Objective To evaluate the effect on postoperative urinary and sexual function of the radical operation with autonomic nerve preserving for mid low rectal cancer.[WT5”HZ]Method [WT5”BZ]Radical operation with autonomic nerve preserving was performed on 188 cases of rectal cancer. Nerves were fully preserved in 98 cases of Dukes A stage,one or both side nerves preserved in 78 cases of stage B, only S 3,4 nerve preserved in 12 cases of stage C.[WT5”HZ]Result [WT5”BZ]The postoperative voluntary micturition developed at 8 5 days for Dukes A; 9 0 days for stage B; and 16 8 days for stage C. Erection and ejaculation remained in Dukes A, in 96% and 100%;stage B, in 47 4% and 50%;stage C, in 0 and 0. The local recurrence after the operation accounted for 12 8%(24/188), among them Dukes A,B,C were 4 1%,21 9%, and 33 3% respectively.The 5 year survival rate were 77 6%,52 6% and 33 3% respectively.[WT5”HZ]Conclusion [WT5”BZ]The radical operation with autonomic nerve preserving for rectal cancer effects a better life quality not at the expense of local recurrence and reduction of 5 year survival rate.

Based on the associated theory of spleen-kidney deficiency about the traditional Chinese medicine and experimental research to discuss myasthenia gravis treated by integrative medicine.According to the theory of traditional Chinese medicine,the mechanism of myasthenia gravis is Qi deficiency of the spleen-stomach.Experimental studies showed supplementing spleen and kidney can improve ATP,muscle glycogen,fat content,other molecular biology material foundations,skeletal muscle cells under hypoxia and mitochondrial structure of animal models and patients,and therefore treat the deficiency of both spleen and kidney.

BACKGROUND: Now, the current study has not yet confirmed two treatment methods of the proximal femoral nail antirotation and total hip arthroplasty for treating intertrochanteric fractures in the elderly, which method has more advantages, and there is no clear conclusion at present. OBJECTIVE: To systematical y compare the repair effect of proximal femoral nail antirotation and total hip arthroplasty for treatment of intertrochanteric fractures in the elderly. METHODS: We searched the related literatures from 2011 to 2015 on Wanfang database, PubMed, MEDLINE and Embase database by computer. We retrieved the journals in China by hand and col ected randomized control ed trials on proximal femoral nail antirotation and total hip arthroplasty in the treatment of intertrochanteric fractures in the elderly. Intraoperative blood loss, operation time, hospitalization time, ambulation time after operation, and Harris score were selected as evaluation indexes. Data were analyzed using RevMan 5.2 software. RESULTS AND CONCLUSION: Final y, 7 Chinese literatures were included, with 811 patients. The results of meta analysis showed that the hospitalization time and ambulation time after operation were significantly shorter in the total hip arthroplasty group than in the proximal femoral nail antirotation group (P < 0.000 01), which reduced the pain of patients. Operation time and intraoperative blood loss were poorer in the total hip arthroplasty group than in the proximal femoral nail antirotation group (P < 0.000 01). No significant difference in Harris hip score was detected between the two groups (P > 0.05). These results confirmed that the internal fixation of proximal femoral nail antirotation in minimal y invasive therapy when reducing operation time and blood loss has a certain advantage. The total hip arthroplasty can make the recovery of hip function earlier, al ow early weight-bearing walking, reduce the patients’ bedridden time and reduce the complications in bed. Thus, the clinicians in the practical work should choose the appropriate therapy with considering the patient’s condition, damage degree, fracture type, and financial capability.

Background The special pathological change of diabetic retinopathy(DR)is microvascular disorder.Angiopoietin-like protein 2(Ang-2)is a new protein associated with genesis of blood vessels.Quercetin has multiple pharmacological action,including improving the microcircularion and the permeability of blood capillary.However,the action mechanism of Ang-2 on DR was unclear.Objective The present study was to investigate the effects of quercetin on Ang-2 and its receptor Tie2 expression in retina with diabetes mellitus.Methods Sixty clean male Wistar rats were randomized into 7 groups and 10 rats for each group,and 10 rats served as blank control group.Streptozotocin of 35 mg/kg was intraperitoneally injected in 60 rats to establish the diabetic models.Quercetins encapsulated by liposome with the doses of 50,150 and 250 mg/(kg · d)(3-5 ml)were used to gavage in different groups of models for 12 weeks,and normal saline solution and calcium dobesilate were used at the same fashion as the negative control group and positive control group,respectively.Twelve weeks later,the animals were sacrificed and retinas were isolated.Expressions of Ang-2 protein and Tie2 mRNA in retinas were detected by ELISA and RT-PCR,respectively.The usage and rearing of the animals complied with the Regulations for the Administration of Affair Concerning Experimental Animals by State Sciences and Technology Commission.Results ELISA showed that the A450 of Ang-2 in 150 and 250 mg/(kg · d)quercetin groups was 0.796±0.057 and 0.842±0.043 respectively and was lower than that negative group(1.012±0.046),showing statistically significant differences(q =2.95,2.698,P＜0.05).RT-PCR assay showed that expression of Tie2 mRNA(Tie2 mRNA/GAPDH mRNA)in retinas was 0.712±0.092 and 0.821±0.087,presenting statistically significant differences in comparison with negative group(1.182±0.098)(q =3.497,2.852,P＜0.05).The expression levels of Ang-2 and Tie2 mRNA in retina were lowest in 150 mg/(kg · d)quercetin group.Conclusions Quercetin can improve the retinal microcirculation by downregulating the expressions of Ang-2 and its receptor in early period of diabetic rats.

