1.Protective effects of total extract of astragalus(TEA) onprimary rats hepatocytes injured by CCl4 or H2O2

Aim To study the protective effect of total extract of astragalus(TEA) on primary rat hepatocytes injured by CCl4 or H2O2 and its mechanism.Methods The primary hepatocytes were isolated by Ⅳ collaganase and injured by CCl4 or H2O2.The contents of malondialdehyde(MDA) and glutathion(GSH),glutathion peroxidase(GSHpx) activity of hepatocytes and the AST and /or ALT level in cultural supernatant were determined by general methods. Results (1) The elevation of MDA content of hepatocytes and AST level in supernatant of cultural hepatocytes,and the loss of GSH content and GSHpx activity induced by CCl4 were restored remarkably by TEA(5~80 mg?L-1);(2)The elevation of ALT level in the supernatant of hepatocytes and MDA content of hepatocytes, and the loss of GSH content and GSHpx activity induced by H2O2 were improved significantly by TEA(5~80 mg?L-1) treatment.Conclusion The results suggest that TEA possess direct protective action on primary hepatocyte in vitro injured by CCl4 or H2O2. These might be associated with its anti-oxidative activity.
3.Effects of autologous blood transfusion on serum neopterin and IL-2 levels in patients undergoing radical gastrectomy

Objective To investigate the effects of autologous blood transfusion on serum neopterin and IL-2 concentrations after radical gastrectomy. Methods Sixty ASA Ⅰ or Ⅱ patients of both sexes (33 male, 27 female) aged 40-65 yrs weighing 51-70 kg undergoing radical gastrectomy for stomach cancer were randomly divided into 2 groups (n = 30 each):homologous blood transfusion group (H) and autologous blood transfusion group (A). The patients were premedicated with intramuscular diazepam 10 mg and atropine 0.5 mg. Anesthesia was induced with midazolam 0.1 mg?kg-1, fentanyl 5 ?g?kg-1, propofol 1 mg?kg-1 and vecuronium 0.12 mg? kg-1 and maintained with isoflurane inhalation (1 % -2 %) and intermittent i. v. boluses of fentanyl and vecuronium. In group H homologous blood 400 ml was infused during gastro-intestinal anastomosis. In group A 400 ml of blood was removed from radial artery after induction of anesthesia, before surgery. Blood volume was maintained by simultaneous infusion of succinylated gelatin 500 ml. The 400 ml autologous blood was infused also during gastrointestinal anastomosis. Venous blood samples were taken before operation (baseline) , at the end of operation and on the 5th postoperative day for determination of serum concentrations of neopterin and IL-2 (ELISA) Results The two groups were comparable with regard to sex (M/F ratio), age, body weight, duration of operation and total blood loss. 4.Phacoemulsification on corneal endothelium cells in diabetic patients with different disease duration

AIM:To observed the variation regularity of corneal endothelial cells in patients with different diabetes duration after phacoemulsification, and investigate the effects of diabetes and its disease duration on corneal endothelial cells.
METHODS: Ninety-seven ( 135 eyes ) cataract patients with diabetes were selected randomly and divided into GroupⅠ( which diabetes duration ≥10a) and GroupII(which diabetes duration <10a) according to their disease duration. Additionally 62 (89 eyes) age-related cataract patients were randomly selected as the control group. The corneal endothelial cell density ( CD ) , proportion of hexagonal cell and coefficient of variation ( CV ) in the three group patients were measured respectively before phacoemulsification and after surgery. And the measurement results were statistically analyzed.
RESULTS:The corneal endothelial CD and proportion of hexagonal cell in the three group were decreased after surgery compared with preoperative. But the CV of corneal endothelial cells was increased on the 1 st wk and in 1st mo after surgery compared with the preoperative. The difference was statistically significant (P<0. 05). The corneal endothelial CD and proportion of hexagonal cell in the two diabetic groups were lower than the control group after surgery. However, the CV of corneal endothelial cells was higher than the control group. The difference was statistically significant ( P < 0.05 ) . There was no significant difference in the corneal endothelial CD, proportion of hexagonal cell and CV between the two diabetic groups before phacoemulsification (P>0. 05). The proportion of hexagonal cell in Group Ⅰ was lower than which in GroupIIafter surgery. While the CV was higher than which in Group II. The difference was statistically significant (P<0. 05).
CONCLUSION: Phacoemulsification has some damage on the corneal endothelial. Since the impact of diabetes on the morphology and function of corneal endothelial cell was related to the diabetic duration. So phacoemulsification has more obvious damage on the corneal endothelial in diabetic patients. And the diabetic duration was longer, the damage on the corneal endothelial in phacoemulsification was more easily.
5.Effects of Chlamydia and Mycoplasma Infections on the Healing of Wounds after Surgery for Haemorrhoids,Fistula and Anoschisis

