1.Clinical application of solution focused approach in the functional gastrointestinal disorders

Objective To application of solution focused approach in the functional gastrointestinal disorders.Methods 64 hospitalized clients with functional gastrointestinal disorders were randomly divided into the observation group and the control group with 32 cases in each group.We applied the normal nursing intervention to the control group,while applied the solution focused approach to the observation group.The anxiety and depression score,score of coping way and gastrointestinal symptoms were evaluated using t test.Results The scores of anxiety and depression,coping way and gastrointestinal symptoms were better in the observation group than those in the control group.Conclusions Adoption of solution focused approach can relieve FGID sufferers'negative emotion,such as anxiety and depression,increase sufferers' coping capacity and symptoms,which demonstrate pivotal significance for rehabilitation of functional gastrointestinal disorders.
2.Cyclooxygenase-2 and choroidal neovascularization

Choroidal neovascularization(CNV)is one of common causes of vision loss.The pathogenesis and development of CNV are a comprehensive process which is regulated by multiple factors and cytokines.Cyclooxygenase-2(COX-2)is an inducible isoform of cyclooxygenase and rate limiting enzyme in the prostaglandin biosynthesis pathway.COX-2 plays an important role in neovascularization by modulating vascular endothelial growth factor(VEGF),migration and apoptosis of endothelial cell.Recently,some experimental studies demonstrated that COX-2 involves in the information of CNV and the inhibitor of COX-2 can suppress CNV.These results provide a new prospect for the prevention and treatment of the CNV.Biological characteristics of COX-2 and its relationship with CNV are reviewed in this article.
3.Effect of nasal continuous positive airway pressure and Bi-phase positive airway pressure on oxygen exchange indexes in newborns with mild to moderate neonatal respiratory distress syndrome

Objective To observe the effect of nasal continuous positive airway pressure (nCPAP) and Biphase positive airway pressure (BIPAP) on the oxygen exchange indexes in newborns with mild to moderate neonatal respiratory distress syndrome (NRDS).Methods After infusion of pulmonary suffactant into lung bilaterally,116 cases with mild to moderate NRDS were treated with nCPAP(nCPAP group,n =75) and BIPAP(BIPAP group,n =41) respectively.Oxygen exchange indexes including arterial partial pressure of oxygen [pa (O2)],modified oxygenation index (P/F),alveolar-arterial oxygen tension difference (A-aDO2),respiratory index (RI),artery/alveolar oxygen partial pressure ratio[pa (O2)/pA (O2)] and pulmonary shunt fraction (Qs/Qt) were measured or calculated at 0 (before treatment),2,8 and 24 h after being treated with nCPAP or BIPAP.Results Six different oxygen exchange indexes of Pa (O2),P/F,A-aDO2,RI,pa (O2)/pA(O2) and Qs/Qt in both groups didn't show statistical significance at 0 hour:pa(O2) (kPa) (6.1 ±0.6 vs 6.1 ±0.6,t =0.11,P >0.05),P/F(kPa) (15.59 ± 1.45 vs 15.71 ± 1.45,t =1.59,P > 0.05),A-aDO2(kPa)(6.04 ±0.64 vs 6.24 ±0.69,t =1.59,P >0.05),RI(6.0 ± 1.0 vs 6.2 ± 1.1,t =1.35,P > 0.05),p,(O2)/pA(O2) (0.24 ±0.02 vs 0.25 ±0.03,t =1.63,P >0.05)and Qs/Qt(%)(11.9±1.6 vs 11.6 ± 1.6,t =1.10,P > 0.05).A-aDO2,RI,pa (O2)/pA (O2) and Qs/Qt of BIPAP group had more improvement than those of nCPAP at 2 and 24 h,and oxygen exchange indexes at 2 h of BIPAP group and those of nCPAP group were:A-aDO2 (kPa) (3.83 ±0.49 vs 4.24 ± 0.67,t =18.26,P < 0.05),RI(2.7 ± 0.5 vs 3.3 ±0.7,t =20.59,P < 0.05),p,(O2)/pA(O2) (0.35 ±0.03 vs 0.32 ±0.04,t =15.35,P <0.05) and Qs/Qt(%) (8.8 ± 1.6 vs 9.8 ±2.0,t =7.68,P < 0.05) ; 24 h indexes in BIPAP group and the indexes of nCPAP group were:A-aDO2 (kPa) (2.29 ± 1.19 vs 3.07 ± 1.67,t=18.43,P <0.05),RI(1.4 ±1.4 vs 2.3 ± 1.6,t=25.02,P <0.05),pa(O2)/pA(O2) (0.49 ±0.10 vs 0.42 ±0.11,t =14.96,P <0.05) and Qs/Qt(%) (6.5 ±3.0 vs 8.5 ±4.4,t =9.59,P <0.05).pa(O2) and P/F of both groups didn't show statistical difference significantly [Pa (O2)(kPa) (12.6 ± 1.0 vs 12.7 ± 1.0,t =3.76,P > 0.05),P/F(28.49 ± 3.17 vs 31.85 ± 2.85,t =3.76.P > 0.05)].Nineteen cases in nCPAP group needed invasive mechanical ventilation,and only 3 cases in BIPAP group underwent the procedure;there was a significant difference (x2 =4.01,P < 0.05).Oxygen exchange indexes in both groups were gradually improved from 2 h,8 h to 24 h,but 22 deteriorated cases showed no improvement,who underwent invasive ventilation.Positive end-expiratory pressure (PEEP) positive airway pressure (EPAP) was (0.42 ± 0.19) kPa in BIPAP group,and (0.56 ± 0.23) kPa in nCPAP group (t =3.45,P < 0.01).Conclusions 1.BIPAP and nCPAP can improve the ability of oxygen exchange in newborns with mild to moderate NRDS,but BIPAP is more effective than nCPAP,which can shorten the possibility of invasive ventilation and reduce the PEEP level.2.p,(O2),P/F,pa (O2)/pA (O2) and RI may be more suitable for evaluating oxygen exchange for neonatal NRDS.

