1.Study on the mutagenesis effects of low-dose sodium arsenite by Ames test

Objective To test whether sodium arsenite can induce in vitro reverse mutation of Salmonella typhimurium histamine-auxotroph mutant. Methods Ames test was carded out with Salmonella typhimurium strains TA97,TA98,TA100 and TA102 by standard method with or without the liver microsomal enzyme activation system (+S9,-S9). Results At concentrations of sodium arsenite from 500.00 to 5000.00 μg/plate, no colonies were seen on the plates of TA97,TA98,TA100 or TA102, with or without the presence of S9. At concentrations of sodium arsenite of 0.01,0.10,10.00 μg/plate and with the presence of S9, twice as many colonies grew on the plates of TA102 as the negative control(P<0.05). Without S9 activation,twice as many colonies grew on the plates of TA100 as the negative control(P<0.05)at concentrations of sodium arsenite of 1.00,10.00 μg/plate(P<0.05). The reverse mutation colonies induced by sodium arsenite in TA98 strain were twice as many as negative control group at concentrations of 0.01,0.10 μg/plate(P<0.05). There was no obvious increase of the strain clones in the other(P0.05). Conclusions With and without S9 activation, the doses of 500.00,5000.00 μg/plate sodium arsenite resulted in a toxic effect and a reduction of the revertants among the strain. At concentrations of 0.01~10.00 μg/plate, sodium arsenite exhibited mutngenesis effects.
2.A case-control study on the risk factors of neural tube defects in Shanxi province.

OBJECTIVETo explore the risk factors of neural tube defects (NTDs) in Shanxi province where the incidence of NTDs had been the highest in China.
METHODS1:1 matched case-control study was used. All the objects collected from hospitals were investigated in standardized questionnaires about susceptible risk factors for NTDs and were genotyped for methylenetetrahydrofolate reductase (MTHFR) C667T polymorphism by PCR-RFLP method. The risk factors of NTDs were analyzed by conditional logistic regression and by SPSS 11.5 statistical software. We also analyzed the univariable and multi-variables in order to independently investigate the MTHFR genotype and maternal periconceptional adverse factors influencing on NTDs.
RESULTSComparing MTHFR gene frequency between case group and control group, our results showed that three genotypes were found in MTHFR C677T. There was significant difference between two groups at 0.01 level (chi2 = 14.61, P = 0.001) about the three genotypes. The MTHFR T allele frequency of mother with NTDs (60.6%) was higher than that of the control (41.4%), and there was significant difference between them (chi2 = 14.59, P < 0.001). By univariate conditional logistic regression analysis, it was indicated that 15 factors were correlated with NTDs (P < 0.05) in the level alpha = 0.05. By multi-variables conditional logistic regression analysis, four factors were connected to NTDs: frequency of pregnancy (OR = 2.87, 95%CI: 1.28 - 6.44), contacting chemical combination in early pregnancy (OR = 16.18, 95%CI: 1.18 - 221.59), frequent taking of germinated potato (OR = 4.66, 95%CI: 1.78 - 12.17) and MTHFR C677T mutation (OR = 2.13, 95%CI: 1.08 - 4.21).
CONCLUSIONFactors as 'frequency of pregnancy', 'history of contacting chemicals', 'taking germinated potatoes in early pregnancy', as well as 'MTHFR C677T mutation' in mothers were important risk factors affecting the occurrence of NTDs in Shanxi province.

3.Long-term survival analysis in 89 adult patients with acute myeloid leukemia of fusion gene aml1/eto positive.

