1.Clinical research in controlling infusion rate during nasal feeding by infusion pump

Objective To investigate and improve the methods of nasal feeding to reduce the occurrence of complication such as diarrhea and vomiting.Methods We separated 140 patients with heavy craniocerebral injury into the control group (50 patients) and the experimental group (90 patients) according to different nasal feeding methods. For the control group traditional method was used. For the experimental group infusion pump was used to control infusion rate. The incidence of complication was compared between two groups. And within experimental group, the same nutrient solutions in respect of type and volume were infused during 10 hours, 16 hours, and 18 hours respectively. The relationship between infusion rate and the occurrence of complication was observed, analyzed and compared to seek for the most suitable enteral nutrition route which incurred the least complication.Results The incidence of diarrhea and vomiting in the experimental group was lower than that of the control group. And there were no significant differences in the occurrence of complication by using infusion pump at three different infusion rates within the experimental group. Conclusions The application of infusion pump in nasal feeding can reduce the incidence of complication and infusing nutrient solution at a slow rate was not necessary.
2.A MORPHOLOGICAL STUDY OF CONNECTIONS BETWEEN ENKEPHALIN IMMUNOREACTIVE TERMINALS AND ?-AMINOBUTYRIC ACID IMMUNOREACTIVE NEURONS IN THE ROSTRAL PORTION OF THE NUCLEUS TRACTUS SOLITARIOUS OF THE RAT

Objective To observe the connections between enkephalin immunoreactive(ENK-ir) terminals and ?-aminobutyric acid immunoreactive(GABA-ir) neurons in the rostral portion of the nucleus tractus solitarious(rNTS) of the rat. Methods Double-immunofluorescent labeling method and the pre-embedding immunohistochemical staining combined with immuno-gold particles labeling technique for electron microscopic detection were used in the present study. Results Under the laser scanning confocal microscope,dense ENK-ir fibers and terminals and some GABA-ir neurons were observed in the rNTS.Close contacts between ENK-ir terminals and GABA-ir cell bodies were also detected.By using the electron microscopic technique,ENK-ir reaction products were found to be mainly localized on the surface of round clear vesicles and dense-cored vesicles in the axon terminals.ENK-ir terminals formed symmetric,which was a dominant pattern,and asymmetric synaptic connections with GABA-ir and immunonegative cell bodies and dendrites.Conclusion The ENK-ir terminals might take part in the transmission and regulation of the taste information in the rNTS through inhibiting,exciting the GABAergic neurons or inhibiting directly the activity of neurons within the rNTS.

4.Extracellular matrix and posterior capsular opacification

Posterior capsular opacification (PCO) is one of the major complications of modern cataract surgery.Multiple studies had demonstrated that the extracellular matrix (ECM) plays an important role in the generation and development of PCO.A variety of ECM components,including fibronectin (FN),laminin (LN),collagen,lumican,osteoponin (OPN),decorin,hyaluronic acid and vimentin,affect the biological properties of lens epithelial cells (LECs) through different mechanisms,such as regulating cell migration,adhesion and proliferation.This review focuses on the role of ECM in PCO formation in order to explore the pathogenesis of PCO and provide a potential therapeutic approach for the prevention and management of PCO.
5.Impacts on chronic fatigue syndrome of qi deficiency syndrome and T cell subgroups in patients treated with acupuncture at selective time.