The intestine microbiota and the host interacts with each other.Resently,the association and causality between gut microbiota and cardiovascular diseases have been well studied.Some underlying mechanisms,especially metabolic pathways are known, including the role of trimethylamine N-oxide in the pathogenesis of atherosclerotic heart disease and heart failure and that of short chain fatty acids and their receptors in blood pressure regulation.
5.Preliminary study of rhBMP-2 induced bone ingrowth at the interface into hollow implants

Objective In order to explore the possibility of bone ingrowth into a hollow prosthesis at the bone-implant interface, and the promoting effects induced by rhBMP-2/carrier compound as well. Methods Bone harvest chamber (BHC) modified form Albrektsson designation, was implanted into meta-physis of tibia of New Zealand rabbit to establish an animal model. 10 ?l rhBMP-2 and rhBMP-2/Healon GV compound, each of which contained 1mg rhBMP-2, were loaded into BHC respectively. The same vol-ume of Healon GV and injection water was also loaded into as control. The tissues in the BHC were harvest- ed at 2, 4 and 6 weeks after implantation respectively. Histological observation with non-decalcified slicing technique was adopted, and bone histomorphometry was analyzed by computerized image analysis system to calculate TBV (trabecular bone volume). The data was compared among the different groups. Results At two weeks after loading, there were woven bone formation in groups of rhBMP-2/Healon GV compound and rhBMP-2, and mature bone trabeculae with marrow-like tissue were observed at 4 weeks; At 6 weeks, new bone formation in the BHC became more intensive. However, the tissues in BHC of Healon GV and injection water at 4 weeks were just like those of the former two groups at 2 weeks, and mature bone trabeculae with marrow-like tissue could only been found until 6 weeks. Conclusion Bone at the interface can grow into the contained space of hollow prosthesis through holes at the wall, and form mature bone. rhBMP-2 loaded in the hollow space can obviously promote this effect and the carrier Healon GV has no negative influence but can increase bone inductive effect.
6.Study on the Spectrum-effect Relationship of Anti-inflammation Effect in vitro of Extracts from Uygur Medicine Hyssopus cuspidatus

OBJECTIVE:To provide reference for clarifying the pharmacodynamic material basis of anti-inflammation effect of Uygur medicine Hyssopus cuspidatus. METHODS:HPLC was conducted to establish the fingerprint of 8 extracts with different po-larities. Using macrophage RAW264.7 as object,the inhibitory effect of extracts with different polarities on inflammatory factor ni-tric oxide(NO),TNF-α and IL-6 in supernatant of cell culture medium was detected,the differences of anti-inflammatory activity in vitro were compared. Grey correlation analysis method was used to analyze the spectrum-effect relationship of peak area of common peaks and anti-inflammation activity. RESULTS:The fingerprint of 8 extracts with different polarities showed obvious differences. Anti-inflammation in vitro results suggested that 30% ethanol extract had the strongest inhibitory effect on inflammatory cytokines in vitro. There were 14 common peaks in the established HPLC fingerprints,the 5 common peaks that was closely related to the in-hibitory effect of inflammatory factors in vitro were peak 8,6,5,10,13,respectively;peak 8 was rosmarinic acid. CONCLUSIONS:The established fingerprint and its spectrum-effect relationship with anti-inflammation activity in vitro can provide certain reference for its pharmacodynanic material basis study.
7.Appliation of Six Sigma Management Method in Hospital Management

