1.Diagnosis and surgical operation treatment for posted sides chronic subdural hematoma in the adult patients

Objective To explore the curative efficacy,methods and prevention of complications of surgical operations treatment of posted sides chronic subdural hematoma in the adult patients.Methods The clinical data of 11 adult patients suffered from posted sides chronic subdural hematoma were analyzed retrospectively.The patients were all treated by different surgical operations.Results The hematomas about 60mL to 110mL were all cleared away by operation.Postoperative hematoma removal or basic cleared.All patients were cured to discharge.11 patients were followed up for 3 -6 months.According to the activities of daily living (ADL)grading system,all patients were classified as normal.Follow -up examination with CT/MR scans in 11 patients showed 10 cases of normal findings, 1 case with the contralateral limitations mild subdural effusion,by coned cranial indwelling drainage tube turn better. Conclusion Reasonable choice different surgical operations of hematoma is suited to adult patients with posted sides chronic subdural hematoma,providing a satisfactory effect and reduce the complicated syndrome and recurrence rate.
2.Variance and resource of intracellular free calcium and extracellular arachidonic acid of host cells during the invasion of Toxoplasma gondii

Objective To explore variance and resource of intracellular free Ca2+ and extracellular arachidonic acid (AA) in different types of passage cells during the invasion of T. gondii. Methods The variance and resource of extracellular AA and intraceUular free Caz+ of Vero and J774A. 1 cells during the invasion of T. gondii were detected by multi-purpose scintillation counter and laser scanning confocal microscope. Data were analyzed using Chi-square test and t test. Results The intracellular free Ca2+ levels in J774A. 1 and Vero cells were both increased after T. gondii infection. The maximal changes of fluorescence intensity were (1 219.7±58.4)% (P<0.01) and (356.3±23.6)% (P<0.05), respectively.The increase of intracellular Ca2+ level in Vero cell was mostly from the release of intracellular Ca2+ store.And the Ca2+ increase in J774A. 1 cell was from both the release of intracellular Ca2+ store and extracellular Ca2+ influx. Extracellular AA levels were significantly increased in both Vero and J774A. 1 cells after T.gondii infection 5.02 and 8. 44 times respectively (t= 3. 124, t = 3. 852, P<0.01). The AA elevations could be significantly inhibited by phospholipase A2 (PLA2) inhibitor pretreating T. gondii. Conclusions The phospholipase C of phagocytic host cell and PLA2 of T. gondii are activated by T. gondii infection,which results in the increase of intracellular free Ca2+ and extraeellular AA level. Combined actions of Ca2+and AA play a major role in the invasion of T. gondii to host cell. While only PLA2 of T. gondii may be activated in nonphagoeytic host cell.
3.Biological characteristics and primary therapeutic response of acute erythroid leukemia

Objective To observe the biological characteristics and analyse primary therapeutic response of acute erythroid leukemia.Methods The data of 28 patients primarily diagnosed as acute erythroid leukemia were analyzed.The patients were divided into with muhilineage dysplasia group and without muhilineage dysplasia group,and the morphology,immunology,cytogenetics and molecular biology characteristics and the complete remission rate of the first induction therapy were compared.Results There were 14 cases(50%)with muhilineage dysplasia,which involved in two lineage or trilineage.In 6 cases by flow cytometry,the myeloid blast immunophenotypes were common expressed.In 8 cases detected by karyotype analysis,5 cases were chromosomal abnormal,including 4 cases were complex chromosomal abnormal,1 case was trisomy 8.In 4 cases underwent WT1 detection,all of them were positive.The complete remission rate of the first induction therapy was 39.29%(11/28),the ratein the multilineage dysplasia group was 35.71%(5/14),and the ratein without multilineage dysplasia group was 42.86%(6/14),the difference had no statistical significance(P>0.05).The complete remission rate of the complex chromosome group was 25.00%(1/4),the intermediate prognostic group was 50.00%(2/4).Conclusions Acute erythroid leukemia had special biological features different from other subtype AML:accompanyed with high frequency of multilineage dysplasia.The abnormality of karyotype were high,and it was often complex karyotype involved with chromosome 5 and/or chromosome 7,which had a low complete remission rate.The complete remission rate of chemotherapy was low,treatment effect was poor.
5.A new aurone glycoside with antifungal activity from marine-derived fungus Penicillium sp. FJ-1.

