BACKGROUND:Toxic effects of excessive thyroid hormone on myocardium can not be ignored with the increased incidence of hyperthyroidism. As a quantitative marker of heart failure, brain natriuretic peptide has a good application prospect to estimate the changes of cardiac function and myocardial damage in hyperthyroid cardiomyopathy. OBJECTIVE:To explore the correlation of brain natriuretic peptide and cardiac function of rabbits with hyperthyroid cardiomyopathy exhibiting different left ventricular geometries. METHODS:Twenty New Zealand purebred rabbits were used in this study. Hyperthyroidism model was established by intraperitoneal injection of levothyroxine (45 μg/kg) for 4 weeks. The success of establishing model is defined upon the appearance of irritation, decreased body weight and increased appetite. Totaly 18 rats were successfuly established. The left ventricular data of hyperthyroidism rabbits were measured by conventional echocardiography. According to the classification of Ganauet al, the 18 hyperthyroidism rabbits were divided into two groups: concentric hypertrophy (n=7) and eccentric hypertrophy (n=11). In addition, ten rats, receiving intraperitoneal injection of 5 mL saline per day, were taken as the control group. RESULTS AND CONCLUSION: Plasma brain natriuretic peptide level of rabbits in the concentric hypertrophy and eccentric hypertrophy groups was obviously higher than that of the control group (alP < 0.01), and the level in the eccentric hypertrophy group was obviously higher than in the concentric hypertrophy group (P< 0.01). Compared with the control group, the average peak diastolic velocity of mitral ring was significantly lower in the concentric hypertrophy and eccentric hypertrophy groups (alP< 0.01), the average peak systolic velocity of mitral ring was also lower in the eccentric hypertrophy group (P < 0.01). The peak systolic and diastolic velocity had a negative correlation with brain natriuretic peptide (alP < 0.01). Experimental findings indicate that brain natriuretic peptide is a sensitive indicator to reflect the cardiac function and myocardial damage in rabbits with hyperthyroid cardiomyopathy, exhibiting different left ventricular geomeies.

Objective To explore the effect of traditional incision and drainage combined with vacuum sealing drainage (VSD)in treating patients with purulent exudation-staged genual septic arthritis. Methods The patients with purulent exuda-tion-staged genual septic arthritis were selected in the Zhengzhou Orthopedics Hospital from January 2010 to December 2015, and they were divided into control group and observation group. Fifty-four patients in the control group were treated with open operative drainage, while fifty patients in the observation group were treated with open operative drainage combined with VSD. The operation time, intraoperative bleeding volume, tourniquet application time, washing time, antibiotic use time, hospitalization time, therapeutic effect and relapse rate were compared between the two groups. Results There was no statistical difference in operation time and tourniquet application time and intraoperative bleeding volume between the two groups(t = - 0. 216,2. 518, 11. 320;P > 0. 05). Compared with the control group, the antibiotic use time and hospitalization time were shorter, the washing time was longer in the observation group(t = 2. 270,- 46. 310,2. 518;P < 0. 05). The occurrence rate of postoperative compli-cations in the control group and the observation group was 9. 3%(5 / 54)and 6. 0%(3 / 50),there was no statistical difference in the occurrence rate of postoperative complications between the two groups(χ2 = 0. 331,P > 0. 05). The fineness rate in the control group and the observation group was 25. 9%(14 / 54)and 22. 0%(11 / 50),there was no statistical difference in the fineness rate between the two groups(χ2 = 0. 219,P > 0. 05). The relapse rate in the control group and the observation group was 33. 3%(18 / 54)and 8. 0%(4 / 50),the relapse rate in the observation group was lower than that in the control group (χ2 =0. 331,P <0. 05). Conclusion Traitional incision and drainage combined with VSD has advantages of shortening hospital-ization and intravenous antibiotic time and decreasing relapse rate in treating purulent exudation-staged genual septic arthritis.

Objective To analyze the magnetic resonance imaging (MRI) features of spinal epidural angiolipoma to improve the diagnostic accuracy of this tumor. Methods The MRI findings in 2 patients with histopathologically confirmed spinal extradural angiolipoma were analyzed retrospectively. Results In both of the cases, the tumors were located in the extradural space of the thoracic intraspinal canal. The spindle-shaped tumor mass extended parallel to the longitudinal axis of the spine. The lesions presented with fat and vascular signal features. One of the tumors grew through the intervertebral foramen with a dumbbell appearance, invading the adjacent diaphragma, vertebral body, pedicle of the vertebral arch and the spinous process. The other tumor consisted of some tortuous and dilated blood vessels with flow void signals. Conclusion Spinal epidural angiolipoma presents characteristic features on MRI, which is considered the best diagnostic modality for this tumor.