This study was aimed to investigate various factors influencing long-term survival in adult AML patients with fusion gene aml1/eto positive. A single institutional retrospective study with long-term follow-up was performed to better define the prognostic factors for AML patients with aml1/eto positive. Newly diagnosed 89 adult AML patients with aml1/eto positive were followed up for 1 to 42 months (median 24 months) from January 2004 to July 2008. Univariate and multivariate analysis of potential factors influencing survival and prognosis were carried out by using Log-Rank and Cox regression method, including sex, age, initial WBC counts, extramedullary leukemic disease, central nervous system leukemia (CNSL), chromosome aberrations, immunophenotype, first induction regimen, chemotherapy course to complete remission (CR), time from induction therapy to CR, negative or positive rate of aml1/eto and allogeneic hematopoietic stem cell transplantation and so on. The results showed that the estimated 5-year overall survival (OS) and relapse-free survival (RFS) were (50.0 +/- 2.3)% and (47.0 +/- 1.9)% respectively in follow-up of 89 patients for 1 - 42 months (mean 24 months). Univariate analysis revealed that initial WBC counts, CNSL, chemotherapy course to CR, time from induction therapy to CR, persistent negative in remission and allogeneic hematopoietic stem cell transplantation were important prognostic factors for long-term surviva1. Multivariate study demonstrated that initial WBC counts, CNSL, CD56 positive, negative or positive rate of aml1/eto, time from induction therapy to CR, persistent negative result of RT-PCR assay in remission and allogeneic hematopoietic stem cell transplantation were all critical factors in relation to OS and RFS. It is concluded that Chinese adult AML patients with fusion gene aml1/eto positive have some different characteristics as compared with patients from other countries, a relatively poor outcome is observed in patients, HSCT should be recommended to adult AML patients.

4.Research progress of rivaroxaban drug metabolism and gene polymorphism

Rivaroxaban is one of the new oral anticoagulants (NOAC) for preventing stroke and systemic embolism in patients with non-valvular atrial fibrillation. It has clear pharmacokinetic parameters, stable plasma concentration, less drug-drug interaction and higher compliance of patients. However, the discrepancy of pharmacokinetics between individuals and drug-induced hemorrhage events frequently occur clinically, therefore the association of gene polymorphism with drug metabolism has become a research hotspot. This article reviews the research progress on pharmacokinetic characteristics of rivaroxaban and its relationship with gene polymorphism, to provide a reference for the individualized rational use of rivaroxaban.
5.A case control study of risk factors for lung cancer

Objective:
To analyze the risk factors for lung cancer and to provide scientific evidence for prevention of lung cancer.
Methods:
A total of 461 newly diagnosed patients with primary lung cancer from January 2015 to June 2017 in Shaoxing were selected as the case group. Six hundred healthy residents with the same residence,distribution of gender and age as the cases were selected as the control group by frequency matching method. A questionnaire survey was conducted to collect demographic characteristics,family history of major chronic diseases,disease history and behavioral factors. The method of case-control study and multivariate logistic regression model were used for statistical analysis of the risk factors for lung cancer.
Results:
The results of multivariate logistic regression analysis showed that after adjusting for age and gender,family history of cancer(OR=6.606,95%CI:3.439-12.689),history of lung diseases(OR=2.836,95%CI:1.208-6.659),high level of stress(OR=2.485,95%CI:1.830-3.376),low consumption of fresh vegetables(OR=3.116,95%CI:2.470-3.930)and smoking(OR=6.029,95%CI:3.716-9.783)were risk factors for lung cancer;after adjusting for age,gender,family history of cancer,history of lung diseases,stress levels and consumption of fresh vegetables,quitting smoking for less than ten years was a risk factor for lung cancer(OR=4.751,95%CI:2.404-9.386)compared with quitting smoking for ten years or above.
Conclusion
Family history of cancer,history of lung diseases,high level of stress,low consumption of fresh vegetables and smoking were risk factors for lung cancer.
6.Hospital Infection Management in Outpatient Stomatology Department:Investigation of Current Situation

OBJECTIVE To understand the current situation of infection management in outpatient stomatology department of our city,and to further strengthen the hospital infection standardization management. METHODS The hospital infection management was investigated in outpatient stomatology department of 18 medical institutions. RESULTS The qualified rate of the city level hospital infection management was 75-100%,and that of the county area level was 40-80%.The disinfection and isolation were not standardized,the standardized prevention consciousness among medical staff was weak,and the hand cleaning and disinfection was insufficient. CONCLUSIONS The stomatology department has a hidden danger to emergence of hospital infection,its infection management should be further strengthened.
7.Clinical significance of combined detection with urinary nuclear matrix protein 22 and cytokeratin 20 mRNA in the monitoring of bladder tumor recurrence