Objective To investigate the survival rate, voiding, sexual function after nerve-sparing surgery with extended systematic extraperitoneal lateral lymphadenectomy in lower rectal cancer. Methods Three hundreds and ninety-two cases with advanced lower rectal cancer who underwent nerve-sparing surgery with radical dissection from 1996 to 2000 were reviewed. Among them,173 cases only cleared in abdominal cavity,219 cases coupled with extraperitoneal lateral lymphadenectomy. Objective To investigate the influence of alprostadil on renal function,oxidative stress,and immune function in early diabetic nephropathy patients.Methods Eighty-six cases of early diabetic nephropathy patients in our hospital during April 2013 to January 2016 were chosen as research subjects,all patients were divided into observation (n =43) and control (n =43) groups according to random data form.Patients in observation group received alprostadil combined with pioglitazone treatment and those in control group received pioglitazone treatment only,both lasted for one month.Before treatment,one month after treatment,renal function indices were detected;serum oxidative stress indices were detected by automatic biochemical analyzer;T lymphocyte subsets in peripheral blood were measured by flow cytometry.Results Before treatment,there were no significant differences in renal function,oxidative stress,immune function between two groups (P ＞ 0.05).One month after treatment,peripheral blood renal function indices such as blood urea nitrogen (BUN),estimated glomerular filtration rate (eGFR),and cystatin C (CysC) levels were lower in observation group than in control group,the level of serum albumin (Alb) was higher in observation group than in control group,the difference was statistically significant (P ＜ 0.05);semm oxidation indices malondialdehyde (MDA),and advanced oxidation protein products (AOPP) were lower in observation group than in control group,human antioxidant indices such as superoxide dismutase (SOD),and total antioxidant capacity (T-AOC) were higher in observation group than in control group,the difference was statistically significant (P ＜ 0.05);peripheral blood CD3 +,CD4 + T lymphocytes and the ratio of CD4 +/CD8 + were higher in observation group than in control group,CD8 +T lymphocyte level was lower in observation group than in control group,the difference was statistically significant (P ＜ 0.05).Conclusions Alprostadil can optimize the renal function and immune function in early diabetic nephropathy patients,reduce the degree of oxidative stress.

Objective To study the relationship between the changes of ten elemental contents in serum and schizophrenia.Methods Calcium (Ca), magnesium (Mg), zinc (Zn), iron (Fe), aluminum (Al), arsenic (As), selenium (Se), copper (Cu), lead (Pb), manganese (Mn) in serum in the of schizophrenic patients and normal subjects are measured by inductively coupled plasma mass spectrometer and inductively coupled plasma emission spectrometer, compared the content of elements between the schizophrenia and normal.Results There were significant differences ( P<0.05) in other elements between two groups except lead.The contents of Ca,Fe,Mg,Zn,As,Se,Cu in the patients with schizophrenia were significantly lower than those in control, while Al, Mn were significantly higher than those in control.Most of elements in serum between case and control groups were correlated (P<0.05).Some of element ratios which have physiological significance were significant statistically (P<0.05), such as: Ca/Mg, Ca/Fe and so on.Conclusion These findings suggest that the patients with schizophrenia might have an abnormal elemental metabolism.Given the relationship among the elements, the element ratios are more valuable in reflecting the relationship between schizophrenia and elements.

Objective To develop an efficient recombinant expression system of alanine aminotransferase ( ALT ) in order to build the foundation for the preparation of feedstock related ALT reference materials.Methods A new human ALT gene was synthesized by optimizing the codons of the nucleic acid sequence encoding human ALT using bioinformatic tools, and then it was cloned into pRSF-Duet expression vector.The recombinant plasmid pRSF-Duet-ALT was transformed into E.coli BL21 and the target protein expression was induced by 2 mmol/L isopropyl β-D-1-thiogalactopyranoside ( IPTG ) .The expression condition for soluble protein was optimized by changing the inducer concentration, shaking speed and induction temperature. The soluble protein was purified by nickel ion affinity chromatography and dextran molecular sieve chromatography, and identified by sodium dodecyl sulfate-polyacrylamide gelelectrophoresis( SDS-PAGE) and Western blot analysis.The activity and stability of recombinant proteins in serum matrix under different storage conditions were detected.Results The usage frequency in E.coli of ALT codons was more than 10%after codon optimization.The expressions of soluble proteins were increased by optimizing the induced expression conditions, including a final concentration of 2 mmol/L of IPTG, and continued incubation with shaking at 150 rpm for 8 h at 25℃.The purified protein was identified as ALT by SDS-PAGE and Western blot with ALT activities of up to 80 000 U/L.Recombinant ALT could be stable for 2-8 d at 2-8 ℃or 25 ℃with a relative standard deviation of less than 5%.Conclusions An efficient recombinant expression system of ALT was developed successfully by codons optimization.The obtained recombinant protein could achieve the requirements of reference material feedstock.
