1.A comparative study between hand-assisted laparoscopic splenectomy and open splenectomy in combination with pericardial devascularization to treat portal hypertension

ObjectiveTo investigate the changes in liver function and the efficacy of either hand-assisted laparoscopic surgery (HALS) or open splenectomy (OS) in combination with pericardial devascularization in the treatment of portal hypertension. MethodsThe clinical data of 94 patients who received splenectomy combined with pericardial devascularization to treat portal hypertension due to cirrhosis from Jan 2002 to May 2008 were analyzed retrospectively. 56 patients received OS and 38patients HALS. The operating time, intraoperative blood loss, postoperative complications, liver dysfunction and mortality were analyzed according to the Child's grading. ResultsThere was no difference in the operating time between HALS and OS (P>0. 05). The intraoperative blood loss and postoperative complications were 5.6% and 10.8%, respectively (P<0. 05). There was no significant difference in the serum ALT between HALS and OS, but there was a significant difference in the ALB (P<0. 05). The AST also had a significant difference on postoperative day 5 (P<0. 05). The serum ALT and AST were elevated after HALS, but there was a significant difference only for AST (P<0.05). The serum ALT and AST in OS were significantly higher after than before operation (P<0. 05). The serum ALB in OS was significantly lower after operation (P<0.05), but it was significantly lower only on postoperative days 1 and 3 (P<0.05) in HALS. ConclusionsCompared with OS, HALS combined with pericardial devascularization caused less damage to the intestinal tract and the liver function. It is a feasible and safe operation and it had fewer postoperative complications.
2.The effect of mental abacus calculation training on form recognition of children: an ERP study

ObjectiveTo explore the influence of mental abacus calculation (MAC) training on children's form recognition.Methods 28 children ( 14 children with MAC training and 14 children with non-MAC traning)were investigated by using event-related potential(ERP) technology.The event-related potentials were recorded when subjects were in the different form shape recognition.Results ( 1 ) The amplitudes of the posterior P1 evoked by the children with MAC training ( (9.59 ± 3.58) μV) were significantly greater than that of the children without MAC training ( (7.06 ± 2.84) μV) ( P < 0.05 ).(2) The amplitudes of right temporal-posterior N170 of children with MAC( ( -9.83 ±2.97) μV) were markedly less than that of children with non-MAC( ( - 12.45 ±2.51 ) μV) ( P < 0.05 ).( 3 ) The amplitudes of prefrontal P3 of children with MAC ( (7.65 ± 2.25 ) μV) were remarkably greater than that of children with non-MAC ( (4.89 ± 3.35 ) μV) ( P < 0.05 ).ConclusionThe brain function of form recognition in children is influenced by mental abacus calculation training.
3.T Endothelial progenitor cells (EPCs) and promote angiogenesis factor levels in peripheral blood in patients with obstructive sleep apnea

Objective To explore the repair possibilities of endothelial progenitor cells (EPCs)in peripheral blood in patients with different extents of obstructive sleep apnea (OSA) through measuring the levels of pro-angiogenic factors and different subgroups EPCs in peripheral blood in patients with OSA. Methods Ninety adult patients with OSA, 30 healthy controls with matched age and gender were enrolled for this study. The subjects performed Polysomnography, were divided in-to four group based on Apnea Hypopnea Index (AHI). The serum levels of HIF-1α, SDF-1αand VEGF were assessed by ELISA. Mononuclear cells were isolated from peripheral blood with density gradient centrifugation, and flow cytometry was used to detect levels of CD133+KDR+EPC, CD133+CD34+EPC, CD34+KDR+EPC and ALDHloCD34+KDR+EPC based on AL-DH activity, and CD133, CD34, PE-KDR related cell surface markers. Results The levels of CD133+KDR+EPC, CD133+CD34+EPC, CD34+KDR+EPC were higher in OSA groups than those of control group, both of which were higher in severe OSA group than those of in mild and moderate OSA groups. The levels of ALDHloCD34+KDR+EPC were higher in mild and moderate OSA groups than that of the control groups, and the levels of ALDHloCD34+KDR+EPC were significantly lower in se-vere OSA group than those of control, mild and moderate OSA groups. Serum levels of HIF-1α. VEGF were significantly high-er in OSA groups compared to those in control groups, both of which were higher in severe OSA group than those of mild and moderate OSA groups. Serum levels of SDF-1αwere significantly lower in severe OSA groups than those of mild, moderate OSA and control groups (P<0.05). Conclusion The mobilization and recruitment of different subtypes of EPCs are obvious-ly increased in patients with OSA, but ALDHloCD34+KDR+EPC with vascular repair capacity keeps to invariability, even de-creases in patients with severe OSA, which results in endothelial damage, and increases the risk of cardiovascular disease.