Objective To detect the expression of urinary nuclear matrix protein 22(NMP22) and cytokeratin 20(CK20) mRNA in the monitoring of bladder tumor recurrence ,and to explore the clinical value of combined detection of urinary NMP22 and CK20 mRNA in the monitoring of bladder tumor recurrence .Methods Enzyme‐linked immunosorbent assay(ELISA) and nested reverse transcriptase polymerase chain reaction(Nested RT‐PCR) were used to detect the expression of NMP22 and CK20 mRNA in the u‐rine of 46 patients with recurrent bladder cancer tumor (recurrent group) ,66 patients without recurrent tumor (no‐recurrent group) and 40 healthy volunteers(control group) respectively .Results The expression of NMP22 and CK20 mRNA in the urine of control group were negative .There was statistically significant difference between the positive expression rate of NMP 22 and CK20 mRNA in the urines of the recurrent group[78 .3% (36/46) ,80 .4% (37/46))] and that of the no‐recurrent group[6 .1% (4/62) , 6 .1% (4/62)] and the control group respectively (P<0 .01) .There was no statistically significant difference between the no‐recur‐rent group and the control group(P>0 .05) .The expression level of NMP22 in recurrent tumor increased obviously along with the progression of the pathological grade and clinical classification of tumor (P<0 .05) .The positive rate of CK20 mRNA expression in the recurrent group increased obviously along with the increase of the tumor number (P<0 .05) .The sensitivity and specificity of NMP22 for monitoring bladder cancer recurrence were 78 .3% and 93 .9% ,of CK20 mRNA were 80 .4% and 93 .9% .Combined de‐tection of both sensitivity and specificity were 95 .7% and 92 .4% .Statistical significance was observed between combined sensitivi‐ty and that of single detection(P<0 .05) .No significant difference was observed between combined specificity and that of single de‐tection(P>0 .05) .Conclusion The positive expression of CK20 mRNA may also indicate tumor multiplicity .Combined detection of NM P22 and CK20 mRNA significantly increases the sensitivity and specificity in clinical detection .
8.Changes of Expressions of CD_(40) and CD_(40) Ligand to Graft-Versus-Host Disease after Allogeneic Hematopoietic Stem Cell Transplantation

Objective To explore the changes of CD40 and CD40 ligand(CD40L) levels and investigate their significances in children with graft-versus-host disease(GVHD)after related-donor human leukocyte antigen(HLA) matching allogeneic hematopoietic stem cell transplantation(HSCT).Methods Nineteen patients with ?-thalassemia major and 1 patient with congenital inherent hemolytic anemia accepted umbilical cord blood transplantation(UCBT) and allogeneic peripheral blood stem cell transplantation(allo-PBSCT),respectively,and all cases were received successfully related-donor human leukocyte antigen matching allogeneic HSCT.Peripheral blood samples were obtained before and after transplantation,the time when GVHD happened,the expressions of CD40 and CD40L were measured by using immunofluresence asssy.Results Four UCBT children and 3 allo-PBSCT children had no acute GVHD.Thirteen children had acute GVHD(degreeⅠ-Ⅳ),the expressions of CD40L on CD4+ and CD8+ T cells in the patients with acute GVHD increased,especially in allo-PBSCT.Five cases of UCBT and 12 cases of allo-PBSCT patients had chronic GVHD,the expressions of CD40L+,CD25+ and CD69+ on CD4+ and CD8+ T cells in patients with chronic GVHD increased obviously.The expression of CD19+CD40+ was lower than normal within 3 months after transplantation.Conclusions The high expression of CD40L+T cells in periferal blood after HSCT was related to the activation and proliferation of T cells in the development of GVHD in HSCT.
9.Effects of sensitized sera on bone marrow transplantation in a murine model

Objective:To investigate the effects of sensitized sera on bone marrow transplantation and clarify the role of antibody in the process of rejection.Methods:Two hundred microlitres sensitized sera or non-sensitized sera were injected into normal BALB/c one day before transplantation.Ten millions (1?107) bone marrow cells from C57BL/6 were transfused to the recipients after lethal irradiation.The donor-reactive antibodies in recipients before transplantation were measured by complement-dependent cytotoxicity (CDC) method.Moreover,the survival analysis and engraftment evaluation of the recipients were carried post transplantation.Results:The CDC results showed that donor-reactive antibodies existed in the recipients which had received sensitized sera transfusion.Eighty percent (80%) of the recipients received sensitized sera transfusion died of bone marrow failure about 10 days post transplantation,while the recipients received non-sensitized sera transfusion were long-term alive.Furthermore,the hematopoietic recovery and percentage of donor chimera analysis declined along with time in the sensitized sera transfusion recipients,and there were significant differences compared with those in the non-sensitized sera transfusion recipients (P
