1.Application study of hsCRP detection in different types of child infections

Objective To investigate the expression level of hyper sensitivity C-reactive protein (hsCRP) in children infected with different pathogens in order to enhance the application value of hsCRP .Methods One hundred and one children patients with infection from March 2010 to March 2015 were selected as the research subjects and divided into the bacterial infection group (67 ca-ses) and mycoplasma infection group (34 cases) according to the hematology and bacterial culture results .Contemporaneous 50 healthy children were selected as the control group .Venous blood was collected for detecting hsCRP ,white blood cell(WBC) and neutrophil percentage(NEUT% ) .Results The levels and abnormal rates of hsCRP and WBC in the bacterial infection group were significantly higher than the those in the mycoplasma infection group and control group (P<0 .05);NEUT% and abnormal rate in the mycoplasma infection group was higher than that in the other two groups (P<0 .05) .The levels and abnormal rates of hsCRP and WBC after treatment in the bacterial infection group were lower than those before treatment ,the difference was statistically sig-nificant(P<0 .05);the area under the ROC curve of hsCRP for diagnosing bacterial infection wa maximal ,which was 0 .68 ,the sensitivity and specificity were 89 .9% and 88 .3% respectively .Conclusion The hsCRP level detection has early diagnostic value for child bacterial infection and can dynamically reflects the curative effect .
2.Plateau Refractory Wounds Treated with Platelet Lysate Gel: A Case Report

Plateau refractory wounds are affected by adverse conditions such as hypoxia, strong ultraviolet rays, and frigid, which can cause serious cell function damage, hinder re-epithelialization, and delay or even stop the healing of wounds. Conventional debridement and drainage, functional dressings and other traditional treatments have poor efficacy on this type of wound. Not only do patients have to endure long hospital stays, but they may also suffer from other complications due to long-term non-healing of wounds. In this paper, we report a patient with a plateau refractory wound that had not healed for 2 months. After two applications of platelet lysate gel treatment, the wound healed completely. This treatment method provides a new treatment approach for patients with plateau refractory wounds, which is worthy of clinical reference.
3.Mathematical Cognitive Ability Predicted by Amplitude of Low Frequency Fluctuate in Patients with Brain Dam-age

Objective To find biological markers to predict the mathematical cognitive ability in order to set patients free from the pain and time-consuming behavioral tests. Methods 86 patients with stroke or brain traumatic injuries were recruited and acquired T1 and rest-ing-state functional MRI imaging data. And a mathematical task (7 calculation items, 2 counting items) and a word-reading task (140 items) was also finished. The partial correlative analysis was made between the score of mathematical task and the amplitude of low frequency fluc-tuation of each voxel of the whole brain with the word-reading performance as controlling task, and AlphaSim correction method was used with corrected P<0.05 (single voxel level:P<0.05;cluster size:>110 voxels). Results There were 5 cerebral regions whose amplitude of low frequency fluctuation significantly correlated with mathematical performance:left inferior parietal lobule (161 voxels, rpeak=0.34), left precu-neus/superior parietal lobule (141 voxels, rpeak=0.31), left middle temporal gyrus (359 voxels, rpeak=0.34), left middle frontal gyrus (491 vox-els, rpeak=0.36), and right middle frontal gyrus (156 voxels, rpeak=0.32). Conclusion The amplitude of low frequency fluctuation of left inferior parietal lobule, precuneus/superior parietal lobule, middle temporal gyrus, middle frontal gyrus, and right middle frontal gyrus could be used as predictors of mathematical cognitive ability for brain-damaged patients.
4.The dynamic expression of CD57 on T cell and clinical significance analysis in acute HIV infection