OBJECTIVETo verify the clinical efficacy on chronic fatigue syndrome of qi deficiency syndrome treated with acupuncture at selective time and explore the effect mechanism.
METHODSEighty patients were randomized into a selective-time-acupuncture group and an acupuncture group, 40 cases in each one. Qihai (CV 6), Guanyuan (CV 4), Hegu (LI 4), Taichong (LR 3), Sanyinjiao (SP 6) and Zusanli (ST 36) were selected in the two groups. In the selective-time-acupuncture group, acupuncture was used at 9:00am to 11:00am. In the acupuncture group, acupuncture was used at any time except in the range from 9:00am to 11:00am. No any manipulation was applied after the arrival of needling sensation. The treatment was given once every day, 10 day treatment made one session and two sessions of treatment were required. The fatigue scale was adopted to evaluate the efficacy before and after treatment in the patients of the two groups. The ratios among CD3+, CD4+ and CD8+ T cells in the peripheral blood were detected before ad b a after treatment.
RESULTSIn the acupuncture group, the total score of fatigue and the score of physical fatigue were reduced after treatment as compared with those before treatment (all P<0.05). In the selective-time -acupuncture group, the total score of fatigue, the s core of physical fatigue and the score of mental fatigue after treatment were reduced obviously as compared with those hefore treatment (all P<0. 01). The improvements in the scores of the selective-time-acupuncture group were superior to the acupuncture group (all P<0. 05). The ratio of CD3+ and CD8+ T cells was increased obviously after treatment in the two groups (all P<0. 05) and the ratio of CD4+ and CD8+ T cells was reduced obviously in the selective-time-acupuncture group (P<0. 05), which was better than that in the acupuncture group (all P<0.05). The total effective rate was 95.0% (38/40) in the selective-time-acupuncture group, which was better than 80.0% (32/40) in the acupuncture group (P<0.05).
CONCLUSIONThe acupuncture therapy at selective time is effective in the treatment of chronic fatigue syndrome of qi deficiency syndrome, which is especially better at relieving mental fatigue. The effect of this therapy is achieved probably by improving the immune function via the regulation of the ratios among CD3+, CD4+ and CD8+ T cells.

7.Clinical studies of L-Arg effect on essential hypertension

AIM and METHODS:To investigate the effect of L-arginine -nitric oxide pathway on patients with essential hypertension via hemodynamics and neuroendocrinology. 24 essential hypertension patients were randomly divided into two groups, group I was given L-Arg, and groups Ⅱ was given normal saline as control. Blood pressure, heart rate, heart funtion, nitric oxide, angiotensinⅡ, endothelin, thromboxane A 2 and prostacyline were measure in all patients. RESULTS: In group Ⅰ arterial pressure decreased, heart rate increased, cardial output, systolic volume and eject fraction increased, total peripheral resistance decreased. NO and PGI 2 levels were inceased. But at 80 min , with NO concentration decreased, SBP,DBP were increased, TPR, FT and AngⅡ were also increased. While HR, CO, SV and EF were decreased. However TXA 2 and PGI 2 showed not much change. CONCLUSION: Exogenous L-arginine produced a vasodilatory effect by increasing NO production ,caused the change of other hemodynamic function . It also indirectly changed plasma ET, AngⅡlevels by negative feed-back and suppressed the accumulation of platelet.

9.Clinical analysis of 78 cases of critical asphyxia neonatorum

Objective To investigate measures of preventing asphyxia neonatorum in order to reduce the rate of asphyxia neonatorum.Methods Clinical datas on a total of 78 cases of critical asphyxia neonatorum from the year 2003~2004 in Longgang Central Hospital were collected and analyzed.Related high risk factors and effective precaution measures of reducing the rate of asphyxia neonatorum were studied.Results The rate of asphyxia neonatorum from January 2003 to December 2004 was 4.22%,of which it was 1.70% of critical asphyxia neonatorum and 8% of treated deaths.The first seven related factors to critical asphyxia neonatorum of this group were:fetal distress(47%),premature delivery(49%),umbilical factors-torsion,prolapse,shortening(32%),breech presentation(26%),high risk PIH(14%),monstrosity(6%),placental abruption and placenta praevia(6%).Conclusion Effective measures of reducing critical asphyxia neonatorum are to strengthen the monitoring of pregnant women and their antenatal care so as to find and treat PIH early and correct breech presentation in time,try to reduce premature delivery,find early and properly treat fetal distress and oafs and prohibit illegal accouchement.
10.Applications of Porous Scaffolds in Muscle Tissue Engineering.

Scaffold is one of the key elements required for tissue engineering. Porous scaffolds have several special advantages for muscle tissue engineering, and they are beneficial to cell survival, myogenic differentiation, and vascular ingrowth. The performance of porous scaffolds is closely related to the property of the biomaterials used. Additionally, the pore size and porosity may affect cell adhesion, proliferation, and differentiation. This review focuses on the application of porous scaffolds in muscle tissue engineering, including their categories, application, and advantages.
