1.Observation of the effect of comprehensive rehabilitation on patients with stroke in different periods

ObjectiveTo observe the effect of comprehensive rehabilitation treatment on clinical nerve function deficiency (CNFD) and activities of daily living (ADL) of the patients with stroke. Methods45 stroke sufferers from different period were divided into three groups (1st group: in one month; 2nd group: one to six months; 3rd group: over six months). Before and after the comprehensive rehabilitation treatment was given, the CNFD, ADL and clinical effect were assessed.ResultsAfter comprehensive rehabilitation treatment, the CNFD of 1st group, and the ADL score of 1st group and 2nd group were improved significantly, and the 1st group's was better than the others. The total effective rate also increased in 1st and 2nd group. Conclusions Early rehabilitation treatment can improve the patient's neural motion function and the activities of daily living.
2.Gd-DTPA Enhancement MR Angiography of Intracranial Occuping and Vascular Diseases

Purpose To investigate the diagnostic valuation of Gd-DTPA enhancement MRA in intracranial occuping and vescular diseases. Methods Gd-DTPA MRA(63 cases), MRA without Gd-DTPA(16 cases)as a control group was performed with Elscint Prestige 2.0T MR,MRA. 56 cases were proved by operation or pathology, 7 cases were proved by clinical observation. Results The shape of aneurysms, the feeding arteries, the draining veins of AVM and diploe angioma, the feeding arteries of occuping were clearly showed by Gd-DTPA enhancement MRA and were not showed by MRA without Gd-DTPA.There wasn't much difference between cavernous angioma with Gd-DTPA enhancement MRA and MRA without Gd-DTPA in the shape and blood supply of cavernous angioma. Conclusion Gd-DTPA enhancement MRA is useful and worthy for showing blood circulation of intracranial occuping and vascular diseases.
3.Chemical constituents from traditional Chinese medicine Siegesbeckia pubescens.

Column chromatography on silica gel was used to study the chemical constituents of traditional Chinese medicine Siegesbeckia pubescens. The chemical structures of the separated compounds were elucidated by spectroscopic data analyses. As a result, eighteen compounds were obtained and identified as 3, 4'-dimethoxy quercetin(1), 3, 3', 4'-trimethoxy quercetin(2), 3, 3'-dimethoxy quercetin(3), 7, 3', 4'-trimethoxy luteolin(4), ursolic acid(5), 2β,19α-dihydroxyursolic acid(6), 2β-hydroxyursolic acid (7), stigmasterol-7-one(8), 5α, 8α-epidioxy-24(R)-methyl-cholesta-6, 22-diene-3β-ol(9), β-sitosterol(10), 2, 6-di(3-hydroxy-4-methoxyphenyl)-3, 7-dioxacyclo [3. 3. 0] octane (11), aurantiamide acetate (12), 3-(m-hydroxyl-p-methoxy)-N-(2'-p-hydroxyl-phenethyl)-2E-acrylamide(13), p-hydroxy benzaldehyde (14), m-hydroxy-p-methoxy benzaldehyde (15), 3, 4, 5-trimethoxybenzoic acid(16), monoethyl malonate(17), and p-hydroxylcinnamic acid(18). Among them, compounds 1-9, 11-18 were isolated from this plant for the first time.

4.Comparison between intravitreal Ranibizumab and Tramicinolone acetonide for macular edema secondary to central retinal vein occlusion

AIM:To compare the efficacy and safety of intravitreal ranibizumab to those of triamcinolone acetonide ( TA ) injection for the treatment of macular edema secondary to central retinal vein occlusion ( CRVO) .
METHODS:This retrospective study included 40 eyes of 40 patients with macular edema associated with CRVO. Twenty patients 20 eyes were treated with intravitreal injection of triamcinolone acetonide (1mg, 0. 1mL), the other 20 patients 20 eyes accepted intravitreal ranibizumab (0. 5mg, 0. 05mL). The change of best corrected visual acuity ( BCVA ) , central macular thickness ( CMT ) , and intraocular pressure ( IOP ) before treatment and at 1, 2wk, 1, 2,3,6mo post-injection in the two groups were observed.
RESULTS:BCVA was improved at 1, 2wk, 1, 2,3,6mo post-injection in the TA group (P<0.05) and ranibizumab group ( P<0. 05 ). No significant difference was found between the two groups ( P > 0. 05 ). CMT decreased significantly within each group ( P < 0. 05 ), and no significant difference between groups was found ( P >0.05). In the TA group, the IOP was significantly higher at 2wk and 4wk than before treatment (P<0. 05). In the ranibizumab group, no elevated IOP was observed at 1, 2wk, 1, 2,3,6mo (P>0. 05). However, the IOP at 1mo was significantly higher in the TA group than that in the ranibizumb group (P<0. 05).
CONCLUSION:Intravitreal ranibizumab is an effective and safe treatment method for macular edema secondary to CRVO. It can effectively improve BCVA and reduce CMT without ocular and systemic complications compared with intravitreal TA.
7.Basic Gait Characteristics of Healthy Adults

