1. Systematic evaluation of Compound Biejia Ruangan Tablets combined with Adefovir Dipivoxil in treatment of chronic hepatitis B with liver fibrosis

To evaluate the clinical curative effect of Compound Biejia Ruangan Tablets (CBRT) combined with Adefovir Dipivoxil in the treatment of chronic hepatitis B with liver fibrosis using Meta-analysis. Cochrane library, PubMed, CNKI, Wanfang databases, and VIP were retrieved comprehensively to collect randomized controlled trials (RCTs) of CBRT combined with Adefovir Dipivoxil in the treatment of chronic hepatitis B with hepatic fibrosis from their inception to October 2017. treatment group was treated with CBRT combined with Adefovir Dipivoxil, and the control group was treated with Adefovir Dipivoxil. All the data were analyzed using Revman 5.3. A total of 19 RCTs and 1 776 patients were included. Meta-analysis results showed that the serum indexes including HA, IV-C, LN, PCIII, ALT, AST, and TBIL of the treatment group, were significantly lower than those of control group. HA [SMD = -1.72, 95% CI (-2.26, -1.17), P = 0.000 01]; IV-C [SMD = -1.10, 95% CI (-1.66, -0.54), P = 0.000 10]; LN [SMD = -1.18, 95% CI (-1.64, -0.73), P = 0.000 01]; PCIII [SMD = -1.52, 95% CI (-1.97, -1.07), P = 0.000 01]; ALT [SMD =-0.48, 95% CI (-0.68, -0.28), P = 0.000 01]; AST [SMD = -1.19, 95% CI (-2.08, -0.29), P = 0.010 00]; TBIL [SMD = -0.98, 95%CI (-1.38, -0.58), P = 0.000 01]; There were no significant difference in serum HBV DNA, and HbeAg negative conversion rate treatment group compared with control group. HBV DNA [RR = 1.21, 95% CI (0.97, 1.50), P = 0.09]; HBeAg [RR = 1.05, 95% CI (0.82, 1.34), P = 0.70]; The total clinical effective treatment group was significantly better than control group. [RR = 1.25, 95% CI (1.15, 1.36), P = 0.000 01]. Compared with the single use of Adefovir Dipivoxil, the clinical curative effect of CBRT combined with Adefovir Dipivoxil in the treatment of chronic hepatitis B with liver fibrosis is better,which can significantly reduce the level of serum liver fiber markers and improve liver function in patients with biochemical indicators and the total clinical efficiency.
2.Determination of furbenicillin sodium and its related substances by HPLC.

Furbenicillin is a broad-spectrum semisynthetic penicillin with strong antibacterial activity against Gram-negative bacteria. Furbenicillin sodium is determined by volumetric method in current criteria. However, the criteria does not contain an assay of related substances of furbenicillin sodium. In this study, we established a method for detection and analysis of furbenicillin sodium and its related substances by HPLC. The analysis was performed with a C18 column under a gradient elution, the detection wavelength was 225 nm, and the column temperature was 35 degrees C. The reliability and accuracy of established method was validated in this study. Pure samples of furbenicillin sodium and its related substances were prepared. The structures, biological activities, and chromatographic retention behaviors of furbenicillin sodium and its related substances were identified using NMR, CLSI agar dilution method, and HPLC. All results in the current study provide ample evidence that this method is able to determine the reasonable limits in the quality-control protocol for furbenicillin sodium.

