1.Determination of Serum Concentration of Deoxyschisandrin by RP-HPLC

OBJECTIVE:To determine the serum concentration of Deoxyschisandrin by RP-HPLC.METHODS:The chr-omatographic separation was performed on Phenomenex C18(250mm?4.6mm,5?m) column with column temperature at 30℃.The mobile phase consisted of methanol - water (80∶20) at a flow rate of 1mL?min-1. The detection wavelength was set at 252nm. RESULTS:The calibration curve of Deoxyschisandrin was linear in the concentration range from 0.52 to 33.28?g?mL-1 (r=0.999 6). The average recovery of deoxyschisandrin was (97.27?4.11)%(RSD=4.23%,n=9).CONCLUSION:The method is simple, accurate and sensitive, and it can be used for the determination of serum concentration of Deoxyschisandrin.

Objective To study into the influence of Ze Xie and Dan Shan root on the immunity regulation and the expression of radical in mice with chronic pelvic infection. Methods The model of mice with chronic pelvic infection was established by putting phenol into the endometrium of the mice. The density change of serum IL -2, TNF, SOD and MDA of the mice was measure by the method of radiation immunity and the method of biochemistry. 4.Practice and effect evaluation of gestational diabetes mellitus specialist practice model

Objective To evaluate the outcomes after implementing gestational diabetes mellitus(GDM) specialist practice model. Methods GDM specialist practice model contained four modules(first visit,follow-up visit,hospital-ization,postpartum),eight aspects of practice contents which included specialist evaluation,diet & exercise survey, diet & exercise prescription,GDM health guidance,weight management during pregnancy. Pregnancy outcomes were compared before and after GDM specialist practice. Results GDM specialist practice model was able to signifi-cantly decrease insulin utilization rate,reduce number of hospitalized patients due to poor blood sugar control,short-en length of hospitalization. Conclusion GDM specialist practice model can effectively save medical costs,and GDM specialist nurses play an important role in management of women with GDM.
5.A clinical research of hirayama disease

Objective To summarize and analyze the clinical features of Hirayama disease.Methods The authors summarized the clinical data of 29 cases of Hirayama disease in recent 9 years.Resuits All of the 29 cases were male and the age of onset was 12-25 years.22 of the 29 patients only had the symptoms of one upper Jimb and7 of them had symptoms of beth upper limbs.Most of them had symptems on the right or the symptoms were on more severe in the right than left.while only one case had symmetric symptoms of both upper limbs.The arm appeared as a slope,this was due to partial muscular atrophy.EMG indicated that the impairment of spinal anterior cells were limited to the arm relevant segments and both sides were involved.Cervical MRI scan suggested spinal atrophy at the level of C5-7 vertebral bodies.being asymmetric in most of the cases.The atrophy was more obvious on the side with more severe symptoms.There was one patient whose MRI showed high T2 signal in the spinal anterior cells of the C5-6 segments.All the cases used cervical supporter and were followed up for 1-5 years.All of them had good prognosis.Conclusions Hirayama disease is more common in men with hidden onset before 20 years of age.Most of the patients noted the symptoms of hands gradually about 2 years after a period of rapid growth in height in adolescence.The predominant clinical manifestations are as follows:atrophy of lower cervical cord in cervical MRI,asymmetric spinal atrophy in horizontal impaction and the degree of atrophy consistent with the symptoms of limbs.EMG could indicate bilateral spinal anterior impairment in lower cervical cord.Rare case may present the abnormal signal in the spinal cord,but pyramid signs were absent.

ObjectiveTo analyze the neurological manifestations of chronic mercury poisoning for the improvement of clinical understanding of this disease.MethodsThe clinical data of 8 cases diagnosed as chronic mercury poisoning admitted in the department of neurology of Xuanwu hospital during the past 5 years were collected and analysed.Results Neurological manifestations of chronic mercury poisoning involved psychological problems,sleep disorders,ataxia,extremities weakness and atrophy,tremor,peripheral neuropathy and paresthesia.ConclusionsThere are various kinds of neurological manifestations of chronic mercury poisoning.Identification of these manifestations in clinical practice and early diagnosis are beneficial for the relief of pains in the patients.
7.Utility of a multiprobe fluorescence in situ hybridization assay in the detection of bladder cancer

