1.Roles and mechanism of interleukin-1? in inhibiting the formation of epidural scar

[Objective]To investigate the role and mechanism of interleukin-1? on fibroblast synthesis of matrix metalloproteinase-3,and explore the impact of interleukin-1? on the formation of epidural scar.[Method]The NIH3T3 cells were divided into three groups:IL-1? group,IL-1? + SB202190 group and control group. After serum-free culture for 20 hours,IL-1? group was treated by IL-1? (10ng/ml) and IL-1? + SB202190 group was treated by the SB202190 (10?mol/L) for 1 hour before treatment with IL-1?(10ng/ml) and the control group was treated with 2% serum. Then those 3 groups were cultured for 24 hours under standard conditions (37℃ in a humidified atmosphere flushed with 5% CO2 in air). The expression of MMP-3 gene was examined by RT-PCR and Western blotting.[Result]The expression of MMP-3 significantly increased in the IL-1? group than that in the control group(P0.05). [Conclusion]IL-1? could stimulate the expression of MMP-3 in NIH3T3 cells which might inhibit the formation of epidural scar. Besides,the author found that the p38 pathway played an major role in this process.
2.Cervical posterior implant fixation for multi-segment cervical spondylotic myelopathy:improves symptoms but diminishes cervical range of motion

BACKGROUND:Cervical Open-door laminoplasty with unilateral mass screw fixation and laminectomy with bilateral mass screw fixation are both procedures that treat cervical spondylotic myelopathy by expanding the space available for the spinal cord. Effectiveness and safety of two operative methods remain unclear. OBJECTIVE:To investigate the biocompatibility of implant and host between open-door laminoplasty with unilateral mass screw fixation and laminectomy with bilateral mass screw fixation to treat multi-segment cervical spondylotic myelopathy. METHODS: Data of 117 patients with multi-segment cervical spondylotic myelopathy (≥ 3 segments) were retrospectively analyzed. Sixty-five cases underwent open-door laminoplasty with unilateral mass screw fixation (laminoplasty group). Fifty-two cases underwent laminectomy with bilateral mass screw fixation (laminectomy group). Clinical outcomes were evaluated by the Japanese Orthopaedic Association scoring system and by recovery rate. Cervical curvature index and cervical range of motion were assessed by X-ray films in both groups. RESULTS AND CONCLUSION:The average folow-up time was 28 months (range 12-59 months) in both groups. No C5 nerve root palsy occurred in the two groups. Japanese Orthopaedic Association scores were greater during final folow-up than pre-fixation in both groups (P < 0.01). No significant difference in Japanese Orthopaedic Association score, recovery rate, and final folow-up cervical curvature index was detected in both groups (P > 0.05). Cervical range of motion was lower during final folow-up than pre-fixation in both groups (P < 0.01). Results confirmed that open-door laminoplasty with unilateral mass screw fixation and laminectomy with bilateral mass screw fixation have similar effectiveness in the improvement of neurological function, relieving pain and reducing complications, but wil greatly reduce the cervical range of motion.

Objective To investigate the effects of Huangqi Sijun Decoction on the levels of thyroxin and cAMP and cGMP in model rats with spleen deficiency syndrome and to explore its therapeutic mechanism.Methods Male SD rats aged three months were randomly divided into 3 groups:normal control group,spleen deficiency model group and Huangqi Sijun Decoction group.The rat models of spleen deficiency syndrome were established by gavage with the decoction of Rhubarb.The serum levels of thyroxin(T4),3,5,3'-triiodothyronine(T3),thyrotropin(TSH)and plasma levels of cAMP,cGMP were determined.Results T3 and T4 were significantly lower but the levels of cAMP and cAMP/cGMP were significantly higher in the spleen deficiency models than those in the normal control group(P
4.Combination therapy with thalidomide plus dexamethasone for patients with reaped or refractory multiple myeloma and for patients on stable phase multiple myeloma