Objective To investigate the effects of extracellular ATP on the recovery of motor function after spinal cord injury in rats.Methods Twenty healthy adult rats were randomly divided into two groups after contusion injury was performed by the weight drop method at T10 with Allen impactor.Group A(ATP group) and group B(control group) each had ten rats.At days 1,3,7,14 and 28 after injury,the recovery of motor function after spinal cord injury in rats was assessed with modified Tarlov scores and slanting board test.Results Modified Tarlov scores and slanting board test were better in group A than in group B after spinal cord injury in rats.Significant difference was found in modified Tarlov scores and slanting board test between the two groups at days 14 and 28 after injury(P

6.The impact of Chitosan Lecithin on the mild cognitive impairment patients' semantic understanding by event-related potential N400

Objective To study the impact of Chitosan Lecithin on the mild cognitive impairment(MCI) patients' semantic understanding by event-related potential N400.Methods 32 patients with MCI were screened from 500 elder people aged Court in Weifang by mini-mental state examination (MMSE) scale,and were divided into two groups:the observation group ( n=16) and the control group ( n=16) by the table of random numbers.The observation group were given Chitosan Lecithin while the control group were given an equal dose of placebo.The intervention time was 2 months.The subjects were asked to conduct semantic judgement by semantic violation experiment.Thirty-two channels electroencephalogram(EEG) was recorded by Neuroacan Nuamps Systerm and analyzed data.Results 1 ) Specific component N400 was found in both groups,which was distributed at the frontal,central and parietal regions.2 ) The observation group reaction time and correct rate were respectively (965.13 ± 178.07 ) ms and ( 92.56 ± 2.36 ) % ,while the control group were respectively ( 1126.13 ± 252.77 ) ms and (85.28 ± 5.73 )%,with significant difference(P < 0.05 ).3 ) Compared with N400 in control group,N400 latency was shorter ( (425.28 ±47.26) ms vs (456.19 ± 37.75 ) ms,F=6.01,P<0.05) and amplitude was higher ( (4.79 ±2.18)μV vs (3.59 ± 1.33) pV,F=5.96,P<0.05).Conclusion Chitosan lecithin has the effect of N400 latency and amplitude,it may be helpful for the patients of MCI in semantic understanding.
7.Establishment of first national standard of Cystatin C

Objective To establish the first national standard of Cystatin C.Methods The candidate standard was prepared from human recombinant Cystatin C,diluted and dispensed aseptically.Homogeneity and stability study were carried out on the automated biochemical analyzer.The target value was assigned by six laboratories using widely recognized immumoassay under strict conditions,and traceable to ERM-DA471.Commutabilities of the candidate standard and its saline dilutions were evaluated according to EP14-Evaluation of Matrix Effects.Excel 2007 was used to analyze the results.Results The preparation had good immunological activity and was proved to be homogeneous and stable (6 months sealed at 2-8 ℃,30 days sealed at room temperature and 37 ℃,30 days open-vial at 2-8 ℃).The assigned value of the preparation was (4.47 ± 0.25) mg/L (coverage factor k =2).The candidate standard and/or its saline dilutions have commutability on the 10 evaluated systems.Conclusion The preparation meets the requirements of national secondary standard and could be used in quality control and evaluation of Cystatin C assays in China.
8. A case of Aspergillus salwaensis-induced spinal infection

