1.Investigation of the Important Perennial Inhaled Allergens for Children with Allergic Asthma in Daqing

Objective To Investigate the important perennial inhaled allergens for children with allergic asthma in Daqing.Method Analyzing the data from 134 patients between 4 and 14 years of age with asthma who have a positive skin test to perennial aeroallergen and also have been carried out Allergen-specific immunotherapy in the last four years.Results The important perennial inhaled allergens for children with allergic asthma in Daqing were house dust,dust mites,polyvalent molds Ⅰ(PMⅠ),animal epithelium and polyvalent molds Ⅱ(PMⅡ).Among them the most important allergens were house dust and dust mites.Conclusion It will have practical significance to prevent and treatment children's asthma by taking measures for these allergens.

3.Correlation analysis between polycystic ovary syndrome susceptibility genes and metabolic phenotypes

Objective To investigate the relationship between polycystic ovary syndrome (PCOS) susceptibility single nucleotide polymorphisms (SNP) and metabolic phenotypes in glucose and lipid metabolism and explore the pathophysiological mechanism of the susceptibility genes.Methods Three of PCOS susceptibility locus 2p16.3 (rs13405728 of LHCGR gene),2p21 (rs13429458,rs12478601 of THADA gene) and 9q33.3 (rs2479106,rs10818854 of DENNDIA gene) were selected and the metabolic phenotypes were compared between different genotypes of SNP in PCOS patients (using dominant model).Results Low-density lipoprotein cholesterol was significantly increased in CC genotype group than in TC + TT groups at rs12478601 of THADA gene [(2.5 ± 0.8),(2.4 ± 0.8) mmol/L; P =0.01].Serum insulin level of 2 hours after 75 g glucose intake was significantly higher in GG + AG groups than that of AA group at rs2479106 of DENND1A gene[(71 ±65),(64 ±50) mU/L;P =0.05],and the prevalence of type Ⅱ diabetes in first-degree relatives of patients were also increased [9.9% (66/666),6.9% (52/751) ; P < 0.05].No association was found between metabolic phenotypes and genotypes of rs13429458,rs10818854,and rs13405728.Conclusions Genetic factors probably have effect on the metabolic characteristics of PCOS.THADA gene is related to lipid metabolism,while DENND1A gene may be involved in insulin metabolism in patients with PCOS.
4.Prevention of Nosocomial Infection in Patients with Mechanical Ventilation in ICU

OBJECTIVE The research aimed to investigate the incidence,clinical characteristics and risk factors of nosocomial infection in patients with mechanical ventilation in ICU,and explore the corresponding prevention procedures.METHODS Active surveillance was carried out by professional fulltime staff.The patients with mechanical ventilation in ICU above 48 hours were selected for the hospital infection surveillance,and the patients after tracheal intubation within 48 hours were also included.RESULTS The incidence of nosocomial infection was 54.28%,and the nosocomial infection were found in 69(65.71%) of the 105 cases.The major complication was mechanical ventilator-associated pneumonia(VAP).The risk factors were more than four times invasive operation and,tracheotomy(P
5.Relationship between Ulcerative Colitis and Lung Injuries.

OBJECTIVETo explore the relationship between ulcerative colitis (UC) and lung injuries by assessing their clinical manifestations and characteristics.
METHODSFrom July 2009 to April 2012, 91 UC patients presenting to Longhua Hospital who met the established inclusion and exclusion criteria were enrolled in this retrospective study. According to the scores of disease activity index, the patients were divided into the mild, moderate, and severe groups. Meanwhile, the records of pulmonary symptoms, chest X-ray image, and pulmonary function were reviewed.
RESULTSSixty-eight (74.7%) patients had at least 1 pulmonary symptom, such as cough (38.5%), shortness of breath (27.5%), and expectoration (17.6%). And 77 (84.6%) had at least 1 ventilation abnormality. Vital capacity value was significantly lower in the severe group than that in the mild group (91.82%±10.38% vs. 98.92%±12.12%, P<0.05).
CONCLUSIONSLung injury is a common extraintestinal complication of UC. According to the theory in Traditional Chinese Medicine that the lung and large intestine are related, both the lungs and large intestine should be treated simultaneously.

