Objective To study the value of MSCT angiography(MSCTA)in diagnosing lower extremities arterial disease (LEAD) of diabetic foot ulcers and analyze the possible influential factors on the onset of diabetic foot ulcers. Methods 80 diabetic patients for MSCTA examination were divided into two groups by whether combined with foot ulcers. The tibial artery calcification score (TACS) and peripheral arterial occlusion index (PAOI) were calculated and the possible risk factors of diabetic foot ulcer were analyzed. Results There were 30 cases suffered with foot ulcer and 18 cases occurred lower extremity arterial occlusion (LEAO), the average PAOI was 4.40(3.60, 5.75).Non-foot ulcer cases were 50, and there were 8 cases suffered with LEAO, the average PAOI was 2.05(1.43, 3.10).Compared to patients without foot ulcer, patients with foot ulcer were older, had a lower BMI, and were more likely to have a history of tobacco use. They usually had higher TACS and severer peripheral arterial occlusive disease (PAOD).Stepwise regression model showed that foot ulcers had significant correlation with TACS and PAOI.Partial correlation analysis showed TACS was independent from PAOI in impacting diabetic foot ulcer. Conclusion MSCTA has important clinical value in diagnosing and valuing diabetic foot ulcer LEAD.Age, smoking history,BMI,TACS and PAOI affect the incidence of diabetic foot ulcers, and TACS and PAOI are the independent risk factors for diabetic foot ulcers.

Objective To explore CTA characteristics of diabetic lower extremities arterial disease (LEAD) with different Fontaine stage. Methods Data of 100 patients clinically diagnosed as diabetic LEAD and underwent lower limbs arterial CTA were analyzed retrospectively. The patients were classified into grade Ⅰ-Ⅳ according to Fontaine classifications.Lower extremities arteries were graded according to CTA.Stenosis degree of lower extremity arteries in different Fontaine stages were analyzed. Results The grades of lower extremity arteries stenosis in different Fontaine stages had statistically significant difference (x2 =186.24,P<0.001).There had statistically significant difference of stenosis degree in different lower extremity arterial levels among different Fontaine stages (superior genicular artery:x2 =69.24,P<0.001;inferior genicular artery:x2 =111.59,P<0.001;dorsalis pedis and arteriae plantaris:x2 =94.15,P<0.001).Grades of stenosis between superior genicular arteries and inferior genicular arteries, superior genicular arteries and dorsalis pedis and arteriae plantaris had statistically significant differences (Z=12.59,P<0.001;Z=10.47,P<0.001).There was statistically significant difference of stenosis grades between inferior genicular artery and dorsalis pedis and arteriae plantaris (Z=12.66,P<0.001).Occluded inferior genicular arteries usually associated with collateral vessels. Conclusion CTA can reflect the clinical stages of LEAD,which is the ideal method in diagnosing of diabetic LEAD.

Objective To investigate the clinical features, diagnosis and treatment of von Hippel-Lindau (VHL) disease in order to improve the understanding of this disease. Methods The clinical features, imaging features, pathological data and treatments of 7 patients in 3 families admitted to our hospital from December 2005 to August 2017 were retrospectively analyzed. Results Ten oprations were performed under the guidance of neural navigation system for the 7 patients. There were 4 enjoying total resection at once;one achieved primary surgical resection of the primary lesions, and residual lesions grew up and the patient accepted surgical treatment at the 7th and 9th years;one patient achieved primary surgical resection of the primary lesion, the residual lesions grew up and surgical treatment was performed 8 months later;another one underwent surgical resection of the primary lesion, and the residual lesion was treated with gamma knife therapy 3 months after surgery. The pathological results of the 10 operations of the 7 patients were proved to be hemangioblastoma. No operative deaths were noted. The symptoms of these patients when they were discharged from the hospital were significantly better than those when they were on admission. The 10 patients were followed up so far,no symptoms of neurological impairment were noted, and they could work and live a normal life. Conclusions Surgical treatment is still the preferred method of VHL,and radiotherapy is effective for small lesions.

Objective:To explore the clinical efficacy of removal of large and medium-sized meningiomas in the sellar region via neuroendoscope assisted supraorbital keyhole approach. Methods:The clinical data of 13 patients with large and medium-sized meningiomas in the sellar region accepted surgery via neuroendoscope assisted supraorbital keyhole approach in our hospital from March 2017 to March 2019 were analyzed retrospectively. The surgical efficacies of these patients were analyzed. Results:Total removal was achieved in all 13 patients: 2 were with Simpson I resection, and 11 with Simpson II resection. The vision was improved in all patients. After surgery, scalp hydrops appeared in 2 patients, transient diabetes insipidus in 2 patients, and recurrent hyponatremia and hypokalemia in one patient; these symptoms were relieved gradually for 2 weeks. No olfactory disturbance was noted in these 13 patients. Follow up for 3-18 months showed no recurrence, and Karnofsky performance status scores were≥80.Conclusion:Surgery via supraorbital keyhole approach is suitable for large and medium sized meningiomas in the sellar region, with advantages of beauty, minimal invasion, few postoperative complications and quick recovery.

OBJECTIVE To prepare curcumin /berberine co -loaded self -microemulsion drug delivery system (CUR/BER- SMEDDS)and evaluate its physicochemical properties and in vitro anti-tumor effects . METHODS Using CUR and BER as model drug, based on the screening of the type of oil phase ,emulsifier,co emulsifier and their mass ratio ,the formulation of CUR/BER - SMEDDS were optimized by central composite design -response surface methodology ,taking particle size and drug loading as evaluation parameters ,with the mass percentage of oil phase and the mass ratio of emulsifier to co emulsifier as factors ,and verification test was conducted . CUR/BER-SMEDDS prepared by optimized formulation were evaluated in terms of physicochemical properties,in vitro dissolution and in vitro anti-tumor effects . RESULTS The optimized formulation of CUR/BER -SMEDDS included that the oil phase was medium chain triglyceride (30.97%),the emulsifier was polyoxyl 40 hydrogenated castor oil (46.77%),the co emulsifier was polyethylene glycol 400(22.26%),and the mass ratio of emulsifier to co emulsifier was 2.10∶1. The validation experiments showed that mean particle size of CUR/BER -SMEDDS was(58.90±5.41)nm,the average drug loading was(94.94±3.87)mg/g,and the relative deviations from the predicted values were -2.90% and -0.14%,respectively. The CUR/ BER-SMEDDS prepared by optimal formulation was light yellow ,clear and transparent liquid after emulsified with water ,and its particles were spherical . The results of dissolution test in vitro showed that after SMEDDS was made ,the cumulative dissolution of CUR in artificial intestinal fluid and artificial gastric fluid and that of BER in artificial intestinal fluid were significantly higher than those of its raw material . Results of in vitro anti-tumor experiment showed that IC 50 values of CUR/BER -SMEDDS for PC -3 cells and DU -145 cells were (17.38±2.84)and(20.89±1.26)μmol/L,which were all lower than those of CUR and EBR significantly (P<0.05).CONCLUSIONS The optimized prepar ation process of CUR/BER -SMEDDS is stable and feasible . The drug release of CUR/BER-SMEDDS is more complete than that of single drug, and has stronger anti -prostate cancer effect in vitro .