Objective To explore the effects of chlamydia and mycoplasma infections on the healing of wounds after surgical operation for haemorrhoids,fistula and anoschisis.Methods Sixty-two patients with haemorrhoids,fistula and anoschisis undergoing the surgical operation during 2000.1 to 2008.10 were collected,and wounds did not heal 40 days after operation and the wound's surface infection occurred after postoperative anti-inflammatory therapy and dressing change.All patients were positive at least for laboratory Ct or Uu,and those with infections caused by other fungi,bacteria,viruses and the other systemic diseases were excluded.The correlation between the infections in 62 cases and sex,surgical types was analyzed.Results In 62 patients,there were 30 cases (48.4%) positive for both Ct and Uu,22 cases for single Ct (35.5%),and 10 cases for single Uu (16.1%),respectively.The infection rate in females was higher than in males (P<0.01).The surgical types included:surgery for 8 cases of haemorrhoids (12.9%),for 18 cases of fistula(29.0%),and for 36 cases of anoschisis(58.1%).respectively.Conclusion There is the possibility of chlamydia and mycoplasma infections in the patients with delayed healing of wounds following the surgical treatment on the anal region.Early diagnosis,early treatment,avoidance of ineffective medicines and repeatedly management of the wounds can shorten the healing period.
6.Ultrasound diagnosis of amnniotic bands syndrome-clinical analysis

Objective To improve the diagnosis of amniotic bands syndrome with ultrasound. Methods All pregnant women underwent ultrasonographic examination to observe the intra amniotic band like structures. Results Among 8771 women undergone ultrasound examination, 60 were found with intra amniotic band structure. Seven cases were diagnosed as amniotic bands syndrome and 53 intra amniotic band like structures included extra amniotic pregnancy(7), amniotic sheets(4), incomplete uterine septum(24), fetal nuchal cystic hygroma(2), fetal nuchal and trunk edema(11) and disclosed extra embryo sac(5). Conclusions Not all the intra amniotic band like structures is amniotic band. But any abnormal structures detected in early stage should be closely followed up.
7.Model of Multidisciplinary Team Treatment on Hepatoblastoma in Children

Objective To study the diagnosis and treatment of the model of multidisciplinary team treatment on hepatoblastoma in children.Methods Retrospective analysis of treatment and long term follow-up of 16 patients with hepatoblastoma(8 cases were boys,8 cases were girls;aged 3 months to 11 years old,average age was 2 years old) during Aug.1998 to Jan.2006 in Shanghai Children's Medical Center.Special in multidisciplinary team treatment,preoperative evaluate and operation.Results The morbility locum of 8 cases were in right liver lobe,8 cases were in left liver lobe.Fifteen cases of the clinical presentation were abdominal mass,1 case was peritonitis.According to International Society of Pediatric Oncology(SIOP) Pretext staging system,43.7%(7/16)cases were stage Ⅱ,43.7%(7/16)cases were stage Ⅲ,12.5%(2/16)cases were stage Ⅳ.In 7 cases of stage Ⅱ,6 cases were accepted tumor total resection,1 case was accepted emergency surgery because tumor was fracture.Patients were given ICE protocal.In 7 cases of stage Ⅲ,6 cases were able to undergo delayed tumor resection following open biopsy and subsequent treatment with chemotherapy 2-3 courses.One case gave up treatment because its had poor response to chemotherapy.In 2 cases of stage Ⅳ,1 case was received liver transplantation due to poor respond to chemotherapy.One case was not received any treatment.In 7 cases of stage Ⅱ,6 cases were alive,1 case died,3-year free survival rate was 100%(4/4).In 7 cases of stage Ⅲ,5 cased were alive,3-year free survival rate was 75%(3/4).Two cases of stage Ⅳ all died.Conclusions Surgery play a very important role in the treatment of hepatoblastoma in children.Chemotherapy has improved the resectability of the tumor.Multidisciplinary team treatment is an effective model for diagnosis and treatment on childhood hepatoblastoma.
8.Therapy of Rhabdomyosarcoma in Children

