1.Ongoing improvement of medical services for large-scale activities as guided by patient satisfaction

A satisfaction survey carried out by Beijing Chao-Yang Hospital during the Beijing 2008 Olympic Games, found differences of patient satisfaction between locals and overseas patients in consulting environment, outpatient procedure, quality of service, quality of diagnosis and treatment. The patient satisfaction of overseas patients was lower than locals. Especially noteworthy is the low satisfaction of the former for the hospital coloring, privacy in the sector, registration, vocal communication, patient condition statement, nurse attitude and service quality. Real-time improvement in these aspects of medical services has harvested obvious progress. Results of the satisfaction survey provided valuable reference materials for large-scale activities, and ongoing improvement of medical services in the long run.
2.Protein MG29 and skeletal muscle aging

OBJECTIVE: To review research advances of Mitsugumin-29 (MG29) protein and its relationship with skeletal muscle aging. DATA SOURCES: A computer-based online search of CNKI (http://www.chkd.cnki.net), Duxiu (http://edu.duxiu.com) and SSReader (http://edu.sslibrary.com) with key words of MG29 and skeletal muscle aging in Chinese; Elsevier SD (http:llwww.sciencedirect.com) and Springer Link (http:llspringer.metapress.comlhomelmain.mpx) with key words of MG29, aging skeletal muscle, sarcopenia. In addition, related articles were manually searched.DATA SELECTION: Inclusion criteria: location, structure and function of MG29; MG29 and skeletal muscle aging; MG29 and exercise; related articles aimed to the content with large impact factor. Exclusion criteria: repetitive studies or Meta analysis.MAIN OUTCOME MEASURES: MG29 has close relationship with skeletal muscle aging.RESULTS: In recent years, extensive researches have been conducted to discover the mechanisms underlying skeletal muscle aging by investigators home and abroad from histology, biochemistry and molecular biology. It is universally accepted that the changes in the cellular homeostatic capacity for Ca~(2+) is a core factor in regulating excitation contraction coupling, and it dynamic balance is the major cause of skeletal muscle aging. MG29, which is located in the membrane of transverse tubule, is a protein that is newly found to modulate Ca~(2+) signal resembling the synaptophysins. Studies have shown that the level of MG29 reduced 50% in old skeletal muscle. Similar phenotypes to aging skeletal muscles have been observed in adolescent muscle null for MG29 which is involved in muscle weakness, increased susceptibility to muscle fatigue, fragmented sarcoplasmic reticulum networks, swollen transverse tubule system, altered calcium sparks, segregated Ca~(2+) reserve and so on. These suggest that MG29 and aging skeletal muscle are closely related.CONCLUSION: It has been confirmed that MG29 is highly correlated with aging skeletal muscle. However, studies concerning MG29 improvement to delay skeletal muscle aging should be conducted subsequently.
3.Effects of Chinese medicine combined with laser therapy for peripheral retinal hole with shallow detachment

AIM:To evaluate the clinical effect of traditional Chinese medicine plus laser photocoagulation on eyes which have peripheral retinal hole with shallow detachment.
METHODS: The 227 cases ( 246 eyes ) of retinal hole with shallow detachment were randomly divided into an integrated Chinese and western medicine group and a western medicine group. The former underwent laser photocoagulation combined with oral Chinese medicine;while the latter was treated with laser photocoagulation alone. Analyses of the effect were performed two months after treatment.
RESULTS:The total cure rate of integrated Chinese and western medicine group was 87. 7%, while that of western medicine group was 71. 6%, significant difference was considered between the two groups (P<0. 01).
CONCLUSION: Chinese medicine combined with laser photocoagulation plays an exact role in eyes which have peripheral retinal hole with shallow detachment, especially when the detached range is greater than 1PD, and it is significantly better than western laser therapy.
4.The zebrafish as a model organism for the research of neonatal hypoxia brain injury