Objective To evaluate the effects of thalidomide in combination with dexamethasone (ID) in patients with relapsed or refractory multiple myeloma and in patients on stable phase. Methods Sixty-two patients with multiple myeloma were studied, include 25 with relapsed or refractory multiple myeloma, and 37 in stable phase. For the relapsed or refractory patients, thalidomide and dexamethasone was given at first three cycles, and then other regiments were given to no-response patients. For the all response patients, the second three cycles TD were enforced. On stable phase patients, thalidomide and dexamethasone were given as the second three cycles of relapsed or refractory patients. Then, thalidomide was given persistently until the disease relapse. Results All of the 25 relapsed or refractory patients were accepted the first 3 cycle TD treatment. The total response rate (VGPR+PR+MR) was 80 %. No complete remission (CR) and near CR(nCR) was gained. For the all response patients, the second three cycles TD were enforced. One patient achieved nCR. Thalidomide was given to all response patients. The median remission time was 6.8 (4~12) months. TD regimen was used to the 37 stable phase patients. The median remission time was 17.5 (8~26) months. The remission time of stable remission patients is longer then that of relapsed or refractory patients (P <0.001). Conclusion The combination of thalidomide and dexamethasone is a feasible and active regimen in the treatment of relapsed or refractory, and stable phase myeloma patients.
5.Recombinant interlukin-2 plus chemotherapy in treatment of multiple myeloma

Objective To study the effect of recombinant interlukin-2 plus chemotherapy in treatment of multiple myeloma. Methods We divided into two groups: chemotherapy plus rIL-2 group and simple chemotherapy group. The patients were given at dose of rIL-2 1×l06 IU/d, iH, qd, for 4 weeks and 4cycles. After 4 cycles the changes of T-cell subsets of their peripheral blood were determined. Response was evaluated according to Blad criteria. Adverse events were graded according to the National Cancer Institute Common Toxicity Criteria,version 3.0. Results The positive percentages of CD_3, CD_4 NK, CD_(69) and CD_4/CD_8ratio after chemotherapy in chemotherapy plus rIL-2 group were significantly higher than those before chemotherapy in the same group and those after chemotherapy in the other group. The overall response rate and complete response rate were 66.7 % and 25.0 % vs 50.0 % and 8.3 % respectively. The side effects of rIL-2 were predictable and manageable. Conclusion Recombinant interlukin-2 plus chemotherapy could increase immune function of patients with multiple myeloma and has a higher response rate as compared with simple chemotherapy.
6.Vitamin D receptor gene polymorphisms of sporadic myasthenia gravis in 267 Chinese Han patients

Objective To explore the associations between vitamin D receptor ( VDR) Fok- Ⅰ and Apa- Ⅰ polymorphisms and myasthenia gravis (MG) in Chinese Han population.Methods Polymorphisms of VDR Fok- Ⅰ and Apa-Ⅰ were determined by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method.The frequencies of genotypes and hyplotypes were compared among 286 normal controls and 267 MG patients in different subgroups classified by gender,age of onset,presence of thymoma,and Osserman classification sat maximal severity in the follow-up.The association between the genotypes and maximal severity of MG and short-term glucocorticoid treatment were also investigated.Results There were no significant differences in frequencies of genotypes and hyplotypes of both Fok-Ⅰ and Apa-Ⅰ between MG group and control group,and among subgroups of MG.The Fok- Ⅰ showed no statistical difference between the patients with better and less improvement after short-term treatment of glucocorticoid.The frequency of Apa-Ⅰ alleles in the patients with better improvement (55/186,29.6% ) significantly differed from the less improved group ( 7/48,14.6%,OR = 2.46,95% CI 1.04-10.43,x2 = 4.400,P = 0.036).The patients with the genotype A A/Aa were more likely to improve better after the treatment(48/93,51.6%) than in the worse group(7/24,29.2%,OR =2.59,95% CI 0.98-14.60,x2 =3.858,P= 0.049).Conclusions Alleles and genotypes of VDR-Fok- Ⅰ and Apa-Ⅰ were not found to be related with MG onset and severity.MG patients with VDR-Apa-A allele may have better improvement short-term treatment of glucocorticoid.
7.Comparative study of the inflammatory factor in patients with COPD in different traditional Chinese medicine syndrome

