreased significantly in 4 cases. For all the cases,complication occurred in only 2 patients,and it disappeared after symptomatic treatment. The shortest survival time was 4 months,and the longest was for 15 months after operation. Conclusions For those patients with advanced portal cholangiocarcinoma,and the tumor could not be surgically resected,percutaneous transhepatic multiple biliary drainage is correct option,which is effective and safe,and it was valuable in relieving malignant obstructive jaundice.
4.Research progress on clinical application of mobile nursing information system

With the development of technology, the new technologies contact with the medical profession increasingly close. Mobible nursing information system (MNIS) is a new technology product of the combination of the nursing profession, which will digitize the introduction of paperless care industry, providing convenient for clinical work, real-time technical service is an important tool for clinical work. The application of MNIS have played an important role in confined the nursing profession, disease management and pharmaceutical management, more users discover problem from different professional perspective, thus the MNIS should be improved for all-round care. This article is consist of the domestic and care information systems development, application and effect, as well as its application in the clinical factors of its research progress in clinical application were reviewed to provide a basis for the development of leadership and decision-making about MNIS, and thus, enhance the mobile application of the effectiveness of care information systems and promoting their improvement.
5.Progress of Drug Treatment for Gastrointestinal Neuroendocrine Neoplasm

Neuroendocrine neoplasm (NEN)is a group of heterogeneous tumor originated from the neuroendocrine system,and gastrointestinal neuroendocrine neoplasm (GI-NEN)is the mostly frequently seen. The incidence of GI-NEN is increasing year by year,but the efficacy of clinical treatment is unsatisfactory. In recent years,new progress has been achieved in medical therapy of GI-NEN,including somatostatin analogues,interferon,mTOR inhibitor,vascular endothelial growth factor inhibitor and chemotherapeutic drugs. Peptide receptor radionuclide therapy also provides an additional option for treatment of metastatic GI-NEN. This article reviewed the recent progress of drug treatment for GI-NEN.
6.Immunohistochemical Study on Bcl-2 and PCNA in Mixed Tumor of Skin

Objective To investigate the expression of bcl-2 and PCNA in mixed tumor of skin. Methods Bcl-2 and PCNA expression in 12 cases of mixed tumor of skin were studied using immunohistochemical S-P method and double-staining. Results Positive reactions of bcl-2 and PCNA were observed in all cases. PCNA staining was positive in tumor cells lining cystic and duct-like structures in tumor nests, but bcl-2 staining was positive in epitheloid tumor cells. Most tumor cells with bcl-2 were PCNA negative and vice versa. Conclusion Aberrant expression of bcl-2 and PCNA in mixed tumor of skin may play a role in tumorigenesis.

9.Effect of acupoint massage on vital signs in patients with coronary heart disease and angina pectoris

Objective To investigate the impact of acupoint massage on the vital signs of patients with angina pectoris in coronary artery heart disease (CHD) during the operation. Methods Using non-concurrent cohort controlled design, 38 cases who with angina pectoris in CHD received interventional therapy, from June to September in 2015, in the therapy rooms of our hospital were selected as control group and received routine nursing. 38 cases who with angina pectoris in CHD received interventional therapy, from October 2015 to January 2016, in the therapy rooms of our hospital were selected as intervention group and received acupoint massage based on routine nursing. Heart rate and blood pressure of patients in two groups were monitored and recorded during the operation. Results Compared with control group, systolic blood pressure of patients in intervention group decreased obviously (143.26±17.67 vs. 153.18 ± 19.35) mmHg (1mmHg=0.133 kPa), diastolic blood pressure decreased obviously (83.16 ± 12.29 vs. 93.82 ± 16.54), and heart rate decreased obviously (71.89 ± 13.90 vs. 79.05 ± 12.63) mmHg (t=2.33, 3.19, 2.35, P<0.05 or 0.01). Conclusions Acupoint massage can effectively reduce heart rate and blood pressure of patients with angina pectoris in CHD during the operation, relieve patients' anxiety, and help for smoothing operation, shortening operation time and reducing operation risk.
10.Cell metabolism of Taxus chinensis var. mairei in suspension cultures treated with methyl jasmonate

Object To study the physiological changes of Taxus chinensis var. mairei (Lemee et L?vl.) Cheng et L. K. Fu in the case of methyl jasmonate (MJ). Methods TTC assay, soluble protein measurement and enzyme analysis were used. Results It was observed that MJ inhibited Taxus cell growth in the view of primary metabolism. MJ induced phenylalanine ammonia lyase(PAL) activity while it inhibited polyphenoloxidase (PPO) activity. Extracellular phenolic content after addition of MJ increased to the maximum at the three days than that of the control group. Conclusion It was demonstrated that MJ induced the transition of Taxus cell from primary metabolism to secondary metabolism. This is favorable for secondary metabolism of Taxus cells. It is important to study the physiology of Taxus cells for revealing the mechanism of MJ.