Many factors,especially perinatal asphyxia,can lead to varying degrees of hypoxia brain injury in fetus or newborns within perinatal period.So far,the mechanisms of neonatal cerebral damage caused by hypoxia during the perinatal period have not been clearly demonstrated,and there have no effective drugs or therapeutic methods to improve hypoxia-induced cerebral damage.This review focuses on the recent progress of zebrafish as a model organism of using in research of hypoxia brain injury,including the anatomic and behavior basis,model making,research strategies and the advantages of neurotrophic drug screening.The application of zebrish in the research of neonatal hypoxic brain injury is promising,and may provide a new tool as research in finding out the therapy strategies of hypoxic-ischemic encephalopathy.
5.Effect of taurine on the expression of endoplasmic reticulum stress-related factors in the brain of zebrafishlarvae after hypoxia reperfusion

ObjectivesTo investigate the expression of glucose regulated protein 78 (GRP78), CCAAT/enhancer binding protein homologous protein (CHOP) and cysteine asparate protease-12 (caspase-12) and neuronal apoptosis in the brain of zebraifsh larvae after hypoxia reperfusion, and the neuroprotective effect of taurine.Methods Five day old post-fertilization zebraifsh larvae were randomly assigned into 3 groups, control group, hypoxia reperfusion group (model group) and taurine group, and the taurine group was further divided into 3 subgroups according to different concentrations (1 mmol/L, 5 mmol/L, 10 mmol/L) with 100 zebraifsh larvae each. The behavior, recovery time and median survival time of those zebraifsh larvae after hypoxia with 1h reperfusion were observed and recorded. The pathological changes and apoptosis of neurons were detected by Nissl staining and terminal deoxynucleotidyltransferase-mediated dUTP nick end-labeling. The expression of GRP78, CHOP and caspase-12 in the brain of zebrafish larvae were detected by Western blot.Results Compared with the model group, the recovery time was shortened, the median survival time was extended, the Nissl stained neurons was increased and the apoptotic neurons were decreased in the taurine groups. GRP78, CHOP and caspase-12 were expressed in model group and taurine group. The expression of GRP78, CHOP and caspase-12 was much lower in taurine group than in model group.Conclusions Hypoxia reperfusion may induce endoplasmic reticulum stress and taurine may be neuroprotective against hypoxia reperfusion by down-regulating GRP78, CHOP and caspase-12.
6.The Value of Low Field Strength MRI in Evaluating the Invasion of Myometrium of Endometrial Carcinoma in Staging Ⅰ

Objective To evaluate the clinical value of low field strength MRI in judging the invasion of myometrium of endometrial carcinoma in staging Ⅰ.Methods 32 cases with endometrial carcinoma approved to be clinical staging Ⅰ pathologically were evaluated by MRI before operation.The staging results of MRI were compared with pathologic results and analysed with statistic methods.Results According to 1988 FIGO staging the staging of the lesions was further carried out,including stage Ⅰa in 6,stage Ⅰb in 18 and stage Ⅰc in 8 cases.Compared with pathologic results,the sensitivity and specificity of MR in evaluating myometium invasion were 92% and 83% respectively,the staging accurate rate was 78%.Conclusion The endometrial carcinoma stage Ⅰ can be staged further by low field strength MRI,MRI has significant advantage in estimating the invasion of myometrium.
7.Characteristics and clinical values of SPECT/CT whole-body bone scanning in detecting bone metasta-ses in patients with lung adenocarcinoma or squamous cell carcinoma

Objective To investigate the clinical values and characteristics of whole body bone ima-ging (SPECT/CT)in detecting bone metastases in the preoperative patients with lung adenocarcinoma or squa-mous cell carcinoma for staging and determining the best treatment plan.Methods Eighty-two preoperative patients with primary pulmonary adenocarcinoma or squamous cell carcinoma performed 99 Tcm-MDP SPECT/CT whole-body bone imaging.One week before surgery,parts of positive lesions performed MRI scan.The differ-ence of the incidence of bone metastasis was analysed by χ2 test.Results In all 82 patients with lung cancer, there were 38 adenocarcinomas and 44 squamous cell carcinomas.Bone metastases were detected in 38 cases, the incidence rate was 46.3%.Of which,among lung adenocarcinoma,the incidence rate was 57.9% (22 /38),and the incidence rate was 36.4% (1 6 /44)in lung squamous cell carcinoma,and the difference was sta-tistically significant (χ2 =1 2.66,P =0.027).The most common area was bilateral ribs,followed by vertebra, pelvis,bones of the extremities and skull.Conclusion Lung adenocarcinoma compared with squamous cell carcinoma is prone to bone metastases,and bone metastases are more common in bilateral ribs.It has important value that whole body bone imaging in screening for bone metastases of pre-operative patients with lung cancer for staging and making the treatment plan.
8.Effects of plant growth regulator uniconazole on plant morphology and biomass allocation of Salvia miltiorrhiza.

