1.The latest treatment advances of severe acquired aplastic anemia

Acquired aplastic anemia is a refractory blood disease for children,and severe acquired aplastic anemia can be a great threaten for life.China is a high incidengce area,children at a period during which this disease can mostly occur.In China,except some congenital aplastic anemia,most children who have this diease is acquired aplastic anemia.In 2009,the Britain society of hematology recommended Guidelines for the diagnosis and management of aplastic anaemia,which represented the latest diagnosis and treatment principle and mainstream view of the clinical methodology about aplastic anemia.They make it clear that the main treatment for the disease are allogeneic hematopoietic stem cell transplantation(allo-HSCT) and immunosuppressive therapy(IST).This article collects the documents in recent 5 years,and provides a overview about the latest advance of the allo-HSCT and IST in the word.
2.Application Analysis of IgM Antibody and DNA Test Results in Children Mycoplasma Pneumonia Infection

Objective To analyze the results of distribution of mycoplasma pneumoniae IgM antibody and DNA test,and ex-plore its application in diagnosis of Mycoplasma pneumoniae infection in children.Methods Retrospectively analysed myco-plasma pneumoniae antibody IgM (MP-IgM)and mycoplasma pneumoniae DNA (MP-DNA)results,114 374 passengers and 8 014 passengers,in 0 ~ 16 year-old children of Shengjing Hospital Affiliated to China Medical University from year 2013 to 2014.Analyzed of the relationship between gender,age,season and the positive rate of the MP-IgM and MP-DNA. Collected 5 106 patients with MP-IgM and MP-DNA results simultaneously and analyze the consistency between two posi-tive indexes in different ages.Results ①MP-IgM and MP-DNA positive rate varied in sex,age and season,the positive rate in female was higher than male.The positive rates of MP-IgM and MP-DNA at the age of 1~3 and 7~ 12 years old were higher than other age groups.The positive rate of MP-IgM in spring and autumn was higher,while MP-DNA was higher in summer and autumn and all differences were statistically significant (P < 0.001).②The positive rate of MP-DNA differed in specimens,the bronchoalveolar lavage fluid had the highest MP-DNA positive rate,49.95%.③The positive consistency of MP-IgM and MP-DNA was associated with age,with 4~6 and 7~12 year-olds the highest,respectively 35.2% and 45.8%. Conclusion ①Incidence of children pneumonia mycoplasma infection was mainly in preschool and school-aged children,more female than male,and autumn highest.② The detection of MP-DNA in bronchoalveolar lavage fluid was the best sample type.③For <1 and 1~3 year-olds,combined detection of MP-DNA and MP-IgM can improve diagnostic rates.
3.Explore Xueshuan Xinmaining tablet effecting on treatment outcome of coronary heart disease based on propensity score.

Xueshuan Xinmaining tablet is a Chinese patent medicine for treating chest pain caused by blood stasis. It is widely used in clinical prevention and treatment of coronary heart disease. In order to understand the treatment effect of Xueshuan Xinmaining tablet in patients with coronary heart disease, we extracted electronic medical record data from 18 large hospitals nationwide. We matched the coronary artery disease patients with or without Xueshuan Xinmaining tablet treatment on gender, age, condition at admission and whether combined with cardiac insufficiency on a one to one ratio. After matching, both groups, patients using Xueshuan Xinmaining tablet (group A) and patients not using Xueshuan Xinmaining tablet (group B), ended up with 1,122 people. In order to evaluate the effectiveness of treatment, the endpoint of effective group was defined as "cure" and "better" while the endpoint of invalid group was defined as "invalid" and "death". Chi-square test showed a statistical significant difference (P < 0.05) between the two groups of patients with coronary heart disease, with a higher efficiency in Xueshuan Xinmaining group. Classic logistic regression analysis showed no statistical significant difference between the two groups on treatment outcome efficiency. Generalized boosted models (GBM) and propensity score (PS) weighted Logistic regression were then applied to balance 45 variables between the two groups. The results showed a regression coefficient greater than 0 and a statistical significant difference (P < 0.05) between the two groups. Based on the existing results of the analysis, we considered that coronary heart disease patients using Xueshuan Xinmaining tablet had a higher efficiency in clinical efficiency than the patients not using Xueshuan Xinmaining tablet. Since this study did not certainly eliminate all the possible confounders and patients from the hospitals included in this study were not yet well represent the overall situation of the source population, the study conclusion only provided drug use reference for clinical doctors for coronary heart disease. Large randomized controlled trials needs to be done for more reliable results to guide clinical rational and effective medication.

