Chinese Journal of Diabetes 2006;14(2):129-132
Objective To analyze the expression of BDNF, NGF,GFAP in Wistar rat pretectel area and determine the relation between neuron damage and neurotrophin change of pretectal area and retinal neurodegeneration in the early stage of diabetes. Methods Male Wistar rats weighing about 200 g were used to create diabetic animal model by injecting STZ. Wistar rats were killed by heart perfusion with 4% FDP at 1-week, 2-week and 1-month after diabetes. The dissected pretectal area was used to determine the expression of BDNF, NGF and GFAP with immunohistochemistry. Results In pretectal area of diabetic rat, the expression of BDNF and NGF began to decrease from 1-week DM, the expression of GFAP began to increase from 2-week DM. Conclusions There are neuron damage and neurotrophin decrease in the pretectal area in the early stage of diabetes. Neurodegeneration and neurotrophin decrease in pretectal area, which is the target tissue of retina, might be one of the reasons resulting in retinal neurodegeneration at the early stage of diabetes.

Objective To investigate the levels of 14 kinds of food allergic specific IgG antibody in eczema and chronic urticaria patients'serum and the role of food allergens in allergic skin diseases and provide scientific basis for clinical treatment and prophylaxis. Methods We used ELISA method to detect the concentration of 14 kinds of food allergic specific IgG antibody in the serum of 900 eczema and chronic urticaria patients and 18 healthy people. Results The food allergen of positive rate in studied group and control group was 90.2% and 11.1%. The positive rate of 14 kinds of food allergic specific IgG antibody in 900 eczema and chronic urticaria patients' serum from high to low was eggs, wheat, shrimp, milk, soja, crab, rice, corn, tomato, chicken, mushroom, beef, pork and ling. The food allergen positive rate in eczema and chronic urticaria patients'serum was 91.9% and 88. 9% ( P >0. 05). The positive rate of shrimp, mushroom and ling in eczema patients'serum was higher than that in chronic urticaria patients'serum ( P <0. 05 ). The positive rate in children's group was 96. 6%, which was higher than other age groups ( P >0. 05 ). The main allergens were egg, wheat and milk in children's group. The sensitization of crab and shrimp increased as people grew up. In 812 positive patients, who were intolerant of three and more than three foods accounted for 53.7%. In 175 follow-up patients, the effective rate was 50.9% in 20 days and 61.1% in 60 days after they avoided the intolerant food. The patients'diet was adjusted according to the test results. In 325 follow-up patients, the effective rate in eczema group was 59.9% in 20 days and 70. 4% in 60 days ( P <0. 05). Thee effective rate in chronic urticaria group was 65.6% in 20 days and 77. 3% in 60 days ( P <0.05 ). Conclusion The intolerant food can be found by detecting serum food allergen specific IgG.It can be used to adjust the diet and relieve the symptom, which is of great importance for preventing and treating allergic skin diseases.

Background The application of femtosecond laser in the corneal refractive surgery has made great progression recent years, but the morphology characteristic of corneal stroma surface after making-flap of femtosecond laser is closely concerned. Objective This study was to analyze the influence of photodisrnption of femtosecond laser on the corneal stroma surface and to investigate the effect of different laser pulse energy on the sudace ultrastructure of corneal stroma. Methods Corneal flaps were made with Visu Max femtosecond laser in 16 fresh porcine eyes with the pulse energy 125 nJ,155 nJ and 195 nJ respectively, and Moria-M2 microkeratome was used as control.Scanning electron microscopy (S-3400N Hitachi High-Technologies Corp) was used to observe and compare the ultrastructural characteristic of corneal stroma bed surface after making of corneal flap. Results The corneal strnma was evaporated and created a smooth surface when photodisrnption happened in the femtosecond laser group.Residual tissue bridges could been exhibited among the cavitation bubbles under the scanning electron microscope.Corneal strnma surface was smooth in the 125 nJ pulse energy group, but some tissue bridges still were visible. In the 155 nJ pulse energy group, much more smooth surface was seen without tissue bridges and mechanical damages on the corneal surface. However, the surface quality was worse and many tissue bridges and grooves existed in the 195 nJ pulse energy group. In addition, different sizes of slots caused by big cavitation bubbles were visible with the round and oval shape. The edges were regular and sharp with small damage zone after cutting with femtoseeond laser. However, many elevated fibril tissues could be seen on the corneal surface after making of flap with microkeratome. Many crimp and irregularity tissues were found on the surface.Blunt surface and indentations appeared in the cutting edge. Conclusions The mierostrueture of corneal stroma surface is more smoother after making of corneal flap with fentosecond laser in comparison with microkeratome.Pulse energy of 155 nJ is preferably during the making-flap with femtosecond laser.

