1.Research on the social determinants of child health inequalities

Objective:To investigate the impact of social determinants of health on the inequality of child health and health care utilization.Methods:Information of 1 118 children aged 0 to 16 is extracted from the Chinese Family Panel Studies(CFPS)year 2008 cross sectional survey data for the analysis.Age standardized concentration curves and concentration indices are employed to assess the inequalities for incidence of low birth weight,self reported good health,adequate timing of breast feeding,health insurance coverage and incidence of catastrophic health expenditures for these children.Concentration indices are decomposed to four levels of social determinants of health(community,family,mother and individual level)to understand their contributions to health inequality respectively.Results:There are health inequalities existing in the investigated children,among which social factors at mother and family level have the largest contribution.Conclusion:To respond to the call by the WHO to achieve health equity through action on the social determinants of health in a generation,the inequalities of health and health care utilization amongst Chinese children should be put on the policy agenda,and social policies should intervene from multiple social dimensions,especially from family and mother levels.
2.Alteration and significance of vascular endothelial growth factor in the serum of patients with glomerular diseases

Objective To study the concentration alteration of serum vascular endothelial growth factor (VEGF) in patients with primary and secondary glomerular diseases (GD), the effect of immune inhibitors on the concentration, and analyze the relationship between VEGF concentration and some clinical indexes. Methods Serum VEGF concentration of GD patients was determined by sandwich ABC-ELISA. Results All of the GD patients had higher level of serum VEGF than the control group. Of the 5 groups divided according to the clinical classification, patients of chronic glomerulonephritis(CGN) group, latent glomerulonephritis(LGN) group, Henoch-Schonlein purpura nephritis(HSPN) group and lupus nephritis(LN) group had higher serum VEGF level than the control group, respectively, with no difference among the above groups. The patients without taking immune inhibitors had higher serum VEGF level than the control group,while no difference was observed between the patients taking immune inhibitors and the control group.Patients with glomerulonephritis had a significantly lower serum VEGF level after receiving immunosuppression aggressive therapy than before. The serum VEGF level of nephrotic syndrome in nephrotic phase was higher than in remission stage. In membranous glomerulonephritis serum VEGF correlated positively with 24-hour proteinuria excretion. The serum VEGF level correlated positively with anti-dsDNA Ab titer and the concentration of ? globulins in LN. Conclusion Cytokine VEGF is involved in the pathogenesis of glomerulonephritis, and in the occurrence and progression of proteinuria. Immunosuppression therapy can inhibit the expression of VEGF. In LN patients, the serum VEGF level is related to LN activity.
3.Analysis on clinical effects and mechanism of compound Muniziqi granules combined with vitamin A acid cream in patients with chloasma

Objective To observe clinical effects of compound Muniziqi granules combined with vitamin A acid cream, and analyze its mechanism of action.Methods 76 cases of patients with chloasma treated in our hospital in March 2010 to February 2014 were selected.They were divided into two groups according to random number table method, 38 cases in each group.Observation group was given compound Muniziqi granules( three times per day,orally) combined with vitamin A acid cream(smear once respectively each morning and evening),control group was taken vitamin C 0.3 g and vitamin E 0.2 g ( three times per day, orally ) comined with vitamin A acid cream ( smear once respectively each morning and evening ) treatment.Observed clinical effects, scores of lesion area and color, detected serum estrogen level in both groups by radioimmunoassay. Results Compared with pre-treatment lesion area and color scores of observation group significantly reduced ( P<0.05 ).Compared with control group post-treatment, lesion area and color scores of observation group significantly reduced(P<0.05).Clinical effective rates of observation group and control group was 94.74%and 75.31%, respectively, the difference was significantly between two groups(P<0.05).Compared with pre-treatment, serum E2, FSH, LH levels of observation group were remarkably reduced(P<0.05).Compared with control group post-treatment, serum E2,FSH,LH levels of observation group significantly reduced(P<0.05).Conclusion The clinical effect of compound Muniziqi granules combined with vitamin A acid cream on chloasma is significant.Its mechanism may be relate to levels of estradiol, follicle-stimulating hormone and luteinizing hormone.
4.Clinical Observation on General Anxiety Disorder Treated by Acupuncture plus Herbal Medicine

