1.Research of the influential factors of ultrasonic cleaning of curettage- aspiration device for uterine cavity

Objective To evaluate the combination influence of the factors such as the ways to putting into the water, addition with multienzyme and temperature of water on the cleaning effect of ultrasonic cleaning of the curettage-aspiration device for uterine cavity. So that we can find out the best condition for cleaning, and provide the scientific evidence for the formulation of the norm of ultrasonic cleaning of the curettage-aspiration device for uterine cavity. Methods According to the factorial design with triple pathogens, three factors were chosen to complete the ultrasonic cleaning of the curettage-aspiration device for uterine cavity, including the sequence of putting into water, addition with multienzyme or not and the temperature of cleaning. The cleaning effect was detected by adopting the eye-measurement or with the hemoglobin reagent. Results The sequence of putting into water first or next had little influence on ultrasonic cleaning;the effect of addition with multienzyme was more significant than without it;the effect of water temperature at 35 centi-degree or 40 centi-degree had no difference. Conclusions It had good benefits for the cleaning effect when the ultrasonic cleaning of the curettage-aspiration device for uterine cavity added multienzyme to the cleansing solution, while the sequence of putting into water first or next and the temperature of water at 35 centi-degree or 40centi-degree shows little influence on the cleaning effect.
2.Effects of environmental intervention on growth hormone and insulin-like growth factor-1 in very low birth weight infants in NICU

Objective To investigate the effects of environmental intervention on growth hormone (GH) and insulin-like growth factor 1(IGF-1) in very low birth weight infants in NICU.Methods 60 cases of very low birth weight infants were randomly divided into the experimental group and control group (30 cases in each group) in NICU from June 2012 to June 2013.By adjusting the light,reducing the noise,focusing the nursing time,the levels of growth hormone and insulin-like growth factor-1 were detected and the growth indexes were measured including body weight,length,and head circumference on the 7th day and 14th day.Results The indicators of growth and development in the experimental group on the 7th day and 14th day after birth were significantly better than those of the control group.The IGF-1 level in the experimental group on the 7th day was significantly higher than that of the control group.The GH and IGF-1 level in the experimental group was significantly higher than that of the control group.Conclusions Effective environmental intervention in NICU can improve the GH and IGF-1 lev-el of very low birth weight infants and improve their quality of life.
3.Determination of 9 Phenoxy Acid Herbicides in Soil by Solid Phase Extraction and LC/MS/MS

A method for the determination of 9 phenoxy acid herbicides in soil by solid phase extraction and LC/MS/MS with matrix correction was developed. The sample was treated by ultrasonic extraction twice with 1. 5% formic acid in acetonitrile. The extract was cleaned up with NH2 solid phase extraction column. A C18 column with formic acid in pure water/methanol mixture as the mobile phase was used under gradient elution. Mass spectral acquisition was done in negative ion mode by multiple reactions monitoring ( MRM ) . Two transitions per compound were used to provide qualitative and quantitative analysis. There were good linear relationships in the range of 2-250 μg/L for 9 phenoxy acid herbicides. The correlation coefficients were in the range of 0 . 9927-0 . 9972 . The method detection limits were in the range of 1 . 66-3 . 82 μg/kg . The average recoveries at three spiked levels ( 10 , 40 , 200 μg/kg ) ranged from 85 . 3% to 110 . 0% with the relative standard deviations (RSD) of 3. 2%-12%.

