1.Thyroid hormone resistance with H435Y mutation in thyroid hormone receptor β: a pedigree analysis

[Summary] The genomic DNA was extracted from peripheral blood leukocyte of the patient with thyroid hormone resistance syndrome and 14 members of his family.The exons 1-10 of thyroid hormone receptor β (TRβ) gene were amplified by PCR.The products of PCR were sequenced directly to detect the gene mutation.The results showed that 3 members of this family were confirmed to have the C→T transition mutation at nucleotide 1 303 site within exon 10 of TRβ gene,and the missense mutation results in the substitution of histidine to tyrosine (H435Y).The heterozygous mutation may lead to the occurrence of thyroid hormone resistance syndrome.

With the development of molecular diagnostic techniques, the study for the etiology of leukemia has been entering the era of the molecular biology.Molecular techniques for leukemia diagnosis, prognosis and individualized therapy are used widely, becoming one of the necessary routine tests for patients with leukemia. So far, molecular techniques for leukemiaincluding cytogenetic diagnosis, molecular genetics, molecular diagnostics based on PCR, mutation detection, as well as a variety of next-generation sequencing, have played an important role in the diagnosis of leukemia, minimal residual disease monitoring, prognosis and targeted therapy.
3.Observations on the Efficacy of Fire Needling plus Moxibustion in Treating Prurigo Nodularis

Objective To investigate the clinical efficacy of fire needling plus moxibustion in treating prurigo nodularis. Methods Ninety patients with prurigo nodularis were randomly allocated to treatment and control groups, 45 cases. The treatment group received fire needling plus moxibustion and the control group, external application of halometasone ointment. Skin lesions and pruritus were scored in the two groups before and after treatment. The clinical therapeutic effects were compared between the two groups. Results There were statistically significant pre-/post-treatment differences in the skin lesion score and the pruritus score in the two groups (P<0.01,P<0.05). There were statistically significant post-treatment differences in the skin lesion score and the pruritus score between the treatment and control groups (P<0.01). The total efficacy rate was 81.8% in the treatment group and 60.5% in the control group; there was a statistically significant difference between the two groups (P<0.05).Conclusion Fire needling plus moxibustion is an effective way to treat prurigo nodularis.
4.Clinical study on the recovery of wrist joint function after radical distal fracture by combined stasis elimination and powder intervention

Objective To study the clinical study of the effect of removing blood stasis and eliminating powder on the recovery of wrist function after distal radius fracture.MethodsA total of 82 patients with distal radius fractures were selected from September 2012 to October 2016.The random number table method was divided into study group and control group.The two groups were given intervention, and the study group (VAS score), the correlation index(swelling time, pain time, fracture healing time), clinical curative effect and the recovery of wrist function were observed and compared with the two groups.ResultsAfter treatment, the VAS scores of the two groups were significantly lower than those before treatment, and the VAS scores of the study group were significantly lower than those of the control group (P<0.05).After the treatment group, the swelling time, pain time (P<0.05).After treatment, the total effective rate was 87.80% higher than that of the control group (68.29%) (P<0.05).After treatment, the two groups were significantly higher than those in the control group (P<0.05), and the wrist function scores of the study group were significantly lower than those of the control group (P<0.05).ConclusionThe combined effect of removing blood stasis and eliminating powder is effective in the treatment of distal radius fractures, which can effectively reduce the pain and reduce the healing time of the fracture and promote the recovery of the wrist function.
5.Determination of lead in workplace air by portable X-ray fluorescence spectrometry.

OBJECTIVETo investigate the permance index ofof portable X-ray fluorescence spectrometer in the determination of lead on filter membrane and to provide data for the determination of lead in workplace air.
METHODSIrradiated with X-ray, the lead would emit specific X-ray fluorescence during the process from the excited state back to the ground state. Rapid determination of lead was completed using fluorescence energy and wave length for qualitative analysis and fluorescence intensity for quantitative measurement. Under set conditions, a series of customized calibration samples were measured to create a standard curve for quantitative analysis of lead on filter membrane.
RESULTSThe regression equation obtained using a portable X-ray fluorescence spectrometer to determine the lead on filter membrane was y=0.004x-0.182 (r2= 0.9999). The linear range was 0.00 -10.40 mg/m3, the minimum detectable concentration was 0.53 µg/m3, and the minimum quantifiable concentration was 1.76µg/m3. The relative standard deviation (RSD) of within-run precision of samples with different concentrations was 0.48%-6.22%, the RSD of between-run precision was 2.51%-5.09%, and the degree of accuracy was in the calibration range of standard samples.
CONCLUSIONPortable X-ray fluorescence spectrometry is a simple, rapid, repeatable, and accurate method for the determination of lead on filter membrane.