Objective To evaluate the clinical application of a muhiprobe FISH (multicolor fluorescence in situ hy-bridization,M-FISH) assay in voided urine specimens for detection of bladder cancer and compare the results with those afforded by urinary cytology.Methods Voided urine specimens from 100 cancer patients with or without surgery and 10 patients with prostate hyperplasia were obtained for urinary cytology and FISH.FISH was performed using a mixture of fluorescent labeler DNA probes for the centromeric regions of chromosomes 3,7,and 17 and 9p21 region.Cystoscopy with biopsy or tumor resection was performed in all patients and compared the pathological results with the cytological and FISH findings.Results The sensitivity of M-FISH were 75.6% for low grade tumors,100% for high grade tumors,with overall sensitivity of 85.5%.Urinary cytology affords an overall sensitivity of 62.9% (33.3% for low grade tumors,96.0% for high grade tumors).Significant difference in overall sensitivity was observed between M-FISH and urinary cytology (P<0.05).The specificity of M-FISH and urinary cytology was 84.6% and 87.8% respectively.No significant difference in specificity was observed between M-FISH and urinary cytology.Conclusion M-FISH improves the sensitivity obtained with urinary cytology for bladder cancer detection with similar specificity,so can detect all high grade infiltrating tumors patients.
8.Diagnostic value of joint detection of biomarkers for the early renal impairment in multiple myeloma patients

Objective To discuss the diagnostic value of serum cystatin-C(Cys-C) and urinary retinol binding protein(RBP) for the early renal impairment in multiple myeloma(MM) patients .Methods According to estimated glomerular filtration rate(eGFR) , the MM patients were divided into two groups :the normal renal function group(eGFR≥90 mL/min ,n=78)marked group A ,the renal impairment group(eGFR<90 mL/min ,n=40)marked group B .130 healthy subjects were selected as control group .The con-centrations of serum Cys-C ,urinary RBP ,serum creatinine(Cr) and serum urea nitrogen(UN) were measured and compared .Results The levels of Cys-C and RBP in group A were significantly higher than those in the control group(P<0 .01) .There were signifi-cant difference of the levels of Cys-C ,Cr ,UN and RBP levels between group B and control group(P<0 .01) .The sensitivity of the combined detection of Cys-C and RBP was 57 .69% ,significantly higher than single index test(P<0 .05) .Conclusion Compared with traditional biomarkers ,Cys-C and RBP are more sensitive and effective indexes for the diagnosis of early renal impairment in the patients with MM ,and jointed detection of the two biomarkers has important value .

Objective To investigate the role of WNK4 gene Exon8 Ala589Ser polymorphism in the pathogenesis of essential hypertension. Methods This study included 259 hypertensive patients and 235 normotensive patients as control subjects. Exon8 Ala589Ser polymorphism in WNK4 was investigated by PCR-RFLP and confirmed by direct sequencing. Genotype, genotype frequencies and allele frequencies of the polymorphism were compared among patients with essential hypertension and control subjects. At the same time, some clinical biochemical variables, such as the blood sugar and lipids in both groups. All of these results were analyzed with Logistic regression analysis to determine whether the risk of hypertension is associated with the polymorphism and the determined biochemical variables. 10.Accuracy of des-γ-carboxy prothrombin(DCP) for diagnosis of primary hepatocellular cancer: a Meta-analysis

Objective To carry out a systematic evaluation for the diagnostic accuracy of Des-γ-carboxy-prothrombin(DCP) in primary hepatocellular cancer(PHC) by Meta-analysis.Methods The published international and domestic studies on DCP in the diagnosis of PHC were searched in multiple databases from its inception until December 2016.A total of seventeen studies were finally selected,among which 1 970 cases of PHC group and 2 588 cases of control group were included.The control group was composed of chronic hepatitis,cirrhosis,tumors in other systems and healthy subjects with physical examination.A meta-analysis was performed using Stata 12.0.Results The overall pooled results of the analysis and the 95% confidence intervals were as follows:the sensitivity(SEN) was 0.79 (0.74 to 0.83),the specificity (SPE) was 0.90 (0.87 to 0.93),the positive likelihood ratio(PLR) was 8.1 (5.8 to 11.1),the negative likelihood ratio(NLR) was 0.24(0.19 to 0.29),the diagnostic odds ratio(DOR) was 34(21 to 55) and the area under the summary receiver operator characteristic curve (SROC) was 0.91 (0.89 to 0.93).The sensitivity combined the detections of AFP and DCP in the diagnosis of PHC was 0.90 with 0.94 of AUCROC and 56 of DOR.Conclusion The serum DCP may exhibit a relatively higher diagnostic specificity for PHC.The combined detections of AFP and DCP could improve the sensitivity and diagnostic accuracy.