4.Effects of 6-week voluntary wheel exercise on sex-hormones and hypothalamic-pituitary-testicular axis in high-fat fed SD rats

Objective To observe the effects of high fat diet and 6?week voluntary wheel running on the structure of hypothalamus, pituitary and testis and sex hormone levels in pubertal male rats. Methods Forty 3?week?old male Spra?gue?Dawley rats were randomly divided into following groups: the control group fed with normal diet (C), training group fed with normal diet ( CE) , control group fed with high fat diet ( D) , and training group fed with high fat diet ( DE) , 10 rats in each group. The groups C and D were bred for 8 weeks freely, and the groups CE and DE were assigned to have vol?untary wheel running twice/d, 1 h/time, 5 d/w, for 6 weeks after a 2?week adaptive feeding. Eight weeks later, blood sample was collected to detect the serum T, E2 , FSH and LH, and the E2 and T of testis were also detected. The histology of hypothalamic, pituitary, and testis tissues was observed by light microscopy. Results ( l) Compared with the group C, the group D had significantly decreased levels of serum T and E2 and testicular T (P<0?05), and significantly increased serum E2, FSH and LH levels (P<0?05). In the group D, vacuolar lipid droplets were increased in the hypothalamus, e?osinophils and basophils were reduced in the pituitary, and the area of seminiferous tubules, percentage of sperm cells, and quantity of Leygid cells were significantly decreased in the testis. (2) Compared with the group D, the serum T and testic?ular T concentrations were increased, but the serum E2, FSH and LH were decreased significantly (P<0?05) in the group DE, and vacuolar lipid drops were increased in the hypothalamus, eosinophil cells were increased in the pituitary gland, and the area of seminiferous tubules was increased but not significantly in the testis. Conclusions 6?week voluntary wheel exercise can improve the high fat diet?induced abnormal secretion of sex hormones, but not effectively improve the histologi?cal changes in hypothalamus, pituitary gland and testis.
5.Analysis of influencing factors on adverse reaction of Shengfu injection based on prospective active safety monitoring.

This study collected demographic data, past history, personal history, family history, dosage, solvent, combined medication information of adverse reaction cases from a prospective, multi center, large sample intensive hospital monitoring, and found the influencing factors with cross-tab analysis. The results showed that in the medication group of 19-45, patients with allergic histories had a higher proportion in ADR; in the medication group of 46-65, male patients with allergic histories had a higher proportion in ADR; indication and non-indication group, patients of 19-45 years old, with combined medications and allergy histories had a higher proportion in ADR; Non-indication medication group, patients with combined medication, higher concentration, out-instruction solvent and dosage, had a higher proportion in ADR. So, the ADRs of Shenfu injection were related to the history of drug allergy, and also related to the indication, dosage, solvent, concentration when it was used.

6.Helicobacter pylori cytotoxin associated protein CagA up-regulating gastrin gene expression

Objective To study whether Helicobacter pylori CagA protein can control gastrin gene expression and the detailed mechanism. Methods First, pcDNA3. 1ZEO (-)/cagA7 was transfected into gastric cancer cell lines AGS and SGC-7901 cells. At the same time, culturing the Helicobacter pylori NCTC11637 and infecting AGS and SGC-7901 cells with it. Next, in the infected and transfecled AGS and SGC-7901 cells, respectively adding the JAK2 signaling pathway inhibitor AG490 and the ERK signaling pathway inhibitor U0126 to inhibit the two signaling pathway. Untreated gastric cancer cells and empty vector transfected cells as the control. Using real-time fluorescence quantitative PCR to detect the levels of gastrin mRNA in transfected and infected cells. Results After AGS and SGC-7901 cells were transfected with pcDNA3. lZE0(-)/cagA7 and infected with NCTC11637, the results showed that the expression of gastrin mRNA increased significantly (P < 0. 05) in transfected and infected cells as compared with the control group, but after adding the inhibitor AG490 and U0126 respectively, the expression of gastrin mRNA decreased significantly(P<0.05). Conclution These results suggest that CagA may up-regulate the expression of the gastrin gene, and CagA is one of the important proteins in regulating gastrin gene expression. The ERK/MAPK and JAK/STAT signaling pathways may be involved in controlling of gastrin gene expression by CagA.
7.Arthroplasty versus joint preservation for displaced 3-and 4-part proximal humeral fractures:a meta-analysis