Abstract: To report a case of Aspergillus salwaensis-induced spinal infection and its laboratory detection. The inflammatory granulation and necrotic tissue samples of a patient with spinal infection were collected from, the Affiliated Hospital of Chengde Medical College on June 17, 2020 for direct smear microscopy and culture, and the isolated strain was identified by microscopy by smear staining, matrix-assisted laser desorption ionization-time-of-flight mass spectrometry (MALDI-TOF-MS), molecular identification and in vitro antifungal susceptibility test. The patient was 62 years old female and presented with recurrent chest and back pain with no obvious cause. The initial diagnosis was spinal infection, after 7 days of treatment with levofloxacin, the effect was not good. Surgery was then performed remove the lesion via posterior thoracic debridement, and fungal hypha was observed under microscope in tissue specimens. The isolated strains had no typical structure, MALDI-TOF-MS was used for identification for many times, but there was no identification result. After 7 days of fluconazole treatment, the patient's condition improved, and her chest and back pain were alleviated compared to before surgery. The patient was discharged and followed up in the outpatient department, the fungus was later identified as Aspergillus salwaensis by sequence analysis of the internal transcribed spacer (ITS) gene sequencing, and the patient's antifungal medication was changed to voriconazole after with the attending physician. The patient consciously recovered well with no pain in the operative area and normal spinal activity at 1 year follow-up. The possibility of spinal fungal infection should be considered in patients with back pain without a clear cause and poor response to routine antibiotic treatment. Direct smear report of microscopic results are very important for guiding clinical antibiotic selection for rare filament fungi with atypical colony and microscopic morphology and unsuccessful MALDI-TOF-MS identification, molecular biological methods such as ITS sequence analysis can be helpful for early identification of the fungal species, improving identification speed.
9.Microsurgical treatment of intraspinal extramedullary benign tumors

Objective To explore the clinical efficacy of using the disposable whole laminectomy, laminoplasty and microsurgical tumor removal to treat intraspinal extramedullary benign tumors. Methods Sixty-four patients, 71 cases adult intraspinal extramedullary benign tumors were removed by the disposable whole laminectomy, laminoplasty and microsurgical tumor removal operation, all postoperative patients were followed up to observe the clinical efficacy. Results Patients with small tumors and large tumors all had a faster recovery of neurological function than preoperative, recent and long-term efficacy were all better than preoperative, and long-term efficacy was superior to short-term efficacy; Patients with giant tumors had a slow-er recovery of neurological function than preoperative, short-term efficacy was not better than preoperative, but the long-term efficacy was superior to preoperative. Conclusion The disposable whole laminectomy, laminoplasty and microsurgical tumor removal is an excellent method of treating intraspinal extramedullary be-nign tumors.
10.Mathematical Cognitive Ability Predicted by Amplitude of Low Frequency Fluctuate in Patients with Brain Dam-age

Objective To find biological markers to predict the mathematical cognitive ability in order to set patients free from the pain and time-consuming behavioral tests. Methods 86 patients with stroke or brain traumatic injuries were recruited and acquired T1 and rest-ing-state functional MRI imaging data. And a mathematical task (7 calculation items, 2 counting items) and a word-reading task (140 items) was also finished. The partial correlative analysis was made between the score of mathematical task and the amplitude of low frequency fluc-tuation of each voxel of the whole brain with the word-reading performance as controlling task, and AlphaSim correction method was used with corrected P<0.05 (single voxel level:P<0.05;cluster size:>110 voxels). Results There were 5 cerebral regions whose amplitude of low frequency fluctuation significantly correlated with mathematical performance:left inferior parietal lobule (161 voxels, rpeak=0.34), left precu-neus/superior parietal lobule (141 voxels, rpeak=0.31), left middle temporal gyrus (359 voxels, rpeak=0.34), left middle frontal gyrus (491 vox-els, rpeak=0.36), and right middle frontal gyrus (156 voxels, rpeak=0.32). Conclusion The amplitude of low frequency fluctuation of left inferior parietal lobule, precuneus/superior parietal lobule, middle temporal gyrus, middle frontal gyrus, and right middle frontal gyrus could be used as predictors of mathematical cognitive ability for brain-damaged patients.
