1.Pharmaceutical Care Performed by Clinical Pharmacist for One Patient with Alcohol Withdrawal and Emergency Dyspnea

Objective:To provide reference for the sedation therapy and clinical monitoring for the patients with liver cirrhosis and alcohol withdrawal.Methods:Clinical pharmacist participated in the treatment process of emergency dyspnea in one patient with liver cirrhosis and alcohol withdrawal,and the thoughts and practice methods of clinical pharmacist participating in clinics were shared.Results:Based on the literatures,clinical pharmacist thought the emergency dyspnea was caused by overdose of sedatives.Therefore,medication monitoring and education on the drugs were provided for the patient,and pharmaceutical suggestions were put forward for the next treatment.Conclusion:Clinical pharmacists can help physicians analyze the causes of adverse reactions,and provide pharmaceutical care and medication education to ensure the safety and effectiveness of drug treatment.

3.The therapeutic effects of constraint-induced movement on patients with upper extremity disorders after head injury

Objective To investigate the therapeutic effects and relative mechanisms when constraint-induced movement therapy (CIMT) is used in patients with upper extremity disorders after head injury, and to compare the effects of CIMT with those of traditional rehabilitation (TR). Methods Thirty-six hemiparesis patients were randomly divided into a CIMT group and a TR group. Patients in the CIMT group were treated with intensive shaping training for 5 h a day for 3 weeks, as well as having their unaffected arms constrained. Meanwhile, the patients in the TR group were treated with traditional rehabilitation with no constraint of their unaffected arms. Motor activity logs (MALs) were compiled, and the Wolf Motor Function Test ( WMFT) and Barthel Index ( BI) were used to evaluate the therapeutic effects before treatment and at 0 d, 1 month and 3 month after treatment. Results Compared with the scores before treatment, the therapeutic effects were significant in the CIMT group, and the effect lasted to the follow-up period. Significant differences were also found between the CIMT group and the TR group in the improvement of dexterity by means of WMFT, and the usage of unaffected upper extremity as recorded in the MAL. There were no significant differences in therapeutic effects between the 2 week and 3 week treatments in the CIMT group. Conclusion CIMT significantly improved upper extremity function and ability in the activities of daily living ( ADL) of hemiplegia patients in the subacute period. The effectiveness of CIMT is superior to that of traditional rehabilitation methods.
4.Association between postprandial triglyceride and the severity of coronary artery disease in patients with metabolic syndrome

Objective To investigate the association between postprandial triglyceride and the severity of coronary artery disease in patients with metabolic syndrome (MS). Methods AlI of 91 patients with MS were recruited for this study. Thirty-one patients with normal fasting and postprandial triglyceride was in MS1 group, 29 patients with normal fasting triglyceride and postprandial hypertrigtyceridemia was in MS2 group, and 31 patients with fasting hypertriglyeeridemia was in MS3 group. Blood triglyceride at the time of postprandial 4 hours was measured and the quantity of coronary artery branch disease was determined by coronary angiography. The relationship between them was analyzed. Result There was significant positive correlation between the quantity of coronary artery branch disease and the level of blood triglyceride at the time of postprandial 4 hours (r = 0.42, P < 0.01 ). Conclusions It is important to detect the level of blood triglyceride at the time of postprandial 4 hours. Prompt intervention maybe decrease the incidence and mortality rate of cardiovascular disease in patients with MS.
6.Selection of Cd~(2+) Binding-peptides and Their Affinities for Heavy Metal Ions

Selection and affinities of Cd binding peptides were assayed by phage random dodecapeptide library and affinity chromatography. Two Cd binding peptides were obtained, it was found that the affinities of Cu~ 2+ ,Co~ 2+ ,Zn~ 2+ ,Ni~ 2+ for Cd binding peptides were higher than that of Cd~ 2+ and Cr~ 2+ after detection of the amplified Cd binding peptides displayed phages to different heavy metal-charged resins; the detoxification of E.coli to Ni~ 2+ and Cd~ 2+ was enhanced when infected by Cd binding peptide displayed phages as compared with those of the control. The interaction of Cd binding peptide displayed phages with heavy metals charged resins was also observed under microscope. The work would be of great value and consequences for the study of interaction between heavy metals and proteins(peptides), as well as thedetoxification and bioremediation of heavy metals.
7.Application of various preoperative imaging examinations for peferator in anterolateral thigh flap