6.Evaluation of phacoemulsification in elderly patients with pseudoexfoliation syndrome

Objective To evaluate the intraoperative complication and postoperative visual acuity effect of phacoemulsification in patients with pseudoexfoliation syndrome. Methods Phacoemulsification and intraocular lens(IOL) implantation was performed on 21 eyes of 19 pseudoexfoliation patients, aged 67-92 years old and (80.24?7.46) years old on average with the history of the disease being (7?4.3) years. Five eyes in Group 1 were complicated with glaucoma. In Group 2, phacoemulsification and IOL implantation was performed on 50 eyes in 42 patients with non-pseudoexfoliation syndrome, 〔aged 61-91 years, mean age (79.52?8.53) years〕. All operations were completed by one surgeon, and followed up for 1 year. 7.Comparative analysis of health information network resource descriptions

Objective To improve the descriptions of health information network resources by analyzing the influen-cing factors of information retrieval.Methods The top 10 papers retrieved on 12 Websites in Chinese and English engines according their titles, keywords and descriptions in the HEAD file of HTML source code with 5 biomedical hot words as the search terms were comparatively analyzed and scored.Results The score of Websites in Chinese and English engines was higher than that of medical and health information network, the score of Chinese network information resources was higher than that of English network information resources .Conclusion The current de-scription of network health information resources is poor .Only by strengthening health information network con-struction and improving the description of health information resources , can more accurate health information be provided for the users.
8.A research on herba houttuyniae bladder irrigation for treatment of urinary tract infection in rabbit

Objective To observe the bacteriostasis of herba houttuyniae liquid in bladder irrigation for treatment of rabbit bladder infection model, and to offer a new way for prevention and treatment of urinary tract infection.Methods Twenty male New Zealand long-eared white rabbits were divided into gentamicin control group and three different concentrations herba houttuyniae groups according to random number table method, each group 5 rabbits. The rabbit bladder infection model was replicated by injecting coli liquid 1 mL (105 cfu/mL) into the urinary bladder through a urethral catheter, and clipping the catheter for 1 hour. After model establishment for 3 days, in the three herba houttuyniae groups, 0.25, 0.50, 1.00 g/mL herba houttuyniae liquid 50 mL were injected into the bladder respectively, and in the gentamicin control group, 160 kU gentamicin was injected into the bladder. Before molding and after molding for 3, 5, 7, 9 days, ear blood in rabbit was taken, and white blood cell (WBC) count and neutrophil percentage (N%) were tested; at the same time, the middle segment morning urine was taken to detect bacteriuria and urine WBC positive rates; after the experiment, the animals were sacrificed, the urinary bladder tissues were taken, and the pathological changes in urinary bladder tissue were observed by a light microscope.Results Before molding, the bacteriuria in animals of each group was negative and the results of urine WBC, blood WBC and N% were within normal range. After molding for 3 days, bacteriuria and urine WBC positive rates reached 100%; with the prolongation of treatment after molding, the above two rates in each group were gradually decreased, and herba houttuyniae 0.50 g/mL and 1.00 g/mL groups appeared statistical significance on the 5th day (bacteriuria positive rate was 60% and 40%, urine WBC positive rate was 60% and 40% respectively, while the gentamicin control group and herba houttuyniae 0.25 g/mL group was still 100% positive, allP < 0.01); on the 9th day, the bacteriuria positive rate in both higher dosage herba houttuyniae groups was 0, in 0.25 g/mL group, it still had the positive rate 20%, showing herba houttuyniae 0.50 g/mL and 1.00 g/mL had stronger inhibitory effect than herba houttuyniae 0.25 g/mL. WBC and N% were increased in each group after molding, and on the 3rd day, WBC and N were significantly higher in each group than those before molding [WBC (×109/L): gentamicin control group 13.00±5.23 vs. 8.95±2.69, herba houttuyniae 0.25 g/mL group 14.52±10.28 vs. 8.73±3.16, herba houttuyniae 0.50 g/mL group 12.97±11.33 vs. 7.83±4.51, herba houttuyniae 1.00 g/mL group 13.03±9.17 vs. 6.45±3.85. N%: gentamicin control group 80.99±14.93 vs. 63.41±7.96, herba houttuyniae 0.25 g/mL group 89.62±9.41 vs. 70.01±9.13, herba houttuyniae 0.50 g/mL group 87.69±11.34 vs. 67.75±8.91, herba houttuyniae 1.00 g/mL group 85.67±12.81 vs. 65.12±4.63]. On the 7th day, WBC and N% fell, and on the 9th day, the WBC and N% were restored to normal range in herba houttuyniae 0.50 g/mL and 1.00 g/mL groups [WBC (×109/L): herba houttuyniae 0.50 g/mL group 8.56±5.38, herba houttuyniae 1.00 g/mL group 7.93±4.82. N%: herba houttuyniae 0.50 g/mL group 65.97±8.35, herba houttuyniae 1.00 g/mL group 65.57±9.12]. Under light microscopy, it was shown that the morphological appearances of tissue cells in various layers of bladder mucosa were normal in herba houttuyniae group, demonstrating that herba houttuyniae fluid in bladder did not cause allergic inflammatory reaction, etc.Conclusion The bacteriostatic action of herba houttuyniae liquid for bladder irrigation in treatment of rabbits with urinary tract infection is better than that of gentamicin.
9.The clinical values of quantitative ultrasound elastography technique in detecting internal carotid artery thickness

