1.Correlation between tear osmolarity and the dry eye severity

Background There still is no a uniform diagnostic criteria for increasing population of dry eye.Studies showed that high tear osmotic pressure is one of causes of a variety of ocular surface discomfort,and the change of tear osmolality has been included in the definition of dry eye syndrome by Dry Eye World Study Group (DEWS) in 2007.It is very clinically significant to study the relationship of tear osmolality with dry eye.Objective This clinical trial was to study the correlation between tear osmolarity and dry eye severity based on Preferred Practice Pattern(PPP)of DEWS.Methods A descriptive study was performed.Fifty-four eyes of 54 patients with dry eye disease were asked to complete an evaluation of dry eye signs and symptoms questionnaire based on PPP.The ocular anterior segment was examined under the slit lamp biomicroscope.Cornealfluorescein staining,tear-film breakup time (BUT),Schirmer test Ⅰ (S Ⅰ t)without anesthesia were performed,and tear sample was collected for tear imageology and tear osmolarity admeasurement.The correlation between dry eye score with tear osmolarity was evaluated using Pearson linear correlation analysis.Written informed consent as obtained prior to any relevant medical procedure.Results The ratio of male and female patients was 1: 2 in this study.The negative correlations were found between tear osmolarity with S Ⅰ t or BUT(r =-0.456,-0.699,P<0.01),and significantly positive correlations were found between tear osmolarity with tear imaging examination,corneal fluorescein staining,conjunctival hyperemia or dry eye symptom score(r=0.545,0.686,0.691,0.803,P<0.01).Conclusions As a diagnostic method,tear osmolarity can objectively reflects dry eye severity.It can be a biomarker in dry eye severity grade when combined with clinical symptom.
2.Application of therapeutic touch in CPAP titration night with obstructive sleep apnea syndrome patients

Objective To evaluate the effect of therapeutic touch on continuous positive airway pressure titration with obstructive sleep apnea hypopnea syndrome (OSAHS) patients.Methods Fortyeight patients diagnosed with OSAHS were randomly allocated to the therapeutic group and the control group with 24 patients in each group.Therapeutic touch was used in the therapeutic group before CPAP titration and the control group received usual care.Anxiety was measured post intervention with State-trait Anxiety Inventory(SAI) and Visual Analogue Scale(VAS).CPAP therapy satisfaction was also evaluated by VAS.Results Compared with the control group,those patients with therapeutic touch showed significant reductions in SAI scores,better sleep quality,and higher satisfaction with CPAP treatment.Conclusions Results suggest that 15-min of therapeutic touch pre CPAP titration night significantly reduced anxiety and improved sleep quality in patients with OSAHS.

Medicinal materials research and development of new drug of traditional Chinese medicine (TCM) research is the premise and foundation of new drug research and development, it throughout the whole process of new drug research. Medicinal materials research is one of the main content of the pharmaceutical research of new drug of TCM, and it is also the focus of the new medicine pharmaceutical evaluation content. This article through the analysis of the present problems existing in the development of TCM research of new drug of TCM, from medicine research concept, quality stability, quality standard, etc are expounded, including medicine research idea value medicine study should focus on the important role and from the purpose for the top-level design of new drug research problem. Medicinal materials quality stability should pay attention to the original, medicinal part, origin, processing, storage, planting (breeding), and other aspects. Aspect of quality standard of medicinal materials should pay attention to establish the quality standards of conform to the characteristics of new drug of TCM. 4.Clinical research of sentinel lymph node imaging for contrast-enhanced ultrasonography in colorectal cancer during operation