In this study, we use pot experiment to evaluate the effect of plant growth regulator on plant morphology and biomass allocation of Salvia miltiorrhiza. Different concentrations of uniconazole were supplied to S. miltioohiza by means of foliar spray. Height, breadth and stem diameter were measured dynamically, the biomass of leaf, stem, flower and fruit, root biomass and biomass ratio were also examined at the harvest time. Owing to the treatment, plant morphology showed significant changes, the height had been greatly reduced and the breadth decreased largely. Meanwhile, the biomass allocation changed too. The biomass ratio of leaf and stem had been notably reduced while the biomass ratio of root had been increased remarkably. It appears that foliar application of uniconazole during vigorous growth period in S. miltioohiza has dramatic effect on dwarfing plant and improving resistant to lodging. This measure could also be applied to condensed cultivation of S. miltioohiza to increase production.

9.Application value of the mathematical model of gastrointestinal decompression after esophagectomy of esophageal cancer

Objective To verify the accuracy of the mathematical model of gastrointestinal decompression after esophagectomy of esophageal cancer and explore predictive value of the mathematical model in the postoperative complications.Methods The retrospective case-control study was conducted.The clinicopatholo gical data of 192 patients with esophageal cancer who underwent esophagectomy in the Beijing Chaoyang Hospital of Capital Medical University between October 2013 and October 2016 were collected.Among 192 patients,160 didn't have postoperative complications and 32 had postoperative complications (7 with postoperative anastomotic leakage,9 with pulmonary infection and 16 with dysfunction of gastralintestinal tract).Patients selected the appropriate surgical procedures according to individual conditions,and then volume of gastrointestinal decompression was recorded daily.According to the regression equation of influencing factors of volume of postoperative gastrointestinal decompression:average daily drainage volume within 5 days (mL)=262.287 + 132.873 × tubular stomach-72.160 × smoking history-27.904 × pathological type of tumor-36.368 × age,predictive value of postoperative gastrointestinal decompression was calculated and compared with real volume of gastrointestinal decompression.Observation indicators:(1) comparison between predictive value and real volume of postoperative gastrointestinal decompression in patients without complications;(2) comparison between predictive value and real volume of postoperative gastrointestinal decompression in patients with complications.Measurement data with normal distribution were represented as (x)±s and comparison was analyzed using the pairedsamples t test.Measurement data with skewed distribution were described as M (range),and comparison was analyzed using the Wilcoxon signed rank tests.Results (1) Comparison between predictive value and real volume of postoperative gastrointestinal decompression in patients without complications:predictive value and real volume of postoperative gastrointestinal decompression in 160 patients without complications were respectively 187 mL (range,58-392 mL) and 207 mL (range,20-570 mL),with no statistically significant difference (Z=-1.106,P>0.05).(2) Comparison between predictive value and real volume of postoperative gastrointestinal decompression in patients with complications:7 patients had postoperative anastomotic leakage,including 1 with cervical anastomotic leakage and 6 with chest anastomotic leakage.The predictive value and real volume of postoperative gastrointestinal decompression in 7 patients with anastomotic leakage were respectively (215±58)mL and (338± 106)mL,with a statistically significant difference (t=-3.139,P<0.05).The predictive value and real volume of postoperative gastrointestinal decompression in 9 patients with postoperative pulmonary infection were respectively (176±61) mL and (239± 111) mL,with no statistically significant difference (t =-1.805,P>0.05).The predictive value and real volume of postoperative gastrointestinal decompression in 16 patients with dysfunction of gastralintestinal tract were respectively (236 ± 60) mL and (357 ± 107) mL,with a statistically significant difference (t =-4.716,P< 0.05).Conclusions The mathematical model of gastrointestinal decompression after esophagectomy of esophageal cancer is correct and feasible.There is a predictive value for patients with postoperative anastomotic leakage and dysfunction of gastralintestinal tract.
10.Clinical value of preoperative serum carcinoembryonic antigen detection in the prediction of esophageal cancer lymph node metastasis