To investigate the efficacy of the formula composed of the herbal drugs to soothe the liver, regulate qi, clarify the heart and eliminate vexation plus acupuncture for general anxiety disorder. Methods: Forty cases in the treatment group were treated with acupuncture plus herbal formula and 38 cases in the control group were treated with oral Doxepin. The assessment was conducted by Self-rating Anxiety Scale (SAS) and the Treatment Emergent Symptom Scale (TESS) before, during and after treatment. Results: The total effective rate was respectively 82.50% in the treatment group and 84.21% in the control group, with a significant difference in SAS assessment (P<0.01) between the two groups before and after the treatment.There were no significant differences in the curative and remarkable effective rate, total effective rate and SAS assessment between the two groups (P>0.05), but the side effect was higher in the control group than in the treatment group (P<0.01). Conclusion: Acupuncture plus herbal formula can have a precise effective effect for extensive anxiety neurosis with mild toxic side effects.
5.Effects of 2,3,4',5-tetrahydroxystilbene-2-O-?-D glucoside preventative administration on expression of nitric oxide synthase in atherosclerotic rats

Aim To investigate the effects of TSG on the content of nitric oxide and the expression of endo-thelium nitric oxide synthase in artery vessels of experimental atherosclerotic rats.Methods Sixty male rats were randomly divided into six groups.GroupⅠ,Normal control;GroupⅡ,Model control;Group Ⅲ,TSG low dose;Group Ⅳ,TSG middle dose;GroupⅤ,TSG high dose;group Ⅵ,Simvastatin.After 12 weeks,blood samples were collected from carotid arteries of rats,and the levels of NO in serum were measured.After that,the aortas were separated from the bodies and placed in-70℃,then the content of nitric oxide synthase was detected,and gene expression of eNOS and iNOSmRNA in artery vessels were respectively measured with RT-PCR method.STATA 8.0 software was adopted in date analysis..Results compared with those of the model group,the level of NO,the activity of NOS and the gene expression of eNOS were increased,and the gene expression of iNOS was reduced by simvastatin and TSG 30,60,120 mg?kg-1?d-1 in the high cholesterol-fed rats,which showed a dose-dependent effect.Conclusion TSG can enhance the expression of eNOS gene and reduce the expression of iNOS gene in aorta vessels of experimental atherosclerotic rats,which may be one of the anti-atherosclerosis mechanisms of TSG.
6.Meta-analysis of Domestic Sparfloxacin for Acute Bacterial Infections with Oral Administration

OBJECTIVE:To assess the clinical efficacy and safety of domestic sparfloxacin for acute bacterial infections.METHODS:Domestic literatures about sparfloxacin for acute bacterial infections were retrieved by computer and their quality was evaluated to extract data(1993~2009).RevMan 4.2.2 software was used for Meta-analysis.RESULTS:A total of 10 RCT were enrolled.The comparisons of 2 groups were homogeneous in terms of clinical cure rate,clinical response rate,bacterial clearance rate and safety.There was statistical significance in comparison of combined effect variable between 2 groups in respect of cure rate,response rate and bacterial clearance rate(P0.05).CONCLUSION:The currently available evidence shows that clinical efficacy of domestic sparfloxacin for acute bacterial infections is better and incidence of ADR was lower.
7.Comparison of different front plastic angle with reinforced plastic endotracheal tube guidance by McGrath -5 video laryngoscope for orotracheal intubation in obesity

