1.Research advances of M3 receptor: a new target for treating and preventing cardiovascular disease.

Cardiovascular disease, with high morbidity and mortality, has been threatening the health of human beings. Therefore, expecting to find a more effective therapeutic method, a plenty of researchers devote themselves to the study of the cardiovascular disease all the time. Since discovered on the heart, M3 receptor of muscarinic acetylcholine receptor (mAchR, M receptor) became a new starting point of the research of the cardiovascular disease. With more and more investigation, many people found that M3 receptor could protect the heart from kinds of cardiovascular disease, which may make it a new hopeful therapeutic point. So, expecting to give support to the reference and encouragement for the study of disease related to M3 receptor in future, this review expounds M3 receptor on the heart from the main following aspects: the effect on the heart, the influence on the cardiovascular disease and the mechanism of M3 receptor involved.

2.Detect of P53,bax and caspase 3 genes expression after optic nerve injury in rats with SYBR green I fluorescence quantitative PCR

Objective Previous study showed that the histopathological basis of visual function damage caused by optical nerve injury is apoptosis of retinal ganglion cells(RGCs). This procedure is regulated by P53, bax and caspase 3 genes. Present study aimed to observe the expression of bax, P53 and caspase 3 mRNA in RGCs after traumatic optic nerve damage in the rats by SYBR green I fluorescence quantitative PCR method. Methods The animal model of optic nerve injury was established in the right eyes of 56 adult Wistar rats by a fluid percussion brain injury device (FPI) . Animal were killed on days 1, 3, 5, 7, 9, 14, 28 days separately after injury. Other 16 Wistar rats were divided into normal control group and sham operation group. The total RNA was isolated from rat fresh retina tissue by Trizol method and was treated by reverse transcription to cDNA using 01igo(dt) 18 as primer and then amplified. The target fragments of bax, P53 and caspase 3 cDNA were linked with carrier pTZ57 R/T to construct recombined plasmids which were transformated to E. Coli DH5α by T/A clone method. Recombined plasmids were extracted with alkaline lysis method and the plasmids were selected in white colonies by ampicillin screening, EcoR I restrictive enzyme analysis, and their specificity was evaluated using DNA sequencing. The standard curves were created by plasmid DNA and the precise expression level of target genes in samples were determined using software. The results were expressed as the ratios of target gene mRNA to GAPDH mRNA. Results The standard curve drawn by pTZ57R/T and target gene presented a good linear tendency with the higher sensitivity and specificity. The expression of P53 and bax mRNA began to increase on the third day after the injury of optic nerve and peaked on the fifth day and started to decline on the seventh day. The expression of caspase 3 mRNA increased from the fifth day through the ninth days after injury and declined on the fourteenth day. The significant differences were found in the expression of P53, bax and caspase 3 between model group and control group (P < 0. 05) . Conclusion The pro-apoptotic protein P53, bax and caspase 3 play an important role in RGCs apoptosis.
3.Contrast-enhanced ultrasonography, baseline ultrasound and spiral computed tomography in the characterization of portal vein thrombosis

Objective To compare the performances of baseline ultrasound(BUS),contrast-enhanced ultrasound (CEUS) and spiral computed tomography (CT) in characterizing of portal vein thrombosis (PVT).Methods The capability of differentiation of benign and malignant thrombi by using BUS,CEUS and spiral CT scan were assessed in 43 patients with PVT.Results The diagnostic accuracies of benign and malignant PVT were 100% with CEUS,60.5% with BUS and 76.7% with CT,respectively.There were significant difference between every two groups( P <0.05).Conclusions CEUS is superior to BUS and CT in distinguishing benign and malignant PVT,thus it is recommended.
4.Identification of different types of biliary strictures after orthotopic liver transplantation by hilar bile duct using ultrasonography

ObjectiveTo explore the feasibility of hilar bile duct using ultrasonography (US) in identification of different types of biliary strictures after orthotopic liver transplantation.MethodsThe US images of hilar bile duct of 62 patients with non-anastomotic biliary stricture (NAS) and 30 patients with anastomotic biliary stricture (AS) were reviewed.Four US feathers including the presence of hilar biliary dilatation,the display of hilar biliary lumen,the thickening of bile duct wall and the echo of bile duct wail were assessed.Results The presence of hilar biliary dilatation,the display of hilar biliary lumen,the thickening of bile duct wall showed a significant difference between NAS and AS groups (P<0.001).The feature with the greatest performance in the diagnosis of NAS was unclear displayed hilar biliary lumen.The sensitivity,specificity and accuracy were 93.7%,86.7% and 92.3%,respectively.Conclusions The observation of hilar bile duct has great vaule in identification of different types of biliary strictures after orthotopic liver transplantation.The display of hilar biliary lumen is more likely to differentiate between NAS and AS
5.Therapeutic Observation of Electroacupuncture at Extensors plus Acupoints Based on Syndrome Differentiation for Wrist-ankle Activity in Post-stroke Hemiplegia