Endophytic fungi which reside in the tissue of mangrove plants seem to play an important role in the discovery of new biologically active substances. During the course of screening for the antimicrobial metabolites from the endophytic fugus Penicillium sp. FJ-1 of mangrove plant Avicennia marina, a new aurone glycoside (1) was isolated by repeated column chromatography on silica gel and recrystallization methods. The structure of 1 was elucidated as (Z)-7,4'-dimethoxy-6-hydroxy-aurone-4-O-β-glucopyranoside, on the basis of spectroscopic analysis. Compound 1 exhibited antifungal activity against Candida sp., with the potency comparable to amphotericin B and much better than fluconazole. Compound 1 can also inhibit extracellular phospholipase secretion in a concentration-dependent manner.

7.Diagnostic and therapeutic advancement of maple syrup urine disease

Maple syrup urine disease (MSUD) is an autosomally recessively inherited disorder of branched-chain amino acid (BCAA) metabolism caused by the defective activity of branched-chainα-ketoacid dehydrogenase complex (BCKD). The disease is characterized by severe ketoacidosis, mental retardation, and neurological impairments. So far, application of tandem mass spectrom-etry and HPLC has allowed newborn screening and early detection, but some patients with variant forms of the disorder will escape detection. Liver transplantation is an effective treatment. However, the rareness of liver donation limits the liver transplantation. In this review, the screening, diagnosis and therapy of MSUD will be discussed.
9.Research on LIU Wan-su’s characteristics on application of han and wen drugs in Xuanming Lunfang by prescription metrology

Objective: To study the nature and flavour of LIU Wan-su’ drug application and the tendency of cold or warm in his prescription; analyze LIU Wan-su’s medical sect by the index of prescription metrology. Methods: Combining the traditional documents with prescription metrology, selecting three doctors before or after LIU Wan-su in Jin and Yuan Dynasty as comparison to expose the characteristic of prescription metrology in his prescription. Results: Wen tonic frequency of each prescription on average, LI Dong-yuan(1.10),ZHU Dan-xi(0.97), LIU Wan-su (0.77), ZHANG Zi-he(0.65); Han tonic frequency of each prescription, LI Dong-yuan (1.34),ZHANG Zi-he (1.00), LIU Wan-su (0.94), ZHU Dan-xi(0.80);Cool and warm drug ratio of four sects in Jin and Yuan Dyanstay, ZHU Dan-xi (0.97),LI Dong-yuan(0.83), LIU Wan-su (0.82), ZHNAG Zi-he (0.58).Cool and warm prescription ratio, ZHANG Zi-he(1.50), LIU Wan-su (0.87), LI Dong-yuan(0.76), ZHU Dan-xi (0.50).Conclusion: As the exponent of "theory of fi re and hot", it will be better to classify LIU Wan-su as " Hejian sect" since his drug application was not limited within cold and cool. To some extent, it is more important to judge a Chinese traditional doctor by syndrome identifi cation rather than only by his drug frequency.
10.Application of detection of C-reactive protein, serum glucose concentration and APACHE Ⅱ grade in systemic inflammatory response syndrome

Objective To study the value of detection of C- reactive protein(CRP), serum glucose concen-tration and APACHE Ⅱ grade for estimating inflammation, pathogenetic condition and prognosis of systemic inflam-matory response syndrome (SIRS) patients.Methods 50 SIRS patients in ICU were selected and were divided into S1 and S2 subgroup according to SIRS diagnosis standard, and 18 patients developed MOBS.30 patients without SIRS, were selected as control group.Detection of CRP, serum glucose concentration and APACHE Ⅱ grade were per-formed.Results CRP(76.19±11.98 mg/L vs.14.04±5.70 mg/L) ,serum glucose concentration(7.10±1.30 mmol/L vs.5.32±1.09 mmol/L) and APACHE Ⅱ grade (20.06±6.39 vs.7.90±3.54) of SIRS group were higher than that of non-SIRs group (P<0.01).CRP, serum glucose concentration and APACHE Ⅱ grade all are higher in group S2 than that of group S1 (91.25±9.89 mg/L vs.62.29±10.38 mg/L,8.32±2.40 mmol/L vs.7.10±1.69 mmol/L,26.72±5.14 vs.18.37±5.70).MOBS incidence and case fatality in S2 subgroup (12/15 and 6/35) were higher than that of S1 subgroup(10/15 and 3/35 ) (P<0.01 ).Conclusion CRP, serum glucose concentration and APACHE Ⅱ grade are increased in SIRS patients and with their increasing, MOBS incidence and case fatality were also increasing.CRP, serum glucose concentration and APACHE Ⅱ grade are significant factors in estimating SIRS patients pathogenetic condition and their prognosis.
