0.05).Conclusions The serum MMP-9 can be tested as an objective index in appraisal the severity of the left to right shunt. The increased level of MMP-9 may be one of important mechanism for developing heart failure process in children with left to right shunt CHD.Monitor the level of MMP-9 will help us find adverse myocardial remodeling earlier and begin treatment immediately to prevent heart failure developing.

AIM: To explore the feasibility of culturing human umbilical vein endothelial cells ( HUVEC ) on acellular corneal stroma and performing the posterior lamellar endothelial keratoplasty ( PLEK ) treating corneal endothelial decompensation.
METHODS: Thirty New-Zealand rabbits were divided into three groups randomly, 10 rabbits for experimental group, 10 for stroma group and 10 for control group. Corneal endothelial cells were removed to establish animal model of corneal endothelial failure. PLEK was performed on the rabbits of experimental group and stroma group, and nothing was transplantated onto the rabbits of control group with the deep layer excised only. Postoperative observation was taken for 3mo. The degree of corneal edema and central corneal thickness were recorded for statistical analysis.
RESULTS:Corneas in experimental group were relieved in edema obviously compared with that in stroma group and the control group, and showed increased transparency 7d after the operation. The average density of endothelial cells was 2 026. 4±129. 3cells/mm2 , and average central corneal thickness was 505. 2±25. 4μm in experimental group, while 1 535. 6±114. 5μm in stroma group and 1 493. 5±70. 2μm in control group 3mo after operation.
CONCLUSION:We achieved preliminary success in our study that culturing HUVEC on acellular corneal stroma and performing PLEK for corneal endothelial decompensation. HUVEC transplanted could survive in vivo, and have normal biological function of keeping cornea transparent. This study provides a new idea and a new way clinically for the treatment of corneal endothelial diseases.

Objective To investigate the clinical value of artery-sparing high-site transvaginal subtotal hysterectomy(TVSH) for the treatment of uterine body leiomyoma. Methods High-site transvaginal subtotal hysterectomy with the preservation of bilateral uterine arteries was performed in 23 patients with uterine body leiomyoma from January 2002 to December 2004 in this hospital. The vesicouterine peritoneum was opened through the vagina to mobilize the uterus outside. With bilateral uterine arteries preserved, the muscular layer was opened 1 cm medial to the uterine horn until the site 3 cm above the isthmus of the uterus. Then wedge resection of the anterior and posterior uterine wall was conducted with the endometrium 2 cm in thickness preserved. Results All the operations were successfully completed without conversions to open surgery. The operating time was 45~80 min(mean,55 min),and the intraoperative blood loss was 80~250 ml(mean,150 ml).The time to first passing flatus was 6~27 h(mean,11 h).The length of postoperative hospital stay was 3~8 d(mean,4 d).The pyrexia rate after operation was 8.7%(2/23). Normal sexual life recovered within 3 months in 18 patients. Conclusions Artery-sparing high-site TVSH has advantages of high safety, effectiveness, minimal invasion, rapid recovery, and fewer complications.

Objectives:To study the impact of nutritional support in non operative management of severe acute pancreatitis. Methods:Sixteen patients with severe acute pancreatitis were provided with stage nutritional support. Results:Fourteen cases fully recovered and 2 cases died. Stage of maintenance equilibration was (57.0?12.5)hours,TPN was (12.1?4.2)days and PN+EN was (12.2?3.5)days. Conclusions:Stage nutritional support might be favourable in improving prognosis and survival rate of severe acute pancreatitis.

ObjectiveTo detect the effect of electroacupuncture combined with acupoint injection of deer melon polypeptide on the bone morpho-genetic protein synovial, BMPs and serum rheumatoid factor IgA-RF, IgM-RF.MethodsA total of 246 patients with Rheumatoid arthritis were divided randomly into control group and combined treatment group, 123 patients in each group. The control group received the treatment of electro-acupuncture, drug and Ultrashort wave therapy, and the combined treatment group continue to receive acupoint injection of deer melon polypeptide after 30 minutes when the treatment of control group was completed. Both groups were treated for 2 weeks. The expression of BMPs, MMP-1 and the ESR, serum rheumatoid factor IgA-RF, IgM-RF were detected. The morning stiffness time, Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and visual analogue scale (VAS) were recorded for outcome assessment.ResultsCompared with control group after treatment, the positive expression rates of ESR (8.79 ±1.61 mm/hvs.12.41 ± 2.57 mm/h,t=3.654), IgM-RF (22.38% ± 5.45%vs.39.57% ± 6.18%,t=2.365), IgA RF (32.84% ± 6.57%vs.49.84% ± 4.20%,t=2.574) in the combined treatment group significantly decreased. Compared with the control group, the expression of BMPs (15.43 ± 2.94vs.7.22 ± 1.52,t=3.914) in the combined treatment group significantly increased, while MMP-1 (8.01% ± 1.54%vs. 15.34% ± 2.47%,t=2.015) in the combined treatment group significantly decreased (P<0.05). Compared with control group, the outcomes VAS (3.37 ± 0.35 vs.5.75 ± 0.39, t=3.251), WOMAC (22.36 ± 3.49vs.30.50 ± 4.11,t=4.021), and morning stiffness (34.32 ± 5.57 minvs. 49.84 ± 4.20 min,t=3.914) in the combined treatment group significantly decreased (P<0.05). The total effect rate (82.11%vs. 71.55%,χ2=12.764) of combined treatment group was significantly higher than control group (P<0.05).Conclusion Electroacupuncture combined with acupoint injection of deer and melon peptide can improve effect rate.

OBJECTIVETo investigate the dosage-efficacy/toxicity relationship of prepared rhubarb, in order to explore the bidirectional effects in hepatoprotection and hepatotoxicity of prepared rhubarb and the objective authenticity for attenuating toxicity by processing.
METHODNormal and pathological animals were adopted simultaneous to investigate the effect of total extracts from prepared rhubarb within a high dose range (2.0, 5.4, 14.7, 40.0 g x kg(-1) x d(-1)) on normal state, biochemical index and histopathology of experimental animals. The factor analytic approach was used to analyze the dosage-efficacy/toxicity relationship of prepared rhubarb.
RESULTThe factor analytic approach was used to extract two common factors from the nine biochemical indexes. The firs common factor was mainly dominated by HA, LN and TGF-β1, and could be explained as fibrotic factors. The second common factor was mainly dominated by ALT, AST and ALP, and could be explained as cellular factor. The results of the factor analysis suggested that prepared rhubarb showed significant bidirectional effects in hepatoprotection and hepatotoxicity, which could protect liver in CC14 injured chronic hepatic injury, but had a certain hepatotoxic effect to normal animals. The pathological examination showed consistent results with the factor analysis. Under comparable dosages, prepared rhubarb showed a stronger liver protecting effect than crude rhubarb, with a lower toxicity.
CONCLUSIONAlthough prepared rhubarb has a certain hepatotoxic effect to normal animals, it has also a significant therapeutic effect to animals with liver injury. The results proved the symptom-based prescription theory and the scientificity of the symptom-based medication. The symptom-based prescription theory is important to correctly realize the dosage-efficacy/toxicity relationship of traditional Chinese medicines and guide the symptom-based medication.