4.Application of FOCUS- PDCA model in hypoglycemia management during hemodialysis in patients with diabetic nephropathy

Objective To evaluate the effect of FOCUS- PDCA model in hypoglycemia management during hemodialysis in patients with diabetic nephropathy. Methods Totally 107 patients with diabetic nephropathy were divided into two groups. Patients in the control group received traditional hypoglycemia management, while FOCUS- PDCA model was implemented in the experimental group in hypoglycemia management, including reducing insulin dosage before hemodialysis,injecting 50%glucose and taking hyperglycemia food during hemodialysis,enhancing the monitoring of blood glucose and health education. Compared the incidence of blood glucose levels and hypoglycemia in two groups of patients at different time of dialysis. Results The frequency of hypoglycemia in the experimental group at the beginning of dialysis [ (9.04±2.08) mmol/L ], 2 hours after dialysis[ (5.78±1.82) mmol/L ]and at the end of dialysis[ (6.94±2.04) mmol/L ] were significantly lower than that in the control group[ (8.60±2.02) mmol/L, (5.20±1.62) mmol/L, (4.97±1.05) mmol/L ], (P <0.05), and the total incidence rate of hypoglycemia was 1.6%(61/3 889), which significantly lower than that 4.8%(185/3 816) in the control group, the difference was statistically significant (χ2=5.288, P < 0.05). Conclusions The application of FOCUS- PDCA model in the hypog1ycemia management of patients with diabetic nephropathy can reduced the incidence of hypoglycemia during hemodialysis. It deserves to be widely used in clinical practice to improve the patients′quality of life.
5.Cox model analysis of long-term survival results from renal transplant recipients over 15 years

Objective To analyze the clinical data of recipients over 15 years after renal transplantation,and to find the factors that affect the long-term survival of recipients after renal transplantation.Method Before June 30,2000,326 renal transplant recipients in our hospital were collected retrospectively.The risk factors which affect the survival of kidney transplant recipients and kidney were analyzed from four dimensions.A Cox model was established to analyze these multi factors.Result Cox hazard model indicated that advanced age (P=0.010,RR =1.052),AMR (P<0.001,RR =18.311),nonadherence (P =0.001,RR =2.854),smoking (P =0.025,RR =2.097)were the risk factors for recipients' survival.Using immunosuppressive regimen FK506 + MMF+ Pred (P =0.019,RR =0.433),or CsA + MMF + Pred (P =0.019,RR =0.413) was the protective factor for recipients' survival.Nonadherence (P<0.001,RR =5.645),and diabetes (P<0.001,RR =3.310) were the risk factors of grafts' survival.Using immunosuppressive regimen FK506 + MMF + Pred (P<0.001,RR =0.236),or CsA + MMF + Pred (P =0.002,RR =0.317) was the protective factor of grafts' survival.Conclusion To enhance the long-term outcome of recipients and grafts,the individualization of immunosuppressive regiments and controlling of the chronic diseases progress by changing the unhealthy life style are cutting on edge.
6.Application of bandage contact lens in pterygium excision combined with conjunctival transplantation in older adults and evaluation on its efficacy

Objective: To discuss the clinical efficacy of bandage contact lens (BCL)in pterygium excision combined with conjunctival transplantation for the older adults,and to explain its advantages.Methods:The clinical materials of 70 older patients (70 eyes ) who underwent the pterygium excision combined with conjunctival transplantation were retrospectively analyzed.They were divided into two groups according to the postoperative treatment.In the observation group,35 cases (35 eyes)wore BCL after operation.In control group,35 cases (35 eyes)were covered with normal eye pad.The scores of blurred vision,photophobia,foreign body sensation, lacrimation,eye-soreness,corneal fluorescence stain (FSC)and epithelial defect areas were observed at 1,3,7 and 10 d afer operation.Results:The scores of blurred vision,photophobia,foreign body sensation,lacrimation,eye-soreness of the patients in observation group were significantly lower than those in control group at 1,3,and 7 d afer operation (P <0.05).The scores of FSC and epithelial defect areas of the patients in observation group were significantly lower than those in control group at 3 and 7 d afer operation (P <0.05).There were no significant differences in blurred vision,photophobia,foreign body sensation,lacrimation,eye-soreness,FSC and epithelial defect areas of the patients at 10 d afer operation between two groups (P >0.05).No complications of infection and implant healing badness were observed in all patients.Conclusion:BCL can significantly reduce the irritations after pterygium excision combined with conjunctival transplantation for the older adults and promote the healing of the corneal epithelium.
7.Effect of Kanglaite Injection on Proliferation and Apoptosis of Human Breast Cancer MCF-7 Cells