Objective To assess the diagnostic value of various preoperative imaging examinations for the point of the lateral femoral circumflex artery perforators through the deep fascia,in order to provide reasonable evidence for the design of location and protocol of anterolateral thigh perforator flap.Mehtods From September,2013 to December,2013,23 patients (including 1 patient with bilateral anterolateral thigh perforator flap) preparing anterolateral flap surgery were randomly divided into 4 groups(every group was 6 side).The patients in different groups were using different imaging techniques respectively,such as handheld doppler (HHD),Color doppler sonography (CDS),MDCT angiography (MDCTA),CDS and MDCTA united image location technology.The flap sizes varied from 8 cm × 5 cm to 28 cm × 12 cm.Moreover the coincidence rate between preoperative and intraoperative location of the perforator vessel was evaluated.Results Ali flaps were survived after the operation.The donor site were primarily closed while partial skin grafting was performed in 8 cases.The appearance and functional resuhs were satisfactory with following up for 3 to 9 months.The rates of coincidence between the preoperative and intraoperative location of perforating branches were HHD 61.53%,CDS 87.50%,MDCTA 85.71%,CDS and MDCTA united image location technology 93.33% respectively.The HHD group showed significant decrease compared with the other groups (x2 =7.92,P < 0.01; x2 =6.15,P < 0.05; x2 =12.54,P < 0.01,respectively).The CDS group had no statistically difference with the MDCTA group (P > 0.05).The united CDS and MDCTA group showed significant increase compared with the other groups (x2 =4.22,P < 0.05; x2 =3.94,P < 0.05).Conclusion CDS and MDCTA united image location technology are more accurate for perforator preoperative location in auterolateral thigh flap,and should be widely used.
8.The Correlation of ACE I/D Gene Polymorphisms with the Antihypertensive Efficacy of Irbesartan Among Hypertensive Patients

Objective To analyze the correlationship of ACE I/D gene polymerphisms with the anti-hypertensive efficacy of irbesartan among essential hypertension (EH) patients in Yunnan han people.Methods One hundred EH patients hypertension were treated with irbesartan 150mg once daily for 4 weeks,and anti-hypertensive efficacy were monitored during the treatment.Results There was a significant difference between anti-hypertensive efficacy of irbesartan among different genotype groups,DD group >ID group >Ⅱ group (P < 0.05) Conclusions There is different therapy responsiveness in diffenent genotypes.ACE I/D polymorphism may be an important hereditary factor that impacts the efficacy of irbesartan.
9.Establishment of a nude mouse model of glioma orthotopic xenograft with double-fluorescent labeling

Objective To establish a stable and real-time monitorable nude mouse model of orthotopic glioma xenograft.Methods U251 glioma cell line was infected by a lentiviral vector containing green fluorescent protein (GFP) and luciferase (Luc) gene.Cells stably expressing fluorescence of GFP and Luc were sorted by flow cytometry.CCK-8 test and Transwell tumor invasion and migration assay were used to compare the biological features between the cells stably expressing GFP-Luc fluorescence and cells without fluorescence.Then the cells were implanted intracranially in the right caudate nucleus of athymic Balb/c nude mice to establish the tumor model.The growth of intracerebral tumor was monitored over time by a bioluminescence imaging (BLI) system.Hematoxylin-eosin (HE) staining was used to evaluate the histopathological features and tumorigenicity of the transplanted glioma cells in the brain of nude mice.Results U251 glioma cell line with stably expressing GFP-Luc fluorescence and the corresponding orthotopic xenograft model were successfully established.There was no statistically significant difference in the proliferation,invasion and migration abilities between the cells with stably expressing GFP-Luc fluorescence and the control cells.This model showed a high tumor formation rate and stable tumor growth,and takes a moderate time to establish this model.Conclusions Compared with the traditional glioma cells,GFP-Luc-transfected human glioma cells are more feasible for the studies of glioma in vivo.The tumor growth and pathological characteristics in this U251-GFP-Luc glioma model are similar to human glioma,and the growth of this tumor can be real-time monitored.It can be used as an ideal animal model for experimental studies of glioma.
10.Contrast-enhanced ultrasonography in evaluation of splenic trauma and injury grading and its clinical apllication

Objective To evaluate the conventional ultrasound (US) and contrast-enhanced ultrasound (CEUS) in diagnosis of splenic trauma including its grading diagnosis. Methods US and CEUS in 42 patients with splenic trauma confirmed by CT and/or operation were performed during 9.2004-10.2007. All the data were compared and analyzed retrospectively. Results Of 42 patients with splenic trauma, 28 cases were detected and 14 cases were missed on US examination, whereas, 40 cases were detected and only 2 mild cases were missed on CEUS examination. The detection rate of lesions with CEUS was significantly higher than that of US (P<0.001) . Ten cases in grading the injury were underestimated by US, however, none of them were underestimated by CEUS. CEUS had good concordance with CT and/or operation in grading diagnosis of 42 cases except two mild cases. Conclusions CEUS has very good concordance with CT and/or operation in detecting injury and grading the splenic trauma compared with conventional US. CEUS as a new imaging technology has made great advance of ultrasoongraphy in evaluation of splenic trauma including injury grading,and it is very useful in clinical application.
