2.Quality control analysis of surgical treatment of lung cancer: a single center's experience

Objective Surgical treatment of lung cancer needs to follow strict quality control standard for the aims of accuracy of pathological staging and potentially improved prognosis.However,there are short of studies related to surgical quality analysis.Methods One hundred and twelve patients were enlisted with the diagnosis of lung cancer and received surgical intervention in 2007.Surgical quality of these cases were retrospectively analyzed in compliance with three international standards,National Comprehensive Cancer Network (NCCN),American College of Surgeons’ Oncology Group (ACOSOG),and International Association for the Study of Lung Cancer (IASLC).Results No surgical death was reported in this group.According to NCCN,ACOSOG and IASLC standards,qualified operations were 82 cases (80.4%),73 cases (71.6%),45 cases (44.1%) in 102 cases with R0 resection,respectively.The median total lymph nodes,median mediastinal nodes,and N1 nodes were 27 (range 0-63),16.5 (range 0-43),and 10.5 (range 0-26),respectively.The median mediastinal node stations resected were 4 (range 0-6).In the mediastinum,stations of 4R,5,6 and 7 presented the highest frequency of receiving lymph node dissection.For N1 stations,10 and 12 were among the top list.For the whole group,1-year survival,3-year survival and 4-year survival were 92% (95% CI,89-95),67% (95% CI,62-72),57% (95% CI,52-62),respectively.Conclusion Quality control is the essential part of surgical treatment of lung cancer,which will facilitate the baseline standardization of clinical research.Since IASLC provides the strictest standard for lung cancer surgery,we suggest that all thoracic surgeons need to follow this standard to secure the accuracy of pathological diagnosis and for a potential better prognosis.
3.miR-21/Sprouty1 function axis regulates the osteogenic differentiation of bone marrow mesenchymal stem cells after postmenopausal osteoporosis

BACKGROUND:Osteogenic differentiation is a complex process involving transcriptional and post-transcriptional regulation by multiple signaling pathways, and the specific mechanisms remain unclear. It is of great significance to study the role of critical miRNAs in the osteogenic differentiation of bone marrow mesenchymal stem cells (BMSCs) in the treatment of osteoporosis and bone defects. OBJECTIVE: To explore the regulatory ability of miR-21/Sprouty1 function axis in the osteogenic differentiation of BMSCs in patients with postmenopausal osteoporosis. METHODS:BMSCs were isolated from healthy people (H-hBMSCs) and patients with postmenopausal osteoporosis (PMOP-hBMSCs), and their osteogenic ability was compared. Expression of miR-21 and Spry1 at gene and protein levels was detected by real-time RT-PCR and western blot assay, respectively. miR-21 expression was upregulated via transfection in PMOP-hBMSCs, and the osteogenic ability and Spry1 expression of the cells were detected, while real-time RT-PCR and western blot were used to detect the expression of osteogenic marker genes, Runx2 and Osterix. RESULTS AND CONCLUSION:Compared with H-hBMSCs, PMOP-hBMSCs osteogenic ability was weakened significantly, miR-21 expression decreased, and Spry1 expression increased, indicating an inhibition to the miR-21-Spry1 function axis. Through the transfection of miR-21 and down-regulation of Spry1, the expression levels of Runx2 and Osterix were increased, and PMOP-hBMSCs osteogenic ability was partially restored.
4.Role of Spry1 in osteogenic differentiation of human bone marrow mesenchymal stem cells under miR-21 regulation

BACKGROUND:Previous studies have found that miR-21 expression is increased during osteogenic differentiation of bone marrow mesenchymal stem cells, but the action and molecular mechanism of miR-21 are stil unclear. OBJECTIVE:To verify the target gene of miR-21, Spry1, and to explore the role of Spry1 in osteogenic differentiation of human bone marrow mesenchymal stem cells. METHODS:Luciferase report was used to verify Spry1 gene targeted by miR-21, and western blot assay was used to detect the expression of Spry1 in the osteogenesis of human bone marrow mesenchymal stem cells. Spry1 expression vector was established and transfected into human bone marrow mesenchymal stem cells. Osteogenesis ability of human bone marrow mesenchymal stem cells was analyzed after Spry1 high expression by alkaline phosphatase, alizarin red staining, RT-PCR and western blot. RESULTS AND CONCLUSION:Luciferase report suggested that Spry1 was a target gene of miR-21. The expression level of Spry1 was decreased in the osteogenesis of human bone marrow mesenchymal stem cells. Increasing expression of Spry1 could inhibit osteogenic differentiation of human bone marrow mesenchymal stem cells. These results indicate that Spry1 as a target gene of miR-21 negatively regulates osteogenic differentiation of human bone marrow mesenchymal stem cells, and plays an important role in bone formation process.
5.Therapeutic Observation of Thunder-fire Moxibustion at Dazhui (GV14) and Shenshu (BL23) plus Cognitive Training for ;Mild Cognitive Impairment Due to Ischemic Cerebral Stroke