Objective To explore the effect of different states of the environment to the rehabilitation process and complications of preterm infants in the different environment in neonatal intensive care unit (NICU). Methods 120 premature infants were divided into observation group and control group with 60 cases in each group by random digital table method. The control group was treated with routine nursing model, while observation group was treated with bird nest nursing, lower noise and using cyclic lights. Oxygen time and hospitalization time, increasing about body weight, sleeping time on the twenty-eighth days and the incidence of complications between the two groups were compared. Results The hospitalization and oxygen time were (41.79±2.51)days and (11.45±5.22)days respectively in the observation group, those were shorter obviously than that in the control group [(45.32±3.85)days and(15.52±5.89)days, t=5.95, 4.01, P<0.01].The growth of body weight was (17.36±2.94) g/d, the sleeping time on the twenty-eighth day was (21.18 ±0.39)hours in the observation group. It was increased than that in the control group [(13.48±4.63)g/d and(19.56±0.45)hours], and the difference was significant(t=5.48, 21.07, P<0.01). The incidences of nosocomial infection and feeding intolerance in the observation group were reduced significantly, the difference was significant ( X2=7.06, 8.71, P<0.01). Conclusions Improving the environment in NICU can prevent the adverse factors on premature infants, promote the recovery and growth of premature infants.
5.Clnical observation, of the effects of lattice ultra pulse carbon dioxide laser combined with traditional Chinese medicine on the treatment of hyperplastic scar.

OBJECTIVETo explore the clinical effects of lattice ultra pulse carbon dioxide laser combined with traditional Chinese medicine ( Fuchunsan ) on the treatment of postburn hyperplastic scar.
METHODSSixty-three patients with hyperplastic scar after burn injury hospitalized from February 2012 to June 2014 in our department were treated with lattice ultra pulse carbon dioxide laser combined with traditional Chinese medicine (Fuchunsan). Patients were divided into early stage group (E, n = 35), middle stage group (M, n = 25), and late stage group ( L, n = 3) according to the formation time of scar, which was respectively 3 weeks to 3 months, longer than 3 months and less than or equal to 6 months, and 3 to 15 years in groups E, M, and L. The number of times of laser treatment of patients in each group was recorded. The degree of scar pain in patients of the three groups was assessed by the Numerical Rating Scale (NRS) before treatment and after treatment for 1, 2, and 3 times. The scar condition of patients in groups E and M was assessed by the Vancouver Scar Scale (VSS) before treatment and after treatment for 1, 3, and 5 times. Patients in group L did not receive VSS assessment but were evaluated by clinical observation only. Photos of scar in treating area were taken before treatment and after treatment for 3 and 5 times to evaluate the clinical effect. Data were processed with t test.
RESULTSPatients in groups E and M were treated with laser for (4.8 ± 1.1) and (7.7 ± 2.1) times respectively. In group L, the treatment was stopped in 2 patients after laser treatment for 5 times, and 1 patient received laser treatment for 12 times. The degree of pain in patients of groups E and M was alleviated significantly after treatment for one time, and the number of patients scoring 1-4 point(s) in NRS increased from 5 cases to 38 cases. After treatment for 2 and 3 times, the increase in the number of patients scoring 1-4 point (s) in NRS was on a small scale. Before treatment and after treatment for 1 time, VSS scores of patients in groups E and M were similar (with values respectively 0.641 and 0. 082, P values above 0. 05). After treatment for 3 and 5 times, VSS scores of patients in group E were respectively (9.2 ± 0.8) and (7.0 ± 1.1) points, which were significantly lower than those in group M [ (9.7 ± 1.0) and (8.2 ± 1.0) points, with values respectively -1.993 and -4.433 , P < 0.05 or P < 0.01]. After treatment for 3 times, the rate of improvement in appearance was respectively 88.6% (31/35) and 72.0% (18/25) in groups E and M, and it was respectively 100.0% (35/35) and 96.0% (24/25) after treatment for 5 times. No significant effect in appearance was found in the 3 patients in group L.
CONCLUSIONSEarly application of lattice ultra pulse carbon dioxide laser combined with traditional Chinese medicine (Fuchunsan) for the treatment of postburn hyperplastic scar is effective.

6.Effects of Dietary Supplementation of Certain Nutrients on Maze Performance and Biochemical Indices in Mice after Exposure to High +Gz

