1.The different characteristic of magnetic resonance angiography of vertebral basilar artery between isolated vertigo and dizziness with ischemic stroke history

Objective To investigate the different characteristics of vertebral basilar artery between isolated vertigo and dizziness with ischemic stroke history by magnetic resonance angiography (MRA) information,and find the independent risk factors of isolated vertigo.Methods From January 2015 to January 2016,isolated vertigo patients from our department were enrolled in study group (vertigo group),and dizzy patients with ischemic stroke history in control group (dizziness group).The patient's general information,and the relevant vascula data of the MRA were statistically analyzed to find the risk factors.Results A total of 118 patients with isolated vertigo was enrolled in the vertigo group,and 74 patients with dizziness after ischemic stroke were used as a control group.There were significant statistical differences in mean diameter of the right vertebral artery,rate of stenosis of basilar artery,vertigo history,the left vertebral artery hypoplasia,basilar artery skewing,and basilar artery hypoplasia between two groups (P < 0.05).Multivariate logistic regression analysis showed that vertigo history (P =0.049,OR =3.822,95% CI =1.004 ~ 14.548),the right vertebral artery hypoplasia (P =0.001,OR =6.083,95% C1 =2.193 ~ 16.876),the left vertebral artery hypoplasia (P =0.006,OR =5.110,95 % CI =1.615 ~ 16.170),and mean diameter of the right vertebral artery (P =O.000,OR =3.143,95% CI =1.724 ~ 5.730) were independent risk factors for isolated vertigo,and basilar artery skewing (P =0.018,OR =O.436,95% CI =0.219 ~ O.866),and degree of basilar artery stenosis (P =0.006,0R =0.634,95% CI =0.459 ~0.877) were the protective factors.Conclusions The right vertebral artery hypoplasia,the left vertebral artery hypoplasia,and mean diameter of the right vertebral artery are independent risk factors for isolated vertigo.Basilar artery skewing and degree of basilar artery stenosis may be the protective factors.
2.Correlation between the characteristics of vertebrobasilar artery on magnetic resonance angiography and isolated vertigo

Objective To investigate the correlation between the morphological characteristics of vertebrobasilar artery on magnetic resonance angiography (MRA) and isolated vertigo. Methods The patients with isolated vertigo were enrolled retrospectively as a case group. Other patients without vertigo were selected over the same period as a control group. The clinical data and MRA data were collected. Univariate analysis and multivariate logistic regression analysis were used to identify the independent risk factors for isolated vertigo. Results A total of 118 patients with isolated vertigo and 179 controls were enrolled. Univariate analysis showed that there were significant differences in left vertebral artery bending degree, basilar artery stenosis rate, previous stroke, dolichoec tatic basilar artery, and left and right vertebral artery hypoplasia between the two groups (all P < 0.05). Multivariate logistic regression analysis showed that previous vertigo (odds ratio [OR] 6.177, 95%confidence interval [CI] 1.945-19.620; P = 0.002), more serious left vertebral artery bending (OR 1.860, 95% CI 1.127-3.069; P =0.015), and left vertebral artery hypoplasia (OR 4.543,95% CI 1.761-11.721; P = 0.002) were the independent risk factors for isolated vertigo, and previous stroke ( OR 0.377, 95% CI 0.162-0.877; P = 0.024), basal artery hypoplasia ( OR 0.401, 95% CI 0.193-0.830; P = 0.014) were its protective factors. Conclusions Previous vertigo, more serious left vertebral artery bending, and left vertebral artery hypoplasia are the independent risk factors for isolated vertigo, and previous stroke and basal artery hypoplasia may be its protective factors.
3.An investigation of the relationship between vascular cognitive impairment and the clinical types of cerebral infarction and leukodystrophy

ObjectiveTo assess the relationship between the location,quantity and leukodystrophy of cerebral infarction and vascular cognitive impairment ( VCI ),and to compare the differences of the severity and domains of cognitive impairment among the different types of cerebral infarction,which will provide the pathophysiological evidence to the mechanism for the development of VCI. MethodThe patients suffering from cerebral infarction or leukodystrophy were recruited from either the ward or clinic in Tianjin General Hospital from April 2009 to April 2010,then assessed the cognition,and classified into different groups according to the location,size,quantity,whether or not they have leukodystrophy and the severity.The Logistic regression analysis was taken with whether or not the patients have had developed to the VCI as the dependent variable,and with the location,size,quantity,whether or not they have leukodystrophy and the severity as the independent variables.The differences of the score of NIHSS,Bathel index and the level of cognitive impairment were compared among patients with different types of lesions.Moreover,the difference of the score of MoCA for each cognitive domain were compared among patients with different types of lesions.ResultsThere was a relationship between key position infarction in the subcortex and white leukodystrophy with VCI.The position infarction in the subcortex and white leukodystrophy ( grade Ⅱ and Ⅲ ) might be the independent risk factors for the VCI( OR =1.752,2.135,3.753; 95% CI 1.533-3.821,1.541-6.787,1.432-6.821,all P <0.05).There were significant differences in the domains of excusive ability,attention,calculation,fluent language and orientation.The average scores of the domain of excusive ability in the subcortical group (3.40 ± 1.90) and the mix group (3.83 ± 1.27) were significantly lower than the cortical disease group(4.28 ± 0.89,t =0.050,0.857,both P < 0.05 ).The average scores of the domains of attention,calculation,fluent and orientation in the mix group were significantly lower than the subcortical group and cortical disease group.ConclusionsThere is a significant association of the key infarction in the subcortex and leukodystrophy with VCI.The patients with both the subcortex infarction and leukodystrophy are susceptible to VCI.The development of VCI is depended on the position of infarction instead of the quantity and size of infarction.The development of VCI is not only related to the white matter disease but also related to the severity of the leukodystrophy.The data highlights the need to small vessel disease.The patients with subcortical lesions mainly impair the executive ability.The patients with mix lesions may have more extensive domains and more severe cognitive impairment.