Objective To observe the clinical efficacy of thunder-fire moxibustion at Dazhui (GV14) and Shenshu (BL23) in treating mild cognitive impairment (MCI) due to ischemic cerebral stroke. Method Sixty patients with MCI due to ischemic cerebral stroke were randomized into a treatment group and a control group, 30 cases each. The control group was intervened by joint treatment plus cognitive training, while the treatment group was by thunder-fire moxibustion in addition to that given to the control group. The two groups were evaluated by the Montreal Cognitive Assessment (MoCA), Mini-Mental State Examination (MMSE) and Wechsler Memory Scale (WMS), the major symptoms were observed, and the clinical efficacies were compared between the two groups. The two groups were both treated for 8 weeks. Result The total effective rate was 80.0%in the treatment group, versus 70.0% in the control group, and the difference was statistically significant(P<0.05). Conclusion The selected acupoints can improve the cognition and activities of daily living (ADL) in MCI patients;thunder-fire moxibustion at Dazhui and Shenshu plus cognitive training can produce a better clinical efficacy than dry cognitive training. Therefore, we can combine thunder-fire moxibustion with modern rehabilitation to enhance the therapeutic efficacy in preventing and treating MCI.
6.Diagnosis and treatment characteristics of radioactive optic neuropathy

AIM:To explore the diagnosis and treatment methods of radioaction-induced optic neuritis ( RION) through the clinical dates of 17 patients.
METHODS: It was a retrospective case series study. From August 2008 to October 2013, 17 cases (24 eyes) of Rion clinical dates from Chinese PLA General Hospital were studied. The diagnosis methods including visual acuity, pupil, fundus, visual field, fundus fluorescein angiography (FFA), visual electrophysiological testing, and head MRI. To analysis the clinical date of patients with diagnosis of RION by statistical description.
RESULTS: The deterioration degree of vision: 13 eyes were classified as Ⅳ, 9 eyes as Ⅲ, 2 eyes as II. Ten eyes RAPD ( + ) , visual electrophysiology is extinguished. The retina of 5 eyes showed flame hemorrhages and cotton wool spots exudation. Optic nerve head edema in one eye. T1 - weighted MRI enhanced in 19 eyes which showed optic nerve of the intracranial and intratubal segments abnormal changed, optic chiasm and pituitary stalk signal abnormalities and enhancement of the optic nerve. Tortuous optic nerves and rough edges were observed in 5 eyes. Treatment effect: 4 eyes of visual acuity improved, 1 eye from blindness to light perception,1 eye from 0. 08 to 0. 2, 1 eye from 0. 4 to 0. 6,1 eye from 0. 04 to 0. 15, the rest of the cases did not see any improvement.
CONCLUSION: The unique clinical manifestation of RION can provide objective basis for clinical diagnosis in time, but there have not been proven any effective treatments.

With widely application of bronchoavleolar lavage , a lot of cases on L.Blattarum infections in respiratory system have emerged in recent years .However , after closely lucubrating pictures of these reported cases and analyzing the results of our animal experiments.It was doubted the reported morphology of L.Blattarum belongs to respiratory ciliated columnar epithelium .This article aimed at guiding our colleagues to distinguish the morphology of Lophomonas blattarum in respiratory system, and,avoid the misdiagnosis.
8.Antibacterial activity of Cu ions released from 316L-Cu antibacterial stainless steel