Objective To study the feasibility of contrast-enhanced ultrasonography (CEUS) in detecting sentinel lymph node(SLN) in patients with colorectal cancer during operation and their clinical significance.Methods Twenty-one patients who underwent radical resection of a primary colorectal cancer were enrolled.During operation,1 % methylene blue was injected around the tumor,SLN mapping was performed.Then the SonoVue was injected at the same sites,CEUS was performed to observe changes of SLN in real time,and marked them.The SLN detection rate and accuracy of CEUS were compared to those of conventional dye guided method.Results The rectal cancer of below peritoneal reflex,the detection rate of CEUS for SLN was 92.31 %,and the accuracy was 88.89%.The detection rate of dye guided method for SLN was 84.62%,and the accuracy was 93.10%,there was no statistically significant difference between CEUS and dye guided method (P >0.05).Conclusions When injected SonoVue in submucosal layer,the good mapping of SLN of patients with rectal cancer of below peritoneal reflex can be obtained,it will play an important role in clinical application to map SLN of patients with rectal cancer of below peritoneal reflex.
5.Clinical Effects of Tacrolimus Combined with Okra Capsule in Treatment of Refractory Membranous Nephropathy

Objective:To investigate the effect and safety of tacrolimus combined with Okra capsule in treatment of the refractory membranous nephropathy.Methods:Selected 60 patients with refiactory membranous nephropathy who were treated in our hospital from March 2014 to October 2015,and they were divided into observation group and control group with 30 cases in each group according to the random number table method.The control group was treated with prednisone,and the observation group was treated with tacrolimus combined with Okra capsule on the basis of the control group,the courses of treatment were 6 months in two groups.The clinical effects were observed in two groups,the levels of urine protein (uPRO),serum Albumin (sALB),serum Creatinine (sCr),serum Alanine aminotransferase (sALT),tumor necrosis factor-α (TNF-α) and transforming growth factor-β 1 (TGF-β1) were detected and compared in two groups before and after treatment,and the adverse reactions were recorded in two groups.Results:The total effective rate of the ob-servation group was significantly higher than control group (P<0.05).Compared with before treatment,the levels ofuPRO,TNF-α,TGF-β1 in two groups were decreased after treatment,while sALB,sCr were increased (P<0.05),and uPRO,TNF-o,TGF-β1 in the observation group were lower than the control group (P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).Conclusion:Tacrolimus combined with Huangkui capsule in the treatment ofrefractory membranous nephropathy has good curative effect,reduce renal damage,and low adverse reaction,worthy of promotion.
6.Construction of ICAT interference lentivirus expression vector and establishment of stable HL 60 cell lin

Objective To construct the ICAT gene interference lentivirus expression vector targeting and to establish stable transfected cell line HL60 .Methods The interference sequence targeted at human ICAT gene was designed and synthesized ,after annealing ,which was connected to PGLV3 interference vector and with PG‐p1‐VSVG ,PG‐p1‐REV ,PG‐p1‐RRE were co‐transfected into 293T cells The lentivirus particles were packaged and generated .The virus titer was detected .HL60 cells were transfected for establishing the stable cell line ;RT‐PCR and Western Blot techniques were used to detect ICAT gene and protein expression in sta‐ble HL60 cells ,then the results were compared with those in the control group .Results The lentivirus expression vector targeted at ICAT was successfully constructed and the virus titer was 2 × 108 U/mL .Stable transfected HL60 cell line was established .The effective interference verification revealed that shICAT could significantly reduce the mRNA and protein level of ICAT ( P<0 .001) .Conclusion The shRNA lentiviral expression vector of ICAT gene is successfully constructed and the HL 60 cell line stably interfering ICAT expression is established .
7.Impact of repetitive transcranial magnetic stimulation on executive dysfunction after stroke

Objective To explore the impact of low frequency repetitive transcranial magnetic stimula tion (rTMS) on executive dysfunction after stroke by magnetic resonance spectroscopy (MRS) and neuropsychological scale.Methods 60 stroke patients with executive dysfunction were recruited and randomly divided into a study group and a control group,with 30 in each.The study group was treated with rTMS in addition to routine treatment,while the control group received routine treatment only.Before and after treatment,the neuropsychological scale and MRS were measured in both groups.Results After treatment,the scores on the LOTCA and FAB of neuropsychological scale in the treatment group were (87.10 ± 3.16) and (11.97 ± 0.48),significantly better than before the baselines.NAA/Cr was (1.64 ± 0.08) and Cho/Cr was (1.17 ± 0.07),which were significantly better than the baselines.In the control group,significant differences were also observed in the scores of the neuropsychological scale and in the indicators of MRS before and after treatment.Conclusions rTMS can effectively improve executive function in patients with cerebral apoplexy by influencing the metabolites related with cognitive function in the brain.Magnetic resonance spectroscopy can show the therapeutic effect at molecular level.

