1.The prognosis value of B-type natriuretic peptide testing in critically ill non-cardiogenic patients

Objective B-type natriuretic peptide(BNP) have been used widely in the diagnosis and prognosis of cardiogenic diseases, but the association between BNP concentrations and non-cardiogenic critical illness is unknown. Our study aims at investigating the role of natriuretic peptide testing for predicting 28-day mortality of non-cardiogenic critically ill patients in emergency department (ED). Methods A total of 255 non-cardiogenic critically ill patients treated in the emergency department were prospectively enrolled and a sample of blood was obtained for BNP measurement from each patient. A variety of clinical and laboratory variables were recorded. After 28 days, the vital status of each patient was ascertained and the association between BNP values at presentation and mortality was assessed. Results 71 patients died during the 28-day observation period. Median BNP concentrations at presentation among decedents were significantly higher than those of survivors (326. 0 ng/L versus 50. 9 ng/L,P <0. 001). The optimal BNP cut point for predicting 28-day mortality was 114. 0 ng/L. In a multivariate analysis (Cox-regression), a BNP concentration greater than 114. 0 ng/L(RR 7. 268, 95% CI 3. 864-13. 672) and an Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ) scores greater than 20 (RR 3. 330, 95% CI 1. 815-6. 109)were the independent predictors of the 28-day mortality. BNP concentration alone had an area under the receiver operating characteristic curve (AUC) of 0. 825 for predicting mortality. Conclusions BNP concentrations at presentation are strong predictor of 28-day mortality in patients with non-cardiogenic critically ill patients in ED, which is better than Rapid Acute Physiology Score (RAPS) , Rapid Emergency Medicine Score (REMS) and APACHE Ⅱ score.

3.Effect of Chinese drugs for activating blood circulation and removing blood stasis on carotid atherosclerosis and ischemic cerebrovascular events.

OBJECTIVETo explore the effect of Chinese drugs for activating blood circulation and removing blood stasis (CDABCRBS) on carotid atherosclerotic plaque and long-term ischemic cerebrovascular events.
METHODSBy using open and control method, effect of 4 groups of platelet antagonists, platelet antagonists + CDABCRBS, platelet antagonists +atorvastatin, platelet antagonists +atorvastatin +CDABCRBS on carotid atherosclerotic plaque and long-term ischemic cerebrovascular events of 90 cerebral infarction patients were analyzed.
RESULTSThrough survival analysis, there was no statistical difference in the effect of the 4 interventions on the variation of carotid stenosis rates or ischemic cerebrovascular events (P > 0.05). The occurrence of ischemic cerebrovascular events could be postponed by about 4 months in those treated with platelet antagonists + CDABCRBS and platelet antagonists + atorvastatin +CDABCRBS. By multivariate Logistic analysis, age, hypertension, and clopidogrel were associated with stenosis of extracranial carotid arteries (P <0.05). Age, diabetes, aspirin, clopidogrel, CDABCRBS were correlated with cerebrovascular accidents (P < 0.05).
CONCLUSIONSWhether or not accompanied with hypertension is an influential factor for carotid stenosis, but it does not affect the occurrence of ischemic cerebrovascular events. CDABCRBS could effectively prolong the occurrence time of ischemic cerebrovascular events.

5.Curative Effect of Praziquantel on Juvenile Paragonimus westermani and the Dynamic Changes of T Lymphocytes in Infected Mice after Treatment

Objective To study the curative effect of praziquantel against juveniles of Paragonimus westermani and the dynamic changes of T lymphocytes and their subpopulations in infected mice after treatment. Methods Mice infected with metacercariae of P. westermani were treated with praziquantel 10 days after infection for examining the curative effect. Results Mice infected orally each with 30 metacereariae of F. westermani were treated with praziquantel at doses of 400,500 and 600 mg/(kg?d) respectively for 3 d, the juvenile reduction rates were 71.01 % ? 10.38% ,82.29% ? 7.92% and 95.83% ? 3.24% respectively; when the mice were infected with 80 metacercariae and treated with a dose of praziquantel 600 mg/(kg?d) for 3 d, the worm reduction rate was 84.49% ?10.91%. T lymphocytes and their subpopulations began to increase at 1 wk and returned to normal at 8 wk after praziquantel treatment. Conclusion The efficacy of praziquantel against juvenile P. westermani was poorer than that against adult worms ; the efficacy was relevant to the drug dosage and the number of metacercariae infected. The T lymphocytes and their subpopulations may be regarded as an index for curative effect.
6.Effect of different antiplatelet or anticoagulation therapy on postoperative pocket hematoma after electrophysiological devices placement

Objective To investigate the effect of different antiplatelet or anticoagulation therapy on postoperative pocket hematoma after electrophysiological devices placements (EPD). Methods The clinic data of 410 patients who took anti-platelet or anticoagulation therapy and needed to be implanted EPD from February 2012 to February 2015 were selected. According to the type of therapy, patients were divided into 4 groups:dual anti-platelet drug (DAP) treatment group (DAP group, 114 cases), low-molecular-weight heparin (LMWH) bridging treatment group (LMWH group, 98 cases), aspirin (ASA) alone group (ASA group, 94 cases) and warfarin group (104 cases). The incidence of pocket hematoma after electrophysiological devices placements was observed. The risk factors of pocket hematoma were analyzed. Results The incidence of pocket hematoma was higher in LMWH group than that in the other 3 groups: 18.4% (18/98) vs. 4.4% (5/114), 3.2% (3/94) and 2.9% (3/104), and there was statistical difference (P<0.05). But there were no significant differences in the incidence of pocket hematoma among DAP group, ASA group and warfarin group (P>0.05). The multiple Logistic regression analysis revealed that LMWH bridging therapy was an independent risk factor for the development of pocket hematoma (OR=7.105, 95%CI 1.872-17.283). Conclusions LMWH bridging therapy can increase the risk of development of pocket hematomas after electrophysiological devices placement.

A simple,reliable RPHPLC method on ODS column with UV detector for deter-mination of the anticoagulant rodenticide coumatetralyl in animal tissue was developed.1,1′-Bi-(2-Naphthol)was used as an internal standard to check the process ofexperiments.The mean recoveries from the spiked rabbit liver were around 90% atthe levels of 0.4-16mg/kg.The coeffecint of variation of 5 determination was 3.2%.The results showed that this method has a satisfactory reproducibility.The methodis practical for examining poisons in poisoning cases.
