1.Effect of Indomethacin suppositories on albumin concentration and insulin resistance of post-operation patients with serious intra-abdominal infection

Objective:To evaluate the effect of Indomethacin suppositories on albumin concentration and insulin resistance of post-operation patients with serious intra-abdominal infection.Methods:38 post-operation patients with serious intra-abdominal infection were divided into two groups randomly.One as treatment group(n=19)and the other as control group(n=19).Albumin,blood glucose,fasting insulin concentration were measured At 72 hours after the operation and 144 hours.Insulin resistance index was calculated using the homeostasis model assessment(HOMA).Finally,the results of two groups were analyzed.Results:The albumin concentration of Indomethacin group at 72 hours was significantly higher than that after 72 hours,Insulin resistance of 144 hours was lower than the control group(P0.05).Conclusion:Postoperative abdominal infection using indomethacin suppository can significantly reduce insulin resistance,and significantly reduced the decomposition of albumin.
2.Thoracic spinal cord injury without radiographic abnormality

Objective 7 cases of thoracic spinal cord injury without radiographic abnormality (SCIWORA) were analyzed to elevate the level of diagnosis and treatment.Methods The type of spinal cord injury were classified into complete injury in 2 and incomplete injury in 5 according to Frankel. 5 cases had MRI examination. Conservative therapy was adopted in 5, and operation was programmed in 2.Results 4 cases recovered completely , 2 cases recovered partly , and 1 case had no recovery.Conclusion MRI provides reliable foundation for diagnosis and treatment to thoracic SCIWORA. The result is satisfactory in those incomplete injury.
3.Perioperative psychological nursing of living-related liver transplantation

Objective To study the effect of perioperative psychological nursing on living-related liver transplantation. Methods The following measures were adopted according to the donors' concrete condition: using nursing procedure in holistic nursing, to assess and determine the main problems in perioperative period, nurses mastered psychological nursing knowledge and skills in individualized psychological nursing, giving pertinent and individualized health education to patients in accordance with their psychological problems during the perioperative period. Results The donors could face the operation with good psychological state after being given fully-integrated and whole-system psychological nursing. Conclusions Psychological nursing is important for living-related liver transplantation during the perioperative period.
4.The effect of biomechanis with different bone density by the pedicle screws fixation

Objective To explore the biomechanis with different bone density by the pedicle screws fixation before and after fatigue test.Thus provide biomechanical basis for the clinical work.Methods A total of 27 fresh lumbar vertebrae samples of adult sheep (L1-L5) were equally divided into three groups,such as group A of decalcified with HCL 0 h,group B of decalcified with HCL 2 h and group C of decalcified with HCL 4 h,by using the random number table method,every groups was 9 models.Four screws was performed on the L4-L5 pedicle of vertebral arch of the three groups.Three groups were performed fatigue test of 250 000 times in 4 direction (flexion,extension,left lateral bending,right lateral bending) by (300+ 105) N load.The range of motion(ROM),the axial compressive stiffness were measured and the results in every group were compared before and after fatigue test.Results After fatigue test,ROM and the axial compressive stiffness had no significant change in group A (P>0.05),but ROM of B group and C group were both increased and the axial compressive stiffness were decreased,Significance was found with and between groups.Conclusion The stability of people who are performed pedicle screws simplly may be decrease when the spine of osteopenia.
5.Comparison of the efficacy and safety of clinical trials of pemetrexed or gemcitabine combined with carboplatin in treatment of the elderly patients with advanced NSCLC

Objective To explore the efficacy and the safety of clinical trials of pemetrexed combined with carboplatin and gemcitabine with carboplatin in treating advanced NSCLC of the elderly patients.Methods A total of one hundred and twenty-eight elderly patients with advanced NSCLC were randomly divided into two groups(n=64).The PC group in which patients were treated with pemetrexed combined with carboplatin,and the GC group in which patients were treated with gemcitabine combined with carboplatin.The effects and the safety were assessed by the following indexes, treatment efficiency,side effects,LCSS. Results The treatment efficiency of the PC group and the GC group were 34.38% and 31.25% after chemotherapy.The difference was no statistically significant between two groups.The difference of LCSS was no statistically significant between two groups.Except hair loss,the incidences of nausea and vomiting,leukopenia,thrombocytopenia and neurotoxicity (grade III-IV )in the PC group (6.25%,3.13%,4.67%,7.81%)were significant lower than those in the GC group(17.18%,20.31%,15.63%,18.75%)(P<0.05).The 1-year and 2-year survival rates of the PC and GC groups were 46.2% VS 46.8% and 13.3% VS 12.5%,respectively,with a median survival of 12.1 VS 1 1.3 months,without a statistically significant difference between two groups.Conclusion PC and GC show similar efficacy for elderly NSCLC patients,however,the toxicities in PC patients are lower than those in GC patients.Thus,pemetrexed combined with carboplatin is an effective therapeutic regimen for advanced NSCLC in elderly patients.
6.Effects of preemptive analgesia with parecoxib sodium on postoperative analgesia and delirium after nerve injury-free surgery for fracture of thoracic and lumbar vertebrae