Objective To compare the different front plastic angle with reinforced plastic endotracheal tube guidance by McGrath -5 video laryngoscope for orotracheal intubation in obesity.Methods 1 20 cases with obesity undergoing general anesthesia,ASA gradeⅠ -Ⅲ who planned intubation guidance by McGrath -5 video laryngo-scope,were randomly divided into three group according to different front plastic angle for endotracheal tube:group A (catheter 60°),group B(catheter 75°),C group(catheter 90°),40 cases in each group.General information and air-way evaluation indexes such as Mallampati classification,the degree of open mouth,neck circumference,thyromental distance,atlanto -occipital joint stretch degree preoperatively were recorded.C /L classification with laryngoscopic exposure,the successful rate of first intubation,time of first intubation,cases of second intubation,the incidence of blood stained catheter and postoperative sore throat and hoarseness were recorded also.Results There were no signif-icant differences of patients with general information and airway evaluating indexes such as Mallampati classification, the degree of open mouth,neck circumference,thyromental distance,atlanto -occipital joint stretch degree among three groups(P >0.05).The successful rate of catheter alignment glottal was 97.5% in group B,which was signifi-cantly higher than that of group A(80.0%)and group C(85.0%)(χ2 =8.36,P <0.05).The time of first intuba-tion was 21 (1 0 -65)s in group B,which was significantly shorter than that in group A[30(1 4 -78)s,Z =3.87,P <0.05]and group C[32(9 -80)s,Z =4.32,P <0.05].All patients in three groups had successful intubation firstly or secondly.The incidence rate of blood stained catheter in group B(2.5%)was significantly lower than that in group A(1 0%)and group C(1 5%,P <0.05 ),but there was no significant difference in postoperative sore throat and hoarseness among three groups(P >0.05).Conclusion McGrath -5 video laryngoscope which guided endotracheal tube with front plastic angle at 75°degree has highest success rate of intubation,shortest intubation time,least compli-cation and is suitable for the application of tracheal intubation in obese patients.
8.Prokaryotic Expression of eBCMA-Fc Fusion Protein, a Soluble BLyS Receptor, with Computer-aided Molecular Design in E. coli

B cell maturation antigen (BCMA) is a receptor of B lymphocyte stimulator (BLyS). Human IgG1Fc fusion proteins with the extracellular domain of BCMA(eBCMA), also called decoy receptors, have beenused as a potential BLyS antagonists to block BLyS activities. In order to design novel BLyS antagonistpeptides, computer-aided homologue modeling was used to construct an eBCMA-Fc fusion protein based on thecrystal structures of BCMA and Fc fragmant. To ensure the activity of eBCMA not to be interfered by Fcfusion, the root mean square distance (RMSD) for eBCMA and Fc were calculated to be 0.036 nm and 0.064nm, respectively, based on molecular docking modeling. An eBCMA-Fc fusion gene was constructed andintroduced into E. coli for expression. As expected, the purified 36 kD eBCMA-Fc fusion protein was able tobind BLyS in vitro at a dosage-dependent manner and demonstrated an anti-proliferative activity induced byBLyS in Daudi cells. The results have provided useful information on the evaluation of computer modeling andthe in vitro biological activity for the design of potential BLyS antagonist peptides.
9.A Novel Monoclonal Antibody With a Mono-specificity for a 46 ku-Cytokeratin

A mAb T2-2 was generated using hybridoma techniques, and its target was identified as a 46 ku-cytokeratin (CK), based on biochemical study and a completely overlapped binding pattern of mAb T2-2 with anti-pan-CKs antibodies. An epithelia-specificity of the mAb T2-2 was determined by screening 68 human normal and 65 tumor tissues using immunohistochemistry. Unlike most of anti-CKs antibodies, the mAb T2-2 recognized a mono-specific epitope only expressed on the 46 ku CK, suggesting that mAb T2-2 is superior to most anti-CKs antibodies that cross-reacted with many different kinds of CKs. In addition, it was found that the mAb T2-2 was multipurpose with a broad applicability to ELISA, immunohistochemistry, immunofluorescence, Western blotting, and was also compatible with various fixation reagents. These results strongly indicate that the mAb T2-2 has potential applications for studying CKs function and for diagnosis of tumor and other disorders.
