1.Progression of Sentinel Lymph Node Biopsy in Breast Cancer

Objective To review the recent studies about sentinel lymph node biopsy in breast cancer. Methods The literatures in recent years on the history, concept, technique and clinical application of sentinel lymph node biopsy were reviewed and summarized.Results There was no unified method for sentinel lymph node biopsy. There was a wide range of detection rate and false negative rate.Conclusion Prospective multi center random clinical trials will help to evaluate the clinical application of sentinel lymph node biopsy.
2.Analysis of causes of death of child - bearing women in Shenyang

Objective To find out about the causes of death of child-bearing women in Shenyang so as to provide basis for the improvement of child-bearing women's reproductive health. Methods A retrospective statistical survey into the death reporting system of Shenyang was conducted and a comparison was made between various age groups of child-bearing women from 1996 to 2000 with regard to the trends of changes in death, the rank order of causes of death and deaths due to malignant tumors. Results From 1996 to 2000, the death rate of child-bearing women in Shenyang was in a state of fluctuation. The case-death ratio in urban areas was 67.24 per 100 thousand people while the case-death ratio in rural areas was 45.97 per 100 thousand. Urban women aged 35 to 49 accounted for most of the deaths among child-bearing women. The rate of death due to cancers in urban areas was markedly higher than that in rural areas. The deaths of child-bearing women due to mammary and reproductive system cancers accounted for one fourths of deaths caused by cancer, with their incidence rate in rural areas being higher than in urban areas. Conclusion More work needs to be done to improve the healthcare of child-bearing women.
3.Diagnosis and managment of 30 cases of phyllodes tumor of breast

Objective To explore the diagnosis and treatment of phyllodes tumor of the breast. Methods Clinical data from 30 patients with phyllodes tumor confirmed surgically and pathologically were analyzed retrospectively. Results Of the 30 cases of phyllodes tumor, there were 16 cases of benign tumor, 9 cases of borderline tumor and 5 cases of malignant tumor according to the standard histological criteria. Local mass excision, total mastectomy, modified radical mastectomy and tumor mass excision were performed in 8, 8 and 13 patients, and 1 patient, respectively. A follow up of 20 cases for 1-24 years found 1 death, but all the others survived. Four patients out of the 20 patients underwent total mastectomy or modified radical mastectomy due to tumor recurrence. Conclusion The preoperative diagnosis of phyllodes tumor is difficult. Intraoperative frozen section examination of the suspicious lesions should be conducted. Multiple slices of the specimen should be performed. Surgical options are closely associated with the prognosis. Wide local excision or total mastectomy is recommended in surgical options.

BackgroundThere are a lot of studies about the wound healing charateristics of cornea after SBK with femtosecond laser.In our study,mechanical microkeratome was preferred for corneal flap.We observed the proliferation of keratocytes by investigating the myofibroblast ( MF) activity.Objective The study was to compare the morphologic and histological changes in the cornea after sub-Bowmans keratomileusis ( SBK) with photorefractive keratectomy ( PRK) and laser in situ keratomileusis ( LASIK)by investigating the express of transforming growth factor-β( TGF-β)and α-smooth muscle actin( α-SMA)and to investigate the wound healing characteristiCs of cornea after SBK.MethodsTwenty-seven adult New Zealand white rabbits were randomly assigned into group A,B and C.SBK waa performed on the right eyes of each rabbit in group A,LASIK for group B and PRK for group C.All the left eyes were used as the normal control group.Histological examinations by light microscopy were performed on day 7,1 months,3 months after surgery.The expression of α-SMA and TGF-β and the number of activated MF were assessed by immunohistochemiatry.ResultsIn SBK group,corneal epithelium cells proliferation around the wound was seen and the numbers of active fibroblasts were increased after surgery.The expression of α-SMA or TGF-β around the corneal flap and the corneal 8troma started at day 7 postoperatively and peaked at 1 months and decreaaed around the corneal flap and the corneal 8troma started at day 7 postoperatively and peaked at 1 months and decreaaed t3mnh.TFβep( SBK:t=2.226,2.158,2.330,P<0.05;PRK:t=4.745,6.524,6.293,P<O.05).The numbersof activ( SBK:t=2.226,2.158,2.330,P<O.05;PRK:t=4.745,6.524,6.293,P<0.05).The numbersof activatedMFs were different fromLASIKstatisticallytoobetweenSBKgroupandPRKgroup ( SBK:t =4.439.5.692,4.175,P<0.05 ; PRK:t=6.330,6.723,5.267,P<0.05 ).Theα-SMAand TCF-βexpressionsin SBKgroupwerelessthan PRK group but more thanLASIKgroup( TCF-β:t =4.691,5.527,t =4.399,P<0.05 ; α-SMA:t =9.637.10.282,8.197,P<0.05).The numhers of MFs in SBK group was less than PRK group before 3 months and were same at 3 months ( t =5.188,4.529,P<0.05 ).Conclusions ComparedwithconventionalLASIK,SBKcanup-regulatethe expression of α-SMA.TGF-β,activated MFs in the corneal flap.which enhance corneal biomechanics and promote healing.However,most of the disadvantages caused by wound healing in SBK still remain compared to PRK.
5.A Study on Relationship between iNOS and p53 Expression in Human Prostatic Carcinoma

Objective To investigate the association of inducible nitric oxide synthetase (iNOS) expression with the expression of mutation type p53 (mt p53) in prostatic carcinoma (PCa). Methods The expression levels of iNOS and mt p53 were in 28 PCa specimens and 10 cases of benign prostatic hyperplasia (BPH) were detected by immunohistochemical method. Results The expression levels of both iNOS and mt p53 in PCa were higher than those in BPH. The expression of iNOS was related to the expression of p53 protein. Conclusion iNOS and mt p53 may play a role in the pathogenesis of PCa.
6.Congenital hypothyroidism characterized by hydrops fetalis:one case report

