2.Dynamic changes and significance of peripheral blood Th17/Treg in chronic hepatitis B patients

Objective:To investigate the significance of T helper type 17 cells(Th17)/T regulatory cells (Treg) in the prognosis of chronic hepatitis B(CHB) by comparing the changes of Th17 cells ,Treg cells and their rate under different therapy in chronic hepatitis B patients.Methods: 40 patients infected with chronic hepatitis B(CHB) were divided into chronic hepatitis B without antiviral(CHBWA) group and chronic hepatitis B antiviral(CHBA) group depending on whether nucleoside antiviral was applied,with 20 cases each,10 cases in asymptomatic hepatitis b carriers(AsC) group.10 cases in health control (HC) group.Flow cytometer(FCM) was adopted to detect peripheral blood Th17 and Treg cell frequency in CHB patients at 0,1 month in CHBWA group ,at 0,1,3 month in CHBA group,AsC patients and HC cases ,then calculating Th17/Treg ratio and observing the dynamic changes at different disease states.Results: After the liver-protecting treatment in CHBWA patients,the Th17/Treg ratio at 1 month(0.39±0.11)decreased compared to 0 month(1.20±0.26)rapidly(P<0.01),and the ratio nearly reached the level of AsC and HC group (P>0.05).Ratio of Th17/Treg and ALT had good positive correlation (r=0.709,P=0).In CHBA patients group,after the liver-protecting and antiviral treatment,patients′ Th17/Treg ratio decreased rapidly in the first month(0.73±0.32) and third month(0.76±0.44),and there′s statistical difference compared with 0 month(1.18±0.27,P<0.01),but there was no significant change of Th17/Treg ratio after being treated for 3 months compared with 1 month,and the Th17/Treg ratios at 0,1,3 month were higher than that in AsC group and HC group(P<0.01).Ratio of Th17/Treg and ALT have good positive correlation (r=0.500,P=0.000),also the ratio of Th17/Treg and HBV DNA were positively correlated (r=0.345,P=0.007).Comparison of peripheral blood Th17,Treg cells frequency,Th17/Treg ratio between CHBWA and CHBA group at 0 month had no statistical significance(P>0.05),while the Th17/Treg ratio in CHBA group was higher than CHBWA group(t=4.471,P<0.01) after 1 month of treatment.Conclusion: In the CHB patients,antiviral treatment will influence the Th17,Tregs cells frequency,resulting the change of Th17/Treg ratio,but also the ratio may be associated with virus removal.

Objective To evaluate the accuracy of head-of-bed elevation in postoperative lung cancer patients, in order to identify the compliance of patients and nurses, and to decrease the postoperative complications and restore rehabilitation. Methods 300 postoperative lung cancer patients were divided into two groups (150 patients in experimental group and 150 patients in control group) by random digital table method. The head of the beds were elevated according to the marking sign on the intravenous infusion stand in experimental group, whereas in control group nurses elevating the bed to the height that patients felt comfortable. The compliance of patients and nurses, the indwelling time of thoracic drainage tube, postoperative complication, and the length of hospital stay were recorded and analyzed. Results The compliance of nurses and patients in the experimental group were 98.7% (148/150) and 95.3% (143/150), respectively, compared to 93.3% (140/150) and 87.3% (131/150) in the control group (X2=5.56, 6.06, P<0.05). Significant difference was found in indwelling time of thoracic drainage tube between the experimental group [(96.0±27.8) h] and the control group [(103.1±24.1) h] . The length of hospital stay in experimental group was (7.9±2.1) d, compared with (8.4±2.2) d in control group (t=-2.45,-1.99, P<0.05). There was significant difference in postoperative complications between experimental group [12.0% (18/150)] and control group [20.7% (31/150)]( X2=4.12,P<0.05). Conclusions Evaluating the head of the bed accurately by the marking sign on intravenous infusion stand stand can promote the postoperative care outcome of lung cancer patients, decrease the postoperative complications and restore rehabilitation.
5.Construction and functional characterization of alentiviralvector mediating RNA interference of DEK gene

Objective To construct the lentiviral vector of RNA interference(RNAi) for DEK,and to detect its effect on breast cancer cell growth.Methods The DEK siRNA was designed and constructed based on DEK sequence using a lentiviral vector.The lentivirul vector containing DEK siRNA was named PSIH-H1-DEK as confirmed by PCR and sequenceing.PSIH-H1-DEK was then packaged with accessory plasmids into lentivirus in 293T cells and selected for 2 weeks with puromycin ( puro ) before the mixed colonies stably expressing DEK siRNA were obtained and the DEK expression was detected by real time PCR( RT-PCR) and Western blotting.The effect of DEK siRNA on ZR75-1 cell growth was determined by cell counting kit.Results Western blot and RT-PCR showed that PSIH-H1-DEK siRNA could suppress DEK gene expression.Suppression of DEK could markedly inhibit the growth of ZR75-1 cells.Conclusion The lentivirus-mediated DEK siRNA is obtained,which will facilitate further research on DEK function in breast cancer development.
6.Risk screening of osteoporosis in community residents and its influencing factors

