1.Epidemiological investigation of constitutional types of Chinese medicine in general population:Base on 21,948 epidemiological investigation data of nine provinces in China

Objects:To describe the distribution regularity of constitutional types of Chinese medicine in general population and analysis the characteristics of different social demography population through epidemiological investigation of constitutional types of Chinese medicine in general population.Methods:Using 21,948 cases large sample data of Chinese medical constitution cross-section survey from general population of nine provinces and municipalities(Jiangsu,Anhui,Gansu,Qinghai,Fujian,Beijing,Jilin,Jiangxi and He’nan),from December 2005 to January 2007.8,448 samples in accord with sample data from 1% nationwide population survey in 2005,which could representative the general population of China were randomly selected.Using the standardized Constitution in Chinese Medicine Questionnaire(CCMQ) to evaluation the scores of all kinds of Constitutional types(Gentleness type,Qi-defi ciency type,Yang-defi ciency type,Yin-defi ciency type,Phlegm-wetness type,Wet-heat type,Blood-stasis type,Qi-depression type,Special diathesis type).Application Discriminant Analysis to distinguish the individual Constitutional types,and analysis the distribution regularity of constitutional types of Chinese medicine in general population.Variables grouped by social demographic,use ?2 test to compare the differences in each sub-group’s constitution composition.Results:In the general population in China,Gentleness type accounted for 32.14 percent,eight pathological constitutions accounted for 67.86 percent,and the former three types of eight pathological constitutions were Qi-defi ciency type,Wet-heat type and Yang-deficiency type,which accounted for 13.42 percent,9.08 percent and 9.04 percent respectively.The composition ratio of constitutional types in different geographical,gender,age,marital status,occupation and education level were different,and the difference was signifi cant(P
2.The relationship between the body mass index and health-related quality of life of 4696 young students aged 18~25 years old

Objective To explore the relationship between the body mass index and health-related quality of life(HRQoL) of young students aged 18~25 years old. Methods 4696 students aged 18~25 years old from nine provinces and municipalities (Jiangsu, Anhui, Gansu, Qinghai, Fujian, Boijing, Jilin, Jiangxi and He'nan) were investigated. The HRQoL of students with different body mass index were compared with rank sum test and lo-gistic regression analysis. Results Obesity and underweight students scored lower in both physical component and mental component scores of SF-36. Compared to students with normal weight, odds ratios (ORs) of impaired HRQoL among obesity students significantly increased in both physical domain (OR=1.86) and mental domain (OR=1.90), while OR of impaired HRQoL among underweight students significantly increased only in physical domain(OR=1.21). Analysis for boys and girls scparately,OR of impaired HRQoL increased in physical domain among underweight boys(OR=1.37), and increased in mental domain among overweight girls(OR=3.85). Con-clnsion Different body mass index have different impact on young students' HRQoL. Both obesity and trader-weight could reduce HRQoL. But obesity have more impact on girls' mental domain than boys, and underweight have more impact on boys' physical domain than girls.

4.A comparative analysis of health-related quality of life in elderly hypertensive patients with different body mass index

