2.Expression of phosphoinositide 3 kinase in rats with acute lung injury induced by oleic acid

Objective To study the expression of phosphoinositide 3 kinase (PI3-K) and its relationship with lung cell apoptosis in rats with acute lung injury induced by oleic acid. Methods 60 healthy male SD rats were divided into 5 groups randomly, with 12 rats for each group. 0.25ml/kg oleic was injected into the tail vein in 4 oleic groups in which the rats were respectively sacrificed after 1h, 2h, 4h, 6h and 24h by exsanguination. In the control group 0.25ml/kg of saline was injected intravenously and sacrificed 2h later. Arterial blood gas, left lung wet/dry ratio, lung index, protein content of BALF, and pulmonary permeability index (PPI) were determined. Fas-L expression of lung tissue was determined by immunohistochemical method and protein expression of PI3-K was determined by Western blot and immunohistochemical method. Results PaO_2 was lowered, and left lung wet/dry ratio, lung index, and protein content of BALF and PPI were increased obviously after oleic acid injection. Fas-L and PI3-K in bronchus epithelial cells, alveolar epithelial cells, pulmonary vessel endothelial cells and macrophage cells were highly expressed in the lung of ALI rats, and peak value of PI3-K appeared earlier than Fas-L. Conclusion Fas-L related apoptosis of bronchial epithelial cells, alveolar epithelial cells, pulmonary vessel endothelial cells and macrophage cells participated in the pathogenesis of earlier period of ALI through PI3-K transduction pathway.
3.Effects of dexamethasone on cell signal transaction system ERK and PI3-K of rat with acute lung injury

Objective The effects of dexamethasone on cell signal transaction system ERK and PI3-K were studied in acute lung injury (ALI) rat in order to provide a theoretical basis for clinical treatment. Methods 36 healthy male SD rats were randomly divided into 3 groups, 12 rats for each group: injured group, dexamethasone treated group and control group. 0.25ml/kg oleic acid was injected via caudal venous into each animal in the former two groups to form ALI models. Then, for dexamethasone treated group, each animal received 1.0mg/kg of dexamethasone 15 min after oleic acid injection; the same amount of saline was given to the animals in injured group, and the animals in control group were injected the same amount of saline at the last two times. All animals were killed by common carotid artery bleeding 2 hours after the last injection. The indexes of PaO2, wet/dry weight of left lung, pulmonary permeability and lung pathology were investigated. The expression of PI3-K, ERK and P-ERK were determined by Western blot and immunohistochemical method. Results PaO2 declined in the animals of injured group, while left lung wet/dry ratio and pulmonary permeability increased obviously, and pulmonary edema and transparent film were observed in injured lung pathology. Compared to injured group, those indexes were improved in dexamethasone treated group. PI3-K, ERK and P-ERK were highly expressed in bronchus epithelial cells, alveolar epithelial cells, pulmonary vessel endothelial cells and macrophage cells of the animals in injured group, the expression of PI3-K, ERK and P-ERK were reduced in dexamethasone group, but the expression in the both groups were still higher than that in control group. Conclusions The higher expression of PI3-K and ERK were involved in the regulation of earlier period of ALI, and dexamethasone may improve ALI by repressing the high expression of PI3-K and ERK.
4.Change in drug resistance of Staphylococcus aureus.

OBJECTIVETo analyze the change in drug resistance of Staphylococcus aureus (SAU) in the PLA general hospital from January 2008 to December 2012, and to provide solid evidence to support the rational use of antibiotics for clinical applications.
METHODSThe SAU strains isolated from clinical samples in the hospital were collected and subjected to the Kirby-Bauer disk diffusion test. The results were assessed based on the 2002 American National Committee for Clinical Laboratory Standards (NCCLS) guidelines.
RESULTSSAU strains were mainly isolated from sputum, urine, blood and wound excreta and distributed in penology, neurology wards, orthopedics and surgery ICU wards. Except for glycopeptide drugs, methicillin-resistant Staphylococcus aureus (MRSA) had a higher drug resistance rate than those of the other drugs and had significantly more resistance than methicillin-sensitive Staphylococcus aureus (MSSA) (P < 0.05). In the dynamic observation of drug resistance, we discovered a gradual increase in drug resistance to fourteen test drugs during the last five years.
CONCLUSIONDrug resistance rate of SAU stayed at a higher level over the last five years; moreover, the detection ratio of MRSA keeps rising year by year. It is crucial for physicians to use antibiotics rationally and monitor the change in drug resistance in a dynamic way.