Objective To explore the management therapy of pediatric rhabdomyosarcoma.Methods Retrospective analysis of treatment of 29 cases with rhabdomyoscroma from Oct.1998 to Jun.2006.Primary sites included 5 cases of neck,1 chest wall,2 abdominal wall,12 retroperitoneal pelvic cavity and 9 extremity.Pathologic types included embryonal type 19(65.5%),alveolar 8(27.6%),pleomorphic 2(6.9%).According to IRS staging system they were Ⅰstage(n=2),Ⅱstage(n=6),Ⅲstage(n=9)and Ⅳstage(n=12).Most tumors were large than 5 cm(n=26).Results Twenty-seven cases were followed up but 2 of the cases were lost,19 cases got complete remission(4 cases relapsed or died after stopping treatment for 3 months to 3 years and 1 case relapsed after giving up treatment for 3 months),4 cases got partial remission(3 cases relapsed after 6 months diagnosed),4 cases got no response(3 of which deteriorated or relapsed and 1 case surved with tumor).Fourteen cases had constant remission between 5-77 months,averaging 22.9 months with average guitting time of treatment 16.9 months.Conclusion Total tumor resection,chemotherapy and radiotherapy play an important role in management of pediatric rhabdomyosarcoma.
9.Experimental study on the effects of butyrate on the apoptosis of colon cancer cell line

Objectives: To investigate the effect of butyrate, the by-products of diet fiber fermented in colon, on the apoptosis of human colon cancer cell and its mechanism for colon cancer prevention. Methods: The subculture of human colon cancer cell line SW1116 was cultured in medium without or with different concentration of butyrate(2、3、4、7 and 10 mmol/L). The cell proliferation rate and apoptosis were measured by MTT and flow cytometer or electro-microscopy respectively after 6、24、48 and 72 cultivation. Results: By the increasing of butyrate concentration and cell culture time, the proliferation of SW1116 cell was inhibited gradually with a highest rate of 59.19% during the study period. The percent of G 1 cell was increased gradually whereas the percent of S cell was decreased. The apoptosis rate of SW1116 was increased gradually with a highest rate of 30.62% at the same time. The ultrastructure of SW1116 cell was changed after butyrate was added to culture medium. Conclusions: Butyrate could not only inhibit the proliferation of colon cancer cell line but also induce it apoptosis.
10.Experimental study on the mechanism of butyrate inducing the apoptosis of colon cancer cell line

Objective: To investigate the mechanism of butyrate,which is the by-products of diet fiber fermented in colon and induce the apoptosis of human colon cancer cell. Methods: The subculture of human colon cancer cell line SW1116 was cultured in medium with different concentration of butyrate(0、2 、3 、4、7 and 10 mmol/L).The mRNA and proteins of bcl-2 or bax in cell lines were detected respectively by RT-PCR and Western Blotting while the concentration of p53 protein and the activity of caspase-3 was detected by flow cytometer and fluorescent quantitative assay respectively after 6、24、48 and 72h cultivation.Results: There was no expression of bcl-2 mRNA and its protein in SW1116 cell during the study periods but the expression of bax mRNA and its protein,the p53 protein concentration and the caspase-3 activity increased gradually when the butyrate concentration was increased and the cultivation time was prolonged. Conclusion: By up-regulating the expression of bax mRNA and its protein,butyrate induce the apoptosis of colon cancer cell on a p53 dependent way.