Object To investigate the features of health related quality of life ( HRQOL) among elderly hypertensive patients with different body mass index (BMI) so s to provide evidence for the health promotion of hypertensive. Methods A cross-sectional survey of health status data from a population of 1281 elderly hypertensive patients from Beijing and 8 provinces of China ( Jiangsu, Anhui, Gansu, Qinghai, Fujian, Jilin, Jiangxi, and Henan) was made in the study. Analysis of variance was applied to compare the differences in HRQOL of elderly hypertensive patients in different BMI categories. Results For the elderly hypertensive patients with low body mass index, the scores of 8 dimensions and scores in physical component summary (PCS) and mental component summary( MCS) of SF-36 were the lowest;except the role-emotional(RE) dimension,the scores of other 7 dimensions,PCS and MCS were significantly different(PF,RP,BP,GH,VT,MH, P<0.01; SF,PCS,MCS, P<0.01). Comparing the scores between any two groups in PCS, the scores of patients with low body mass index were significantly lower than normal weight group (62.30 ± 22. 12 vs72.40 ±20.86, P<0.05) .overweight group (62.30 ± 22.12 vs 74.97 ± 19.20, P<0.05 ) and obese group (62.30 ± 22.12 vs 74.57 ± 19. 22, P < 0.05 ). Comparing the scores between any two groups in MCS,the scores of patients with low body mass index were significantly lower than normal weight group (68. 81 ± 20. 48 vs 74. 70 ± 20.41, P < 0. 05), overweight group (68. 81 ± 20. 48 vs 78.08 ±18.23, P<0.05) and obese group (68.81 ±20.48 vs 78. 69 ± 17.77, P<0.05); the score of normal weight group was significantly lower than the overweight group (74. 70 ±20. 41vs 78. 08 ±18.23, P<0.05). Gender Stratification showed that females with low body mass index had worse HRQOL than males. Conclusion The HRQOL of elderly hypertensive patients with low body mass index is significantly lower than normal weight patients, overweight patients and obese patients, the health promotion of elderly hypertensive patients with low body mass index should be paid more attentions, especially females with low body mass index.
5.Development and Validation of the Japanese Version of the Constitution in Chinese Medicine Questionnaire (CCMQ)

Objectives : The objective of this study is to develop a Japanese version of the Constitution in Chinese Medicine Questionnaire (CCMQ) in Chinese, which is comprised of 60 items with 9 sub-scales, and evaluate its reliability and validity. Methods : We conducted a survey of 130 participants in the Toyama area of Japan from Dec. 2005, to Feb. 2006. A test-retest method was used. Feasibility was evaluated by the response times to the questionnaire, and the response rates of the CCMQ items. Internal consistency within the sub-scales was assessed by Cronbach's α coefficient. Reproducibility was confirmed between the first and second occasions using weighted kappa and Spearman correlation. Lastly, criterion validity was evaluated by correlation between CCMQ and SF-36 sub-scales. Results : Response time was 8 minutes on average and its rate was nearly 100%. Internal consistency was achieved for each of the 9 sub-scales with a 0.65 to 0.79 α coefficient. Reproducibility ranged from 0.41 to 0.81 for the items, and from 0.79 to 0.88 for the sub-scales. Regarding the criterion validity, the “Gentleness type” sub-scale was positively correlated with SF-36 (0.46, P<0.001), while other 8 pathological constitutional types of the CCMQ were negatively correlated with SF-36 (-0.35 to -0.50, P<0.001) as expected. Conclusions : We developed a Japanese version of the CCMQ and found acceptable levels of reliability and validity using a survey of 130 subjects in Japan. This suggests that the CCMQ could be a useful tool in comparing the constitution profiles between Chinese and Japanese.

6.Health-related quality of life and its affecting factors in the elderly individuals: data from 9 provinces of China

Objective To investigate the health-related quality of life (HRQOL), demographic characteristics, and health behaviors of the Chinese elderly to find out high-risk population and behaviors. Methods Data was collected from a cross-sectional survey performed in Jiangsu, Anhui, Gansu,Qinghai, Fujian, Beijing, Jilin, Jiangxi, and Henan province. MOS SF-36 was used for HRQOL assessment, t test was used for HRQOL comparison between the elderly and the general population. Multiple stepwise linear regression analysis was used to evaluate the affecting factors. Results The HRQOL among the Chinese elderly were PF ( Physical Functioning) 79 ± 21, RP ( Role-Physical ) 68 ± 40, BP ( Bodily Pain) 72 ± 23, GH ( General Health) 57±22, VT ( Vitality ) 69 ± 20, SF ( Social Functioning) 79 ± 23, RE ( Role-Emotional ) 72 ±40, M H ( Mental Health)76 ± 18, which were lower than Sichuan Norm on 7 dimensions ( P < 0. 05 ; except on M H dimension) and lower than Hangzhou Norm on 5 dimensions (P < 0. 05 ;except on GH, VT, and MHdimensions). The important affecting factors included physical exercise, education level, medical history of chronic disease, age, race, marriage, body mass index ( BMI ), sleeping habits, and gender. Conclusions Compared with the general population, the HRQOL in the elderly might be lower. The health policy and community heahhcare services should focus on the elderly individuals with insufficient exercise, lower education level, chronic disease history, and ethnic minority, or widows. Health education should encourage them to improve physical exercise and sleeping behavior.
7.Items selection in development of quality of life instrument for patients with esophageal cancer