6.Pregnancy outcomes of patients with polycystic ovary syndrome undergoing in vitro fertilization and embryo transfer

Objectives To study pregnant outcomes of patients with polycystic ovary syndrome (PCOS) undergoing in vitro fertilization and embryo transfer (IVF-ET),and analyze the differences of pregnant outcomes in patients with various phenotypes of PCOS.Methods From Jan.2005 to Feb.2010,631 PCOS patients (PCOS group)and 1423 patients with tubal infertility (control group) who underwent IVF-ET with matched age and body mass index were selected in Center for Reproductive Medicine of the Provincial Hospital Affiliated to Shandong University.Retrospective study was carried out,and pregnancy outcomes were compared between two groups.Results The rates of abortion and preterm birth in PCOS group were significantly higher than those in control group [22.7% ( 143/631 ) vs.18.69% (266/1423) and 11.2% (38/339) vs.6.4% (51/794) respectively,all P <0.05 ].The rates of gestational diabetes mellitus were 1.5% (5/339) in PCOS and 0.6% (5/794) in control group,respectively; the rates of pregnancy induced hypertension syndrome were 4.7% (16/339) in PCOS and 3.0% (24/794) in control group; gestational days were(272 ± 13) days in PCOS and(273 ± l0)days in control group; the rates of neonatal deformity were 0.6% (2/339) in PCOS and O.8% (6/794) in control group; weight of newborn infants in the two groups was(3.5 ±0.5 ) kg; and there was no significant difference between two groups in the above index ( all P > O.05 ).Ovulatory PCOS patients had similar abortion rate [ 18.6% (19/102) ] and preterm birth rate [ 8.2% (4/49) ] when compared with those of control group (P > 0.05 ).Conversely,oligo-ovulatory PCOS patients showed higher abortion rate [ 23.4% ( 124/529 ) ] and preterm birth rate [ 11.7% (34/290) ] than those of control group ( P < 0.05 ).Conclusions PCOS patients after IVF-ET have an increased abortion rate and preterm birth rate.However,ovulatory PCOS did not present various pregnancy complications.Non-polycystic ovary PCOS patients have worse pregnancy outcome.Ovarian dysfunction might be related to obstetric complications.

8.Individualized monitoring and treatment for children with severe enterovirus 71 infection of different stages

Objective To investigate the clinicalmanifestation,monitoring and therapeutic measure of severe enterovirus 71 ( EV71 ) infection in children.MethodsForty-five cases of severe EV71 infectionwere admitted in our PICU from May 2010 to Sep 2011.The vital sign and arterial blood pressure,central venous pressure,mixed venous oxygen saturation,dynamic non-invasive heart function and urine volume were monitored.Forty-five cases were divided into 3 stages according to clinical manifestation:( 1 ) nervous system involvement stage; (2) respiratory system involvement stage; ( 3 ) circulatory system involvement stage ( compensation and decompensation).We adopted individualized remedy measure according to different stages.ResultsIn 45 cases,38 cases discharged from hospital,the cure rate was 84.4%.Among all the 38 cases,nervous system involvement was found in 19 cases,respiratory system involvement was found in 12 cases,circulatory system involvement was found in 7 cases.Seven cases died,who had circulation failure.ConclusionWe should identify severe EV71 infection early.Positive control of high fever,appropriate liquid treatment,control of high blood pressure,early respiratory support,preventment of circulation failure are the key measures for treatment.Individualized monitoring and treatment are effective in children with severe EV71 infection.
9.Use of recombinant human luteinizing hormone for ovulation stimulation in in vitro fertilization-embryo transfer