Objective To observe the effect of electroacupuncture at extensors plus acupoints based on syndrome differentiation on the wrist-ankle activity in post-stroke hemiplegia. Method A total of 168 patients with post-stroke hemiplegia were randomly divided into a control group and a treatment group, 84 cases each. Both groups were prescribed with Western medication, based on which, the control group received electroacupuncture at acupoints based on syndrome differentiation, and the treatment group received electroacupuncture at extensors plus acupoints based on syndrome differentiation. The changes of electromyography (EMG) in the two groups after the intervention were observed, and the clinical efficacies were compared between the two groups.Results The total effective rate was 95.1% in the treatment group versus 91.0% in the control group, and the between-group difference was statistically significant (P<0.05). After the intervention, the EMG of extensor carpi radialis in the treatment group was significantly different from that in the control group (P<0.05); the between-group difference was statistically insignificant in comparing the EMG of tibialis anterior muscle (P>0.05), but the intra-group differences were statistically significant in both groups(P<0.05). The value of M-amp/H-amp was changed significantly in both groups after the treatment (P<0.01), but the between-group difference was statistically insignificant after the treatment (P>0.05).Conclusion Electroacupuncture at extensors plus acupoints based on syndrome differentiation can increase the motor unit of carpi radialis and decrease the spasm of gastrocnemius in post-stroke hemiplegia, and consequently produce a remarkable clinical efficacy.
6.Conventional ultrasonography and contrast-enhanced ultrasonography of normal rabbit biliary ducts

BACKGROUND:The experimental animal models can be used to in-depth investigate the effect of ultrasound and contrast-enhanced ultrasound in the diagnosis and treatment of ischemic biliary lesions. But there is no report on the phase timing standard of rabbit model contrast-enhanced ultrasound that used in various hepatobiliary diseases. OBJECTIVE:To investigate the ultrasonic appearance of normal rabbit biliary ducts with conventional ultrasonography and contrast-enhanced ultrasonography, and the criterion of contrast-enhanced ultrasonography phase timing of rabbit liver. METHODS:The conventional ultrasonography of hepatobiliary system was performed on 10 healthy New Zealand rabbits, and contrast-enhanced ultrasonography was performed on duct wal s that displayed in conventional ultrasonography, then the features were analyzed. RESULTS AND CONCLUSION:(1) The bile duct of normal New Zealand rabbits showed a portal to biliary caliber ratio of 3.59±0.54. (2) The detection rates of common and middle lobe bile ducts were significantly higher than that of lateral lobe bile ducts on both conventional ultrasonography and contrast-enhanced ultrasonography. (3) The phases of contrast-enhanced ultrasonography of normal rabbit liver were divided into arterial phase (10-20 seconds), portal phase (21-30 seconds) and late phase (31-180 seconds). The normal duct wal presented hyper-enhancing at arterial phase and persistent iso-enhancing at portal and late phases. The establishment of these parameters of normal New Zealand rabbits not only lays a foundation for the application of contrast-enhanced ultrasonography on biliary ischemia but also expands to other hepatobiliary models.

9.Clinical study on diagnosis and prognosis assessment of ischemic-type biliary lesion with contrast-enhanced ultrasound

Objective To evaluate the value of contrast-enhanced ultrasound (CEUS) in diagnosing ischemic-type biliary lesion (ITBL) and analyze the relation between the enhancement patterns of bile duct wall of ITBL and its outcome.Methods 36 patients confirmed with ITBL (24 cases),anastomotic biliary stricture (3 cases),cholangitis (4 cases),biliarysludge (1 cases),and acute rejection (4 cases),who underwent CEUS examination,were enrolled in this study.The images were retrospectively analyzed in consensus by 2 readers.After reviewing the images,the readers were asked to make a diagnosis of ITBL.The diagnostic standard was hypo-or non-enhancement of hilar bile duct wall in arterial phase on CEUS.Results The diagnostic sensitivity,specificity,accuracy,positive predictive value and negative predictive value were 66.7%,83.3%,72.2%,88.9%,and 55.6% for reader 1;62.5%,83.3%,69.4%,88.2%,and 52.6 % for reader 2,respectively.The interobserver agreement was good (κ =0.83).In 24 ITBL patients,the ratio of mortality or retransplantation with non enhancing hilar bile duct wall in arterial phase was much higher than that with enhancing hilar bile duct wall (non-enhancement 54.5%,hypo-enhancement 20%,hyper-or iso-enhancement 12.5%).Conclusions CEUS had diagnostic value of ITBL.Non-enhancing hilar bile duct wall in arterial phase on CEUS predicated the poor outcome.
10.The relationship of radiation dose and image quality with tube voltage selection for adult upper airway radiography

Objective To investigate the relationship between the tube voltage and radiation dose as well as image quality in adult upper airway digital radiography (DR).Methods We used CDRAD2.0 contrast details phantom and PMMA to simulate adult upper airway. With different tube voltages,the phantom was exposed using automatic exposure control system (AEC).The entrance surface dose( ESD),dose area product(DAP) and mAs in every exposures were recorded.The image quality factors(IQF) of all images were calculated. Results With tube voltage increasing,ESD,DAP,mAs decreased and IQF value enhanced.There were statistically significance ( F =45.15,26.41,29.26,56.53,P < 0.05 ).ESD,DAP,mAs significantly increased with tube voltage less than 75 kV,began to decrease when tube voltage more than 75 kV,tended to be on the balance in 75 -80 kV.At the same time,the fluctuation of IQF value was no statistical difference in 50 - 75 kV of tube voltage,but was statistical significance in 75 -90 kV( F =11.35,P <0.05 ).So,the image quality of adult upper airway with different tube voltage had no significant difference.Conclusions The appropriate tube voltage was 75 kV to 80 kV in the upper airway DR.The IQF value can be provided as the clinical evaluation index of image quality.