Objective To explore the influencing factors of high risk osteoporosis (OP) population. Methods Using multi-stage cluster sampling method, 1 district was selected from each of the 6 cities randomly, then 3 communities were selected from each district randomly. Questionnaire investigation and physical examination were applied to community residents aged 40-69 years old. The questionnaire mainly included basic information, OP risk assessment (using the 1 minute test method developed by the International OP Foundation), health status and lifestyle information (recent 12 months). The relationship between high-risk groups and influencing factors was analyzed by unconditional logistic regression. Results Totally 6577 valid questionnaires were collected, 2069 were male (31.46%) and 4508 were female (68.54%). A total of 166 cases (2.52%) were found as self-reported OP patients at the age of 40-69, self-reported rate of male (1.55%) was lower than that of female (2.97%) (χ2=11.719, P<0.01), the rate was higher among people aged 60-69 years than people aged 40-49 (χ2=44.766, P<0.01);3400 cases (52.28%) were found as high-risk individuals,there were more male at higher risk than female (OR=1.845). Among different age groups, the 50-59 and 60-69 years age groups had more high risk people than that of 40-49 years group (with OR=2.171 and OR=2.854 respectively). Comparative analysis was also conducted for the following factors: degree of education [college or above (OR=0.517) compared with primary school or junior middle school], occupation [compared with civil servants, technical management personnel (OR=2.289), business services (OR=2.224), farming/forestry/fishing (OR=2.258) and production/transportation staff (OR=2.552)], self-reported history of chronic disease (OR=1.596), fracture (OR=4.061), body pain (OR=2.286) and foot cramps (OR=1.923), as well as calcium/vitamin D intake (OR=1.357), increased sunlight (OR=1.256), daily walking steps>5000 (OR=1.336) and BMI>24.00 (OR=1.322), they were all related to a increased proportion of high risk population. Conclusion The proportion of OP high-risk groups is higher among people aged 40-69. Gender, age, cultural level, occupation, bad health status and lifestyle changes are closely related to higher OP risk. Community residents OP screening should be continued to further study the risk factors of OP.
7.Relationship of gene polymorphisms of angiotensin convertion enzyme, aldosterone synthase and α-adducin with subclinical renal lesion

Objective To investigate the relationship of gene polymorphisms of angiotensin eonvertion enzyme (ACE), aldosterone synthase (CYP11B2)and α-adducin with subclinical renal lesion. Methods I/D polymorphism of ACE gene, -344T/C polymorphism of CYP11B2 gene and 460G/T polymorphism of α-adduein gene were detected by polymerase chain reaction (PCR) and restrictive fragment length polymorphism(RFLP) in 604 normotensive subjects and 1081 primary hypertensive patients whose creatinine (Cr) were less than 2mg/L. The primary hypertensive and normotensive subjects were divided respectively into normal group (Ccr≥60ml/min) and subclinical renal lesion (Ccr<60 ml/min) group, according to creatinine clearance rate (Ccr) calculated by Cockcroft-Gault equation. Results ANOVA, contingency X2 and partition of chi-square were selected. The frequencies of different genotypes of ACE, CYP11B2, and α-adducin were in agreement with Hardy-Weinberg equilibrium in our study. Normal renal function group (A group, n=512) and subclinical renal lesion group (B group, n=92) in normotensive subjects, and normal renal function group (C group, n=828) and subclinical renal lesion group (D group, n=252) in hypertensive patients were compared. The patients in B and D groups were older than those in A and C groups (P<0.01). But there were no significant differences in the age between B and D groups, and between A and C groups. The frequency of ACE-DD genotype in D group was the highest (22.6%) among four groups and the frequency of α-adducin-TT genotype in A group was the lowest (13.3%) among four groups (all P<0.01). The differences of genotype frequencies of ACE and α-adducin genes among other three groups were not significant. No significant difference was found in frequencies of genotypes of CYP11B2 among four groups. Conclusions Subclinical renal lesion is increased with the aging. ACE-DD genotype is related with hypertension and subclinical renal lesion, while α-adducin-TT genotype is related with hypertension and subclinical renal lesion. Association between the genotypes of CYP11B2 and subclinical renal lesion is not found.
8.Effects of stretch on transient outward potassium and inward rectifier po-tassium current in cultured neonatal rat atrial myocytes