Objective To investigate the effects of preemptive analgesia with parecoxib sodium on postoperative analgesia and delirium after nerve injury-free surgery for fracture of thoracic and lumbar vertebrae and to promote the postoperative rehabilitation of the patients. Method 80 patients meeting the criteria were selected. and randomly divided into observation group and control group.40 patients each group. The observation group used parecoxib sodium for preemptive analgesia. while the control group used sufentanil. and the analgesia effects and the incidences of delirium were observed. Results The differences in operative time and intra-operative blood loss between the patients of the two groups were statistically insignificant. In 2 h. 6 h. 12 h.24 h and 48 h after the surgery.the VAS score and the accumulative time of intravenous self-controlled analgesia pump being pressed of the observation group were significantly lower than those of the control group. and the differences were statistically significant (P<0.05). The first time for the patients of the observation group to press the intravenous self-controlled analgesia pump is (3.84±0.62) h after the surgery, is significantly later than that of the control group (1.05±0.47)h.and the difference is statistically significant (P<0.05). The incidence of delirium in 7 days after the surgery in the patients of the observation group was 10.00%. and is significantly lower than that of the control group (25.00%) (P<0.05). Conclusion Using parecoxib sodium for preemptive analgesia before nerve injury-free surgery for fracture of thoracic and lumbar vertebrae can elevate the postoperative analgesia effects of the patients.decrease the incidence of postoperative delirium, and is highly safe and consequently worthy of clinical application.
7.Comparison of two treatment reconstruction modalities for hilar bile duct stricture

Objective To explore an effective treatment for inflammatory hilar bile duct stricture. Methods The clinical data with patients of intra hepatic cholelithiasis complicated with hilar bile duct stricture that received plastic repair of hilar bile duct stricture(PHBDS) with pedicled cholecystic graft or Roux en Y cholangio jejunostomy (RYCJ) in our hospital during recent 10 years were retrospectively analyzed. The peri operative conditions, postoperative morbidity of cholangitis and recurrence of hepaticolithiasis were observed . Results The peri operative morbidity of biliary fistula and inflammatory ileus were not significantly different between the two groups. The postoperative morbidity rate of cholangitis was 5.66% and 21.88%,(P=0.010) and recurrence rate of hepaticolithiasis was 3.77% and 16.67%,(P=0.021) in PHBDS group and RYCJ group respectively . Conclusions PHBDS is an easy and safe operation. RHBDS can preserve the physiologic function of Oddi′s spincter, so the reflux of intestinal contents to bile duct is avoided , and the results are better than the RVCJ.
8.Observation on the therapeutic effects of ACCF and ACDF on cervical spondylotic myelopathy and evaluation on the postoperative complications

Objective To investigate the clinical therapeutic effects of different surgical treatments on cervical spondylotic my-elopathy and the occurrence of postoperative complications .Methods 65 patients with cervical spondylotic myelopathy admitted in our department between January 2010 and January 2013 were retrospectively analyzed ;and among them ,31 underwent anterior cer-vical discectomy and fusion(ACDF) and 34 underwent anterior cervical corpectomy and fusion (ACCF);the general surgical condi-tions ,JOA scores and functional grades of Nurick ,incidences of dysphagia and so on in the patients of the two groups were investi-gated .Results The operative time and intra-operative blood loss of the patients of the ACDF group were better than those of the ACCF group[(88 .70 ± 9 .03)min ,(125 .46 ± 12 .62)min ,(94 .26 ± 10 .34)mL ,(133 .98 ± 12 .09)mL] ,and the differences were sta-tistically significant(P<0 .05) .The postoperative lengths of stay of the two groups were similar ,and the differences in preoperative and postoperative JOA scores and functional grade of Nurick of the two groups were statistically insignificant (P>0 .05);but the differences between the preoperative and postoperative JOA scores and functional grade of Nurick in 6 months after the surgeries and those before the surgeries of the same groups were statistically significant (P<0 .05) .At the same observation time points ,the incidences of dysphagia and the thicknesses of soft tissue before the cervical vertebra in the operated segment of the observation group were all lower than those of the control group ,and the differences were statistically significant (P<0 .05);in the patients of either group ,no loosening ,translocation or non-fusion of the bone graft was observed .Conclusion The two surgeries in treatment of cervical spondylotic myelopathy van both obtain satisfying clinical therapeutic effects ,but ACDF has a lower incidence of postop-erative complications ,and doctors should choose a suitable surgery based on the actual conditions .
9.Observation study of clinical efficacy of Bevacizumab combined with Chemotherapy for advanced and metastatic Cancer