Objective To reasonably control the inventory and shorten the purchase time on the premise of ensuring clinical application. Methods By Six Sigma management method, the purchases process of equipment parts was improved in hospital so that the best approach for the equipment parts purchase was provided. Results The time to the purchase of equipment parts have an average reduction of 10.5 days. Conclusion There is a significant difference between the two groups (before and after the improvement). It shows that the Six Sigma is an effective method to improve accessory purchasing management processing for hospital.
8.Clinical study of occasio for antibiotic prophylaxis in hip arthroplasty surgery

[Objective]To study the best time for infusing antibiotics as prophylaxis against infection in hip arthroplasty.[Method]Ninty cases of hip arthroplasty were divided into 6 groups randomly with 15 cases each.Ampicillin(UNASYN,1.5g) and cefuroxime axetil(Zinacef, 2.0g) was administered by intravenous infusion respectively one day or one hour before operation or 30 minutes after the operation began.Blood samples from peripheral vein,cancellous bone of acetabulum when reaming,medullary canal of femur when rasping and catheter right after the operation were harvested.Serum Bactericidal Activities(SBAs) were determined by minim dilution technique and were compared between the groups.[Result]SBAs of UNASYN and Zinacef are similar to the bacteria most commonly causing infection after prosthesis implantation such as staphylococcus.Percentage at SBA≥1:8 of UNASYN and Zinacef is significantly more at the beginning of operation and when reaming acetabulum than the other groups(P
9.Biomechanical testing of neotype retropharyngeal cervical hook-plate for atlantoaxial instability

[Objective] To analyze the mechanics of neotype retropharyngeal cervical hook-plate(RCHP)for atlantoaxial instability with biomechanical testing.[Methods]Six fresh adult craniocervical specimens(C0~3)used for testing the biomechanical instability were all putted into following five test courses:intact,instability(Type ⅡOdontoid fracture),RCHP,Anterior C1、2 transarticular screw fixation,brooks.Repeated statistical analysis of the data was performed using the SPSS 13.0 software package.Measures were performed with SNK testing method to determine whether significant differences existed,P=0.05.[Results]Biomechanical testing showed that,compared with the controlled group and the injured group,each fixation course significantly decreased ROM in all test modes.In the ROM of flexion,there was no significant difference between RCHP and Brooks(P=0.525),but both of them was higher than anterior C1、2 transarticular screw fixation(P
10.Empirical study on intensity strength of weight-loading of a newly designed femur prosthesis with cage on the tension side

Objective To inspect the loading strength of a newly designed femur prosthesis with cage on the tension side for improving its design and clinical application. Methods A straight cementless femur prosthesis of total hip joint with cage on the proximal tension side was designed according to the geometry parameter of proximal femur of Chinese people. The concepts of proximal press-fit fixation, hypo-distal fixation, interface osteointegration and bone fusion with the cage were adopted in the prosthesis. Ti-6Al-4V forging workblanks were used in manufacturing with CAD/CAM technology. One prosthesis was taken randomly from finished products of each No.0, 1, 2, 4 group(with different) matched by (+0, +2) head with 26 mm diameter. The weight-loading test was done at 1 body weigth (BW), 3-8 BW respectively on INSTRON 4507 200kN univeral experiment machine universal material study machine with the prosthesis in a extraversion position of 18?. Results No plastic deformation and crack appeared when any of the prosthesis bear 1, 3-8 BW loading. The ultimate loading limit of No.1 and No.4 prosthesis was 57.6 kN and 81.8 kN respectively. Destroy first appeared at the stress collection area of the neck when the size of neck was relatively small. When the size of neck was big enough, break occurred at the stress collection area of the cage on the tension side. Conclusion The intensity strength of weight-loading of the femur prosthesis can not be destroyed even with a cage on the proximal tension side. It can endure the weight loading of hip in daily activities, and even strengthen due to the bony growing in proximal femur.