Objective To evaluate application of recombinant human luteinizing hormone (r-hLH)used in ovarian stimulation of assisted reproductive technique and impact on outcome of pregnancy. Methods From Apr. To Jul. 2009, 123 patients with low LH level ( < 1 U/L) at day 3 of menstruation and downregulation of pituitary function undergoing in vitro fertilization-embryo transfer (IVF-ET) in Reproductive Medical Center, Provincial Hospital Affiliated to Shandong University were enrolled in this study, whom were classified into 66 cases treated by r-hLH in r-hLH group and 57 cases without r-hLH treatment in non-r-hLH group. In the mean time, 145 patients with normal level of serum LH ( 1-2 U/L) not given by r-hLH treatment and undergoing IVF-ET were matched as control group. Total amount of gonadotropin, estradiol levels and LH levels on the administration of human chorionic gonadotropin ( hCG), number of oocytes retrieved, number of 2PN zygotes, rate of high quality embryos, the rates of implantation and clinical pregnancy were compared among these three groups. Results The level of serum LH on the day of hCG administration were ( 1.59 ± 0.77 ) U/L in r-hLH group, (0.54 ± 0.25 ) U/L in non-r-hLH group and (2.39 ± 1.01 ) U/L in control group, which reached tatistical difference between every two groups (P < 0.05). The rates of high quality embryo were 59.36% in r-hLH group, 57.79% in non-r-hLH group,which were significantly lower than 65.94% in control group, respectively (P < 0. 05 ). The rates of 2PN were 67.62% in r-hLH group and 68. 32% in control group, which were significantly higher than 62. 84% in non-r-hLH group, respectively ( P < 0.05 ). The rates of implantation of 29.77% in r-hLH group were significantly higher than 18.26% in non-r-hLH group ( P < 0.05 ). The total amount of gonadotropin,estradiol level on the day of hCG administration, the number of oocytes retrieved, and clinical pregnancy rate were not significantly different among those three groups ( P > 0.05 ). Conclusion The administration of recombinant human uteinizing hormone in patients who are profoundly suppressed after down-regulation with long protocol can get more quality embryos, the higher rates of 2PN and implantation.
10.Clinical characteristics of influenza A caused by H1N1 virus strain in critical children with acute respiratory distress syndrome and emergency treatment

Objective To describe the characteristics of and emergency treatment for and outcomes of critical ill children with 2009 influenza A caused by H1N1 virus strain. Method A prospective observational study of 3 pediatric patients with severe influenza A of H1N1 virus strain complicated with acute respiratory distress syndrome (ARDS) from November to December 2009. Results The H1N1 virus strain was confirmed by using realtime reverse transcription polymerase chain reaction (Real-time RT-PCR). Two patients survived and one died. Fever and cough were the onset symptoms. The systemic responses to influenza A at first were relatively mild. The tragic deterioration occurred all of a sudden with cyanosis all over the lips and dyspnea. The roentgenography showed bilateral multiple tabular pulmonary effusion and diffuse opaque shadows. The length of time required to confirm the diagnosis of ARDS from the symptom onset was 4 to 6 days. All patients were severely hypoxic with the ratio of PaO2 to 0.7-0.9 fraction of inspired oxygen (FiO2) to be 70- 100 mmHg at admission to PICU. In order to avoid injury to the lung, the protective ventilation strategy was carried out with low tidal volume (6 mL/kg) and adequate pressure,and conservative fluid management. Conclusions The H1N1 strain influenza virus A is characterized by pyrexia, cough and other respiratory symptoms in the early stage of critically ill children. In a few days, cough increased along with a sudden burst of cyanotic lips and shortness of breath, highly suggesting ARDS. Timely oxygen therapy and respiratory support, conservative fluid management, and the prophylaxis of secondary infection may be the essential measures. More clinical data are needed to clarify the critical features and to evaluate the emergency therapy for H1N1 influenza A in critically ill children.