ObjectiveTo study the dynamic express of CD57 on T cell of PBMC and clinical significance in acute HIV infection.MethodsSeventeen patients with acute HIV infection were enrolled study randomly diagnosed from 2006.11 to 2009.12 and 15 healthy donors as control group.The PBMCs from 1th,3th and 6th during acute infection were collected.The proportion of CD3+CD57+T lymphocytes,CD3+ CD8+CD57 +T lymphocytes and CD3 + CD4 + CD57 + T lymphocytes were evaluated by flow cytometric analysis with three or double color staining.The relationship between the proportion of CD57+ T phenotypes and virus load and CD4+T cells count was analyzed.ResultsThe proportion of CD57+T lymphocytes in PMBC in 1th,3th and 6th during acute HIV acute was 15.24% ±1.49%,13.51% ±2.45% and 14.65% ±1.83%,respectively,and was higher than normal control group and the difference was significantly(P<0.0001 ).The proportion of CD8+ CD57+T lymphocytes was 7.79% ±2.10% and 9.88% ±2.36% in 1th and 3th month during acute infection,respectively.The proportion of CD8+ CD57+T lymphocytes in 1th and 3th month during acute infection were positive relationship with virus load in corresponding time,and R2 was 0.3700 and 0.3768,and P value was 0.0096 and 0.0088,respectively.The proportion of CD8+CD57+T lymphocytes in the 1th and 3th month during acute infection was negative relationship with CD4+T lymphocytes count.The R2 was 0.3768 and 0.4235,and P value was 0.0215 and 0.0017,respectively.In 6 rapid progressors and 11 no rapid progressors on the 1th month after HIV infection,CD8+ CD57+T lymphocytes percentage was 11.20%±2.21% and 6.16% ±1.09%,respectively,and CD4+CD57+T lymphocytes percentage 2.79% ±0.31%and 1.40% ±0.30%,respectively.Both CD8+CD57+T and CD4+CD57+T in rapid progressors were higher than no rapid progressors,and P value was 0.0338 and 0.0106,respectively.ConclusionCD57+ T lymphocytes percentage in peripheral blood increase in acute HIV infection patients,in which the increasing CD8+CD57+T lymphocyte may mirror the dynamic of HIV replication and CD4+T cell count.The CD57 high express on T lymphocyte on the early HIV acute infection predicts rapid progression.
5.Comparative study of arterial spin labeling and dynamic susceptibility contrast of solitary brain metastasis at 3.0T MR

Objective To evaluate the perfusion findings of solitary brain metastasis using MR arterial spin labeling (ASL)imaging,and make a comparison with dynamic susceptibility contrast (DSC)imaging.Methods Twenty-three patients with pathologically proven solitary brain metastasis from lung cancer underwent ASL and DSC imaging.The ASL cerebral blood flow (CBF),DSC regional CBF (rCBF)and DSC regional cerebral blood volume (rCBV)were measured and compared between the tumor and the contralateral brain parenchyma.Pearson correlation test was performed to assess the relations between ASL and DSC measurements.Results Brain metastasis showed ring-like hyper-perfusion on ASL and DSC mappings.ASL CBF values of the tumor and the contralateral brain parenchyma were(54.872±21.131)mL·min-1·100 g-1and (21.109±4.788)mL·min-1·100 g-1,respectively (P<0.01).DSC rCBF values of the tumor and the contralateral brain parenchyma were (1.094 8±0.309 3)mL·min-1·100 g-1and (0.495 7±0.154 1)mL· min-1·100 g-1,respectively (P<0.01).DSC rCBV values of the tumor and the contralateral brain parenchyma were (1.753 1±0.545 5)mL/100 g and (0.729 9±0.215 8)mL/100 g,respectively (P<0.01).There was a strong correlation between ASL CBF and DSC rCBF (r=0.898,P<0.01),and there was no correlation between ASL CBF and DSC rCBV.Conclusion ASL can evaluate the CBF of brain metastasis as DSC dose. It requires no contrast agent administration and can be used in daily clinical practice.
6.Effects of two different core temperature monitoring methods on cardiopulmonary bypass time and coagulation function in open heart surgery

Objective To explore the effect of two different core temperature monitoring methods on cardiopulmonary bypass and coagulation function in open heart surgery to provide a reference for monitoring the core body temperature in open heart surgery. Methods One hundred and forty patients undergoing open heart surgery in this hospital from June to December 2016 were divided into the control group(n= 70) and observation group (n= 70). The control group monitored the temperature of nasopharynx and bladder. The observation group monitored the temperature of rectum and nasopharynx. The temperature falling time of cardiopul monary bypass, time of blocking ascending aorta,time of rewarming, total time of cardiopulmonary bypass were recorded during operation. The coagulation function was monitored on 1 d before surgery and at the end of surgery,including thrombolytic time (TT), prothrombin time (PT) and activated partial thromboplastin time (APTT). Results The temperature falling time,rewarming time, total time of cardiopulmonary bypass in the control group were more than those in the observation group, the difference was statistically significant(P<0.01);there was no statistically significant difference in coagulation function indicators(TT,PT,APTT) before operation between the two groups (P> 0.05). At the end of the operation, the coagulation function indicators (TT, PT, APTT) had statistically significant difference between the two groups (P<0.01). The coagulation function indicators in the observation group were better than those in the control group. Conclusion Using the rectal temperature for monitoring the core tempera ture in the patients undergoing open heart surgery under cardiopulmonary bypass is better than using bladder temperature, which can shorten the time of cardiopulmonary bypass and improves coagulation function.
7.Clinical study on central venous catheter drainage and intrapleural injection of urokinase in the treatment of tuberculous pleurisy