[ABSTRACT]AIM:Toinvestigatetheeffectsofmechanicalstretchontransientoutwardpotassiumcurrent(Ito), inward rectifier potassium current ( IK1 ) and action potential duration ( APD) of cultured neonatal rat atrial myocytes . METHODS:Neonatal rat atrial myocytes were isolated and cultured on silicone sheeting with or without stretch for 24 h. The silicone membrane area was increased by 12%in stretched group.The cells without stretch served as control .Ito, IK1 and APD were recorded by the technique of whole-cell patch clamp.RESULTS:Compared with control group, Ito density in stretched myocytes was significantly reduced [(1.6 ±0.4) pA/pF vs (12.1 ±2.9) pA/pF, P<0.01], whereas IK1 density was increased [(-10.8 ±0.8) pA/pF vs (-8.8 ±0.9) pA/pF, P<0.01].The APDs at 50%and 90%levels of repolarization ( APD50 and APD90 ) in the stretched cells were obviously decreased than those in non-stretched cells [(10.5 ±1.4) ms vs (15.5 ±2.4) ms, (30.0 ±2.8) ms vs (56.3 ±3.6) ms, P<0.01].CONCLUSION: Stretch stimulation leads to the reduction of Ito density, the increase in IK1 density and the shortness of APD in cultured rat atrial neonatal myocytes , which may contribute to atrial electrical remodeling induced by pressure overload .
9.Relationship between Thyroid Hormones and the Nature of Thyroid Nodules

Objective To detect serum levels of thyroid hormones in patients with thyroid nodules (TN), and investi-gate their relationship with the nature of TN. Methods A total of 245 patients with TN were recruited in the study. Accord-ing to levels of thyroid antibodies and postoperative pathological results, all patients were divided into nodular goiter (NG) group, thyroid adenoma (TA) group and thyroid cancer (TC) group. TC group was further classified as the TC with increased level of thyroid antibodies (TC-AB+group) and the TC with normal level of thyroid antibodies (TC-AB-group). The serum levels of free T3 (FT3), free T4 (FT4) and thyroid stimulating hormone (TSH) were detected for all patients before operation, and differences of thyroid hormones were analyzed between different groups. Results The serum level of TSH was signifi-cantly higher in TC group than that of NG group and TA group (P<0.05). The serum level of FT3 was significantly lower in TC-AB+group than that of NG group and TC-AB-group. The serum level of FT4 was significantly lower in TC-AB+group than that of NG group, and the serum TSH level was significantly higher than that of other groups (P<0.05). Conclusion The increased serum levels of TSH were found in some patients with TC, which may partly attribute to their coexistence with autoimmune thyroiditis and subsequent hypothyroidism. The increased serum TSH level may not be the inherent characteris-tics of TC.
10.Association between the change of daily living activities and symptoms of depression in Chinese middle-aged and elderly people

Objective To explore the association between the change of daily living activities and the suymtoms of depression in Chinese middle-age and elderly people.Methods People without symptoms on depression,emotional or spiritual diseases or memories related diseases in baseline survey (2011),those who were aged ≥45 years in the China Health and Retirement Longitudinal Survey (CHARLS) with two waves from 2011 to 2013 were invited to participate in this CHARLS study.Information related to demographic characteristics and health status of the subjects were collected through household-interview.Depression symptom was assessed with the Center for Epidemiological Studies of Depression (CES-D10) scale.Activities of daily living function was assessed by both Basic Activities of Daily Living (BADL) and Instrument Activities of Daily Living (IADL) scales.Symptoms of depression was compared and the differences in various BADL/IADL changing groups were analyzed with Log-rank test.Association between the changes of daily living activities and depression symptom was analyzed by Cox regression model.Results Results from the log-rank test showed that the differences of depression symptom hazard ratio were statistically significant in different BADL/IADL changing groups.Results from the Cox regression analysis showed that the BADL/IADL in the deteriorating groups was associated with the higher hazard ratio of depression symptom (P<0.01),with the HR value (95%CI) for BADL and IADL deteriorate groups as 1.45 (1.20-1.76) and 1.64 (1.36-1.98),respectively,when comparing to the ‘stablized'group.This phenomenon appeared an urban-rural difference.Statistical significances on IADL also happened in both urban and rural areas while BADL only occurred in rurals.Conclusion There appeared an association between the change of daily living activities and depression symptom in Chinese middle-age and elderly people.Difference between IADL and BADL was also noticed between urban and rural areas.