Objective To investigate effects of Kanglaite injection on proliferation, cycle and apoptosis of human breast cancer MCF-7 cells;To discuss its relevant mechanism. Methods Logarithmic growth phase cells were divided into control group and Kanglaite-treatment group (10, 20, 40μL/mL). Cells were cultured in RPMI-1640 for 24 h before drug treatment. The inhibition rate of Kanglaite injection on proliferation of human breast cancer MCF-7 cells was detected by MTT assay. Apoptosis and cell cycle of MCF-7 cells were detected by flow cytometry. Changes in cell nucleus were determined by Hochest staining assay. Protein expressions of Bcl-2 and Bax were detected by ELISA and Western blot. Results Kanglaite injection for 12 h, 24 h or 48 h resulted in a significant inhibition of MCF-7 cells proliferation (P<0.05, P<0.01);Compared with the control group, Kanglaite injection-treated cells showed increased percentage in G2/M and G0/G1 phases (P<0.001, P<0.01), but showed decreased percentage in S phase (P<0.01), and apoptosis rate increased (P<0.05, P<0.001). Kanglaite injection significantly decreased protein expression of Bcl-2, and enhanced protein expression of Bax of MCF-7 cells (P<0.01, P<0.001). Conclusion Kanglaite injection can inhibit the proliferation of human breast cancer MCF-7 cells, decrease cell cycle and induce apoptosis, the mechanism is related with decreasing protein expression of Bcl-2 and enhance the protein expression of Bax.

ObjectiveTo discuss the effect of neonatal behavioral neurological assessment (NBNA) and developmental screening test (DST) on newborns with hypoxic ischemic encephalopathy (HIE). MethodsNBNA was performed for 60 newborns with HIE within 12-14 days and DST was also performed at 6th month after birth.ResultsMild and severe degree HIE newborns with low NBNA scores had unfavorable prognosis. Conclusions NBNA and DST can provide evidence for early interference for newborn's HIE.

Objective To observe the effect of Ningxia aqueous extracts of fruitless lycium sprout (AEFLS) on cardiomyo cyte antioxidation and apoptosis-related protein expression in aging mice.Methods The natural aging C57BL/6J mice with 13 months old were randomly divided into aged control group,AEFLS low dose group (AEFLS1),AEFLS middle dose group (AEFLS2)and AEFLS high dose group(AEFLS3).The AEFLS1,AEFLS2 and AEFLS3 groups were respectively given with 5,10,20 mg/kg AEFLS gavage,while the aged control group was given with the normal saline gavage,for continuous8 weeks.The xan thine oxidase assay and thiobarbituric acid method were used for the determination of SOD and MDA in heart tissues.Western-blot and immunohistochemical method were used to detect the expressions of Bcl-2,Bax and Capase-3 in heart tissue.Results Compared with the aged control group,the MDA level in the AEFLS2 and AEFLS3 groups was decreased,while the SOD activity was increased,the difference was statistically significant (P<0.01).The Western-blot result showed that compared with the aged control group,the optical density value of Bcl-2 in the AEFLS2 and AEFLS3 groups was increased,but the optical density values of Bax and Capase-3 were decreased (P<0.01);the immunohistochemical results showed that compared with the aged control group,the immunopositive(IP) expressions of Bcl-2 protein in heart tissues in the AEFLS2 and AEFLS3 groups were increased (P<0.01),while the IP expressions of Bax and Capase-3 were decreased (P<0.01).Conclusion Middle and high doses of AEFLS can increase the antioxidative ability of myocardial tissue,up-regulates the Bcl-2 expression,down-regulates the Bax and Capase-3 expressions and plays anti-cardiomyocyte apoptotic role.