Objective To study the therapeutic efficacy and toxicity of Bevacizumab combined with Chemotherapy in patients with advanced and metastatic cancer.Methods Fifty-nine patients of advanced metastatic cancer (Forty-two patients of refractory metastatic colorectal cancer and 17 advanced adenocarcinoma) were treated with normalized chemotherapy combined with Bevacizumab.Patients with refractory metastatic colorectal cancer were treated with Bevacizumab combined with FOLFIRI or FOLFOX4,and patients with advanced adenocarcinoma of NSCLC were treated with Bevacizumab in combination with docetaxel and cisplatin for 4-6 cycles,Bevacizumab was used until progressive disease (PD).During treatment,adverse effect were assessed with Common Toxicity Criteria V3.0 developed by National Cancer Institute.Results Among the 42 patients of metastatic colorectal cancer,27 were treated with First-line 1 with complete remission (CR),13 partial response(PR),4 stable disease(SD) and 9 PD,the effective rate (ER) was 51.9% ( 5/15 ),disease control rate(DCR) was 66.7% (18/27);Fifteen patients were treated with second-line PR,of which 5 PRD,4 SD,6 PD.ER was 33.3% (5/15),and DCR was 60.0% (9/15).The effective rate were slightly higher in the First-line treatment group than that in the second-line treatment group,however,the difference was not statistically significant (x2=1.335,P=0.248 ).Among the 17 patients of NSCLC,of which 1 CR,7 PR,5 SD,4 PD.RR was 47.1% (8/17),and DCR was 76.5% (13/17).The major toxicities were one grade 3 hemoptysis observed in one patient,grade 3 thrombosis was observed in one patient.Other common adverse effects,were epistaxis,hemoptysis,hypertension and proteinuria,which were not severe and could be well tolerated.Conclusion Bevacizumab Combined with Chemotherapy was effective in short term for the patients with advanced and metastatic cancerMost patients could tolerate the side effects.Further studies should be done to prove the long-term effects.
10.Effect of Berberine on endoplasmic reticulum stress and insulin resistance in skeletal muscle of metabolic syndrome rats and its mechanism

Objective To investigate the effect of Berberine on insulin resistance and its mechanism in Otsuka Long-Evans Tokushima Fatty(OLETF) rats with metabolic syndrome(MS).Methods LETO(Long-evans Tokushima Otsuka)rats(the control group receiving standard normal diet,n=10)and diabetic OLETF rats(the MS group receiving high-fat diet for 24 weeks,n=30).Rats in the MS group were randomly divided into 3 subgroups(n=10,each subgroup).Each subgroup was gavaged with normal saline,high-dose Berberine(100 mg · kg-1 · d-1)and low-dose Berberine (50 mg · kg-1 · d-1) respectively,and the high-fat diet remained unchanged.After 6 weeks of berberine treatment,body weight,blood glucose and lipid metabolism parameters were determined.The oral glucose tolerance test(OGTT) and insulin tolerance test(ITT) were used to detect insulin resistance.Expression levels of the protein and mRNA of 78 kDa glucose-regulated protein (GRP7 8),Caspase-12 and CCAAT/enhancer-binding protein(C/EBP) homologous protein(CHOP) in skeletal muscles were detected by Western blot and RT-PCR.Results After Berberine treatment,the body weight,fasting plasma glucose,fasting insulin[(28.9 ± 2.0) mU/L,(31.5± 2.4) mU/L vs.(36.9 ± 4.7) mU/L],total cholesterol,triglycerides,and low-density lipoprotein cholesterol were decreased,while the high-density lipoprotein cholesterol(HDL-C) levels were increased in MS rats with high-dose berberine and low-dose berberine as compared with the control group (P < 0.05) respectively.Berberine treatment could reduce the protein and mRNA expression levels of GRP78,Capase-12 and CHOP in the skeletal muscle of MS rats(P<0.05).Conclusions Berberine may alleviate insulin resistance in rats with metabolic syndrome by reducing endoplasmic reticulum stress in skeletal muscle.