Objective To investigate the relationship between isolated nocturnal hypertension and left ventricular hypertrophy. Methods In the inhabitants of 14 villages in Jingning County, Zhejiang Province, we performed 24-hour ambulatory blood pressure monitoring with SpaceLab monitors and measured 12-lead resting electrocardiogram using an electronic recording system of GE company, Left ventrieular hypertrophy was diagnosed with the criteria of Sokolow-Lyon voltage amplitude and Cornell product. Isolated nocturnal hypertension was defined as a nighttime ( from 22:00 to 4:00) blood pressure of ≥ 120/70 mm Hg( 1 mm Hg = 0. 133 kPa). Isolated daytime ( from 8:00 to 18:00) hypertension was a diurnal blood pressure of ≥ 135/85 nun Hg. When both conditions were present or absent, subjects were classified either as having combined day-night hypertension or as normotensive on ambulatory measurement. Analysis of variance and multiple regressions were used for statistical analysis. Results 647 participants (53.9% being female, average age 47. 8 years) included 72 patients with isolated nocturnal hypertension, 33 with isolated daytime hypertension and 248 with day-night sustained hypertension. Compared with normotensive subjects, patients with isolated nocturnal hypertension and day-night sustained hypertension had a higher Sokoiow-Lyon voltage amplitude and Comell product. However, after adjustment for sex, age, body mass index, drinking and smoking habits, serum total cholesterol, fasting blood glucose and the use of antihypertensive drugs, only day-night hypertensive patients had a significantly higher Sokolow-Lyon voltage (32. 8 mV, P =0. 0003 ) and Cornell product (1371 mV×ms, P =0.0004) than normotensive subjects (29.0 mV, 1114 mV×ms).Regardless of whether Sokolow-Lyon or Cornell criteria were used, both nighttime and daytime systolic and diastolic blood pressure were independent risk factors of left ventricular hypertrophy (P < 0. 01 ). However, the prevalence of left ventricular hypertrophy in patients with isolated nocturnal hypertension ( 23.6% ) study was not statistically different from that in normotensives ( 17.4%, P = 0. 24). Conclusion In our current cross-sectional study, isolated nocturnal hypertension was not independently related to left ventricular hypertrophy diagnosed with ECG criteria.

Abstract: Objective To investigate the clinical and laboratory characteristics of pulmonary infection caused by Nocardia otitidiscaviarum. Methods The clinical data of a patient with pulmonary infection caused by Nocardia otitidiscaviarum were reported, and the clinical characteristics, laboratory characteristics and drug sensitivity of pulmonary infection caused by Nocardia otitidiscaviarum were summarized in combination with the relevant literature at home and abroad from January 2010 to December 2022. Results A 67-year-old female patient was admitted to the hospital on June 30, 2020 because of "repeated chest tightness and shortness of breath for 3 years, aggravated cough, expectoration and fever". The sputum, alveolar lavage fluid and blood of the patient were collected for culture, and the detected pathogenic bacteria were identified. There are pathogenic bacteria growing in sputum and alveolar lavage fluid, which are identified as Nocardia otitidiscaviarum by Autof ms mass spectrometer. According to the results of pathogenic bacteria and the patient's condition, meropenem combined with compound sulfamethoxazole tablets were given anti-infection treatment, and the patient's condition improved and discharged. Conclusion The clinical manifestations and imaging features of nocardiosis are lack of specificity, and are prone to misdiagnosis and missed diagnosis. Etiology is the key to disease diagnosis, and clinical examination and culture should be conducted in time.