Objective To develop the specific module of quality of life instrument for patients with esophageal cancer. Methods with the structured group ( nominal group and focus group) methods and the qualitative and quantitative theory and methodology in developing rating scales, items were preliminary screened, evaluated and modified. And the data measuring from 28 cases of esophageal cancer patients and 25 doctors/nurses were analyzed by 4 statistical procedures: method of coefficient of variation, correlation analysis, patients'importance rating procedure and doctors' importance rating procedure. Results By above statistical procedures and advises of doctors, thus 19 - items specific module was formed finally. Conclusions The specific module developed on the strict procedures has good content representativeness and validity.
8.Effect of finasteride on reducing hemorrhage in holmium lasterenucleation of prostate

Objective To evaluate the effect of preoperative oral finasteride on patients with benign prostate hyperplasia (BPH) undergoing holmium lasterenucleation of prostate (HoLEP).Methods A total of 156 BPH patients from Department of Urology in Shanghai Ninth People's Hospital from Sep.2010 to Mar.2012 was analyzed retrospectively.79 patients receiving oral 5 mg/d finasteride before operation were selected as medication group,and the 77 patients without taking finasteride were selected as control group.The perioperative data,including operation time,amount of washing fluid during operation,preoperative and postoperative changes of hemoglobin level and postoperative bladder washing time were compared between two groups.Results Compared with control group,the changes of hemoglobin level after HoLEP,amount of washing fluid during operation,and postoperative bladder washing time with normal saline were significantly decreased in experimental group[(1.08±0.27) g/L vs.(1.55±0.32) g/L,(27.51±3.67) L vs.(36.89±6.47) L,(24.85±4.17) h vs.(35.87±5.10) h,all P<0.05].Conclusions Oral finasteride before HoLEP can reduce perioperative bleeding and the volume of bladder irrigation with normal saline.

10.Stratified analysis of the relationship between traditional Chinese medicine constitutional types and health status in the general population based on data of 8 448 cases.

Objective: To examine the relationship between traditional Chinese medicine (TCM) constitutional types and health status among groups of different age or gender in the general population of China. Methods: Data of 8 448 cases were randomly sampled from a database of 21 948 cases of a cross-sectional survey on the TCM constitutional types and health status which was carried out in 9 provinces or municipalities of China (Jiangsu, Anhui, Gansu, Qinghai, Fujian, Beijing, Jilin, Jiangxi and Henan) according to gender and age structure of the Chinese population in 2005. Scores of health-related quality of life scale--the Medical Outcomes Study 36-Item Short-Form Health Survey (MOS SF-36)--were analyzed by Nemenyi test to compare the health status of individuals with different constitutional types. Results: Compared with the gentleness type, the MOS SF-36 scores of the 8 types of pathological constitution were significantly low (P<0.05) among groups of different age or gender. The MOS SF-36 score was the lowest in men of the qi-deficiency, qi-depression and blood-stasis types, while it was the lowest in women of the phlegm-dampness, qi-depression and qi-deficiency types. For the age group of 15 to 34, the special diathesis, qi-depression and blood-stasis types had the lowest MOS SF-36 scores; for the age group of 35 to 59, the qi-deficiency, qi-depression and blood-stasis types had the lowest MOS SF-36 scores; for the age group of over 60, the qi-deficiency, qi-depression and phlegm-dampness types had the lowest MOS SF-36 scores. Conclusion: In groups of different gender or age, the MOS SF-36 scores of the 8 types of pathological constitution were significantly lower than that of the gentleness type, indicating a deficient health status. The health status of different types of constitution showed different characteristics in groups of different gender or age.