ObjectiveTo study normal gait characteristics of healthy adults in different age groups.Methods90 healthy adults were divided into three groups according to their ages. It was 20~39 year group, 40~59 year group and 60~70 year group. They received a gait analysis with the gait analysis system based on digital video and image processing which could provide temporal-spatial parameters and kinematic parameters. The gait data of the three groups were compared. Relationships between gait speed and other gait parameters were investigated.ResultsThere were statistically significant differences in some parameters among three groups. Gait speed was significantly correlated with stride time, stride length, stance time (%), cadence, maximum flexion of hip and knee in swing phase.ConclusionThe normal gait patterns of healthy adult established with the gait analysis system based on digital video and image processing can be used as the base line to compare with abnormal gait.
8.Cellular fibrous histiocytoma: a clinicopathologic analysis of 27 cases.

OBJECTIVETo study the clinicopathologic characteristics of cellular fibrous histiocytoma (CFH) with emphasis on diagnosis and differential diagnosis.
METHODSClinical and pathologic features were reviewed in 27 cases of CFH (encountered during the period from 2008 to 2012) along with outcome analysis. Immunophenotyping was performed with EnVision method.
RESULTSThe patients included 13 males and 14 females. The age at presentation ranged from 15 to 61 years (mean, 34 years; median, 32 years). The tumor occurred in the extremities (n = 14), head and neck (n = 7), and trunk (n = 6). Histologically, the tumors were located in the dermis. Some cases showed wedge like extension into the subcutaneous adipose tissue. On high power, they consisted of dense fibroblasts and myofibroblasts. Other cell components such as psammoma-like histiocytes, hemosiderin-containing macrophages or touton-type giant cells were rare. The spindled tumor cells were arranged mostly in intersecting fascicles. Focal storiform architecture was not uncommon. In addition, a few cases showed prominent hemangiopericytoma-like pattern. There was no prominent cellular atypia but increased mitotic figures were not difficult to find. Two cases exhibited necrosis. By immunohistochemistry, the tumor cells showed variable expression of alpha smooth muscle actin. CD34 positive cells were present in some cases, but were distributed mostly in the periphery or bottom of the lesions. They were all negative for desmin, h-caldesmon, S-100 protein and cytokeratin. Follow-up in 19 cases revealed local recurrences in 5 cases and bilateral pulmonary metastases in 1 case after repeated recurrences.
CONCLUSIONSCFH is a cellular form of benign fibrous histiocytoma which has a risk for local recurrence after incomplete excision. Distant metastasis can occur in rare examples. However, there were no morphological parameters predicting the risk of recurrence or metastasis. Increased awareness of the clinocopathological features and immunophenotypes of CFH is helpful in avoiding misdiagnosing the disease as malignant tumors, especially dermatofibrosarcoma protuberans.

9.Identification of liposoluble constituents in Yushu tablets by UPLC-ESI-IT-TOF/MS.

In order to identify the chemical constituents of Yushu tablets comprehensively, we studied the chemical constituents of CHCl3 extract from Yushu tablets by the ultra performance liquid chromatography-electrospray ionization-ion trap-time of flight mass spectrometry (UPLC-ESI-IT-TOF/MS). It showed that there were more than 100 compounds separated, and forty-nine peaks among these were identified on the basis of high resolution mass spectrometry data and literature data reported. Determination of twelve peaks was further confirmed by standard substances. These components assigned to the different plant sources mainly included phenylpropanoids, triterpenoids, quinones and m-trihydroxybenzene compounds. By analyzing the chemical components of CHCl3 extract from compound Chinese medicine Yushu tablets comprehensively, this study provided the foundation for studying chemical components, pharmacodynamic substance and quality control of Yushu tablets.

10.Application of magnetic resonance imaging in the evaluation of microwave ablation for liver cancer

Objective To investigate the value of magnetic resonance imaging(MRI)in the evaluation of microwave ablation(MA)for liver cancer.Methods The clinical data of 51 patients with liver cancer who received MA at the General Hospital of PLA from November 2005 to June 2009 were retrospectively analyzed.The MRI findings of 65 nodules before and after the treatment of MA were evaluated by serological examination and needle biopsy of liver.Results All patients received MRI within 1 month after MA.Of all nodules.63 were presented with high signal intensity on T_1-weighted images(T_1 WI)and low signal intensity on T_2-weighted images (T_2 WI)and with peripheral ring-like enhancement.They were diagnosed as complete coagulation necrosis,and the alpha-fetoprotein(AFP)level of the patients decreased from 333.83μg/L before MA to 37.68 μg/L after MA.Two nodules were presented with low signal intensity on T_1 WI and high signal intensity on T_2 WI,and they were diagnosed as local residual.All patients were followed up l month after MA,and 5 nodules showed enhancement with the same image characteristics as local residual.They were diagnosed as local recurrence of liver cancer by needle biopsy of liver and AFP level detection.New intrahepatic nodules in 23 patients and an abdominal nodule in 1 patient were detected with an increase of AFP level(mean,120.16 μg/L).Conclusion MRI Can exactly evaluate the efficacy of MA in the treatment of liver cancer.