Objective:The changes of serum inflammatory factors in patients with chronic obstructive pulmonary disease (COPD) with different traditional Chinese medicine (TCM) syndrome types were compared, and the characteristics and significance of inflammatory factors in COPD were discussed from the perspective of traditional Chinese and western medicine.Methods:A total of 100 patients with COPD who met the inclusion criteria and were admitted to Dongzhimen Hospital of Beijing University of Chinese Medicine from September 2021 to September 2022 were selected and divided into phlegm turbation obstructing lung group ( n=50) and lung and kidney qi deficiency group ( n=50) according to TCM syndrome types. Twenty healthy subjects in the same period were selected as control group. Serum levels of monocyte chemoattractant protein-1 (MCP-1), macrophage inflammatory protein-1α (MIP-1α), interleukin-6 (IL-6), erythrocyte sedimentation rate (ESR), C-reactive protein (CRP) and other inflammatory factors were compared in each group. Results:The MCP-1, IL-6, ESR, CRP, white blood cell count (WBC) and procalcitonin (PCT) of COPD patients in phlegm turbation obstructing lung group were significantly higher than those in lung and kidney qi deficiency group (all P<0.05). The WBC, MCP-1, MIP-1α, IL-6, ESR and CRP of COPD patients in the lung and kidney qi deficiency group were significantly higher than those in the control group (all P<0.05). In the phlegm turbation obstructing lung group, the MIP-1α, MCP-1, IL-6, ESR, CRP, WBC, and PCT were significantly higher than those in the control group (all P<0.05). Conclusions:Patients with COPD have inflammatory reactions, and the inflammatory reactions of patients with phlegm turbation obstructing lung syndrome are more obvious than those with lung and kidney qi deficiency syndrome. The inflammatory factors MCP-1, MIP-1α, IL-6, ESR, CRP, WBC, PCT and other indicators could be used to judge the degree of COPD inflammation, which had certain clinical guiding significance for different syndrome types of COPD patients.
8.Eukaryotic expression of Leptospira interrogans lipL32/1-ompL1/1 fusion gene encoding genus-specific protein antigens and the immunoreactivity of expression products.

OBJECTIVETo construct the eukaryotic expression system of L.interrogans lipL32/1-ompL1/1 fusion gene and to identify the immunoreactivity of expression products.
METHODSPCR with linking primer was used to construct the fusion gene lipL32/1-ompL1/1. The P.pastoris eukaryotic expression system of the fusion gene, pPIC9K-lipL32/1-ompL1/1-P. pastorisGS115, was constructed after the fusion gene was cloned and sequenced. Colony with phenotype His(+)Mut(+) was isolated by using MD and MM plates and His(+) Mut(+) transformant with high resistance to G418 was screened out by using YPD plate. Using lysate of His(+) Mut(+) colony with high copies of the target gene digested with yeast lyase as the template and 5'AOX1 and 3'AOX1 as the primers, the target fusion gene in chromosome DNA of the constructed P. pastoris engineering strain was detected by PCR. Methanol in BMMY medium was used to induce the target recombinant protein rLipL32/1-rOmpL1/1 expression. rLipL32/1-rOmpL1/1 in the medium supernatant was extracted by using ammonium sulfate precipitation and Ni-NTA affinity chromatography. Output and immunoreactivity of rLipL32/1-rOmpL1/1 were measured by SDS-PAGE and Western blot methods, respectively.
RESULTSAmplification fragments of the obtained fusion gene lipL32/1-ompL1/1 was 1794 bp in size. The homogeneity of nucleotide and putative amino acid sequences of the fusion gene were as high as 99.94 % and 100 %, respectively, compared with the sequences of original lipL32/1 and ompL1/1 genotypes. The constructed eukaryotic expression system was able to secrete rLipL32/1-rOmpL1/1 with an output of 10 % of the total proteins in the supernatant, which located the expected position after SDS-PAGE. The rabbit anti-rLipL32/1 and anti-rOmpL1/1 sera could combine the expressed rLipL32/1-rOmpL1/1.
CONCLUSIONAn eukaryotic expression system with high efficiency in P.pastoris of L.interrogans lipL32/1-ompL1/1 fusion gene was successfully constructed in this study. The expressed fusion protein shows specific immunoreactivity, which can be used as a potential antigen for developing a novel vaccine of L.interrogans.