Objective To explore the possible effects of nutritional supplements on brain function as reflected by Water Maze test performance in mice after +Gz exposure.Method Mice were arranged into control group (group A),+Gz group without nutritional supplementation (group B) and +Gz plus nutritional supplementation group (group C).Each group contains 12 mice.Mice in group A were not exposed to +Gz while mice in both group B and group C were exposed to 8 min +10 Gz.Distilled water was gavaged to group B mice 3 h before +Gz exposure.On the day before +Gz exposure pyridoxol fortified water was given and 3 h before exposure mixed amino acids solution were gavaged to group C mice.Water Maze test was done and scores were recorded in all groups.After the Water Maze test was completed,blood was collected through the eyes for serum amino acid determinations and brain tissue was collected by decollation for monoamine determination and γ-glutamyl transferase (GGT) activity evaluation. Result After +Gz exposure,longer completion time and more mistakes were observed in Water Maze test in group B as compared with group A and a trend of improvement in group C was noticed. The ratio of brain 5-HT to dopamine(DA) was significantly reduced in group C as compared with group B.Gamma glutamyl transferase (GGT) activity in brain tissue in group C and group B increased significantly. Conclusion High sustained +Gz exposure significantly reduces Water Maze test performance in mice (longer completion time and more mistakes).It seems that there is a trend of improvement in Water Maze performance in mice in dietary nutritional supplementation group,which might be due to significant reduction in ratio of brain 5-HT to DA in mice with nutritional supplementation.
7.Differences of tacrolimus' dosage and concentration of individuals in morning periods after renal transplantation

Objective To study the influential factors of tacrolimus'dosage and concentration differences between individuals in morning periods after renal transplantation.Methods The clinical data consisted of 118 receptors in morning periods after renal transplantation,whose immune suppressions were tacrolimus,mycophenolate and hormone.At 3,7,14 and 30 d after operation,all the receptors'weight,dosage of tacrolimus,dosage of hormone,diarrhea,blood fat,liver function,renal function,albumn and erythrocrit were recorded respectively,and at the same time their concentrations of tacrolimus and genetic polymorphisms of CYP3A5,MDRl 3435,MDR1 2677 and MDRl 1236 weredetected.Multiple linear regressions were performed.Results The fitting degrees of stepwise regression equations were low.At 3,7,14 and 30 d after operation,the adjusted R2was 0.284,0.267,0.417 and 0.324,respectively.From the aspect of pharmacogenomics,the main factors rela-ted to the differences of tacrolimus'dosage and concentration included MDR1 2677,MDRl 1236 and MDR13435,which varied intensively.Age,albumn,renal function,blood fat and liver function were important factors too.Conclusions The main reasons of the differences of tacrolimus'dosage and concentration between individuals in morning periods after renal transplantation are medicines and changes of internal environment after operation.The genetic polymorphisms of MDR1,age,albumn,renal runetion.blood fat and liver function are important factors too.
8.The correlation between patient selection and heterotopic ossification after cervical artificial disc replacement

Objective To analysis the correlation between patient selection and heterotopic ossification (HO) after cervical artificial disc replacement.Methods Data of 48 patients with cervical spondylosis (34 cervical spondylotic myelopathy and 14 nerve-root type) who had undergone Bryan cervicadisc replacement from December 2003 to December 2008 were reviewed retrospectively,and all the patients had been followed up for more than 5 years.There were 21 males and 27 females with an average age of 42 years old (range,20-53 years).There were 38 single level replacement (C3-4 3 cases,C4-5 5 cases,C5-6 28 cases,C6-7 2 cases),9 double level replacement (C4-5,C5 6 4 cases;C5-6,C6-7 5 cases) and 1 three level replacement (C3-4,C4 5,C5-6) as a total of 59 surgical segments.The occurrence of HO was defined by McAfee classification on cervical lateral X-ray.Four factors were used in patient selection including gender,range of motion (ROM) of the target level,alignment of the functional spine unit (FSU) of the index level,and the disc height ratio between surgical level and the adjacent levels.The correlation between these four factors and HO was evaluated by logistic regression.The receiver operating characteristic (ROC) curve and area under the ROC curve were used to evaluate the significant result of logistic regression and the optimal diagnostic value.Results 48 patients were all followed up for an average period of 70.3 months (range,60-120 months).The occurrence rate in this study was 33.9% (20/59 segments).Only one factor,the disc height ratio of the target level and its adjacent levels,presented statistical correlation with HO.ROC analysis showed that the area under the ROC curve of disc height ratio was 0.813 and the optimal diagnostic threshold was 0.9.Conclusion The disc height ratio of the target level and adjacent levels was the only patient selective factor correlated with the occurrence of HO.For those with disc height loss exceeding 10% comparing to adjacent levels,it is not indicated for cervical artificial disc replacement with Bryan prosthesis.
9.Effect of Xylitol on the COX-2 Expression of Renal Tubule in Diabetic Rats