Objective To observe the therapeutic effect of central venous catheter drainage and intrapleural injection of urokinase on tuberculous pleurisy patients.Methods 60 hospitalized patients with tuberculous pleurisy were selected,and they were divided into two groupsby simple random grouping method.Both two groups received 3HRZE/6HR anti-tuberculosis treatment.30 patients in the observation group were treated with central venous catheter drainage and intrapleural injection of urokinase.30 patients in the control group were treated with conventional pleurocentesis.The duration of pleural effussion drainage,incidence of pleural thickening,hospitalization time and expense,and the adverse reaction rate were observed during treatment.Results In the observation group,the curative effect at 1 week was 46.7%,the duration of pleural effussion drainage was (20.5 ± 6.7)days,the incidence rate of pleural thickening was 26.7%,the hospitalization time was (9.4 ± 2.7) days,the hospitalization expense was (6 675.4 ± 1 818.4) RMB,the incidence rate of adverse reaction was 3.3%.In the control group,the curative effect at 1 week was 20.0%,the duration of pleural effussion drainage was (25.1 ± 7.7) days,the incidence rate of pleural thickening was 46.7%,the hospitalization time was (10.3 ± 2.8)days,the hospitalization expense was (7 508.9 ± 1 692.1) RMB,the incidence rate of adverse reaction was 20..0%.There were statistically significant differences between the two groups in the curative effect at 1 week (x2 =4.800,P =0.028),duration of pleural effussion drainage (t =2.484,P =0.016),incidence of pleural thickening (t =4.444,P =0.035) and incidence rate of adverse reaction (x2 =4.043,P =0.044).No statistically significant differences were observed between the two groups in hospitalization time(t =1.270,P =0.209) and expense (t =1.838,P =0.071).Conclusion In comparison to conventional pleurocentesis,the treatment of central venous catheter drainage and intrapleural injection of urokinase for tuberculous pleurisy is markedly efective,it is safe and Worthy of popularizing in clinical application.
8.Research progress in the pathogenesis of extramedullary disease in multiple myeloma

Multiple myeloma (MM) lesions are mostly localized in the marrow. Extramedullary disease in multiple myeloma (MM-EMD) is defined as malignant plasma cell infiltration away from the bone marrow or adjacent soft tissue, may occur at the initial diagnosis or during the consultation. MM-EMD may be found at initial diagnosis or during the treatment. MM-EMD has high invasiveness and poor prognosis, with clinical behavior distinct from marrow-restricted myeloma. However, its pathogenesis has not been elucidated. In general, the obstructed homing of myeloma cells, enhanced invasiveness, the degradation of extracellular matrix, and increased angiogenesis capacity may be involved in the occurrence of MM-EMD. Tumor genetic abnormalities and changes in the bone marrow microenvironment play important roles in the above pathogenesis.
9.Relationship between perceived organizational support and occupational pressure of general practitioners: the mediating role of professional identity

Objective:To investigate the current situation of general practitioners' occupational pressure in Heilongjiang Province, and explore its relationship with professional identity and perceived organizational support. It aims to provide references for the construction of general practitioners and the development of general medical education.Methods:The general situation questionnaire, perceived organizational support scale, occupational pressure scale, and professional identity scale were applied through network and site surveys for collecting data. A total of 288 questionnaires were collected with 263 valid questionnaires (91.3%). Pearson correction analysis and bootstrap analysis were performed for data analysis.Results:The average score of occupational pressure among general practitioners was (115.95±22.40), and the results of Pearson correction analysis showed that perceived organizational support was negatively correlated with occupational pressure ( r=-0.413, P<0.01) and positive correlation with professional identity ( r=0.587, P<0.01). There were also significant negative correlations between professional identity and occupational pressure ( r=-0.442, P<0.01). Moreover, the mediating effect showed that professional identity had a certain mediating effect on the relationship between perceived organizational support and occupational pressure (LLCI=-0.2039, ULCI=-0.0760, P<0.05). Conclusion:General practitioners' occupational pressure was at a high level, which can be reduced through improving the perceived organizational support and professional identity.
10.Impact of achievement goals on the academic achievement of medical students

Objective:To investigate the impact of achievement goals on the academic achievement of medical students.Methods:In May to June of 2019, an online questionnaire survey was performed for data collection, and the stratified sampling method was used for sampling. Demographic factors, Achievement Goal Scale, and Academic Achievement Scale were used in this study, and descriptive statistics, the Pearson correlation analysis, and the multivariate linear hierarchical regression analysis were performed to analyze the impact of achievement goals on the academic achievement of medical students.Results:For these medical students, the mean value of achievement-near to the goal orientation was (3.45±0.77), the mean value of achievement-dodge the goal orientation was (3.43±0.80), and the mean value of mastery goal orientation was (3.85±0.66). Achievement-near to the goal orientation had a significant positive impact on academic achievement ( β=0.336, P<0.01), and mastery goal orientation also had a significant positive impact on academic achievement ( β=0.540, P<0.01). Conclusion:The achievement goals dominated by mastery goal orientation is the most beneficial dimension for the development of medical students.