Epiretinal membrane (ERM) is a relatively common macular disease that forms along the surface of the internal limiting membrane (ILM) of the retina to some reason.The pathogenesis is not clear.Microincision vitrectomy surgery has been confirmed as a minimally invasive and very safe modality of treatment.Deciding when to perform a vitrectomy can be difficult.There are many factors which can affect the postoperative visual acuity,such as age,the thickness of macular,integrity of photoreceptor inner segment/outer segment (IS/OS) junction.

6.Effect of levosimendan on pulmonary artery pressure in patients with pulmonary hypertension undergoing mitral valve replacement

Objective To investigate the effect of levosimendan on pulmonary artery pressure in patients with pulmonary hypertension undergoing mitral valve replacement.Methods Twenty-four ASA Ⅱ or Ⅲ and NY-HA class Ⅱ or Ⅲ patients,aged 35-60 yr,with mean pulmonary artery pressure (MPAP) > 30 mm Hg,undergoing mitral valve replacement were randomly divided into 2 groups (n =12 each):control group (group C) and levosimendan group (group L).In group L,a loading dose of levosimendan 24 μg/kg was injected intravenously after aortic unclamping,followed by infusion of levosimendan at a rate of 0.2 μg· kg-1 · min-1 until 1 d after operation.Group C received the equal volume of normal saline.HR,MAP,MPAP,pulmonary capillary wedge pressure (PCWP),cardiac index (CI) were recorded at 5 min after induction (T0),at the end of CPB (T1) and at 1 h after operation (T2),and the pulmonary vascular resistance (PVR) and rate-pressure product (RPP) were calculated.The improvement in pulmonary hypertension was recorded.Results PCWP was significantly lower and CI higher at T1,2 in both groups,and HR was significantly higher at T1,2 and MPAP lower at T2 in group C,and MPAP and PVR were significantly lower at T1,2 in group L than at T0 (P < 0.05).HR,MPAP and PVR were significantly lower and CI was significantly higher at T1,2,RPP was significantly lower at T2 and the improvement in pulmonary hypertension was higher in group L than in group C (P < 0.05).Conclusion Levosimendan can improve pulmonary hypertension without increasing the myocardial oxygen consumption and with a significant increase in myocardial contractility in patients with pulmonary hypertension undergoing mitral valve replacement.
7.Epidemiological investigation of an outbreak of anthrax in Gongliu County, Xinjiang Uygur Autonomous Region

Abstract
A suspected case of cutaneous anthrax was reported by Gongliu County Disease Control and Prevention Center, Ili Kazak Autonomous Prefecture, Xinjiang Uygur Autonomous Region on August 19, 2021. Then, an epidemiological survey was performed by a joint investigation team consisting of professionals from Xinjiang Uygur Autonomous Region Center for Disease Control and Prevention, intermediate-level trainees from the Field Epidemiology Training Program of Chinese Center for Disease Control and Prevention, and professionals from Ili Kazak Autonomous Prefecture Center for Disease Control and Prevention. A total of 11 cutaneous anthrax cases were identified, including 8 suspected cases and 3 clinically diagnosed cases, and all cases were villagers in Y Village, X Township, Gongliu County, without severe case or deaths found. The onset of the first case occurred on July 27, and the onset of the last case occurred on August 16. The main clinical manifestations included ulcerative eschar on hands and exposed skin of the upper extremity. A Bacillus anthracis isolate was detected in meat samples from infected cattle. Epidemiological surveys showed that villagers did not report infected cattle to related sectors and privately slaughtered and ate meat from infected cattle without any effective protective measures, resulting in this outbreak. It is recommended to strengthen health education for people raising, selling and slaughtering livestock, and publicize zoonotic disease control knowledge, including anthrax, and establish an effective surveillance and response system for anthrax for immediate identification and treatment of epidemics.
8.Comparison among different noncontact specular microscopes for the detection of corneal endothelial cell density