Objective To investigate the clinical value of preoperative serum carcinoembryonic antigen (CEA) detection in the prediction of esophageal cancer lymph node metastasis.Methods The clinical data of 111 patients with esophageal cancer who were admitted to the Chaoyang Hospital of Capital Medical University between December 2010 and January 2014 were retrospectively analyzed.Patients received preoperative serum CEA examination and enhanced CT of the chest.The surgical procedures were selected according to the condition of patients, including radical resection of esophageal cancer via left thoracic approach, transabdominal right thoracic approach (open and laparoscopic surgeries), cervico-thoracic-abdominal triple incision (open and laparoscopic surgeries) and transabdominal incision.The international standard was used for tumor location and TNM stage of esophageal cancer.The count data and comparison of ordinal data in the univariate analysis were analyzed using the chi-square test, Fisher exact probability and rank-sum test, respectively.The multivariate analysis was done using the stepwise logistic regression.The ROC curve was used for evaluating diagnostic value of serum CEA examination and enhanced CT of the chest.All the 111 patients were divided into 4 groups according to the interquartile range results of the CEA examination, and the lymph node metastasis rates of 4 groups were compared by the chi-square test.Results All the 111 patients underwent successful radical resection of esophageal cancer after preoperative serum CEA detection and enhanced CT of the chest, including 40 via left thoracic approach, 56 via transabdominal right thoracic approach, 8 via cervico-thoracic-abdominal triple incision and 7 via transabdominal incision.There were 3 patients with upper thoracic esophageal cancer, 52 with middle thoracic esophageal cancer, 36 with lower thoracic esophageal cancer and 20 with cancer of gastro-esophageal junction.The postoperative pathological type included 84 squamous cell carcinomas, 23 adenocarcinomas and 4 other carcinomas.There were 44 patients with negative lymph node metastases and 67 with positive lymph node metastases.The positive rate of elevated serum CEA in the 111 patients was 36.04% (40/111).Tumor location, pathological type and N stage of tumor were clinical pathological factors affecting the positive rate of serum CEA of patients (Z =6.815, 6.608, 16.928, P <0.05).N stage of tumor was an independent risk factor affecting the positive rate of serum CEA of patients by multivariate analysis [OR =2.206, 95% confidence interval (CI) :1.370-3.552, P < 0.05].The T stage of tumor and serum CEA level were risk factors affecting lymph node metastasis of esophageal cancer by univariate analysis (Z =18.971, x2=10.081, P <0.05), and those were also independent risk factors affecting lymph node metastasis of esophageal cancer by multivariate analysis (OR =3.558, 3.936, 95% CI: 1.798-7.041, 1.480-10.469, P <0.05).The lymph node metastasis rates of esophageal cancer were 46.43%, 48.28% , 55.56% and 92.59% when CEA level≤ 1.75 μg/L, 1.75 μg/L < CEA level ≤ 2.68 μg/L, 2.68 μg/L < CEA level ≤4.21 μg/L and CEA level > 4.21 μg/L by the stratified analysis, respectively, with a significant difference among the 4 groups (x2=16.026, P < 0.05).The areas under the curve of CEA level and enhanced CT of the chest for lymph node metastasis were 0.687 (95% CI: 0.590-0.785) and 0.689 (95% CI: 0.591-0.788) by ROC curve, which were significantly different from the area under the guides (P <0.05).The areas under the curve of CEA level and enhanced CT of the chest for lymph node metastasis were 0.785 (95% CI: 0.697-0.873, P < 0.05).Conclusions Serum CEA detection not only has certain predictive value for lymph node metastasis of esophageal cancer, but has a higher predictive value combined with enhanced CT of the chest.There is a risk of lymph node metastasis for patients with deep tumor invasion and elevated CEA level, and the range of lymph node dissection should be expanded.