BACKGROUND:316L-Cu antibacterial stainless steel is made by adding a certain amount of copper into the stainless steel fol owed by a special heat treatment to uniformly disperse copper-rich precipitates in stainless steel substrate, thereby harvesting the antibacterial properties. OBJECTIVE:To evaluate the antibacterial activity of the Cu ions released from 316L type Cu-bearing antibacterial stainless steels against Porphyromonas gingivalis, thereby providing biomedical evidence for its clinical application. METHODS:The medical 316L stainless steel samples at a surface area to volume ratio of 0.1 cm2/L were soaked in simulated body fluids at 37 ℃ for 1-10 days. A graphite furnace atomic absorption spectrophotometer was employed to detect the amount of Cu release in the simulated body fluids each day and then the rate of Cu release per day could be determined. The antibacterial activities of the steel samples were evaluated by a standard film-covered method under a scanning electron microscope. RESULTS AND CONCLUSION:The daily Cu releasing amount from the 316L-Cu stainless steel within 10 days was significantly higher than that of 316L stainless steel, and al the values remained nearly constant. With time, the sterilizing rate of 316L-Cu stainless steel was gradual y increased, and reached 100%until the 10th hour. Porphyromonas gingivalis showed some morphological changes at 3 hours after treated with 316L-Cu stainless steel, appeared with cleavage at 6 hours, and mostly disintegrated into pieces at 9 hours. The results indicated that the 316L-Cu antibacterial stainless steel showed excel ent antibacterial property against Porphyromonas gingivalis, slowly release Cu irons, and alter the surrounding microenvironment, which is a highly promising biomaterial and has good clinical value.
9.Value of 18F-FDG PET/CT in staging of pancreatic cancer

Objective To evaluate the value of 18F-FDG PET/CT in tumor staging in patients with pancreatic cancer.Methods A total 77 patients (from June 2010 to August 2015;44 males, 33 females, age range 36-83 years) who underwent 18F-FDG PET/CT examination for pancreatic cancer and confirmed with pathology were enrolled in this retrospective study.All patients had not been treated before the PET/CT scanning and received surgery or biopsy 4 weeks after the scanning.Two-sample t test and ROC curve analysis were used for data analysis.Results 18F-FDG uptake was higher in 94.8%(73/77) of pancreatic lesions than that in normal pancreatic tissue.The range of SUVmax of pancreatic lesions was 2.4-13.4(mean: 6.2±2.4).SUVmax of patients with smaller primary lesion (minor axis≤2.0 cm) was significantly lower than that of larger lesion group (minor axis >2.0 cm;t=-2.661, P<0.05).A total of 46 patients underwent lymph node excision, and the mean number of excised lymph nodes per patient was 13.8±9.2.About 56.5%(26/46)cases with lymph nodes metastases were confirmed with pathology.When the cut-off value of minor axis of regional lymph nodes was 0.45 cm, ROC curve showed that the sensitivity, specificity and AUC were 84.8%(39/46), 65.2%(30/46) and 0.788, respectively.When the cut-off value of SUVmax of regional lymph nodes was 2.05, the sensitivity, specificity and AUC were 54.3%(25/46), 80.4%(37/46) and 0.759, respectively.18F-FDG PET/CT changed 18.2%(14/77)of patients′ treatment plan.Conclusions 18F-FDG PET/CT is a useful tool in pancreatic cancer staging.Though 18F-FDG PET/CT has no significant advantages in N-staging, it really helps to make a more accurate M-staging for clinical decision.
10.Effects of vibrating the abdomen on peripheral blood gastrointestinal hormones and gastrointestinal motility of young anorexia model rats

Objective:To observe the therapeutic effect of vibrating the abdomen on anorexia model rats,as well as its effects on cholecystokinin octapeptide(CCK-8)and motilin(MTL)in the peripheral blood. Methods:Forty young rats were randomly divided into a normal group(n=10)and a modeling group(n=30).Rats in the normal group were fed common feed.The anorexia model was established by the etiological simulation method in the modeling group,and these rats were further randomly divided into a drug group,a vibrating abdomen group,and a model group 3 weeks after the anorexia model was induced,with 10 rats in each group.The drug group was given Jian Wei Xiao Shi Pian by intragastric administration at a dose of 0.72 g/(kg·bw)(0.72 g drug was dissolved in 10 mL purified water).The normal group and the model group were given purified water once a day in the morning.The vibrating abdomen group was treated with vibrating the abdomen once a day for 21 times.The body mass,food intake,serum CCK-8,MTL,gastrin(GAS),neurotensin(NT)levels,and the intestinal propulsion rate of rats in each group were measured. Results:Compared with the model group,the body mass,food intake,serum MTL and GAS levels,and the small intestine propulsion rate increased significantly,and the serum CCK-8 and NT levels,the gastric residual rate decreased significantly in the vibrating abdomen group and the drug group(P<0.05).There were no significant differences between the vibrating abdomen group and the drug group(P>0.05). Conclusion:Vibrating the abdomen increases the food intake and body mass of anorexia model rats,reduces the residue of gastric contents,improves the small intestine propulsion rate,and therefore has a good therapeutic effect on anorexia.The mechanism may be related to inhibiting the secretion of CCK-8 and NT in plasma and promoting the release of MTL and GAS in serum.