AIM: To evaluate the alterations of maternal hepatic drug-metabolizing enzymes and antioxidative function in tobacco-induced intrauterine growth retardation (IUGR). METHODS:Pregnant rats were assigned to control group and tobacco group. IUGR model was produced by smoking from gestational days (GD) 9 to GD 20. Fetuses were removed by laparotomy on GD 21. The fetal development parameters such as fetal body weights, litter size and placental weights were recorded. Subcelluar fractions of liver were prepared by differential centrifugation. Aim To study the effect of emodin on IL-8 secretion and NF-?B activation of HT-29 cells,and explore the molecular mechanism of emodin.Methods The cytotoxicity of emodin was assessed by WST;NF-?B activation was detected with co-focal microscopy by immunofluorescence;the production of IL-8 was investigated by ELISA.Results Emodin with the concentration of 10~80 ?mol?L-1 could decrease the mass production of IL-8 Secretion of HT-29 cells stimulated by IFN-?+LPS in a dose-dependent manner.Emodin with various concentrations could inhibit NF-?B activation dose-dependently.Conclusions Emodin inhibited IL-8 secretion and NF-?B activation of HT-29 cells stimulated by IFN-?+LPS.
10.Relationship between coal burning type of arsenic poisoning and essential trace elements

Objective To investigate the effects of essential trace elements selenium, zinc, copper, iron,cobalt, chromium and molybdenum upon arsenic poisoning caused by coal-burning. Methods Stratified random sampling method was used to conduct epidemiological investigation on 139 arsenic exposed residents(including nonpatient, light, moderate and severe patients) in an area polluted by coal-burning arsenic in Xingren county of Guizhou province as exposure group. Control group included 34 residents who lived about 13 km away from the endemic area of arsenic contamination. Inductively coupled plasma-optical emission spectrometry (ICP-OES) was used to analyze arsenic, selenium, zinc, copper, iron, cobalt, chromium and molybdenum in coal, soil, rice,corn, chilli, hair, blood and urine. Results Arsenic content in coal, soil, corn and chilli of polluted area were 4.894,146.551,0.522,1.440 mg/kg, respectively. These arsenic content were significantly higher than those in control area which were 1.980,50.167,0.296,0.948 mg/kg, respectively(P < 0.05 or < 0.01) . The content of selenium in soil of the diseased area(5.038 mg/kg) was significantly lower than that in soil of control area(8.948 mg/kg, P <0.05). The content of copper, iron, chromium in soil and iron in corn were 44.114,5731.500,98.323,89.996 mg/kg, respectively. These elements content were significantly higher than those in control area which were 13.473,1298.430,36.839,57.391 mg/kg, respectively (all P < 0.05) . Hair and urine arsenic levels were 1.985mg/kg and 149.593 μg/g Cr in exposed group, respectively. These arsenic levels were significantly higher than those in control group which were 0.670 mg/kg and 49.853 μg/g Cr, respectively(all P < 0.01) . Hair selenium level in exposed group(1.706 mg/kg) was significantly lower than that in control group(2.405 mg/kg, P < 0.01). Hair levels of iron and chromium, blood level of eopper and the ratio between copper and zinc in exposed group were 88.295,8.933 mg/kg, 1.053 mg/L and 0.074, respectively. These element levels and elements ratio were significantly higher than those in control group which were 47.970,4.099 mg/kg, 0.934 mg/L and 0.065, respectively(P < 0.01 or < 0.05). Hair selenium level was negatively correlated with the progression of arsenism(r = - 0.414, P < 0.01) .Hair levels of iron and chromium, the ratio between copper and zinc in blood were positively correlated with the progression of arsenism(r = 0.271,0.261,0.250, all P < 0.01) . Conclusions Low selenium, high copper, high iron and high chromium coexists in arsenic polluted area. In exposed group, hair selenium is low, hair iron and chromium, blood copper and ratio between copper and zinc are high. These element changes with environment trend.These element changes are associated with the occurrence and development of the disease caused by coal-burning.