Background Clinical research showed that the corneal endothelial cell density value from different corneal specula microscopies exist diversity.The relevant literature of SP02,Tomey EM-3000 and SP3000P is still seldom up to now. Objective This research was to assess the repeatability of endothelial cell density measurements by SP02,Tomey EM-3000 and SP3000P respectively and the agreement among 3 kinds of endothelial microscopes.MethodsFifty-four healthy volunteers with the age 17-38 years old were included this research.The written informed consent was obtained from each subject before examination.The corneal endothelial cell densities in the right eyes were analyzed with SP02,Tomey EM-3000 and SP3000P respectively for 3 times under the automatic mode,and the analytical procedure of SP3000P measurement were divided into automatic mode SP3000P (A) and manual correction modes SP3000P( M).The repeatability of each specula microscopy was analyzed by calculating the intraclass correlation coefficients (ICC) and coefficient of variation ( CV ),and the 95% confidence intervals and plotting Bland-Altman graphs were used to analyze the agreement among these methods.ResultsThe mean corneal endothelial cell densities in the population <24 years were significantly higher than the ones ≥ 24 years (t =3.692,P<0.05 ),but no statistical difference was found between different gender ( t =0.335,P =0.739 ).The mean corneal endothelial cell densities were ( 3058 ± 260 ),( 2954 ± 229 ),( 2668 ± 258 ),( 2734 ± 268 ) cell/mm2 ; the ICCs were 0.957,0.940,0.972 and 0.972 and the CV were 0.063,0.061,0.056,0.058 for SP02,Tomey EM-3000,SP3000P (A) and SP3000P ( M ) respectively.The 95% confidence intervals were ( - 100.8 - 306.8 ),( 162.6 - 617.4 ),( 109.9-494.1 ) and ( -0.6 - 132.6 ) cell/mm2 for between SP02 and Tomey EM-3000,SP3000P ( A ) and SP02,SP3000P(A) and Tomey EM-3000,SP3000P(A) and SP 3000P(M) respectively.ConclusionsSP02,Tomey EM-3000 and SP3000P(A) have good repeatability in the measurement of corneal endothelial cell density,however the outcome is different.Therefore,it is not interchangeable for the detection of corneal endothelial cell density.The differences of corneal endothelial cell density obtained from these instruments shall be paid high attention for their differences.SP3000P(A) and SP3000P(M) can be used interehangeably and SP3000P(A) is a preferable choice due to its convenience and quickness.
9.Study on the Effect of Auricular Point Sticking on Sleep Quality of Patients Undergone Liver Transplantation

Objective To observe the effect of auricular point sticking on the sleep quality of patients undergone liver transplantation.Method The sleep quality was assessed by using sleep quality scale. The patients undergone liver transplantation were randomized into two groups, both receiving auricular point sticking, while the insomnia zone was selected in the observation group and non-insomnia zone was selected in the control group, 4 weeks as a treatment course.Result In 4 weeks after the transplantation, there were significant inter-group differences in comparing the scores of unfreshing sleep, difficulty falling asleep, nightmairs, medications, post-insomnia reactivity, and the total score of the self-rating scale of sleep (SRSS) (P<0.05); in 2 months after the transplantation, there were significant differences in comparing the scores of sleep latency, sleep efficiency, sleep disturbance, sleep medications, daytime dysfunction, and the global score of Pittsburgh Sleep Quality Index (PSQI) between the two groups (P<0.05), indicating that the sleep quality in the observation group was markedly improved. Three months after transplantation, there were no significant differences in comparing the sleep indexes between the two groups (P>0.05).Conclusion Auricular point sticking at the insomnia zone can markedly improve the short-term sleep disorder in patients after liver transplantation, and can reduce the dose of sleep medications.
10.Optimization of processing technology for xanthii fructus by UPLC fingerprint technique and contents of toxicity ingredient.

The experiment's aim was to optimize the processing technology of Xanthii Fructus which through comparing the difference of UPLC fingerprint and contents of toxicity ingredient in water extract of 16 batches of processed sample. The determination condition of UPLC chromatographic and contents of toxicity ingredient were as follows. UPLC chromatographic: ACQUITY BEH C18 column (2.1 mm x 100 mm, 1.7 microm) eluted with the mobile phases of acetonitrile and 0.1% phosphoric acidwater in gradient mode, the flow rate was 0.25 mL x min(-1) and the detection wavelength was set at 327 nm. Contents of toxicity ingredient: Agilent TC-C18 column (4.6 mm x 250 mm, 5 microm), mobile phase was methanol-0.01 mol x L(-1) sodium dihydrogen phosphate (35: 65), flow rate was 1.0 mL x min(-1), and detection wavelength was 203 nm. The chromatographic fingerprints 16 batches of samples were analyzed in using the similarity evaluation system of chromatographic, fingerprint of traditional Chinese medicine, SPSS16.0 and SIMCA13.0 software, respectively. The similarity degrees of the 16 batches samples were more than 0.97, all the samples were classified into four categories, and the PCA showed that the peak area of chlorogenic acid, 3,5-dicaffeoylquinic acid and caffeic acid were significantly effect index in fingerprint of processed Xanthii Fructus sample. The outcome of determination showed that the toxicity ingredient contents of all samples reduced significantly after processing. This method can be used in optimizing the processing technology of Xanthii Fructus.