Objective:To observe the effect of xylitol on the cyclooxygenase(COX)-2 expression of renal tubule in diabetic rats.Methods:The Wistar rats were randomly divided into normal control group(group NC),diabetes control group(group DC),5% xylitol-treated group(group 5%),10% xylitol-treated group(group 10%)and 20% xylitol-treated group(group 20%).At the end of 8 weeks,the expression of COX-2 in kidney tissue,the level of serum uric acid,allantoin and creatinine were tested in rat groups.Results:The levels of serum uric acid and allantoin were higher in group 5% and group 10% compared with that of group DC.The differences in levels of serum uric acid and allantoin were statistical significance between group 10% and group DC(P < 0.05),whereas,the lower levels of serum uric acid and allantoin were found in group 20% compared with those of group DC(P > 0.05).The levels of urine uric acid and allantoin were lower in group 5% and group 10% than those of group DC(urine uric acid,P> 0.05 and allantoin,P< 0.05),whereas,group 20% had higher levels of urine uric acid and allantoin than those of group DC(P< 0.05).The fractional excretion of uric acid(FEUA)was lower in group 5% and group 10% compared with that of group DC(P < 0.05).The FEUA was higher in group 20% than that of group DC(P < 0.05).The expression of COX-2 was significantly increased in group 5% and group 10% compared with that of group DC(P < 0.05),but the expression of COX-2 decreased in group 20%(P < 0.05).Conclusion:The lower and mediate doses of dietary xylitol could aggravate the tubular injury through increasing the level of serum uric acid and the expression of COX-2 in renal tubule.The higher doses of xylitol could increase the excretion of uric acid and down-regulate the expression of COX-2 in renal tubule.
10.Effect of homeopathic bidirectional-traction reduction device and traction table in surgical treatment of femoral intertrochanteric fractures in the elderly

Objective To compare the outcomes of homeopathic bidirectional-traction reduction device and traction table in surgical treatment of femoral intertrochanteric fractures.Methods A retrospective case control study was made on 94 cases of femoral intertrochanteric fractures treated from July 2015 to December 2015.There were 26 males and 68 females,aged 60-75 years.According to the Evans classification,the fractures were type Ⅱ in 24 cases,type Ⅲ in 32 and type Ⅳ in 38.Fifty-seven cases sustained chronic diseases.According to the random number table,the subjects were assigned to receive homeopathic bidirectional-traction reduction (homeopathic reduction group,48 cases) and traction table reduction (control group,46 cases).All fractures were fixed with proximal femoral nail antirotation.Operation time,reduction time,tluoroscopy time,blood loss and rate of closed reduced cases were recorded.Fracture union and rotation of the affected femur to the tibia were detected after operation.Functional outcome was evaluated using the Harris score at the final follow-up.Results All fractures were reduced closely in homeopathic reduction group,while 15 fractures in control group were reduced via a small-incision anterior approach.Operation time,reduction time and fluoroscopy time in homeopathic reduction group were (62.9 ± 12.1) min,(6.8 ± 1.5) min and (11.3 ± 5.6) s respectively,significantly less than the corresponding data in control group (all P < 0.05).One month after operation,rotation of the affected femur to the tibia was (2.8 ± 1.2) ° in homeopathic reduction group,significantly less than that in control group [(11.5 ± 4.7) °] (P < 0.05).Bone union was observed in all cases.At the final follow-up,Harris hip score was (92.6 ± 7.6)points in homeopathic reduction group,significantly higher than that in control group [(87.3 ± 6.5) points] (P < 0.05).Conclusion For the elderly patients with intertrochanteric fractures,homeopathic bidirectional-traction device has advantages of high rate of closed reduction,shorter operation time,less radiological exposure and satisfactory function recovery of the affected hip joint.