Objective To investigate the clinical values of quantitative ultrasound elastography technique in detecting internal carotid artery thickness .Methods One hundred and forty healthy objects who have conducted physical examination in our hospital from February 2012 to August 2015 were selected ,and divided in to three groups:the healthy control group (n=40;IMT<0 .9 mm) ,the IMT thickening group (n=60;0 .9 mm≤ IMT < 1 .3 mm) and plaque group(n= 40 ;IMT ≥1 .3 mm) .All the three groups conducted routine ultrasound and quantitative ultrasound elastography ,and the clinical data investigation and laboratory ex‐amination were carried out at the same time .Results The TC ,LDL‐C values in the plaque group and intimal thickening group were significantly higher than the control group (P<0 .05) .The AC values in the plaque group and intimal thickening group were signif‐icantly higher than the control group (P<0 .05) ,while the AI and PWVβvalues were significantly lower(P<0 .05) .Linear corre‐lation analysis showed that IMT were positively correlated to the AI ,PWVβ (r=0 .512 ,P <0 .05 ;r= 0 .483 ,P<0 .05) ,and the AC were negatively correlated (r= -0 .713 ,P<0 .05);Logistic analysis showed that LDL‐C ,AC ,PWVβwere the major independ‐ent risk factors for IM T (P<0 .05) .Conclusion Ultrasound elasticity quantitative detection of neck vessels is safe and can non‐in‐vasive dynamically observe blood vessel wall changes during the progression of atherosclerosis ,thus it's helpful in the judge and i‐dentification of internal carotid artery thickness .
10.Pyrosequencing for detection of drug resistant relevant mutation in the polymerase gene of Hepatitis B virus and its clinical application

Objective To explore the accuracy and clinical application of Pyrosequencing for detection of drug resistant relevant mutation in the polymerase gene of Hepatitis B virus (HBV).Methods Compared with Sanger sequencing,the accuracy and sensitivity of Pyrosequencing were assessed.Pyrosequencing was used to determine the serum of 1164 patients with chronic Hepatitis B and its results were analyzed.Results The sensitivity of Pyrosequencing was 1 × 103 KIU/L,the same as Sanger sequencing.But Pyrosequencing was more stable and specific than Sanger sequencing to detect low rate of mutation with over 10% mutation.The mutations couldn't be detected in 248 patients with chronic Hepatitis B,but positive results occurred in 919 patients,among them,61.5 % without mutation,38.5 % with mutation (including 47.5% of the single locus mutation and 52.5% of the combined mutation).Other sites had different degree of mutations except the sites of rtI169T and rtA194T; the main mutation sites in patients with chronic Hepatitis B were rtM204V/I (32.1%),rtL180M (19.8%),rtA181V/T (6.7%) and rtN236T (5.3%),in which fractional mutation had high ratio in the sites of rtA181V/T (6.7%) and rtN236T (5.3%).Conclusions Pyrosequencing has good sensitivity,specificity and accuracy and can early detect the drug resistant relevant mutation in the polymerase gene of HBV,which is suitable for monitoring patients with chronic Hepatitis B for the treatment of nucleoside analogues (acid).The different sites and frequencies of mutations may be associated with different habits of doctors for using different anti-HBV drugs.