6.Abnormity rates of blood lipids and uric acid and their relationship analysis in health examination population in Yangzhou city

Objective To investigate the abnormity of blood lipids and uric acid ,and to analyze their relationship in health exami-nation population over 18 years in Yangzhou .Methods Totally 28 202 individuals receiving the health examination from October 2011 to August 2012 in the physical examination center of this hospital were enrolled in our study .Total cholesterol(TC) ,triglycer-ide(TG) ,low density lipoprotein cholesterol(LDL-C) ,high density lipoprotein cholesterol(HDL-C) and blood uric acid(UA) were detected ,and the relationship between age and sex with the abnormity of blood lipids and uric acid and the relationship between UA levels and blood lipids were analyzed .Results Among the detection population ,the detection rates of abnormal TG ,abnormal TC , abnormal HDL-C ,abnormal LDL-C and hyperuricemia were 31 .5% ,32 .1% ,15 .9% ,18 .4% and 17 .3% respectively .Compared with females ,males had the higher rates of abnormal blood lipids and UA (P<0 .05) .The rate of abnormal blood lipids was in-creased with age increase before 50 years old ,the > 50 -70 years old group had the highest abnormity rate of blood lipids ,then which was gradually decreased after 70 years old .The morbidity rate of hyperuricemia was increased with age .Compared with nor-mal UA group ,the hyperuricemia group had the higher rate of abnormal blood lipids (P<0 .01) .Conclusion The health examina-tion population over 18 years have the higher abnormity rate of blood lipids and uric acid ,which is significantly correlated with age and sex ;the increase of uric acid has certain correlation with abnormal blood lipids metabolism .Both blood lipids and uric acid are the risk factors of cardiocerebralvascular diseases ,rational dietary structure and periodic physical examination have the important significance to prevent the occurrence of these diseases .

9.Coronary stent placement in myocardio bridge

Objective To investigate the methods and efficacy of coronary stent placement in patients with myocardial bridge. Methods 4 patients with angina and ST-T changes in ECG were involved in the study. Coronary angiography showed there were 75%~90% systolic stenosis of LAD in all patients. Two of them were regarded as isolated myocardial bridge, and the others were regarded as myocardial bridge associated with atherosclerosis. The length of artery pressed by myocardial bridge was in the range of 10~18 mm. All patients who had repeated angina accepted stenting therapy. One of the stents was Crossflex, three were Nir stents. Stents size: (3.0~3.5)?(12~20)mm. All stents were expanded with 8~10 atm for 30~40 seconds. Results After stenting, coronary angiography showed no residual stenosis. In the follow-up 6~18 months there were no complaints of angina. In two of them, coronary angiography showed there were no restenosis after one year.Conclusion Coronary stenting is an optional method to treat patient who have myocardial bridge and have no efficacy after taking medicines, especially for the patients who have both myocardial bridge and atherosclersis. But futher study in large populations is still necessary.
10.Quantification of posterior capsular opacification after cataract surgery

AIM: To evaluate posterior capsular opacification (PCO) with hydrophobic acrylic intraocular lens (IOL, Sensar AR40e) and silicone IOLs after cataract surgery, to use a software program developed to provide an objective assessment of the amount of PCO in the digital images of the posterior capsule to quantify PCO. METHODS: Ninety-eight eyes underwent standardized phacoemulsification and "in the bag" IOL placement, were randomized to receive a three piece lens of hydrophobic acrylic or silicone, but lens materials were different in one case. In year 1 and 2, digitized retro-illumination images were taken from the posterior capsule. Images were analyzed by POCO software program, removing the Purkinje light reflexes, contrast enhancement, filtering to enhance low-density PCO. RESULTS: The percentage of PCO were 0.32±0.13 of hydrophobic acrylic IOLs in year 1, compared with 0.39±0.17 of silicone (P=0.37). In year 2, the percentage of PCO were 0.42±0.20 with hydrophobic acrylic IOLs and 0.34±0.18 with silicone IOLs (P=0.50). Of those patients with PCO in year 1 and 2, severity grades were 0.50±0.30 and 0.82±0.58 of hydrophobic acrylic cases, compared with 0.63±0.35 and 0.55±0.35 of patients with silicone IOLs (P=0.52,P=0.69) with no statistical significance.CONCLUSION: The POCO system is capable of producing an objective and repeatable measure of PCO that is relevant to assessing techniques of PCO prevention.