9.Effect of transcranial direct current combined with mirror neuronal rehabilitation training

Objective To observe the effect of transcranial direct current stimulation (tDCS) with mirror neuronal rehabilitation training system (MNST-V1.0) in post-traumatic unconscious patients after severe craniocerebral injury. Methods A prospective, self controlled and open-label method was used. Thirty-six post-traumatic unconscious patients with severe craniocerebral injury from January 2016 to July 2017 were selected. Four cases of the patients did not complete the treatment and the last 32 cases completed the study. All patients were given routine wake-up therapy, and tDCS combined with MNST-V1.0 (20 min/time, 1 time/d, 6 times/week, a total of 8 weeks) was given at the same time. The Glasgow coma scale (GCS), JFK coma recovery scale and Four coma rating scale before treatment and 2, 4, 8 weeks after treatment were recorded. Results The scores of open reaction, language and motor response score of GCS 2, 4, 8 weeks after treatment were significantly higher than those before treatment:(1.56 ± 0.82), (2.06 ± 1.01) and (3.11 ± 1.45) scores vs. (1.00 ± 0.45) scores, (2.23 ± 1.06), (2.56 ± 1.08) and (3.02 ± 1.04) scores vs. (1.00 ± 0.61) scores, (2.79 ± 1.12), (3.22 ± 1.33) and (4.44 ± 1.07) scores vs. (1.00 ± 0.54) scores, and there were statistical differences (P < 0.01 or <0.05). The scores of hearing, vision, movement, speech response, communication and arousal of JFK coma recovery scale 2, 4, 8 weeks after treatment were significantly higher than those before treatment, and there were statistical differences (P<0.01). The scores of open reaction, sport reaction, brainstem response of Four coma rating scale 2, 4, 8 weeks after treatment were significantly higher than those before treatment, and there were statistical differences (P<0.05); there was no statistical difference in respiratory score of Four coma rating scale before and after treatment (P>0.05). Conclusions The tDCS combined with MNST-V1.0 can improve the consciousness level in post-traumatic unconscious patients with severe craniocerebral injury, and have the effect of promoting awakening.
10.A combination of lenalidomide, bortezomib, and dexamethasone (RVD) provides deep-er responses in patients with newly diagnosed multiple myeloma

Objective: This study investigated the efficacy and safety of a combination of lenalidomide, bortezomib, and dexametha-sone (RVD) in patients with newly diagnosed multiple myeloma (NDMM). Methods: The clinical features and responses of 48 patients with NDMM who were treated with RVD from January 2015 to May 2019 in Beijing Chaoyang Hospital were retrospectively analyzed. Results: The median age of the 48 patients was 59 years (range: 34-79). Among these, 44 patients were Durie-Salmon stageⅢ, 15 were ISS stageⅡ, 19 were ISS stageⅢ, and 12 had plasmacytoma; 32.5% of all patients were cytogenetic high-risk. All patients re-ceived a median of four cycles (range: 1-9) of the RVD regimen as induction treatment. The overall response rate was 97.9%, with 35.4% of patients achieving complete response (CR) or better. The rate of very good partial remission (VGPR) or better was increased from 64.1% (after two cycles) to 84.6% (after four cycles). The mean collection of CD34+cells was 4.2 (± 2.6)×106/kg. Negative minimal residual disease (MRD), as indicated by next-generation flow (NGF), was achieved in 20.6% of patients after induction. Two patients with positive MRD after induction became MRD negative after transplantation. Two patients developed grade 3 or 4 hematologic toxic-ity. No nonhematologic toxicity of grade 3 or 4 was observed. Conclusions: In patients with NDMM, RVD treatment resulted in signifi-cantly improved response rates and exhibited an acceptable risk-benefit profile, with no adverse impact on stem cell collection. RVD combined with transplantation significantly improved the negative rate of MRD, as indicated by NGF.