Objective To explore the differential diagnosis of hydrops fetalis and the rare presentations of neonatal congenital hypothyroidism. Methods The data of one congenital hypothyroidism diagnosed neonate with hydrops fetalis leading to birth asphyxia and respiratory failure were retrospectively analyzed. The relevant literatures were reviewed. Results A Uyghur female infant by cesarean delivery at gestational age of 38+5 week for intrauterine distress, presented general edema with cyanosis and dyspnea after birth. Trachea cannula was used to assist ventilation. At one-day old, the thyroid function examination showed that the serum thyroid stimulating hormone was>100 mU/L and the free thyroid was 6 . 56 pmol/L. Moreover, ultrasonographic examination indicated the thyroid aplasia. The clinical symptoms were improved after the treatment with the levothyroxine tablets replacement, and breathing machine was removed at 8-day old. The dosage of drug was adjusted by clinical manifestation and laboratory monitoring. The patient was discharged at 18-day old with the medicine and was followed-up. Conclusions Congenital hypothyroidism can be the pathogenesis of hydrops fetalis and its differential diagnosis should be paid attention.
7.Hospital infection and its risk factors in elderly acute leukemia patients with multifactor logistic-regression analysis

Objective To investigate the characteristics and risk factors of hospital infection in elderly patients with acute leukemia and provide basis for its prevention and treatment. Methods The hospital infection rate,infection sites,pathogenic bacteria and infectious factors of 116 elderly patients with acute leukemia between January 1999 and January 2008 were analyzed retrospectively,and compared with groups of non-elderly patients in the corresponding period. Results The incidence of hospital infection [62.9 %(73/116)]and death[52.1 %(38/116)]in elderly patients were higher than that of non-elderly ones (P <0.01). The most commonly infective sites were oral cavity and respiratory system. The multiple and serious infection in elderly patients was more than that of non-elderly noes. The common pathogenic bacteria were mainly grant-negative one. The results of multivariate aralysis showed that the absolute neutrophil count in peripheral blood,cycles of chemotherapy,latest infections,stages of treatment,length of hospitalization and seasons on hospitalization were found as independent risk factors for hospital infection in elderly patients with acute leukemia. Conclusion The incidence of hospital infection and death in elderly patients was high. The absolute neutrophil count in peripheral blood,cycles of chemotherapy,latest infections,stages of treatment,length of hospitalization and seasons on hospitalization are independent risk factors for elderly patients with acute leukemia.
8.Analysis of long-term survival related elements in acute promyelocytic leukemia

Objective To investigate various factors influencing long-term survival of the patients with APL. Methods The clinical data of 62 cases with APL were analyzed retrospectively. Univariate and multivariate analysis of the potential factors influencing survival and prognosis were carried out with Log-Rank and Cox regression method, including sex, age, initial WBC count, initial Plt count, the level of LDH,first induction regimen, length from induction therapy to CR, and post-remission therapy. Results 62 cases were followed up for 6 to 102 months. 5-year OS and relapse-free survival (RFS) were (77.1±6.2)% and (71.4± 3.68)%, respectively.Univariate analysis revealed that initial WBC count, first induction regimen, length from induction therapy to CR and post-remission therapy were important prognostic factors for long-term survival. Multivariate analysis demonstrated that initial WBC count and post-remission therapy were associated with RFS and OS. Conclusion The post-remission therapy combining ATRA, As2O3 and chemotherapy would significantly improve the long-term survival of APL patients entering CR.
9.Construction and implementation of quality control index for clinical safety of Chinese medicine injection.

In order to ensure the authenticity and accuracy of traditional Chinese medicine injection safety monitoring data, Chinese medicine injection safety monitoring quality control indicators, including the monitoring center, monitoring personnel, hardware conditions, monitoring progress and the number of patients into the group, original documents and archives management, electronic data, adverse events, quality management were constructed. Its application in the creation of major new drugs technology major projects, 10 kinds of traditional Chinese medicine injections clinical safety monitoring quality control work, found the missing case surveillance, not reported adverse events, only reported adverse reactions, electronic data reporting lag, lack of level of efforts to control the problem, and corrected, the traditional Chinese medicine injection safety monitoring of quality control and quality assurance, and subsequent Chinese medicine safety monitoring quality control to provide the reference.

10.Clinical observation on acupuncture combined with tuina manipulation for thoracic facet joint disorder

Objective:To observe the clinical effect of acupuncture combined with tuina manipulation for thoracic facet joint disorder. Methods:A total of 93 eligible cases were randomly allocated into an observation group and a control group. Patients in the observation group (n=46) were treated with acupuncture combined with tuina manipulation, whereas patients in the control group (n=47) were treated with tuina manipulation alone. The treatment was done once a day, for a total of 3 times. Therapeutic efficacies were then evaluated according to scores on signs and symptoms.
Results:After treatment, there were intra-group statistically significant differences in scores of eight signs and symptoms (all P<0.01); and the between-group differences were statistically significant in scores of eight signs and symptoms (all P<0.05). After three times of treatment, the recovery rate in the observation group was 54.4%, versus 25.4% in the control group, showing a statistical significance (P<0.05).
Conclusion:Acupuncture combined with tuina manipulation can obtain better effects than tuina manipulation alone for thoracic facet joint disorder and is therefore worth further clinical application.