BACKGROUND:Currently, the treatment of proximal humeral fractures mainly contains joint preservation (conservative treatment, open reduction and internal fixation) and arthroplasty, but how to choose the treatment is stil controversial. OBJECTIVE:To evaluate and compare the clinical outcomes of joint preservation versus arthroplasty in the treatment of displaced 3-or 4-part humeral fractures in randomized control ed trials using meta-analysis. METHODS:Medline (January 1966 to December 2013), PubMed (January 1980 to December 2013), Embase (January 1990 to December 2013), Science (January 1990 to December 2013), Springer (January 1990 to December 2013), China National Knowledge Infrastructure (1994 to 2013), and Wanfang database (1982 to 2013) were searched for randomized control ed trials addressing joint preservation and arthroplasty for 3-or 4-part proximal humeral fractures. Articles meeting the inclusion criteria were included. The related data were extracted and loaded onto Comprehensive Meta-Analysis Software for meta-analysis. RESULTS AND CONCLUSION:Seven articles with 320 patients (165 patients undergoing joint preservation and 155 patients receiving arthroplasty) were accepted in this mete-analysis and al of them were high-quality English researches by modified Jadad Scale. Meta-analysis results displayed that the random-effects mean Constant score across al types was 55.9 (95%CI:50.7-61.1;P<0.001). Constant score was higher in the joint preservation group than in the arthroplasty group (P<0.01). The study displayed significant heterogeneity (I2 0.001). In the meta-analysis, Constant scores were decreased with increasing rate of male to female and rate of tuberosity resorption. These results suggested that 3-or 4-part proximal humeral fractures demonstrate improved Constant scores when treated with joint-preserving options compared with arthroplasty. Moreover, age, fracture pattern, gender ratio and complication rate are significant predictors of the Constant score. Given the observed heterogeneity and variance in treatment techniques in the included studies, more randomized control ed trial studies are needed to definitively recommend joint-preserving techniques versus arthroplasty for treatment of 3-or 4-part proximal humeral fractures.=88%, Q statistic=107.6, Q=13;P<0.001). 8.Extraction of the Volatile Oil from Huoxiang Zhengqi Prescription Process with Supercritical CO_2 Fluid:Optimization of the Technical Condition by Orthogonal Test

OBJECTIVE:To establish the optimal technical condition for the extraction of volatile oil from the volatile herbs of Huoxiang zhengqi prescription with supercritical CO2 fluid(SFE-CO2).METHODS:The technical condition for the extraction of volatile oil from the volatile herbs of Huoxiang zhengqi prescription were optimized using L9(34)orthogonal design table with extraction pressure,the extraction temperature,the pressure and temperature of separator Ⅰ as factors and with the yield of the volatile oil as index.RESULTS:A highest extraction yield of volatile oil from the volatile herbs of Huoxiang zhengqi prescription was achieved(2.057%)under an optimal technical condition as follows:with extraction pressure at 25 MPa,the extraction temperature at 30 ℃,the pressure for separator Ⅰ at 9 MPa and the temperature of separator Ⅰ at 45 ℃.CONCLUSION:Extraction of volatile oil from the volatile herbs of Huoxiang zhengqi prescription by SFE-CO2 is characterized by high extraction yield,stable,pure operation,safe,and by which the biological activity of the volatile oil can be maintained,thus it is a potential extraction and separation method.
9.Research Analysis on Medication Compliances of Tuberculosis Patients

OBJECTIVE:To probe into the status quo of medication compliances and problems in the medication of patients with tuberculosis so as to direct their use of drugs.METHODS:Information on medication compliances of tuberculosis patients who experienced regular physical examinations was investigated by interview and questionnaires,and which were analyzed statistically.RESULTS:Of the562cases investigated,54%followed or almost followed medication directions;46%failed to follow normal drug therapy for different reasons.CONCLUSION:The pharmacists should do their utmost to expand the range of pharmaceutical care so as to improve patients'medication compliances.
